Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 3 of 11 Review of Current of Reports P-13-04 MICHAEL,MCGREAL, POLICE OFFICER 02/18/04 Description of Incident: Officerwas stopped behind a Chevy Venture Van ata stop sign in Devon Avenue Market Access Road at Tonne Road. The driver of the van felt there wasn't enough room for traffic to get by her van so she backed up twice and struck fully marked Squad #241. Collision caused a puncture in plastic cover on right front bumper. Supervisor's Comments & Recommendations: This type of accident is not always avoidable, as the case with this collision. Because of the unpredictability of the driver's actions, the officer did not have enough time or space between vehicles to take evasive action. LPC Comments & Recommendations: Concur P-14-04 THOMAS FROELICH, POLICE OFFICER 03/01/04 Description of Incident: While parking Squad #231 at 1670 Lsndmeier Road to conduct surveillance on a suspicious vehicle, the officer backed his squad into position when he struck a garbage dumpster. Squad sustained a broken right taillight lens. No damage to th_e_dumpster. Supervisor's Comments & Recommendations: This occurrence would have been avoided if more caution was used by the officerduring his maneuver. LPC Comments & Recommendations: Driver Refresher Training Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 4 of 11 P-15-04 MICHAEL MUTI, POLICE OFFICER 03/03/04 Description of Incident: Officer was backing his squad car into a parking space at 700 Nicholas in order to run radar on Higgins Road. He apparently struck or ran over a five-foot piece of metal conduit. Officer related he did not realize this until he proceeded forward and heard it scraping on the ground. The pipe was imbedded in the left corner of the rear bumper. Supervisor's Comments & Recommendations: It is the opinion of Officer Houston of the Traffic Unit, that this crash was unavoidable. LPC Comments & Recommendations: Concur FS-01-04 PAUL FRIEDMAN, FLEET SERVICES 03/10/04 Description of Incident: Employee pulled truck (unit#704) out of the wash bay with the dump body raised and it did not clear the door header. Dump bed, cab shield, roller on garage door and garage door were damaged. Supervisor's Comments & Recommendations: Employee must be-more aware of his surroundings before moving a vehicle. LPC Comments & Recommendations: Driver Refresher Training Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 5 of 11 P-16-04 RICK MEYERS, POLICE OFFICER 03/16/04 Description of Incident: Officer was conducting surveillance on 1500 Chase from the parking lot at 1440 Chase. When Officer backed up Squad #243 he struck cinder block, damaging the right rear bumper. Scuff and scrape marks to the right rear passenger side bumper. Supervisor's Comments & Recommendations: Check behind the squad before backing up. LPC Comments & Recommendations: Driver Refresher Training F-13 MATTHEW BONIL,LA, FIREFIGHTER 03/21/04 Description of Incident: After returning from a call at 1298 Aldrin, FF Bonilla realized his pagerwas missing. He checked the gear, rig and the fire scene, but did not find it. Supervisor's Comments & Recommendations: Do not carry the pager. Use tether to secure pager to clothing. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 6 of 11 W-01-04 JOHN NICHOLAS, PUBLIC WORKS 03/26/04 Description of Incident: After leaving a job site at Biesterfield & Beisner employee was waiting at stoplight in Unit #805 (Ford F-250) and saw another vehicle attempting to leave the same jobsite. He checked mirrors before backing up to allow room for the other vehicle, but did not see the car that had pulled in behind his vehicle. No damage to Unit #805. Supervisor's Comments & Recommendations: Do not back up unless another person is there to assist. Do not try to provide additional space, let the other equipment wait several light cycles. LPC Comments & Recommendations: Report sent back to be rewritten by the appropriate supervisor. P-17-04 JOHNNELL WILLIAMS, POLICE OFFICER 04113/04 Description of Incident: Officer was backing out of the driveway area of 2241 Pratt in Squad#239, when he struck a yellow concrete post with the front right quarter panel of the squad. ($500.00 damage) Supervisor's Comments & Recommendations: Officer Williams needs to be aware of his environment and any possible hazards that may exist. LPC Comments & Recommendations: Driver Refresher Training Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 7 of 11 P-18-04 RANDALL NELSON, POLICE OFFICER 06/03 — 04/04 Description of Incident: Officer reported he discovered one of his police stars missing. He used the star only on his Honor Guard uniform, which he last wore in June 2003 for the Village parade. Supervisor's Comments & Recommendations: Officer must take better care of department equipment. LPC Comments & Recommendations;: Concur F-14-04 JOHN E. FORDE, FIREFIGHTER 04/02/04 Description of Incident: The landline extension cord for the Quint 8 swung back towards the vehicle and as the vehicle was pulling out of the station, the rear tires hit the cord. Supervisor's Comments & Recommendations: Try to check to be sure that cord and plug are clear of the vehicle and do not pull out of station until this is checked. Ensure that plug devise does not hit the floor when ejected from outlet of vehicle. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 8 of 11 P-19-04 FRANK VRCHOTA, POLICE OFFICER 04/03/04 Description of Incident: Officer responded to a call at 928 Little Falls for a report of a wild cat that had kittens in the complainant's garage. The Officer put on the Department issued animal bite gloves and placed the kittens into a cardboard animal box. Upon attempting to place the parent cat into the box,the animal became highly agitated. The cat twisted its head around and bit the officer on the right thumb, through the protective glove. The cat fled the area. Supervisor's Comments & Recommendations: Recommend reviewing effectiveness of bite gloves, which are in each police vehicle. LPC Comments & Recommendations: Police supervisors are looking into different types of protective gloves. F-15-04 MARK BLACKALLER, FIREFIGHTER 04/06/04 Description of Incident: Chicago Communications Service was installing communication cable for the police and fire departments. The cable had dropped down and caught the first section of the overhead door, thus preventing the door from opening all the way. As the Tower Truck was pulling into garage the basket (which would have blocked the view for the driver to --see-the cable) came in contact with about 2"of the bottom section of the overhead door. This incident damaged three panels of the overhead door. Supervisor's Comments & Recommendations: Do not allow work to be done on or around the overhead door while energized. LPC Comments & Recommendations: Concur -- A bill was sent to Chicago Communications Service for the damages. Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 9 of 11 F-16-04 HARVEY SPEIDEN, FIRE INSPECTOR 04/0704 Description of Incident: As employee was conducting an inspection at 930 Perrie Drive, it was difficult to distinguish between stairs and landing. 'The stairs and landing are painted the same color and there isn't a handrail. The employee missed a stair, which caused pain to his lower back. Supervisor's Comments & Recommendations: When walking down stairs, determine where landing area is. Whether or not the stairs and landing area are the same color, take extra precautions to determine the last stair and the landing area. LPC Comments & Recommendations: Concur P-20-04 MATTHEW FIORI'TO, POLICE OFFICER 04/18/04 Description of Incident: Officer was backing up Squad #246 at 109 loka in Mt. Prospect (employee's driveway) when squad came in contact with his personal vehicle, which was parked in the roadway. Driver's side quarter panel damaged on Squad #246. Supervisor's Comments & Recommendations: Exercise due caution when backing up. LPC Comments & Recommendations: Driver Refresher Training Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 10 of 11 F-17-04 PATRICK MCMANUS, FIREFIGHTER 04/21/04 Description of Incident: Screen on the Compaq laptop screen was observed to have small lines and a kaleidoscope effect. Possibly the small LED lamp that attaches to the screen put pressure on the liquid crystal inside the screen and caused this effect. The cost to replace is $1,400.00. Supervisor's Comments & Recommendations: Remove lamp from the laptop. LPC Comments & Recommendations: Lamps are being removed from the screens. FS-02-04 THOMAS MORONY, AUTOMOTIVE MECHANIC 04/23/04 Description of Incident: Employee was working on Engine#112, changing a RR tire using a 1" pneumatic impact gun (approx 35-40 lbs.)After installing the wheel/tire and tightening it with the impact gun in a crouching position, Tom could not stand back up, because of lower back pain. Tom states no heavy lifting was involved. Supervisor's Comments & Recommendations: No recommendations, because the supervisor can't identify what caused the occurrence. LPC Comments & Recommendations: No Comment Loss Prevention Committee Minutes for May 13, 2004 Meeting Page 11 of 11 F-18-04 JOHN KELLY, FIREFIGHTER 04/23/04 Description of Incident: Boat#7 was being operated on Busse Lake for training. The propeller struck something on the bottom of the lake twice. This damaged the throttle arm where it hinges on the motor. Estimated cost to repair or replace is $250.00. Supervisor's Comments & Recommendations: We can try to avoid shallow areas in the: lake. The problem is that we can not see into the water. Even if we could we would not be able to avoid all obstacles. LPC Comments & Recommendations: No Comment
