Dear President Craig:
Following discussion with Vito Sammarco, we have prepared this Agreement, in duplicate, for
installation of emergency vehicle pre-emptive devices (hereinafter EVPD ) on the proposed
traffic signal at Devon Avenue and Ellis Street.
Accordingly, the Village agrees that it will pay for one hundred (100%)percent of the costs for
the new EVPD, including installation, (estimated $5,000.00)on Cook County's traffic signal.
Said costs will be billed to the Village following installation of the EVPD.
Further, by execution of the Agreement, the Village of Elk Grove and its Fire Department
acknowledge that they shall not have access to or on County's traffic signal and as such, agrees to
use Cook County's Electrical Maintenance Contractor for repair of said EVPD, enter into an
Agreement with said Contractor for EVPD repair and pay directly to the Contractor for said
repair costs as they may occur.
Should the Village of Elk Grove be agreeable to the terms of this Agreement, please countersign
two copies of this letter and return them to this Department, Attention: Gerald Wulkowicz,
