HomeMy WebLinkAboutRESOLUTION - 21-80 - 2/12/1980 - PERMIT FOR DRIVEWAY/COOK COUNTYRESOLUTION NO. 21-80 A RESOLUTION AUTHORIZING THE VILLAGE PRESIDENT TO SIGN A PERMIT WITH THE COOK COUNTY HIGHWAY DEPARTMENT FOR THE CONSTRUCTION OF A DRIVEWAY ENTRANCE, 15" CULVERT, AND SIDEWALK (GULLO, NORTHWEST CORNER OF LIVELY BOULEVARD AND LANDMEIER ROAD) NOW, THEREFORE, BE IT RESOLVED by the President and Board of Trustees of the Village of Elk Grove Village, Counties of Cook and DuPage, Illinois: Section l: That the Village President is hereby authorized to sign a Permit with the Department of Highways of the County of Cook for the construction of a driveway entrance, 15" culvert, and a sidewalk on Landmeier Road, a copy of which is attached hereto and made a part hereof as if fully set forth. Section 2: That this Resolution shall be in full force and effect from and after its passage and approval according to law. PASSED this 12th day of February ig80. The Village of Elk Grove hereby accepts full responsibility for the future maintenance, replacement, removal and relocationof the sidewalk mentioned herein. This permit does not call for the installation of any utilities in the R.O.W. of Landmeier Pd. The general contractor, before starting the job, will deposit with the Cook County Highway Department, Permit Division, insurance as required on Foran "B." A performance and R.O.W. restoration bond in the amount of $5,000.00 issued by on bchalf of Gallo Interratiornl Develrp- ment Co. has been deposited with the LPe mit Division of the Cook County F:ignway Department. That said applicant shall furnish all material to do all work required, and pay all costs which may be incurred in connection with such work, and shall prosecute the same diligently and with- out delay to completion within the time hereinafter stated. SEE STAND 2. That the construction work will be completed by the day of , 19 , and shall be in accordance with the Standard Specifications for Road and Bridge Construction of the Illinois Department of Transportation adopted July 1, 1976 and the Supplemental Specifications thereto of The County of Cook. 3. That upon completion of said work, said applicant shall, within 10 days, at its own cost, restore said highway substan- tially to the same condition in which it was before said work was commenced and shall remove all debris, rubbish, materials, apparatus, tools and equipment, as well as all excess excavated materials, from the right-of-way of said highway, all to the satisfaction_of t1le County Superintendent of Highways for the County- o -f' Cook- 4. That should future construction and operation of said highway by The County of Cook, require alteration or change of location of the improvement referred to herein, such change shall be made by the applicant upon the written request of The County of Cook, without expense to the said County. 5. That the said applicant , � successor or assigns shall assume all risk and liability for accidentsand damages that may accrue to persons and property, during the prosecution of the work or any time thereafter, by reason of the location, construction and work referred to herein, and said applicant_agree .to indemnify and save harmless the County of Cook, from any such claims for damages and from all costs and expenses incurred on account thereof and in connection therewith. 6. All trenches and openings made in Cook County Highway Department Right -of -Way shall be backfilled with sand or limestone screenings adequately compacted in accordance with either Method 2 or Method 3 apecified in Article 603,08 of the State Standard Specifications, and all pavement replaced under the supervision of the Cook County Highway Department, Permit Engineer. 7. This opening must be inspected daily and additional bituminous material must be placed, daily if necessary, to maintain the patched area at the same elevation as the adjacent undisturbed pavement surface, for a period of not less than _180 days. No depressions or dips in the patched area will be permitted during the above period of time. All auger pits shall be a minimum of ten (10) feet from the edge of pavement. 8. That all tunnels shall be securely braced and backfilled by pressure Grouting. 9. That wool or steel sheeting shall be used to protect base course and pavement if -deemed necessaryby th3 County Permit Engineer. 10. That if it is found during the progress of the work, that subterranean conditions prohibit the construction of said improvement in and along the alignment as outlined in the plans, it is expressly understood that construction operations shall cease, if directed by the Permit Engineer, until proposed revised alignment has been approved by the Cook County Highway Department. 11. That without further action, The Cook County Highway Department reserves the right to make connections to the proposed sewer for the purpose of draining the highway. 12. That the Contractor or Applicant shall be responsible for providing positive drainage.
