4H IMS, the VILLAGE OF NX GROVE VIII AGE
located in the County of COOK State of Illinois,
wishes to g9ga% nat a 240 Sanitary ;Sever and Appurtenances thereto in Busse
goad (HoMs 63) from Pratt Boulevard to Landreier Road.
WHICH by law comes under the jurisd!lction and control of the Del.artment of
Public Works and Buildings of the S1;.ate of Illinois.

dMREAS, a permit from said Department in required before
said work can be legally undertaken by said Village
now
THEREFORE, be it resolved by the Vi_ 11M of Alk Grove Village
County of P. k ,_,a, State of Illinois
FIRST; That we do hereby request from the Department of
Public Works and Buildings, State of Illinois, a permit authorising
the Inland Lakes sever & Water Co. _,. to proceed with the work herein
described.
SECOND; That we hereby pledge the good faith of the
Village of Elk Grove Village
and guarantee that all work shall be performed in accordance with the
conditions of the permit to be granted by the Department of Public Works
and Buildings of the State of Illinois, and to hold the State of Illinois,
Department of Public Works and Buildings, harmless on account of any
damages that may occur to persons or property during the prosecution of
such work, and assume all liability for damages to persons or property
due to accidents or otherwies by reason of the work which is to be
performed under the ,provision of said permits
THIRIM That we hereby state that the proposed work is,
is not, to be performed by the employeos of the lg�d Lakes Sever & Water Co.
FOURTH; That the proper officers of the Village of Elk Grove Village
are hereby instructed and authorized to sign said permit in behalf of
the Village of Elk Grove Village and Inland Lakes Sever & Water Oo.
