Parrin, Village Manager All Department Heads FROM: Loss Prevention Committee SUBJECT: Loss Prevention Committee Meeting Minutes —November 13, 2003 The meeting was called to order at 8:35 A.M. at the Village Hall. Present: Jack Gray, Chairman Larry Hammar, Deputy Police Chief Jim Forde, Deputy Fire Chief Pam Kuc, Housing Inspector Excused: Diane Hunter, Finance Department Secretary The minutes were approved for the meeting of September 11, 2003 Review Current Reports: 27 Incidents Tabled Reports: F-14-03 Robert Denna F-15-03 Marc Skowronski F-16-03 Brian McVeigh Incident occurred on 5/4/03, but was received by Finance on 6/17/03 due to computer problems and a new lieutenant needing to review all reports. The delay had nothing to do with any union issues. P-32-03 Don Mercil Committee questioned the supervisor's remarks of prevention ("reduce the number of rounds fired at the range or wear a shoullder pad where the butt of the gun hits the shoulder") would alleviate the injury. After further investigation regarding this issue it was found that out of 99 individuals at the range, only 1 was injured. LPC Comments: This injury is not a system wide problem, no need to change procedure at carbine training in Marsielles. New Business: ► Driver Refresher Training: Date for class is Wednesday, December 17, 2003. ► Confined Space Entry—Joint entryand rescue drill was planned for Public Works and Fire this past summer. Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 3 of 16 Review of Current of Reports W-11-03 VICTOR MORALES, WATER & SEWER 08/28/03 Description of Incident: Employee was working a noisy "Vactor Machine" during a water main break at 231 Tower. Employee states that when he inserted hearing protection into his ears, it caused his ear to plug with wax. Supervisor's Comments & Recommendations: Use earmuff type hearing protection if wax is problematic. LPC Comments & Recommendations: No Comments F-27-03 MICHAEL PIANTKOWSKI, FIREFIGHTER 08/28/03 Description of Incident: While at a Swift Water Rescue Drill (8/26-8/28), FF used a dry suit& SCUBA equipment. By Saturday he noticed a rash with large bumps developing on his right arm, right side of his chest, right hip and right leg. By Sunday it became red, inflamed and tender. It is assumed that something in the water at either Park Ridge or Gurnee caused these rash symptoms while the employee was participating in the water rescue classes. Supervisor's Comments & Recommendations: No comments were noted on the "On the Job Injury' form. LPC Comments & Recommendations: No Comment Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 4 of 16 F-28-03 MELVIN B. STILLSON, FIREFIGHTER 09/02/03 Description of Incident: E114 was making an extremely tight left hand turn through a construction zone onto eastbound Nerge and driver didn't realize there was a problem until returning to the station. One wheel chock missing and one mounting bracket bent. Approximate replacement cost $150.00 Supervisor's Comments & Recommendations: I would do my best to avoid driving in this construction zone. Have Village staff inspect the area to make sure emergency vehicles can get through. As of now, 1041 and 1045 Nerge are not accessible to our tower. Driveways are too narrow and need to be wider. LPC Comments & Recommendations: Driver Refresher Course FS-04-03 LEONARD STRIECH, FLEET SERVICES 09/02/03 Description of Incident: Leonard was picking up debris in the parkway at 1635 Biesterfield when he got stung in the arm by a bee. His arm felt okay that day, but swelled up overnight. Supervisor's Comments & Recommendations: No comments are noted. LPC Comments & Recommendations: No Comment Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 5 of 16 S-05-03 WALTER LAMOUNTAIN, STREETS 09/02103 Description of Incident: While working on Pratt Blvd., Walter was moving hose for air hammer when it slipped out of his hand. To prevent it from falling on the ground his foot slid causing left knee to twist. Supervisor's Comments & Recommendations: Be more aware of surroundings and foot placement. LPC Comments & Recommendations: Concur GO-03-03 KIMBERLY HINSLEY, FINANCE 09/04/03 Description of Incident: Kim hurt her shoulder lifting a PC while at the Fire Admin building. She felt something pop in her shoulder at the time of the accident, then the next morning reported that the pain had traveled down into her arm overnight, Supervisor's Comments & Recommendations: I don't believe that any significant action can be taken to avoid a repeat of this rare occurrence. Kim is usually very careful to lift properly when moving computers and other equipment. I can remind her about proper lifting techniques and refer her to a previous newsletter (distributed by jack Gray) where I believe the topic of lifting was covered. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 6 of 16 5-06-03 TODD GORE, STREETS 09/09/03 Description of Incident: While Todd was kneeling on one knee and bending over reaching for a sign base (which was under and between two shelves) he felt a strain in his left shoulder blade. ABMC states he sustained a neck strain. Incident occurred at 666 Landmeier Road paint room. Supervisor's Comments & Recommendations: I instructed Todd to make a sign, which reads "Do Not Use The Corner For Storage". LPC Comments & Recommendations: It would be best to instruct proper lifting and reaching techniques. SR-05-03 JEFF REYNOLDS, WATER & SEWER 09/09/03 Description of Incident: Employee indicates illness is job-related, stress is the doctor's initial diagnosis. Additional testing is in process and as of yet no medical documentation has been submitted to support this claim. "No medical documentation was submitted to support this claim." Supervisor's Comments & Recommendations: An injury of this type is almost impossible to avoid, as this is a requirement of the job to perform this type of work. The reaction of the employee to this type of work is individual and cannot be accurately gauged until the work is performed. Absent a physical cause for this incident, employee may need stress management courses. LPC Comments & Recommendations: TABLED (Lack of sufficient information for LPC to comment on at this time. Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 7 of 16 W-12-03 JAMES SCHAUDEK, WATER & SEWER 09110103 Description of Incident: Jim was using the post pounder to install a sign base on Shirra Court. When he removed the pounder to reset the base the chuck that goes between the pounder and the base stuck in the pounder. While relocating the pounder to the next area, the chuck fell out of the pounder and hit his right foot above the steel toe. The chuck we are told weighs 27 pounds and fell from about a two-foot height onto his toe. Supervisor's Comments & Recommendations: The crews will be warned to keep their eye on the chuck during the removal process and keep their legs and feet out from underneath the pounder and the chuck. LPC Comments & Recommendations: Concur P-40-03 STEVEN PRINDL.E, POLICE OFFICER 09/10/03 Description of Incident: Officer responded to an audible alarm at 961 Mississippi. Concerned forthe safety of an elderly resident who could be inside he entered the home. He noticed a fog in the room and observed several "Raid" bug foggers. The inhalation caused him to cough and have a bad taste in his mouth. Supervisor's Comments & Recommendations: Officer entered house to offer assistance, and to check on an unsecured residence after being summoned by a concerned neighbor. He did not detect hazard until entry had already been made. Officers on occasion are unknowingly exposed to such hazards while carrying out their required duties. Due to this I feel the incident was unavoidable. The homeowner could have prevented this matter if proper precautions were taken to placard and secure home during bug fogging. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 8 of 16 LF-02-03 STAN MUNSON, PUBLIC WORKS 09/12/03 Description of Incident: Siding on south side of building was damaged while pulling away trailer at 1635 Biesterfield (west garage) that was parked very close to the building. Supervisor's Comments & Recommendations: More caution of the surroundings in the work area. LPC Comments & Recommendations: Driver Refresher Course GO-04-03 JANE SLOGER, HEALTH DEPT. NURSE 09/12/03 Description of Incident: While reviewing use of "EpiPen", the pen discharged into her right thumb. Needle punctured right thumb bone and exited at nail bed. ABMC x-ray & tetanus shot given. Supervisor's Comments & Recommendations: Employee reviewing use of "Epipen". Inservice training on use of "Epipen" could have prevented incident. Because of short expiration dates on "Epipens" they will no longer be ordered by Community Health Nurse. LPC Comments & Recommendations: TABLED (Why do we have Epipens? If they are necessary what will be used in place of the Epipen?) Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 9 of 16 GO-05-03 PATRICIA HORNIK, FINANCE DEPT. 09/12/03 Description of Incident: Pat accidentally tripped over an open desk drawer. She bruised her right leg, but did not require medical attention. Supervisor's Comments & Recommendations: Not to leave drawers/cabinets open if not in use. Be more aware of the surroundings. LPC Comments & Recommendations: Concur F-29-03 ANDY LAIRD, FIREFIGHTER 09/20/03 Description of Incident: Unknown how scratches occurred. Most likely caused by tree branches that rubbed against body or driver's side of ambulance #8. Supervisor's Comments & Recommendations: Need to keep ambulance away from curb when parking or driving to prevent the box portion from hitting tree limbs. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 10 of 16 F-30-03 JOSEPH HOLLERBACH, FIREFIGHTER 09/22/03 Description of Incident: FF Hollerbach was doing a "dummy drag" as part of his morning physical training. While carrying a 150 Ib. dummy, he slipped on muddy ground. He regained his balance and did not fall, but did feel a twinge on his left side. Tuesday his side felt stiff and went to ABMC due to pain. Was diagnosed with pulled muscle and prescribed a muscle relaxer for pain. Supervisor's Comments & Recommendations: This supervisor is unaware of the environment or conditions that may exist at the Oak Lawn training academy. I am unable to make a determination whether there are any unsafe acts or conditions. Appears to be an environmental hazard. LPC Comments & Recommendations: Do not concur with comment of environmental hazard. LPC feels more caution when being subjected to lifting unusually heavy objects should be used. FS-05-03 JOHN MEYERS, STREETS 09/22/03 Description of Incident: Employee is experiencing numbness and tingling in his right hand, thumb,forefinger and middle finger. Supervisor's Comments & Recommendations: It is unknown what caused this condition. it is possible that a form of carpal tunnel may exit in the right arm area from a great deal of computer keyboard use or possibly his duties as a mechanic and crew leader in the past. LPC Comments & Recommendations: This incident is being reviewed by Gallagher Basset Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 11 of 16 P-41-03 DION ZINNEL, POLICE DEPT, COMMANDER 09/25/03 Description of Incident: Employee was northbound Arlington Heights Rd. in median lane passing a squad that was on a traffic stop in the curb lane approx. 500 ft. north of Elk Grove Blvd. He slowed his vehicle preparing to make a U-turn to respond to an emergency call. He was struck in the rear by another northbound vehicle. Over $500.00 damage to both vehicles. Supervisor's Comments & Recommendations: Unforeseen event caused by other driver. LPC Comments & Recommendations: Report should have been filled out by the supervisor not by the employee. H-01-03 JEFF PRIEBE:, HEALTH DEPT. 09/26/03 Description of Incident: Wheel well left rear passenger side dented/chipped paint. Unknown how damage occurred, it was observed by fleet services personnel during maintenance of vehicle on 9/26/03. Supervisor's Comments & Recommendations: The damage to the vehicle was unavoidable. Although employees attempt to avoid areas where a car might be damaged, they cannot control all parking situations. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 12 of 16 GO-06-03 LLOYD MADZEL, FINANCE DEPT. 10/02/03 Description of Incident: Touch pad water meter scanner was lost. it was in his jacket pocket and must have fallen out. Replacement cost $400.00 Supervisor's Comments & Recommendations: The water meter scanner should have been attached to belt. LPC Comments & Recommendations: Concur F-31-03 GEORGE EILERS, FIREFIGHTER 10/06/03 Description of Incident: Employee was participating in a RIT Team Drill. He participated on many different levels at different times of the drill. He had been pulling, lifting and dragging different people at different times of the drill. He had worked on the roof, lowered through the roof and climbed ladders. George is not certain where the injury took place. He noticed after drills were concluded his shoulder was beginning to hurt. Supervisor's Comments & Recommendations: When performing a drill that requires strenuous activity, limit the number of evolutions a firefighter may participate to one. Having a firefighter participate in two, three or more evolutions pushes the limits of that firefighter for that given time. Firefighter becomes fatigued and is more susceptible to injury. It is important for firefighters to be physically familiar with techniques, but not at the expense of a possible injury. LPC Comments & Recommendations: In training drills the fire department will rotate firemen being used as "victim" instead of using the same one several times. Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 13 of 16 F-32-03 PHILIP HAVERLARD, FIREFIGHTER 10/10/03 Description of Incident: Engine (E114) was backing into a cul de sac to turn around. While vehicle was backing to the left, the right front corner destroyed a mailbox and post at 1722 Biesterfield Road. Mailbox/post was destroyed and minor damage occurred to the front fender and door. Supervisor's Comments & Recommendations: Pull the engine forward into the middle of the road before backing to one side or go around the block. Phil was using a "backer" on the left rear of the vehicle, but from there he could not see the right front corner. LPC Comments & Recommendations: Driver Refresher Course P-42-03 JOHN NOWACKI, POLICE DEPT. SGT. 10/10/03 Description of Incident: Employee was on a marked police motorcycle with all its emergency lights activated in order to stop traffic. Vehicle driven by elderly female failed to stop and tried to drive around the motorcycle. While driving around motorcycle the vehicle struck it. Supervisor's Comments & Recommendations: No recommendations as they pertain to the motorcycle. Police motorcycle clearly marked with emergency lights on. Elderly motorist (F/W, DOB: 03/14/22) should not be allowed to operate a motor vehicle on the roadway. LPC Comments & Recommendations:, Concur Loss Prevention Committee Minutes for March 13, 2003 Meeting Page 14 of 16 BS-08-03 MARK STANKOVSKY, BUILDING SERVICES 10/14/03 Description of Incident: Vehicle#606 was backing into loading dock while removing seatbelt, door was opened and still in reverse. Door got caught on hand railing. Repair cost $800.00 Supervisor's Comments & Recommendations: Never open the doors until the vehicle has come to a complete stop and you have the shift lever into park and parking brake has been applied. LPC Comments & Recommendations: Driver Refresher Course BS-09-03 DUANE WESTBERG, BUILDING SERVICES 10/18/03 Description of Incident: Duane noticed the flood light/power box located on the front lawn at 901 Wellington was broken off its base. Approximate repair cost is $135.00 Supervisor's Comments & Recommendations: Since it is unknown how or what caused the damage an opinion cannot be provided. It is possible either the landscaping contractor or vandals caused the damages. LPC Comments & Recommendations: Concur
