RESOLUTION NO. 10-88 <br />A RESOLUTION AMENDING SECTIONS 8.12, ON-THE-JOB INJURIES; 8.13, <br />LIGHT DUTY WORK STATUS; 8.14, WORKER'S COMPENSATION AS PROVIDED <br />UNDER THE ILLINOIS REVISED STATUTES OF THE PERSONNEL RULES AND <br />REGULATIONS OF THE VILLAGE OF ELK GROVE VILLAGE <br />BE IT RESOLVED by the President and Board of Trustees of the Village <br />of Elk Grove Village, Counties of Cook and Du Page, Illinois as follows: <br />Section 1: That Sections 8.12, On -the -Job Injuries; 8.13, Light <br />Duty Work Status; and 8.14, Worker's Compensation, of the Personnel Rules <br />and Regulations of the Village of Elk Grove Village be and the same is <br />hereby amended to read as follows: <br />WORKMEN'S COMPENSATION BENEFITS <br />APPLICATION: To All Village Employees <br />PURPOSE: To establish and set forth Village Policy with regard <br />to circumstances related to injuries and/or illnesses <br />arising out of or in the course of employment with the <br />Village of Elk Grove Village. <br />POLICY: The Village and all Village employees operate under and are <br />subject to the Illinois Worker's Compensation Act and the <br />Illinois Occupational Diseases Act. <br />It is the Village's policy to provide a safe and healthful <br />work environment. <br />Employees are required to perform their jobs in a safe, <br />attentive manner. <br />Notwithstanding the above requirements, employees may <br />occasionally suffer an accident injury or illness arising <br />out of, or in the course of, employment. In such cases, the <br />Village will provide and pay for all necessary first aid and <br />any reasonable medical services required as a result of the <br />injury or illness. <br />In accordance with the aforementioned laws, employees may <br />elect to secure their own medical treatment for an injury or <br />illness arising out of, or in the course of employment with <br />the cost for such services paid by the Village. <br />A. REPORTING REQUIREMENT <br />An employee who is injured or becomes ill as a direct result <br />of their work must report the injury or illness to their <br />supervisor immediately, unless physically una a to do so. <br />The Supervisor is re uired to complete the "Supervisor's <br />Investigation Report form and submit it to the Finance <br />Department IMMEDIATELY for insurance and recording purposes. <br />The report must be complete. <br />If the report cannot be submitted to the Finance Department <br />that same day, the Supervisor is reluired to verbally <br />notify his/her Department Head and t�nance Department <br />that the incident has occurred. The report must then be <br />completed and submitted within twenty-four (747—hours. <br />B. COMPENSATION FOR JOB RELATED DISABILITY <br />-1- <br />
