A RESOLUTION AUTHORIZING THE MAYOR TO SIGN A RENEWAL
LICENSE/AMENDMENT AGREEMENT ISSUED BY FOREST PRESERVE DISTRICT
OF COOK COUNTY, ILLINOIS BETWEEN THE FOREST PRESERVE DISTRICT
AND THE VILLAGE OF ELK GROVE VILLAGE TO LOCATE, OPERATE AND
MAINTAIN AN ENTRANCE SIGN ON DISTRICT PROPERTY
NOW, THEREFORE, BE IT RESOLVED by the Mayor and Board of Trustees of the
Village of Elk Grove Village, Counties of Cook and DuPage, State of Illinois as follows:
Section 1: That the Mayor be and is hereby authorized to sign the attached document
marked:
RENEWAL LICENSE/AMENDMENT AGREEMENT
ISSUED BY
FOREST PRESERVE DISTRICT
OF COOK COUNTY, ILLINOIS
a copy of which is attached hereto and made a part hereof as if fully set forth.
Section 2: That this Resolution shall be in full force and effect from and after its passage
and approval according to law.

RENEWAL LICENSE/AMENDMENT AGREEMENT
ISSUED BY
FOREST PRESERVE DISTRICT
OF COOK COUNTY, ILLINOIS

License Agreement No. 1665 dated June 5, 2000 (the "License Agreement") by and between the Forest Preserve District
("the District') and the Village of Elk Grove Village ("Licensee"), which authorized Licensee to locate, operate and
maintain an entrance sign on District property is hereby amended as follows:
I. Effective Date. The phrase "this License Agreement has been fully executed" in Section 15 of the License
Agreement shall be deleted and replaced with the phrase "this License Agreement and the License Amendment
dated have been fully executed".
2. Facilities. The phrase "Village entrance sign" (the "Facilities") is hereby amended to "an internally illuminated
village gateway sign and underground electrical duct according to the plans attached hereto as Exhibit A."
3. Fee. The word "none" in Section 3 of the License Agreement shall be deleted and replaced with the sentence "A
License Fee of $3,500, along with a $500 Application Fee and a$3,500 Youth Education Land Acquisition and
Restoration Program Fee for a total Fee of $7,500 shall be paid by Licensee and remitted to the District along with
two (2) executed copies of this License Amendment dated prior to the District's
execution of this License".
4. Miscellaneous. Unless otherwise expressly modified by this License Amendment dated the
terms -of the License shall remain effective and unaltered.
IN WITNESS WHEREOF, the parties have signed this License Amendment as of the dates set forth by their signatures, to
be effective as of the Effective Date.
