Acting Chairman Alan J. Shapiro, DDS called the meeting to order
at 8:00 p.m. in the Charles J. Zettek Municipal Building, 901
Wellington Avenue, Elk Grove Village, and noted for the record that the
following were present: Alan J. Shapiro, DDS, and Ted E. Thompson.
The Fire and Police Commissioners, Alan J. Shapiro and Ted E.
Thompson was given continued notice to schedule the next Public
Hearing on Police Officer Patrick Hayes. (see attached minutes)
Tonights hearing was continued due to Officers Hayes attorney Joseph R.
Mazzone was unable to attend due to other obligations.
The Commissioners then turned to their regular meeting.
It was moved by Ted Thompson and seconded by Dr. Shapiro that the
minutes of April 22, 1996 be approved as submitted and that the Board
Dispense with the Reading thereof. The motion was voted upon by
acclamation; whereupon the Acting Chairman declared the motion carried.
PRESENTATION AND APPROVAL OF INVOICES
Peter A. Felice $ 320.00
Retainer fees for April & May 196 at $160 ea.
Police Consultants 3,546.00
for Multiple Interviews Assessments Reports
May 9, 23, 30 & June 5 - 197 at $18 ea.
Total of Bills $ 3,866.00
It was moved by Ted Thompson and seconded by Dr. Shapiro that the
Board authorize the disbursement of funds for payment. The motion
was voted upon by acclamation; whereupon the Acting Chairman declared
the motion carried.
NEW BUSINESS
None
CORRESPONDENCE
1. Five(5) written Reprimands were issued by the Chief of Police on
three(3) different dates, April 26, April 30 and May 30, 1996 to
five(5) different Police officers for several different failures:
one(1) failure to perform a duty
Two(2) failure to appear in court when required
or assigned
one(1) disobedience of an Order, written or oral
One(1) failure to be prompt for duty assignment
including roll call and court appearance
2. Notice of suspension issued by the Chief of Police on May 9, 1996,
to a police officer for one(1) for failure to appear in court when
required or scheduled.
3 . Copy of letter from the President of Police Pension Board, Eugene
P. Diemer reference the findings the Board voted on for Robert
Arnold.
4. Letter tO the Fire and Police Commission from Robert Arnold
reference the decision of the Police Pension Board that he be
return to work as a police officer.
5. A letter from David L. Schuman, police applicant, who took the
written Exam on March 24, 1996 and did not pass. He is asking
for test results.
6. Letter from Cpl. Jonathan L. Brown, Marine Corp. wanting to take
the Police Written Exam late.
7. Copy of letter sent to Robert A. Alexander, Assistant Principal
for Student Activities, Elk Grove High School reserving dates and
times for the Firefighter Orientation and written Exam.
S. Letter of notification of Retirement from Lt. Clyde Hood effective
June 14, 1996.
9. Copy of letter sent to Lt. Clyde Hood from Fire Chief MacArthur
thanking him for the thirty plus years for his service.
10.Copy of the publication from Illinois Professional Firefighters
Association, The Size Up.
OLD BUSINESS

PENDING
Six month probationary review was scheduled for Police Officer
Rlingsporn, but was delayed and rescheduled for July 22, 1996 at 8:00
p.m.
Next meeting is scheduled July 22, 1996, at the Charles J. Zettek
Municipal Building, 901 wellington Avenue at 7:00 p.m.
ADJOURNMENT
It was moved by Ted Thompson and seconded by Dr. Shapiro that the
meeting be adjourned. The motion was voted upon by acclamation;
whereupon the Acting Chairman declared the motion carried and the
meeting adjourned at 8:45 p.m.

NOW COMES OFFICER PATRICK HAYES, by and through his attorney, Joseph
R. Mazzone of the Law Firm of SCHENK, DUFFY, QUINN, McNAMARA, PHELAN,
CAREY AND FORD, LTD. and requests that this Board continue the hearing currently
set for June 24, 1996 at 7:30 p.m. and in support thereof would state that counsel for Officer
Hayes has previously been obligated to a continuing hearing before the Board of Fire and
Police Commissioners of the Village of Addison.
WHEREFORE, Officer Patrick Hayes prays this Board continue the hearing
previously set for June 24, 1996 at 7:30 p.m. to a date and time certain.

Joseph Mazone, and by agreement of the parties, this matter is
continued to August 26, 1996 at 7 :00 p.m.
Patrick Hayes shall remain suspended without pay, pending the
hearing.
