A RESOLUTION AUTHORIZING THE VILLAGE PRESIDENT AND VILLAGE
CLERK TO EXECUTE AN INSTRUMENT OF CONVEYANCE CONVEYING
CERTAIN PROPERTY TO ELK GROVE TOWNSHIP
WHEREAS, the Village of Elk Grove Village is the owner of certain
real estate legally described on Exhibit A (attached hereto) said
property being donated to the Village pursuant to an Annexation Agree-
ment and Amendment thereto, said Annexation Agreement being between
the Village, River Forest State Bank and Trust Company, and Trustee,
under Trust Agreement dated November 18, 1970 and known as Trust No.
1688, and SBL Associates; and
WHEREAS, it is the desire of the President and Board of Trustees
of the Village of Elk Grove Village to convey said property to the Elk
Grove Township as long as the property is to be primarily used as a
mental health center to provide care and services, program and facili-
ties to persons needing mental health treatment, including the mentally
retarded, and/or the alcoholic and drug addicted and incidentally used
for other community health programs and services.
NOW, THEREFORE, BE IT RESOLVED by the President and Board of
Trustees of the Village of Elk Grove Village, Cook and DuPage Counties,
Illinois, as follows:
Section 1: That the Village President and Village Clerk be and are
hereby authorized to execute a Quit Claim Deed, a copy of which is
attached hereto as Exhibit A, thereby conveying the real estate legally
described therein to Elk Grove Township subject to the covenant and
condition that said real estate shall be used primarily as a mental
health center to provide care, services, programs and facilities to
persons needing mental health treatment, including the mentally retarded
and for those addicted to alcohol or drugs and incidentally used for
other community health programs and services.

Section 2:
That if any breach of the covenant and condition
provided in Section 1 shall occur the Village shall have the right to
cause title to said real estate to REVERT TO THE VILLAGE OF ELK GROVE
VILLAGE IN THE FOLLOWING MANNER:
a) The Village Clerk shall, by certified mail, to the
Clerk of the Elk Grove Township, give notice that
said covenant and condition has been breached.
b) That if such breach has not been cured within ninety
(90) days from the receipt of said notice the Village
Clerk shall register a Declaration of Breach of
Covenant with the Registrar of Titles of Cook County,
Illinois.
c) That upon the registration of such declaration title
to the realty shall revert to the Village of Elk Grove
Village.
Section 3: That the Village President and the Village Clerk are
hereby authorized to execute such other documents as are deemed
necessary to effectuate the aforegoing after receiving a certified copy
of the ordinance of Elk Grove Township declaring the transfer of title
to such real estate to the township to be necessary, convenient and sub-
ject to the covenant, condition and right of reversion provided for in
sections 1 and 2 hereof.
Section 4:
That all Resolutions in conflict with the provisions
hereof are hereby repealed.
Section 5: That this Resolution shall be in full force and effect
from and after its passage and approval according to law.

QUIT CLAIM DEED
THE GRANTOR, the Village of Elk Grove Village, a Municipal
Corporation created and existing under and by virtue of the laws
of the State of Illinois, for the consideration of Ten ($10.00)
Dollars, in hand paid, and pursuant to authority given by the
Board of Trustees of said Village CONVEYS and QUIT CLAIMS unto
Elk Grove Township, a Municipal Corporation, created and existing
under and by virtue of the laws of the State of Illinois having
its principal office in the Village of Elk Grove, County of Cook
and State of Illinois all interest in the following described
Real Estate situated in the County of Cook and State of Illinois,
to wit:
That part of the Southwest Quarter of Section 31, Township
41 North, Range 11, East of the Third Principal Meridian,
described as follows: Beginning at the Intersection of the
East line of Route 53 and a line 50.0 feet Northeasterly
(measured at right angles) of and parallel with the South-
easterly extension of the center line of Nerge Road; thence
Northerly along the East line of Route 53 aforesaid, 317.87
feet; Thence Easterly at right angles thereto 231.50 feet;
Thence Southerly parallel with the East line of Route 53
aforesaid 434.79 feet to the aforesaid line of 50.0 feet North-
easterly of the Southeasterly extension of the center line
of Nerge Road; Thence Northwesterly along said 50.0 foot
line 259.35 feet to the point of beginning, Cook County,
Illinois.
Subject to the following covenant and condition and right of
reversion:
This conveyance is subject to the covenant and condition that
said real estate shall be used primarily as a mental health
center to provide care, services, programs and facilities to
persons needing mental heath treatment, including the mentally
retarded and for those addicted to alcohol or drugs and inciden-
tally used for other community health programs and services.
In the event any breach of the covenant and condition provided above
shall occur the Grantor, Village of Elk Grove Village shall have the right
to cause title to said real estate to REVERT TO THE VILLAGE OF ELK GROVE
VILLAGE IN THE FOLLOWING MANNER:
