Chairman Shapiro called the meeting to order at 6:25 p.m. in the Charles J. Zettek Building,
901 Wellington Avenue, Elk Grove Village and noted for the record that the following were
present: Chairman Alan J. Shapiro, DDS, Secretary E. Butch Ehrke and Commissioner
Michael P. Kontek.
It was moved by Commissioner Ehrke and seconded by Commissioner Kontek that the
minutes of August 17, 2004 meeting be approved as submitted and that the Board dispense
with the reading thereof. The motion was moved upon by acclamation whereupon Chairman
Shapiro declared the motion carried.
PRESENTATION AND APPROVAL OF INVOICES
1) Police Consultants, Inc. Invoice 4022 dated 9-15-04 in the amount of$1000.00 for
psychological evaluation of two police candidates.
2) Theodore Polygraph Service Invoice 14580 dated 9-7-04 in the amount of$125.00 for
polygraph examination of police candidate.
3) Police Consultants, Inc. Invoice 4010 dated 8/30/04 in the amount of$1500.00 for
training and certification of fire and police commissioner.
4) Police Consultants, Inc. Invoice 4006 dated 8/23/04 in the amount of$500.00 for
psychological evaluation of police candidate.
5) Theodore Polygraph Service Invoice 14561 dated 9/1/04 in the amount of$250.00 for
polygraph examination of two police candidates.
It was moved by Commissioner Ehrke and seconded by Commissioner Kontek that the Board
authorize the disbursement of funds for payment. The motion was voted upon by acclamation
whereupon the Chairman declared the motion carried.
CORRESPONDENCE
1) Note from Village Manager dated ,9/17/04 authorizing FPC to hire one firefighter.
2) Notice of Suspension dated 8/23/04 issued to police officer for violating Rules and
Regulations, Section 26.1.1 A, Rules of Conduct, Rule 49 (Failure to appear in court
when required or scheduled).
3) Written Reprimand dated 8/31/04 issued to police officer for violating Rules and
Regulations, Section 26.1.IA, Rules of Conduct, Rule 05 (Failure to perform a duty).
4) Letter to fire candidate requesting written confirmation of his decision to withdraw
from our hiring process.
5) Letter to Human Resources advising that firefighter has completed his probationary
period and should be granted permanent employee status effective 9/02/04.

6) Appointment letter to police officer candidate with start date effective 10/04/04.
7) Written Reprimand dated 9/15/04 issued to police officer for violating Rules and
Regulations, Section 26.1.IA, Rule 29 (Failure to follow medical roll procedures).
8) Notice of Suspension to police officer for violating Rules and Regulations, Section
26.1.1A, Rule 49 (Failure to appear in court when required or scheduled).
9) Letter to police candidate requesting written confirmation of her decision to decline
our offer of employment.
10)Letter to firefighter congratulating him on his retirement.
11)Letter from fire candidate confirming his decision to withdraw from our hiring
process.
OLD BUSINESS -None
NEW BUSINESS
1) Chief Schmidt and Deputy Chief Lebert stopped in to meet new commissioner.
2) Meeting with Chief Goostree and Deputy Chief Miller regarding schedule for next
firefighter entrance exam.
3) Instructed secretary to send certified letters to police candidates and fire candidates
that have not responded to our request for background information.
4) Commission instructed secretary to fax background information packet to John
Kelly to verify our compliance with privacy laws.
PENDING
1) Police Officer Hiring Process—four vacancies; authorization to replace two at this
time. Current eligibility list effective until June 1, 2006.
2) Firefighter Hiring Process—sir:vacancies; authorization to replace one at this
time. Current eligibility list effective until December 27, 2004.

ADJOURNMENT
It was moved by Commissioner Ehrke and seconded Commissioner Shapiro that the meeting
be adjourned. The motion was voted upon by acclamation whereupon Chairman Bantner
declared the motion carried and the meeting adjourned at 9:30 p.m.
