6.CONSENT AGENDA
a. Consideration of a request from Northwest Presbyterian Church, located at 301 Ridge
Avenue,to waive FY 2016/17 Vehicle License fees in the amount of$50.
(Northwest Presbyterian Church is requesting a fee waiver for FY 2016/17 Vehicle
License fees for two passenger vehicles ($25 each) for a total waiver of$50.
(It has been past practice of the Village Board to grant fee waivers for governmental
and non-profit organizations.
(The Director of Finance recommends approval.)
b. Consideration of a request from Living Hope Church, located at 777 Meacham Road, to
waive permit fees for two (2)temporary banners in the amount of$50.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.(The Director of Community
Development recommends approval.)
c. Consideration of a request from School District 54, to waive FY 2016/17 business license
fees in the amount of$100.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.

d. Consideration of a request from High School District 214,to waive FY 2016/17 business
license fees in the amount of$450.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.

e. Consideration of a request from The Salvation Army, located at 1000 Pratt Blvd., to waive
FY 2016/17 vehicle sticker fees in the amount of$1,325.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Finance recommends approval.)
f. Consideration of a request from the Elk Grove Park District, located at 499 Biesterfield
Road,to waive FY 2016/17 vehicle sticker fees in the amount of$2,300.
(The Elk Grove Park District is seeking a waiver of FY 2016/17 vehicle sticker fees for 4
passenger vehicles($100), 13 Class-13 trucks($650), 18 Class-D/F trucks ($1,350), and 2
Class-H/J/K trucks($200) for a total waiver of$2,300.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Finance recommends approval.) l-
g. Consideration of a request from the Elk Grove Park District, located at 499 Biesterfield
Road,to waive FY 2016/17 business license fees in the amount of$2,950.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Finance recommends approval.)
h. Consideration of a request from Meet Chicago Northwest Convention Bureau for payment
of annual dues for FY 2016-17 in the amount of$100,269.13.
(The annual dues rate is based on 10%of the 4%hotel/motel taxes collected.
(The Village collected $1,504,037 in hotel/motel tax revenues in FY 2015-16, a decrease
of$58,987 or 4%from the prior year.
(Compared to last year, this year's distribution to the Meet Chicago Northwest decreased
by $3,932.40.)
L Consideration of a request to hold a Public Hearing before the Plan Commission to consider
the petition submitted by SBA Towers for a Special Use permit to build a
telecommunication antenna in the I-1 Industrial District on the property located at 190
Gordon Street.
(SBA Towers is seeking the Special Use for the purpose of locating a telecommunication
facility at 190 Gordon Street.
(The proposed facility would include a 70' monopole and a 55' x 75' leased area.
(A date for the public hearing has not yet been determined.)

j. Consideration to adopt Ordinance No. 3453 granting a variation of Section 7E-9(A)of the
Zoning Ordinance to permit the construction of a twelve foot(12') high palisade fence
along portions of the north and east property lines located at 2065 Tonne Road, Elk Grove
Village (Commonwealth Edison).
(This item was discussed at the May 24, 2016 Village Board meeting and currently appears
under Unfinished Business.)
k. Consideration to adopt Ordinance No. 3454 which amends Section 6-4-5-1(B)of the
Village Code as it pertains to parking restrictions on certain designated streets within the
Village.
(The existing Code prohibits on-street parking in this area from 7:00 p.m.to 6:00 a.m. on
certain designated streets within the Village, primarily in the area north of Higgins Road.
(This amendment extends the hours of permitted parking,thereby prohibiting on-street
parking from 11:00 p.m.to 5:00 a.m.)
1. Consideration to adopt Ordinance Number 3455 amending Ordinance Numbers 3133, 3291
and 3420 all of which pertain to the Village's Emerald Ash Borer Infestation Tree
Replacement Program by authorizing the Mayor and Village Clerk to adopt a
Comprehensive Amendment to said Agreements.
(On April 28, 2015,the Village amended the Tree Harvesting Agreement dated August 21,
2007 with St.Aubin's.
(The long tern impact of the EAB infestation on Ash trees continues to be significant.
(The Village and St. Aubin deem it necessary and appropriate to again amend the original
Tree Harvesting Agreement for the purchase and planting of 600 additional trees.
(Ordinance Numbers 3133, 3291 and 3420 will be rescinded in favor of a new agreement.
(St. Aubin's under the new amended agreement will provide a total of two thousand, seven
hundred fifty(2,750)4"trees between 2016 and 2020 at an established yearly price.
(These new trees plus the 2,050 trees planted to date provide a grand total of 4,800 trees
planted by the end of 2020.
(The Director of Public Works recommends approval.)
in. Consideration to adopt Resolution No. 18-16(attached)authorizing the Mayor to execute a
Letter of Receipt stipulating the terms of an agreement for an application seeking a Cook
County Class 6B Property Tax Exemption status pursuant to the Cook County Real
Property Classification Ordinance as amended February 15, 2006 for certain real estate
located at 55 North Lively Boulevard.
(Synergy Mechanical Solutions(applicant) is applying for Cook Count Class 6B Property
Tax Abatement for property located at 55 North Lively Boulevard.
(The Applicant is currently located 1380 Louis Avenue in Elk Grove Village.
(Due to business expansion the Applicant intends to relocate operation to 55 North Lively
in Elk Grove Village.
(The Company currently employs eight full time positions with an additional four full time
positions to be created.
(The Applicant is a manufacturer's representative for HVAC equipment. The Applicant

intends to use the building for its office and warehousing.
(The Applicant intends to upgrade the building totaling approximately $48,348.00. Items
include: replace dock drive and apron; replace parking lot drive and apron; seal coat and
stripe parking lot; confirm all storm structures are functioning; remove gravel along dock
and parking lot drive in and replace with grass;remove boulders; provide fagade,
landscaping,and signage plan.
(The goal of Class 613 is to attract new industry, stimulate expansion and retention of
existing industry and increase employment opportunities. The eligibility requirements for
6B status are new construction, substantial renovation or buildings that have been vacant
for a period of time.
(This site qualifies as it involves occupation of an abandoned building for less than twenty-
four(24)months continuous vacancy with a purchase for value, special circumstances, and
significant rehabilitation.
(Property approved for Class 613 status allows the owner of the property to have the
assessment level lowered for a period of twelve years. The assessment schedule is 10%of
fair market value for ten years then 15% in the eleventh year and 20% in the twelfth year.
Industrial property is currently assessed at 25%of fair market value in Cook County.
(The Director of Business Development and Marketing recommends issuing a Letter of
Receipt.)
n. Consideration to adopt Resolution No. 19-16 (attached)authorizing the Mayor to execute a
Letter of Receipt stipulating the terms of an agreement for an application seeking a Cook
County Class 6B Property Tax Exemption status pursuant to the Cook County Real
Property Classification Ordinance as amended February 15, 2006 for certain real estate
located at 1125 Lunt Avenue.
(The Law Firm of Elliot and Associates is applying for a Cook County Class 613 Property
Tax Abatement on behalf of their client Ninja Dog, LLC (Applicant)doing business as
Gorilla Paper, Inc. for the property located at 1125 Lunt Avenue.
(The Applicant currently lease approximately 6,000 square feet of space at 190 Joey Drive
in Elk Grove Village. Due to business expansion the Applicant intends to purchase the
30,000 square foot building located at 1125 Lunt Avenue and utilize it for sales and
distribution of paper and printing supplies.
(The Applicant estimates that due to the introduction of a new product line the business
will generate approximately $1.5 million in retail sales annually. The savings from the 613
will be used to launch the new program targeting direct sales to restaurants. The Applicant
currently generates a minimal amount of sales tax through on-line sales.
(The Applicant intends to upgrade the building totaling approximately $30,000.00.
(Items include: update fagade and landscaping per application packet; sealcoat and stripe
parking lot; install ADA parking stall shall be in compliance with details. $250 FINE sign
required; storm catch basin shall be cleared of all debris; sump pit shall be emptied;replace
curb stops as needed; rear drainage ditch shall be restored to grass line ditch; all trees shall
be cleared and any debris and/or trash removed; and a stoop and accessible walk shall be
installed at the rear man door.
(The goal of Class 6B is to attract new industry, stimulate expansion and retention of
existing industry and increase employment opportunities. The eligibility requirements for
6B status are new construction, substantial renovation or buildings that have been vacant

for a period of time.
(This site qualifies as it involves occupation of an abandoned building for less than twenty-
four(24)months continuous vacancy with a purchase for value, special circumstances, and
significant rehabilitation.

o. Consideration to adopt Resolution No. 20-16(attached)authorizing the Mayor to execute a
Letter of Receipt stipulating the terms of an agreement for an application seeking a Cook
County Class 6B Property Tax Exemption status pursuant to the Cook County Real
Property Classification Ordinance as amended February 15, 2006 for certain real estate
located at 650 Morse Avenue.
(The Law Offices of Sarnoff& Baccash, on behalf of Tradewater, LLC (Applicant), is
seeking a Cook County Class 6B Property Tax Exemption for 650 Morse Avenue.
(The Applicant intends on purchasing the building and using it for their operation of
processing ozone depleting refrigerants for carbon credits.
(The Applicant is currently located in Chicago but due to business expansion they plan to
open a facility in Elk Grove Village. The new facility will employ three to five full time
employees.
(The subject property consists of an approximately 12,000 square foot building that has
been 100%vacant since January 2015.
(The Applicant intends on spending $900,000 to refurbish both the interior and exterior of
the building. Including: rehabilitating the side drainage ditch shall be cleared of all trees
and invasive brush; restore to a grass lined ditch; remove and replace the dock and drive-in
door in accordance with the ICRC detail; remove and replace the parking lot apron per the
apron detail this may require coordination with the 1361 Jarvis property owner; a parking
lot shall be installed in accordance with Village code, including storm water detention;
remove and replace the main building entrance; his entrance shall be ADA accessible;
update fagade, landscaping, and sign per application packet.

p. Consideration to adopt Resolution No. 21-16 ascertaining the Prevailing Rate of Wages for
Workmen on Public Works Projects.
(The State of Illinois requires the Village to adopt this Resolution each year to determine
the prevailing rate of wages for laborers performing construction of public works projects.
(The rates are determined by the Illinois Department of Labor throughout Cook and
DuPage Counties.)
