TRUSTEES OF THE VILLAGE OF ELK GROVE VILLAGE, ILLINOIS HELD AT THE CHARLES J. ZETTEK MUNICIPAL BUILDING, 901 WELLINGTON AVENUE ON TUESDAY, May 11, 1999
Village President Craig B. Johnson called the meeting to order at 8:40 p.m.
1. ROLL CALL:
Present: President: Craig B. Johnson;
Trustees: Nancy J. Czarnik, Bart K. Dill, Patton L. Feichter, Samuel L. Lissner, James P. Petri, Chris Prochno
Absent: None
Also Present: Village Manager, Gary E. Parrin; Village Attorney, George B. Knickerbocker; and Village Clerk, Patricia S. Smith
A quorum was present.
2. INVOCATION
The Reverend Mark Clem, Pastor of Elk Grove Baptist Church delivered the invocation.
3. APPROVAL OF MINUTES:
Motion to approve the minutes of the Regular Meeting held on April 27, 1999 as submitted.

4. REPORT OF THE VILLAGE PRESIDENT AND BOARD OF TRUSTEES
La Magdalena Restaurant – It was the consensus of the Board to allow outdoor seating and the use of planters as a barrier from the pedestrian walkway. Mr. Fernandez, La Magdalena owner
was directed to consult with the Director of Engineering and Community Development as to the type of planters he would be using.
Alexian Brothers Medical Center – The President and Village Manager met with Michael Schwartz, President of ABMC and Brother Valentine for an up-date on the hospital. They discussed
the concerns of the Village regarding the work trailer. Brother Valentine mentioned the possibility of a new building in that area among other items.
Proclamation – President Johnson issued a proclamation declaring May 20, 1999 as “Buddy Poppy Day”.
Peer Jury Program – A plaque of recognition was awarded to Daniel Duda, who will be graduating from Elk Grove High School this year, for his time and efforts on the Peer Jury.
Trustee Petri reported on his attendance at Elk Grove High School on April 29, 1999 for Community Service Awards; and the Prayer Breakfast at Queen of the Rosary Church on May 6, 1999.
Trustee Prochno reported on her attendance at the Prayer Breakfast on May 6, 1999. Trustee Prochno commented that Daniel Duda has committed hundred of hours to community service especially
with the Senior Center. A reminder was noted of the Memorial Day service at the Elk Grove Park District Pavilion on Monday, May 31, 1999 at 10:45 a.m. to honor the Veterans of Foreign
Wars who have given their lives for their country.
Blood Donor Awards – President Johnson explained that earlier this evening awards were presented to Blood Donors who have given one gallon or more and that the 19 empty milk containers
located at the front of the Council Chambers represent the amount of
blood donated by Village resident and Plan Commission member Fred Geinosky. The President thanked all of the donors.
Trustee Feichter – President Johnson presented a proclamation honoring Trustee Feichter for his accomplishments as an educator and especially by his coaching his 1999 Constitution Team
to a 1st Place Finish from among the best competitive teams in the entire United States and the District of Columbia.
5. APPROVAL OF WARRANT
Motion to authorize payment of invoices as shown on the April 30, 1999 Accounts Payable Warrant in the amount of $382,504.70.

C. Elk Grove Library/Waive fees – Grant a request to waive the $24 business license fee for its coin-operated vending machine.
D. Prince of Peace United Methodist Church/waive fee – Grant a request to waive the $100 business license fee for their Mother’s Day Out Preschool.
E. Gem Construction, Inc., Wood Dale, IL – Award a purchase contract to the lowest responsive and responsible bidder for the removal and replacement of sidewalks, driveway aprons, curb
and gutter under the 1999 Private Participation Concrete Replacement Program at a per unit cost not to exceed the following:

Renew a maintenance contract and time-and-materials contract for maintenance of Village-owned traffic signals, street lights and traffic signal pre-emption system in a total contract
amount not to exceed $29,283.36
G. Marcott Enterprises, Inc., Elmhurst, IL /non-toxic debris Award a service contract to the lowest responsive and responsible bidder for the hauling and disposal of non-toxic debris
from Village-owned sites in a total contract amount not to exceed $68,500.
H. John Neri Construction Co., Inc., Addison, IL – Award a contract to the lowest responsive and responsible bidder for the 1999 Sanitary Sewer Replacement Project in an amount not
to exceed $122,040.
I. ICMA Annual Conference – Authorize the Village Manager and Assistant Village Manager to attend the International City/County Management Association annual conference in Portland,
Oregon from September 25 to September 29, 1999 in an amount estimated at $1,600 per person.
J. Fulton Contracting, Roselle, IL – Waive the formal bidding process; and
Award a contract to install and modify the Village’s five (5) outside sirens in an amount not to exceed $19,600.
K. FY 1999/00 Insurance – Authorize the following Health Care items to be effective June 1, 1999 through April 30, 2000
HMO ILLINOIS – Authorize the Village Manager to enter into a contract to provide HMO Medical benefits at the following monthly rates:

Lloyds of London - Renew a contract to provide Specific Stop Loss Re-Insurance coverage at a cost not to exceed $268,986.
Renew a contract with Lloyds of London to provide Aggregate Stop Loss Re-insurance coverage at a cost not to exceed $17,040.
Arthur Gallagher and Co.- Renew a contract for consulting services at a cost of $5,000.
Cost Care Consultation Services - Renew a contract through Gallagher Bassett at a cost of $5.25 per participant per month.
Flex-Spending Plan - Renew the Flex-Spending Plan administered by Gallagher Bassett at a cost of $5.45 per participant per month.
