APR 2 1 2005 OF THE BOARD OF FIRE AND POLICE COMMISSIONERS
OF THE VILLAGE OF ELK GROVE VILLAGE

Chairman Shapiro called the meeting to order at 7:00 p.m. in the Charles J. Zettek Building, 901
Wellington Avenue, Elk Grove Village and noted for the record that the following were present:
Chairman Alan J. Shapiro, DDS, Secretary E. Butch Ehrke and Commissioner Michael P. Kontek.
It was moved by Commissioner Kontek and seconded by Commissioner Ehrke that the minutes of
February 15, 2005 meeting be approved as submitted and that the Board dispense with the reading
thereof. The motion was moved upon by acclamation whereupon Chairman Shapiro declared the
motion carried.
PRESENTATION AND APPROVAL OF INVOICES
1) Police Consultants, Inc. —Invoice 4114 dated 2/17/05 in the amount of$500.00 for
psychological evaluation of police candidate.
2) Police Consultants, Inc. —Invoice 4121 dated 2/28/05 in the amount of$500.00 for
psychological evaluation of police candidate.
3) Federal Express—Invoice 5-385-37716 /tracking 849955011251 dated 2/23/05 in the
amount of$14.31 for delivery of overnight letter to fire candidate.
4) Ottosen Trevarthen Britz Kelly & Cooper, Ltd. —Invoice dated 2/28/05 in the amount
of$135.00 for professional services related to police officer appeal hearing.
5) Daily Herald—Invoice T3486801 dated 2/26/05 in the amount of$21.25 for
publication of legal notice related to police officer appeal hearing.
6) Theodore Polygraph Service, Inc. —Invoice 14787 dated 2/21/05 in the amount of
$375.00 for polygraph examination of fire and police candidates.
7) Theodore Polygraph Service, Inc. —Invoice 14788 dated 2/24/05 in the amount of
$250.00 for polygraph examination of fire and police applicants.
8) Theodore Polygraph Service, Inc. —Invoice 14793 dated 3/1/05 in the amount of
$125.00 for polygraph examination of fire applicant.
9) Police Consultants—Invoice 4130 dated 3/10/05 in the amount of$1000.00 for
psychological evaluations of two firefighter candidates.
10)FedEx—Invoice 837303237076 dated 2/23/05 in the amount of$16.48 for delivery of
overnight letter to firefighter candidate.
11)FedEx—Invoice 837303237087 dated 2/23/05 in the amount of$14.48 for delivery of
overnight letter to firefighter candidate.
It was moved by Commissioner Ehrke and seconded by Commissioner Kontek that the Board
authorize the disbursement of funds for payment. The motion was voted upon by acclamation
whereupon the Chairman declared the motion carried.

CORRESPONDENCE
1) Letter to police candidate advising that he did not pass his psychological evaluation.
2) Memo to George Knickerbocker regarding Mr. Arnold's request for a retirement
commission card.
3) Notice of Suspension dated 2/17/05 issued to police officer for violating Rules and
Regulations, Section 26.1.1 A, Rules of Conduct, Rule 11 (Incompetence or
inefficiency in the performance of duty). One-day suspension to be served on
03/02/05.
4) Notice of Suspension dated 2/17/05 issued to police officer for violating Rules and
Regulations, Section 26.1.1A, Rules of Conduct, Rule 02 (Any action or conduct which
impedes the Department's efforts to achieve its goals or brings discredit upon the
Department), and Rule 05 (Failure to perform a duty). Two-day suspension to be
served on 2/24/05 and 2/25/05.
5) Notice of Hearing on March 3, 2005 for police officer suspension appeal.
6) Notice of Filing to Chief Schmidt from police officer's attorney, Joe Mazzone.
7) Memo from Chief Schmidt regarding the performance of probationary police officer
hired on 7/6/04.
8) Letter from Stanard & Associates promoting their services for promotional testing.
9) E-mail from Val &Jeff Perlik regarding public perception of EGV police personnel
and FPC response.
10)Memo to Chief Schmidt regarding the scheduling of the next sergeant's promotional
testing process.
11)E-mail from Val & Jeff Perlik regarding village policy of police officers accepting free
coffee, lunches, etc. from local businesses.
OLD BUSINESS
1) Awaiting response from Village Attorney to FPC memo dated 2/17/05 regarding issuance
of retirement commission card to former police officer.
NEW BUSINESS
1) Six-month review for probationary police officer
2) Meeting with Chief Schmidt
PENDING
1) Police Officer Hiring Process—four vacancies; authorization to replace one at this time.
Four names remain on the current eligibility list scheduled to expire on June 1, 2006.
2) Firefighter Hiring Process—six vacancies plus one on military leave; authorization to
replace two at this time. Thirty-two names are listed on the current eligibility list
scheduled to expire on February 2, 2007.

ADJOURNMENT
It was moved by Commissioner Ehrke and seconded Commissioner Kontek that the meeting be
adjourned. The motion was voted upon by acclamation whereupon Chairman Shapiro declared the
motion carried and the meeting adjourned at 7;45 p.m.
