Zettek Building, 901 Wellington Avenue, Elk Grove Village and noted for the record that the following were present: Chairman Alan J. Shapiro, DDS, Secretary E. Butch Ehrke and Commissioner Michael P. Kontek. It was moved by Commissioner Kontek and seconded by Commissioner Ehrke that the minutes of November 16, 2004 meeting be approved as submitted and that the Board dispense with the reading thereof. The motion was moved upon by acclamation whereupon Chairman Shapiro declared the motion carried. PRESENTATION AND APPROVAL OF:INVOICES 1) Police Consultants, Inc., Invoice 4072 dated 12/01/04 for $500.00 for psychological evaluation of police officer candidate. 2) Police Consultants, Inc., Invoice 4062 dated 11/15/04 for$4,234.50 for administration of firefighter entry examination to 122 candidates. 3) Police Consultants, Inc., Invoice 4067 dated 11/17/04 for $500.00 for psychological evaluation of police officer candidate. 4) The Clinic—Invoice dated 11;18/04 in the amount of$495.00 for employment medical examination for police officer candidate. 5) The Clinic—Invoice dated 11!30/04 in the amount of$495.00 for pre-employment medical evaluation for police officer candidate. 6) Northwest Neurology—Invoic:e dated 11/23/04 in the amount of$300.00 for neurological evaluation of firefighter candidate. 7) Police Consultants, Inc., Invoice 4082 dated 12/15/04 in the amount of$500.00 for psychological evaluation of police officer candidate. 8) The Clinic—Invoice dated 12,121/04 in the amount of$495.00 for pre-employment medical evaluation for firefighter candidate. It was moved by Commissioner Ehrke and seconded by Commissioner Kontek that the Board authorize the disbursement of funds for payment. The motion was voted upon by acclamation whereupon the Chairman declared the motion carried. CORRESPONDENCE 1) Letter from Dr. Boyajian announcing his retirement effective March 1, 2005. 2) Letter from Firefighter requesting a copy of his personnel file. 3) Appointment letter to Police Officer; hire effective date: January 3, 2005. 4) Retirement congratulations letter to police sergeant retiring on 12/3/05. 5) Retirement congratulations letter to police sergeant retiring on 12/31/04. Minutes of the Regular Meeting December 21, 2004 Page 1 of 3 6) Memo from Chief Schmidt requesting promotion of next two candidates on Sergeant's Eligibility List (effective 12/6/04 and 1/3/05). 7) Copy of memo from Chief Schmidt to all Police Personnel advising the promotion of two police officers to the rank of sergeant. 8) Pass/Fails letters sent to all candidates that participated in the entry firefighter examination on 11/14/04. 9) Letter to police candidate advising that he did not pass his psychological evaluation. 10)Email from Village Manager advising that we do not charge employees for copies of their personnel file. 11)Letter to police candidate advising;that he did not pass his psychological evaluation. 12)Letter to police candidate advising that he did not pass his psychological evaluation. 13)Appointment letter to Police Officer; hire effective date January 3, 2005. 14)Memo to Chief Schmidt requesting comments/constructive criticism on the performance of probationary officer. Final probationary review to be conducted on 1/25/05. Memo from Chief Schmidt to Command staff requesting performance report and recommendation regarding probationary officer. Memo from Chief Schmidt to Probationary Officer advising hire of his scheduled final probationary review on 1/25/05. 15)Memo to Village Clerk regarding the rescheduling of the January 18, 2005 regular meeting to January 25, 2005 at 7:00 p.m. 16)Letter dated 12/17/04 from Firefighter announcing his resignation effective 12/21/04. OLD BUSINESS -None NEW BUSINESS 1) Discussed future medical evaluations and the replacement of Dr. Boyajian. Alexian Brothers Occupational Health has contacted our office numerous times. Commissioners instructed secretary to contact Alexian to set up appointment for a presentation of their offerings,. PENDING 1) Police Officer Hiring Process—one vacancy; authorization to replace one at this time. Eight names remain on the current eligibility list that will expire on June 1, 2006. 2) Firefighter Hiring Process— seven va.ancies; authorization to replace two at this time. Eligibility list will expire on December 27, 2004.
