A RESOLUTION ADOPTING A PROGRAMS & SERVICES
SELF EVALUATION (ADA 1990)
NOW, THEREFORE, BE IT RESOLVED by the President and Board of Trustees
of the Village of Elk Grove Village, Counties of Cook and DuPage, State of
Illinois as follows:
Section 1: That the Village President and Board of Trustees hereby
adopt the document entitled:
Village of Elk Grove
Programs & Services - Self Evaluation
Americans with Disabilities Act of 1990
a copy of which is attached hereto and made a part hereof as if fully set
forth and is ordered to be placed on file in the office of the Villace
Clerk.
Section 2: That this Resolution shall be in full force and effect
from and after its passage and approval according to law.

VILLAGE OF ELK GROVE
PROGRAMS & SERVICES - SELF EVALUATION
AMERICANS WITH DISABILITIES ACT OF 1990
Under Title II of the Americans With Disabilities Act, public
entities employing 50 or more persons are required to perform a
self-evaluation of Village policies, practices, and procedures.
The purpose of the self-evaluation is to ensure that individuals
with disabilities are offered an equal opportunity to participate
and benefit from Village programs and services. Any area that is
inconsistent with the ADA's non-discrimination requirements must
be identified and corrected through a self-evaluation document.
Village Staff performed the self-evaluation of Village
programs and services against a checklist of eight (8)
non-discrimination requirements under TITLE II of the Act. The
evaluation examined application/registration forms, written
policies, rules, and regulations of programs and services,
program brochures and other official publications. For each
requirement, current practices not in compliance are identified
along with the modifications necessary to correct them. Those
procedures that are in compliance with any given requirement are
noted as such.
ADA Non-Discrimination Requirements
1 . Qualified disabled individuals are to be given an
opportunity to participate and benefit from a program or
service. This includes the provision of reasonable
auxiliary aids if necessary and desired by the
individual to result in the equal provision of the
service (Auxiliary aids for the hearing and visually
impaired include sign language interpreters, large print
materials, readers, etc. ) .
A. Disabled Assistance
If requested, the Village will provide reasonable
auxiliary aids including a sign language
interpreter, large print materials, readers, etc.
In addition, clipboards are provided at all
counters of the municipal complex to
assist disabled individuals in filling out
forms/applications.
B. Meeting Notice
A statement appears on all meeting notices_
and agendas issued by the Village
for any public meeting. That statement
notifies individuals who are in need of
reasonable accommodations to contact
the Village Clerk's office preferably
no later than five days before the
meeting.
C. Purchasing
A statement was added to the "Instructions to
Bidders" informing vendors to contact the Finance
Department prior to a bid opening if special
accommodations are necessary (sign language
interpreter, large print material, reader, etc. )
D. Telecommunications Device For the Deaf (TDD)
A TDD has been installed at the Finance Counter.
Individuals calling the TDD line will be able to
communicate with all Village Departments by
leaving a message and having the appropriate
staff person return the call as soon as possible.
The Northwest Central Dispatch has a TDD line
available for emergency (9I1) access.
E. Newsletter & Other Village Publications
In future editions of the Elk Grove Villager
and other Village publications a section will be
added notifying the public that reasonable
accommodations will be provided at meetings
for persons with disabilities when requested
in advance.
2. Contractors who bid and/or provide services to
the Village are to be made aware that they must not
discriminate on the basis of a disability when
performing activities on behalf of the Village.
A. Award of Contracts
The "Instructions to Bidders" contains a
requirement for vendors to recognize the ADA
guidelines.
In addition, Purchase Order forms are in the
process of being revised to include ADA
requirements.
3. The Village may not discriminate in the selection
of sites or location of meeting room facilities so
as to exclude or impair program objectives with _
respect to disabled persons.
A. Meeting Room Policy
The meeting room policy and application
forms will be amended to accommodate
organizations who have disabled
members when requesting a public meeting
room. Currently, meeting rooms are
available in the municipal complex
and Meacham & Greenleaf Fire
Stations. The meeting rooms in the
Municipal Complex are handicap
accessible while the fire station meeting
rooms are not.
organizations who have disabled members will
be scheduled a meeting room in the municipal
complex. If necessary, organizations
scheduled for a meeting room in the Municipal
Complex will be relocated in order to
accommodate organizations with disabled
members. Since the Village 's meeting room
policy was approved by resolution, the
current policy will need to be amended by
resolution.
CPR classes are currently scheduled in
the Greenleaf Fire Station. Classes with
disabled participants will be relocated
to a meeting room at the Municipal Complex.
4. Licensing and certification programs must
not discriminate against qualified
disabled individuals with respect to
their disability.
The Village does not in policy,
practice, or procedure discriminate
against qualified disabled individuals
when applying for a license or
certification from the Village.
5. Eligibility criteria for programs and
services must not discriminate against
the disabled unless the exclusionary
criteria is necessary for the provision
of the service or program. In addition,
the imposition of special requirements
for the disabled is prohibited.
Disabled individuals are not excluded
from Village services or programs. As
stated previously, reasonable accommodations
will be provided if necessary and desired
by the disabled individual. Special
requirements are not imposed upon
disabled individuals when participating
or benefiting from a program or service.
6. Disabled individuals are not to be
assessed a surcharge to cover costs
related to the provision of auxiliary
aids.
Any costs in the provision of reasonable
auxiliary aids will be borne by the Village.
7. Services are not to be denied to
individuals or entities who have a
relationship or association with a
disabled individual .
The Village does not by policy, practice or
procedure deny services to individuals or entities
who have a relationship or association with a
disabled individual.
8. Employers are prohibited against
implementing discriminatory
personnel policies with respect to
disabled individuals.
A. Employee Awareness
Management and Department Heads have attended
various seminars as well as an in-house seminar
on Americans With Disabilities Act issues.
Literature on the proper etiquette in meeting
and providing services to individuals with
disabilities will be distributed to those
employees who have regular contact with the
public.
B. Job Descriptions
Job descriptions are currently being revised to
include the essential functions of the position.
C. Hiring Practices
1 . During an interview Village employees
will not ask questions regarding an
applicant 's disability or previous
worker's compensation claims.
2. Applicants who request reasonable
accommodations (i.e. interpreter,
reader, etc. ) for applications and/or
interviewing process will have the
request met.
3. Village employment application forms note
that job descriptions are available for
an applicant to determine if they can
perform the essential functions of the
position. If accommodations are necessary
for the applicant to perform the job, the
applicant will be instructed to request the
type of accommodation.
4. It is current practice of the Village to
offer employment conditional upon passing
an employment physical.
5. Village employees who have become disabled
as defined by the Act will remain employed
if the essential functions of the job can
be performed without undue hardship for the
Village to accommodate their disability.
