(ELMHURST ROAD PHASE II - SECTION 2) NOW, THEREFORE, BE IT RESOLVED by the President and Board of Trustees of the Village of Elk Grove Village, Counties of Cook and DuPage, State of Illinois as follows: Section 1: That the Village President be and is hereby authorized to sign the attached documents marked: "FIRST SUPPLEMENTAL AGREEMENT FOR CONSULTANT SERVICES ELMHURST ROAD PHASE II - SECTION 2" a copy of which is attached hereto and made a part hereof as if fully set forth and the Village Clerk is authorized to attest said documents upon the signature of the Village President. (31 & 3035)N-89 Limits: Landmeier Road to Elmhurst Road County: Cook This is a SUPPLEMENTAL AGREEMENT between the VILLAGE OF ELK GROVE VILLAGE, hereinafter referred to as the "VILLAGE", and TRITON CONSULTING ENGINEERS, LTD., 830 North Meacham Road, Schaumburg, Illinois 60173- 4922, a firm of Consultants, hereinafter referred to as the "CONSULTANT". The VILLAGE and the CONSULTANT entered into an Agreement on December 29, 1988 to furnish certain professional services in connection with the proposed improvement of Elmhurst Road and Illinois Route 72, which work is hereinafter referred to as the "PROJECT". The VILLAGE proposes to engage the CONSULTANT to furnish additional engineering services for the preparation of Contract Plans, right-of-way plats, and legal descriptions. The VILLAGE has authority under Section 4-201.4 of the Illinois Highway Code to engage such services, and the CONSULTANT represents that he/she is in compliance with Illinois Statutes relating to professional registration of individuals and to corporate practice, if a corporation, for rendering such services. The CONSULTANT, in signing this AGREEMENT, certifies that he (she) has no financial or other interests in the outcome of this project. The CONSULTANT (if owned by an individual) in signing this AGREEMENT certifies that he (she) is not in default on an educational loan as provided in Public Act 85-827. Under penalties of perjury, the CONSULTANT certifies that 363595778 is their correct Federal Taxpayer Identification Number and they are doing business as a corporation. Additional meetings and coordination was required for the following agencies: Agency Amoco Oil Company Universal Chemical Company Burger King Restaurant Perk's Hot Dog Restaurant Cook County Highway Department Item 4: Drainage Design Purpose Right -of -Way and Roadway Impacts Right -of -Way and Roadway Impacts Right -of -Way and Roadway Impacts Right -of -Way Impacts Landmeier Road improvements The change in the type of project from rehabilitation to reconstruction provided the opportunity to improve the drainage system along Illinois Route 72 and Elmhurst Road. Improvements to the drainage system included replacement of all laterals to meet size and cover requirements, reduction of the inlet spacing to meet updated standards, and partial replacement of the mainline storm sewer. , Item 5: Preliminary Lighting Plans Although the Plan Preparation Advisory was followed concerning the preparation of lighting plans, there were significant changes made by IDOT Bureau of Electricity after system layout had been approved. These changes included lighting system layout, lighting system limits, circuit diagram, maintenance responsibility and transfer box details. Item 6: Preliminary Roadway Plans Following the preliminary plan submittal IDOT's policy on pavement design was updated to a Mechanistic Pavement Design method. The roadway pavements were changed from full -depth bituminous to hinged -joint P.C. Concrete based on this policy change. Plats and Legals 26 168 hrs. Modifications to the right-of-way and temporary easement limits were required due to the detailed design analysis following the approval of the Project Report. Additional temporary easements were required to reconstruct the pavement and driveways within tight right-of-way limitations. In addition, the complexity of each of the parcels was not anticipated in the Original Agreement. The original estimate for preparing the plats, legal description, and monumentation for each of the parcels was $1550 per parcel, but the actual costs incurred were $2300 per parcel. Ten of the eleven temporary easements were located in parcels requiring partial takings. Parcel 0024TE was located in a new property and required ordering a title report. Ludlow and Associates, the registered land surveyor subconsultant, has submitted $2,965 in bills for review of this additional work. Item 8: Preliminary Specifications & Estimate A. Building Removal and Demolition Special Provision When the original Agreement was executed, it was not anticipated that the special provisions for the demolition of the two (2) restaurants and one (1) garage would be made part of the Contract Documents for the roadway project. Additional work was required to prepare six (6) Special Provisions and provide a cost estimate for each item. B. IDOT Recurring Special Provision Change In January 1991 the format of the IDOT's Recurring Special Provisions hours was changed following 'submittal of the preliminary plans.
