Parrin, Village Manager All Department Heads FROM: Loss Prevention Committee SUBJECT: Loss Prevention Committee Meeting Minutes — January 9,2003 The meeting was called to order at 8:45 A.M. at the Village Hall. Present: Jack Gray, Chairman Larry Hammar, Deputy Police Chief Dave Miller, Deputy Fire Chief Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Excused: None The minutes were approved for the meeting of November 14, 2002 Review Current Reports: Seventeen (17) reports were reviewed. See attachments Tabled Reports: None Old Business: LPC Bulletins: Jack Gray asked if the Fire Department would like to take over on the osL s Prevention Bulletin and its distribution. Dave Miller will discuss it with Doug Goostree and have an answer for the committee at the next meeting. New Business: Driver Refresher Training: Officer K. Loss Prevention Committee Minutes for January 9, 2003 Meeting Page 3 of 11 Review of Current of Reports F-49-02 LT. SCOTT MILLER, FIREFIGHTER 07-11-02 Description of Incident: Damage occurred to the VHF radio base station sometime between 07/11/02 and 10/11/02. Cause was from lightning strike. Supervisor's Comments & Recommendations: Unavoidable — Act of God LPC Comments & Recommendations: No Comment W-09-02 ROBERT COOK, PUBLIC WORKS 10/21/02 Description of Incident: After gate had been opened, employee pulled into the lift station at 1491 Haar Lane. Bob was looking out the passenger side window, when the wind blew the driver's side gate partially closed. When the truck continued forward, the mirror struck gate, which caused damage to door of Unit#828. Supervisor's Comments & Recommendations: Be sure gates are fully open and are secured so wind will not blow them shut. Currently no mechanism is in place to hold gates open and since they are very light,they will move when the wind blows. Will look into a %vay to secure gates open. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 9, 2003 Meeting Page 4 of 11 F-50-02 SCOTT KORONA, FIREFIGHTER 11/09/02 Description of Incident: A finger lancet was not properly placed or fell out of the Sharps container in the drug bag, leaving it loose in the side pocket of the bag. The sharp portion of the lancet had punctured the exterior of the side pocket, exposing the tip. Korona was handling the bag when the side pocket area contacted his right shin area and he felt pain. Korona notified me, cleaned his wound and continued assisting with the call. After the call Korona went to ABMC for exam, exposure form completion and blood draw. Supervisor's Comments & Recommendations: Instruct paramedics who are coming on shift to check that the Sharps container is properly secured and that no sharps are already out in the side pocket during the morning checkouts. Also, when depositing a sharp in the container the container should be removed and then the sharps placed in it in order to visually confirm the proper placement and secure prior to placing the container in the side pocket. LPC Comments & Recommendations: Concur Comment: Although we concur, it is the opinion of the committee that this incident is more poor housekeeping, rather than unsafe conditions. F-51-02 LT. JOHN FORDON, FIREFIGHTER 11/11/02 Description of Incident: While on a call at 646 Dauphine Court, the patient was combative and lunged and fell. Lt. Fordon tried to catch her and felt a pain in his right shoulder. Supervisor's Comments & Recommendations: We should have more personnel surround a psych patient in case of erratic behavior. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 9, 2003 Meeting Page 5 of 11 P-42-02 DAVID STRUWING, POLICE OFFICER 11/13102 Description of Incident: Ofc. Struwing was assigned to assist SSD investigators with a narcotics investigation. The offenders took off and fled south on Rt. 355. The offenders exited their vehicle behind 1013 Charlela, jumping a fence. Ofc. Struwing along with other officers chased the offenders over the fence. In doing so he received a cut from the fence to the palm of his right hand. ABMC cleaned and dressed the cut to the right palm. Supervisor's Comments & Recommendations: This was an unavoidable situation. The officer was injured in the course of his duties in foot pursuit of two felony suspects while jumping a chain link fence. LPC Comments & Recommendations: Concur P-43-02 DAVID KLINGSPORN, POLICE OFFICER 11/19/02 Description of Incident: (See P-46-02 for Vehicle Accident Report) Ofe. Klingsporn was responding to a reported carfare at Devon and Elmhurst Roads. He had emergency equipment activated on unit#235. Upon approaching the intersection of eastbound Devon at Rt. 83 on a green light, another vehicle disobeyed a red light, pulled in front of the officer's squad,which resulted in a collision. Moderate front end and driver side front fender damage. Officer also complained of general soreness-went to ABMC. Supervisor's Comments & Recommendations: Continue to educate the general public in recognizing and complying with situations involving yielding to emergency vehicles. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 9, 2003 Meeting Page 6 of 11 P-44-02 KRISTIN JULIAN, POLICE OFFICER 11/23/02 Description of Incident: Officer was making a left turn into the driveway at 945 Criss Circle in order to check the rear of the building. The officer's focus was on the front of the building. She overshot the driveway while turning and struck a rock with the front bumper. Minor scratches and scrapes to the front passenger side lower bumper Supervisor's Comments & Recommendations: Full attention must be given to driving while checking a building. LPC Comments & Recommendations: Driver Refresher Course G-03-02 JUDY POHLMAN, FINANCE 11/25/02 Description of Incident: On her way into work, Judy stepped off the sidewalk onto the Village's black asphalt parking lot. The spot Judy stepped on was not salted and she didn't see the patch of ice. Judy's foot slipped out from under her and she fell flat on her back. She experience immediate pain in her lower back and left hip area. Alexian Occupational Health Clinic did X-rays and released her to go back to work. Supervisor's Comments & Recommendations: The"black ice"on the parking lot caused the accident. The employee probably could not have prevented this incident, because "black ice' is difficult to see. It is difficult to know how thoroughly the parking lot was salted. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for ,January 9, 2003 Meeting Page 7 of 11 P-45-02 NICHOLAS OLSEN, POLICE OFFICER 12/03/02- 12104/02 Description of Incident: Unit #258 was parked overnight when it was struck by unknown vehicle that may have been making a delivery to Village Hall or plowing the parking lot. Minor damage. Supervisor's Comments & Recommendations: Vehicle was struck while parked LPC Comments & Recommendations: Concur S-12-02 STANLEY MUNSON, STREETS 12104/02 Description of Incident: While putting branches through chipper a branch spun around and hit Stanley in the side of face (cheekbone). Stan sustained a "laceration to the right cheek requiring three stitches per ABMC. Supervisor's Comments & Recommendations: This is not a preventable accident. The chipper blades turned the branch and just happened to slap Stanley in the cheek. Nothing unsafe on the employee's part took place LPC Comments & Recommendations: Disagree —failure to wear protective dear caused this accident Note: Wearing protective gear should be policy Loss Prevention Committee Minutes for January 9, 2003 Meeting Page 8 of 11 S-13-02 STEVEN STORC, STREETS 12/05/02 Description of Incident: While getting out of front-end loader, lower rung on entry ladder broke causing Steve's right knee to twist. Also, his right arm and right shoulder are sore. Supervisor's Comments & Recommendations: This was not a preventable accident, just something that broke unexpectedly. LPC Comments & Recommendations: More thorough inspections should be implemented to periodically check for any damage to the equipment. W-10-02 MARK ANGELLOTTI, PUBLIC WORKS 12/06/02 Description of Incident: During excavation for a water main repair, employee broke gas line. Appears large rock that was displaced during excavation may have broken the gas line. Gas line was much shallower than normal. Damaged portion approximately 12-18 inches deep. Supervisor's Comments & Recommendations: To an extent this type of accident is unavoidable. Typical excavations allow for the removal of concrete, gravel and rocks from underneath a drive apron without striking the utility services. Typically utility services are at least 18"deep. When time permits utilities should be hand located or hydro-excavated, this was an emergency repair. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 9, 2003 Meeting Page 9 of 11 F-52-02 TOM WIND, FIREFIGHTER 12/08/02 Description of Incident: Very minor damage to air line coupler at the 1000 Oakton Street Station 8. Air-line was not disconnected from Engine 8, which caused filter to break at the connection. Supervisor's Comments & Recommendations: This has been an on-going problem specific to this apparatus for many years. It has been checked by Fleet Services for a leak and none found. Further review will have the same result. Install an auto ejector for air-line on apparatus to help prevent this occurrence, as top air pressure is a necessity for emergency vehicles. For now a sign has been placed on the steering wheel regarding the attached air-line. LPC Comments & Recommendations: Concur BS-O6-02 DUANE WESTBF_RG, BLDG SERVICES 12/09/02- 12/10/02 Description of Incident: Duane observed unknown vandal(s) had damaged the plaque in front of the bronze sled with children statue located near the fountain area. Supervisor's Comments & Recommendations: Police afternoon shift was requested to increase patrol and/or place an unmarked squad with officer in the area. For the last week in addition to this damage, numerous holiday lights and timers have been intentionally disconnected. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 9, 2003 Meeting Page 10 of 11 F-53-02 LT. SCOTT PETERSON, FIREFIGHTER 12/16/02 Description of Incident: Lt. Peterson was walking around his vehicle to decide the best area to extricate the patient. The antenna from the vehicle struck him in the lower area of his left eye. Peterson did have his helmet shield down at the time of the accident. Treated at ABMC. Supervisor's Comments & Recommendations: In the future, employees should try to remove hazards when possible. The conditions during the night make the scene more dangerous. A possible solution for eye protection may be to wear eye goggles to prevent injury. The face shield does not afford the protection of goggles. This accident occurred when the antenna entered under the face shield. LPC Comments & Recommendations: Disagree -- Goggles do not completely cover the face as well as a face shield would. Also employee should be more aware of surroundings. S-14-02 EDWARD FRANZEN, STREETS 12/18/02 Description of Incident: While Ed was stepping out of the sweeper#724 to pick up a coil of rope that would clog up the sweeper, his left foot landed half on the curb and half off the curb. It slipped off and he twisted his left knee. Ed went to Alexian Brothers Clinic for x-rays, because his left knee was very swollen. Supervisor's Comments & Recommendations: Advised employee to first look for a level surface to exit on. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 9, 2003 Meeting Page 11 of 11 P-46-02 DAVE KLINGSPORN, POLICE OFFICER 11119102 Description of Incident: (See P-43-02 for Injury Report, same incident) Accident occurred at Busse & Devon, while officer was responding in Unit#235 to a car fire at Devon & Elmhurst. Emergency equipment was activated and officer had the green light. The offending vehicle disobeyed the red light and collided into Unit#235. Moderate front end and driver side front fender damage. Supervisor's Comments & Recommendations: Continue to educate the general public in recognizing and complying with situations involving yielding to emergency vehicles.
