CONSENT AGENDA A) Consideration of the following: - To approve the Balancing Change Orders for- the Biesterfield Road restriping project in the amount of $29,977.45; and, - To approve the Final Pay Estimate for the Biesterfield Load restriping project to Palumbo Brothers, Inc., hillside, Illinois, in the amount of $29,977.43. (The project was completed at a cost of $347.55 under the bid price. (A report from the Director of Engineering and Community Development recommending approval is attached.) B) Consideration of the following renewals and costs associated with the Village's self -insured group medical and dental programs: - To authorize the renewal of the agreement with Gallagher Bassett to administer the Village's self -insured medical and dental programs at a cost of $13.70 per participant per month for medical claims and $5.�n per pariticipant per month for dental claims; - To renew the Village'F contract wlt.b Lloyds of London to provide specific stop Loss Coverage for a sixteen -month pezr.od at a cost of $.384,624; - To renew the V•i.11age'�: contract with Lloyds of London to provide Aggregate Stop Loss Coverage for a ,sixteen -month period at a cost, of $18,000; . Agenda for VB Meeting 12-11-90' - To authorize the payment of $4,000 to Arthur Gallagher and Co. for consulting services; - To authorize Cost Care Consultation Services through Gallagher Bassett at a monthly cost of $4.80 per medical plan participant. (The profession service fees, consulting fees, and specific and aggregate stop .loss coverage casts are shared by the Village, the Library, the Park District, and Northwest Central Dispatch system, based on participation levels.) C) Consideration to approve the rollowing monthly rates for individual employee, employee plus one (1) dependent, and employee plus two (2) or more dependents in HMO Illinois and Health Chicago HMO as alternatives to conventional health insurance: 1991 Participation Monthly Carrier Level Premium HMO Illinois Employee only $120.48 Employee + 1 dependent 230.87 Employee + 2. or more dependents' 355.59 Health Chicago HMO Employee only $135•99 Employee + 1 dependent 289.58 Employee + 2 or more dependents 383.49 Delta Dental Irmo $ 12.89 _ Employee only Employee + 1 dependent 25.65 Employee + 2 or more dependents 46.34 D) Gonside.ration of the following: - A request from School District #59 (attached) to waive the 1991 Alarm User License fees in the amount. of $ 200; and, A request f..rom the Elk Grove Park District (attached) to waive the 1991 Alarm User License fees in the amount of $175, (School District #59 requests the waiver for alarms at five elomentary s.hools and Grove Junior High, The Park District requests the waiver for •alarms at seven locations. Agenda for V73 Meeting 12-11-90 (It has been the past praq.tice of the Village Board to waive the.3e fees for other governmental agencies. (A report from the Assistant Director of Finance, Is attached.) E) Consideration to adopt RESOLUTION NO. 1-91 (attached), a Resolution rescinding Resolution No. 61-89 _ and authorizing the Village President and Village clerk to execute a Joint Agreement for the., Federal project Elmhurst Road F.A.U.S. (Devon Avenue to Greenleaf). (Resolution No. 61-89 was adopted by the Village Board on•September 26, 1989 and related to a Joint Agreement with the Department of Transportation for right-of-way acquisiticn costs in an amount not to _exceed $30,000 for the Elmhurst Road F.A.U.S. project. Since the Raid costs will approximate $38,000', a revised agreement is required. The revised agreement includes ROW -acquisition costs up to $40,000. (This Joint Agreement provides for the .100 percent reimbursement of all costs associated with land acquisition fo. the project. (A report from the Director of Engineering and Community Development is attached recommending approval of the Joint Agreement.) F; Consideration to adopt RESOLUTION NO. 2-91 (attached), a resolution authorizing Village officials to sign a work permit from the Illinois State Department of Transportation for work performed.by the Village or its contractors in State-owned rights -of -way. (The State of Illinois annually requests municipalities to adopt such a resolution prior to work being performed in State rights -of -way, including emergency work on broken watnrmains or sanitary sewers. (The resolution is adopted annually by the Village.) . Agenda for VB Meeting 12-11-90 -4 - REGULAR AGENDA 7. The existing free-standing sign on the lot advertises the Midas Shop only at that location although the strip center is comprised of a Midas Muffler, oil Express, and Fender Menders. The petitioners require exposure of their businesses 6n the Busse frontage because of the, layout of the complex. (A report from the Director of Engineering and Community Development recomwending approval is attached.) 8) Consideration of a request (attached) from Charles J. All required work has been completed and with the approval of the attached documents the Village can accept the improvement, close out the contract and release the contractor (Palumbo Bros., Inc.) from further responsibilities on the project. The project's engineering, design and inspection was performed by the Village's Engineering Division. Please note that the Balancing Change Order which covers all changes to the project lists a net change to the contract of $347.55 under Bid Price. The project was completed at a total cost of $29,977.45 while the contractor's Bid Price was $30,325.00. r respectfully recommend apprcval of the Pinal Pay Estimate and Balancing Chan qe Order, and, acceptance of the Biesterfield Road Nestriping project. I would request that this matter be presented (!) the President and Board of Trustees at the January 8, 1991 Village Board meeting. All right of way costs were to be paid for by the r1linois Department of Transportation and this Agreement authorized funding UP to $l 0, 000.
