Vi13EREAS, the Village of Elk Grove Village
located in the County of Cook and Du Page , State of Illinois,
wishes to locate construct, operate rand maintair. watermains, sewer lines,
driveways alleys street returns sidewalks and street lights constructed
WM Village of Elk Grove Village or under the supervision of said Village
in the year 1966 on the State Highways in the Village of Elk Grove Village,

MICH by law comas under the jurisdiction and control of the Department of
Public Works and Buildings of the State of Illinois.
and
TMEREAS, a permit from said Department is required before
said work can be legally undertaken by said Village or supervised by the Village
now
THEREFORE, be it"Tesolved by the President and Board of Trustees
of the Village of Elk Grove Village,
County of Cook and Du Page State of Illinois
FIRST: That we do hereby request from the Department of
Public Works and Buildings, State of Illinois, a permit authorizing
the Village of Elk Grove Village ,to proceed with the work herein
described.
SECOND: That we hereby pledge the good faith of the
Village of Elk Grove Village
and guarantee that all work shall be performed in accordance with the
conditions of the permit to be granted by the Department of Public Works
and Buildings of the State of Illinois, and to hold the State of Illinois,
Department of Public Works and Buildings, harmless on account of any
damages that may occur to persons or property during the prosecution of
such work, and assume all liability for damages to persons or property
due to accidents or otherwise by reason of the work which is to be
performed under the provision of said permit.
THIRD: That we hereby state that the proposed work 3
is not, to be performed by the employees of the Village of Elk Grove Village
FOURTH: That the proper officers of the Village of Elk Grove Village
are hereby instructed and authorized to sign said permit in behalf of
the Village of Elk Grove Village, Illinois
I, Eleanor G. Turner hereby certify the above to be a
true copy of the resolution passed by the Board of Trustees of the
Village of Elk Grove Village
County of Cook and Du P+age State of Illinois
