Chairman Shapiro called the meeting to order at 7:00 p.m. in the Charles J. Zettek Building, 901
Wellington Avenue, Elk Grove Village and noted for the record that the following were present:
Chairman Alan J. Shapiro, DDS, Secretary E. Butch Ehrke and Commissioner Michael P. Kontek.
It was moved by Commissioner Kontek and seconded by Commissioner Ehrke that the minutes of
March 15, 2005 meeting be approved as submitted and that the Board dispense with the reading
thereof. The motion was moved upon by acclamation whereupon Chairman Shapiro declared the
motion carried.
PRESENTATION AND APPROVAL OF ]INVOICES
1) Alexian Brothers Corporate Health—Invoice 150857 dated 4/5/05 in the amount of
$619.00 for medical evaluation of firefighter candidate.
2) Ottosen Trevarthen Britz Kelly &Cooper—Invoice dated 3/31/05 in the amount of
$1495.50 for professional services relating to 3/3/2005 Appeal Hearing.
3) Alexian Brothers Corporate Health—Invoice 150638 dated 03/19/05 in the amount of
$1246.00 for medical evaluation of firefighter candidate and police candidate.
4) Theodore Polygraph Service—Invoice 14827 dated 3/30/05 in the amount of$375.00
for polygraph examinations for th-ee firefighter candidates.
5) Police Consultants, Inc. —Invoice 4137 dated 3/18/05 in the amount of$500.00 for
psychological evaluation of firefighter candidate.
It was moved by Commissioner Ehrke and seconded by Commissioner Kontek that the Board
authorize the disbursement of funds for payment. The motion was voted upon by acclamation
whereupon the Chairman declared the motion carried.
CORRESPONDENCE
1) Email from Lily Soldman of ABOH inquiring as to whether or not we want actual lab
and physical results on candidates or just a"fit for duty" form.
2) Appointment letter sent to police officer hired on 4/4/05.
3) Letter to police candidate requesting return of his completed background packet.
4) Letter to police candidate requesting written confirmation of her phone message asking
to be removed from our eligibility list.
5) Letter from firefighter announcing;his retirement effective March 16, 2005 after more
than 27 years of service. Retirement congratulations letter from Chief Goostree to
firefighter. Retirement congratulations letter from FPC to Firefighter.

6) Memo from Chief Goostree to Village Manager requesting authorization to hire new
firefighter. Note from Village Manager authorizing FPC to proceed with replacement.
7) Letters to four firefighter candidates requesting completion of their background packets
and polygraph examination..
8) Appointment letters to two firefighters hired on 3/21/05.
9) Emails from J Perlik regarding village policy on free coffee/donuts for police officers
and supervisors.
10)Revisions to the Police Policy& Procedure Manual.
11)Findings& Order prepared by John Kelly for the 3/3/2005 police officer appeal
hearing.
12)FPC memo to appropriate village department heads advising findings and decision
relating to Suspension Hearing.
13)Response from Chief Schmidt regarding Robert Arnold's request for a retirement
commission card
14)Third letter to Police Candidate requesting completion of background information
packet.
15)Memo to Chief Goostree requesting comments/constructive criticism regarding the
performance of probationary firefighter hired on 5/24/2004. Final probationary review
scheduled for 8:30 p.m. on May 10, 2005.
16)Legal notice regarding sergeant's examination.
17)Letter from police candidate confirming his decision to withdraw from our hiring
process.
18)Second letter to police candidate requesting written confirmation of decision to
withdraw from our hiring process.
OLD BUSINESS
1) Discussed response from Chief Schmidt regarding issuance of retirement commission
card to former police officer. Decision made to deny request.
2) Discussed Perlik emails. Informational copies will be forwarded to Mayor and Police
Chief.
NEW BUSINESS
1) Discussed sufficiency of"fit for duty" form from ABHS vs. complete medical results
for retention in employee file. Decision made to obtain and retain medical results for
each new employee since ABHS destroys test results after two years.
2) Illinois Fire & Police Commissioners Association Spring 2005 Seminar; no attendees.
3) Discussed Police Sergeant Promotional Process. Reviewed individual oral interview
phase. Will finalize format at 5/10/05 meeting.

PENDING
1) Police Officer Hiring Process—ttv-ee vacancies; no authorization to replace at this time.
Two names remain on the current eligibility list scheduled to expire on June 1, 2006.
2) Firefighter Hiring Process—five vacancies plus one on military leave; authorization to
replace one at this time. Twenty-nine names remain on the current eligibility list
scheduled to expire on February 2, 2007.
ADJOURNMENT
It was moved by Commissioner Ebrke and seconded Commissioner Kontek that the meeting be
adjourned. The motion was voted upon by acclamation whereupon Chairman Shapiro declared the
motion carried and the meeting adjourned at 7:30 p.m.
