Present: Jack Gray, Chairman Larry Hammar, Deputy Police Chief Jim Forde, Deputy Fire Chief Pam i Housing Inspector Diane Hunter, Finance Department Secretary Leslie Sass, Asst. to the Chief of Police The minutes were approved for the meeting of May 13, 2004 Review Current Reports: 31 Incidents Tabled Reports: None Old Business: ➢ Confined Space Training for Fire and Utilities: J. Gray will check with Scott Bernholdt and J. Denna for training exercise date and time. ➢ "On Your Watch" class was held on August 31, 2004 with 21 employees attending. Positive comments were received regarding this class. Another class can be offered in late September if there is interest. ➢ EVOC class was held on August 26, 2004 at the Allstate Arena with seven (7) police officers in attendance. The Allstate Arena will no longer be available to us in the future. J. Gray is checking on new sites. WILLIAM MARK SELLERS, FIRE 04/28/04 Description of Incident: H & H Electrical employees park in the grass on the utilities right of way. Tire tracks were traced back to their parking area. Someone drove their vehicle on FD property damaging two control boxes for the underground sprinkler system flooding the area & parking lot. Supervisor's Comments & Recommendations: Closer supervision of contractors working in the Village. Provide designated parking areas. LPC Comments & Recommendations: Concur W-02-04 JOSH FARMER, PUBLIC WORKS-UTILITIES 05/24/04 Description of Incident: While excavating at 1351 Jarvis,the unmarked cables were struck. One cable nicked and partially broken and one was cut. SBC indicated the severed cable was abandoned and the other they elected to repair at their own expense. Supervisor's Comments & Recommendations: No recommendations LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 4 of 18 S-03-04 NEAL SHERMAN, STREETS/BLDG/FLEET SVC. 05/24/04 Description of Incident: Employee jammed his index finger of the right hand fingerwhile putting branches in truck. Supervisor's Comments & Recommendations: This is the most unusual reporting of an "injury on duty" that I have ever received. Employee has limited recollection of the event and worked over two weeks before reporting the incident to the Department of Public Works. Note: Employee also works at Jewel Foods. LPC Comments & Recommendations: No Comment SR-02-04 JAMES SCHAUDEK, WATER & SEWER 05/27/04 Description of Incident: While operating sod cutter, employee felt pain in his left elbow. This particular piece of equipment is very jerky and rough on the arms. Supervisor's Comments & Recommendations: Was a long day of cutting sod, it could have happened to anyone. LPC Comments & Recommendations: No Comment Loss Prevention Committee Minutes for 'September 9, 2004 Meeting Page 5 of 18 FS-04-04 GREG BATOR„ FLEET SERVICES 05/29/04 Description of Incident: Employee lost pager during his weekend standby duty. Supervisor's Comments & Recommendations: Better mounting system of pager, maybe with a safety tether. LPC Comments & Recommendations: Pager was recovered. Committee concurs with Supervisor's comments. P-21-04 ERIC PERKINS, POLICE 05/31104 Description of Incident: Employee was checking buildings and parking lots over the Memorial Day Holiday. As he turned into the parking lot at 2480 Estes, the front, right tire of squad #237 struck a metal storm drain. After the impact his squad struck a parked vehicle in the driveway of the business. Squad had grill damage. Supervisor's Comments & Recommendations: Officer Perkins was advised that caution must be used when operating a motor vehicle. He was also advised to check for obstructions before turning into private parking lots. LPC Comments & Recommendations: Driver Refresher Training Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 6 of 18 F-20-04 JOHN FORDE, FIREFIGHTER 06/02/04 Description of Incident: John states he has been feeling a general soreness in his neck for the last 2 months. While enroute to this incident he began to feel a numbness and tingling in the two fingers of his left hand. He reported this to me upon their return to quarters. He was sent to ABMC Occ. Health for evaluation. Doctor feels this injury may have come from either climbing into or while riding on the vehicle. Supervisor's Comments & Recommendations: I can't say how to prevent this in the future without knowing exactly what caused this injury. It may be possible that the configuration of the back of the seat puts pressure on a certain area of the back that could cause muscle or nerve inflammation. However, it could be possible that John's history of a back or neck problem may have made him more susceptible to this kind of injury. LPC Comments & Recommendations: Concur S-04-04 MICHAEL WAGNER, STREETS 06/08104 Description of Incident: While tipping a ladder to fold it up a chisel fell off the top rung and struck employee in the head. Five sutures to his scalp and a tetanus shot was given. Supervisor's Comments & Recommendations: Store equipment/tools appropriately after use. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 7 of 18 W-03-04 SCOTT BERNHOLDT, PUBLIC WORKS 06/08/04 Description of Incident: Nicorwas excavating to repair a leaking gas service when they struck the EGV streetlight cable. The EGV line was mismarked due to errors in "As-Builts" maps. Supervisor's Comments & Recommendations: Likely that this type of accident will continue since contractor appears to have not followed plans and contract inspection did not catch the contractor. EGV personnel operated the equipment correctly. When time permits, locaters will be encouraged to trace additional sections of the cable to begin connecting the "As-Builts"that are not"As- Builts". LPC Comments & Recommendations: Concur F-21-04 KEVIN COTTE, FIRE 06/09/04 Description of Incident: FF Cotte was attending PADI certification class with Arlington Hts. Fire Academy. On his first deep-water dive he was instructed how to submerge. He did so and felt a sudden pressure in both ears and a nosebleed. He was told this was normal by his instructor. FF Cotte dove again on 06/11/04 and felt pressure. On 6/12/04 he felt dizzy and had pain in his left ear. He called his primary doctor for an appointment Supervisor's Comments & Recommendations: Have the instructor use less weight on diver's weight belt to alleviate the problem. LPC Comments & Recommendations: Committee disagrees with supervisor's comments. We feel that FF Cotte wasn't pressurized properly. Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 8 of 18 F-22-04 LANCE HARRIS, FIRE Report Recvd after 06/15/04 Description of Incident: While on an auto accident on northbound 53 at Biesterfield FF Harris lost his pager. Supervisor's Comments & Recommendations: Either clips that fit better or carry your pager in your pocket or don't carry your pager on duty. LPC Comments & Recommendations: Concur S-05-04 KRISTIN KIEFER, STREETS/BLDG/FLEET SVC. 06/17/04 Description of Incident: Employee was pruning roses and was stung by a bee on her left leg. She has allergic reactions to stings. Supervisor's Comments & Recommendations: No comment. This is a common occurrence. We removed the bees nest from the area. RICK GARCHIE, FIRE Report Recvd 06118104 Description of Incident: The laptop computer in Ambulance#125 has a broken door to CDROM. Not reported by Fire Dept. Was discovered by IS Personnel. Supervisor's Comments & Recommendations: Turning the computer so it faces the passenger's seat will get this door out of the way. The passenger then has full view of the screen. However,the status change buttons may need to be moved so the driver can access them. LPC Comments & Recommendations: Need to be more careful. P-22-04 TIMOTHY KELINFELDT, POLICE 06/19/04 Description of Incident: While chasing an offenderthrough backyards at 1040 Arkansas, the officer turned his left knee. He reported the injury the following day. He indicated that there is no swelling and only minor discomfort. Supervisor's Comments & Recommendations: Officer was attempting to catch a violator by giving chase through yards. Due to the dampness the grass was wet and uneven. The officer could not have perceived this act as unsafe or unreasonable. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 10 of 18 P-23-04 MICHAEL GASPARI, POLICE 06/23/04 Description of Incident: Officer was traveling southbound on 1355 when he heard a stone or similar object strike the windshield on the driver's side. This caused the windshield of Unit #255 to crack. Supervisor's Comments & Recommendations: Officer never saw the object heading toward his vehicle so he was never able to take evasive action. LPC Comments & Recommendations: Concur W-04-04 ROBERT GRAUF, WATER & SEWER 06/30/04 Description of Incident: Employee was performing assigned tasks at Well House#11 (1689 Virginia)and felt pain in left knee as he ascended a ladder. Pain persisted through remainder of the day and by the next day found difficulty only when climbing stairs and/or ladders. Supervisor's Comments & Recommendations: No recommendations at this time. Accident was not preventable LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 11 of 18 F-24-04 MAUREEN DIEGEL, FIRE DEPT. CLERK 07/01/04 Description of Incident: While lifting a package the employee felt pain in lower abdomen. ABMC reports she "strained" her left side abdominal wall & lower back area. Supervisor's Comments & Recommendations: Maureen lifted a package that was too heavy for her capabilities. I told herto ask for help to lift packages. LPC Comments & Recommendations: Concur P-24-04 FRANK GOTTARDO, POLICE 07/03104 Description of Incident: Officer was responding to a call at Cleannont and Tone Roads of a man bleeding. While attempting a left turn, squad #238 skidded on wet pavement striking the curb with right front tire. Supervisor's Comments & Recommendations: Utilize proper speed based on exiting conditions. LPC Comments & Recommendations: Driver Refresher Training (Supervisor did not fill out report, the officer did.) Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 12 of 18 F-25-04 CLINT CUNZ, FIRE 07/05/04 Description of Incident: The VHF portable radio fell into a pond during a mutual aide call in Schaumburg. Radio was dried out and the contacts were cleaned. Radio is back to working order. Supervisor's Comments & Recommendations: Try to keep the portable away from water. Secure in a pocket. LPC Comments & Recommendations: Concur F-26-04 MICHAEL PESTEL, FIRE 07/06/04 Description of Incident: When driver closed the door of Ambulance#8 (unit#126)the driver's side rearview mirror fell out and broke upon hitting the ground. Supervisor's Comments & Recommendations: Report incident to Ford Motor Company to determine if this is a defective part. LPC Comments & Recommendations: It was not a defective part. It will be charged to the insurance account not maintenance account. Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 13 of 18 VS-02-04 MARCELINO GARCIA, STREETS 07/08/04 Description of Incident: While pulling up a bucket of roof patch with a rope at 666 Landmeier, the employee injured his left shoulder. Left shoulder joint strain per clinic. Supervisor's Comments & Recommendations: The employee was shown at other locations to use a 1 gallon container, not to try to pull up a 3 '/s gallon container thru the roof hatch. This accident may not have happened if the employee was using a smaller container. LPC Comments & Recommendations: Employee did not follow instructions of supervisor. P-25-04 THOMAS FROELICH, POLICE 07/11/04 Description of Incident: Officer was patrolling the parking lot at 910 Lincoln Square in Squad#236. As officerwas driving southbound through the lot, his :squad was struck by a vehicle backing out of a parking space. The driver of the vehicle stated that he did not see the squad car. Supervisor's Comments & Recommendations: Officer could not avoid accident. Offending driver claims to have valid insurance, but presented an expired insurance card. LPC Comments & Recommendations: Concur (Supervisor did not fill out report, the officer did.) Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 14 of 18 F-27-04 ROBERT MARSHALL, FIRE 07/13/04 Description of Incident: FF Marshall was using the K-12 saw to cut a ventilation hole at 800 E. Devon. While cutting the roofing material, the saw kicked back on him and he felt pain in his left shoulder. Employee finished his cut and began assisting other FF's with stripping the roofing material. He discontinued working when he began feeling "pins and needles and numbing sensations" to his left hand. Supervisor's Comments & Recommendations: FF Marshall was wearing all of his protective equipment when this injury occurred. He was in proper position and he was applying proper technique when the injury occurred. The K-12 is a powerful tool and even when used properly, can grab and kick back. He did a fine job of controlling the tool, but unfortunately he injured his shoulder in the process. LPC Comments & Recommendations: Concur P-26-04 MICHAEL GARRISON, POLICE 07/15/04 Description of Incident: Officer was operating motorcycle unit #273 and was attempting to maneuver back into traffic after clearing a traffic stop. As he entered the roadway from the median and attempted to get into the outside lane, the vehicle positioned directly ahead of him abruptly stopped resulting in the motorcycle's crash bar clipping the rear of the vehicle. Supervisor's Comments & Recommendations: Be more careful when operating motor vehicle and always be prepared and ready to safely stop. LPC Comments & Recommendations: Driver Refresher Training Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 15 of 18 S-06-04 JAMES SCHWARTZ, STREETS 07/22/04 Description of Incident: Employee was backing dump truck #740 out of the garage when he backed into the inspector's car, which was parked in the no parking zone. Front passenger mirror broken, door dented and glass broken. Supervisor's Comments & Recommendations: Should have had someone guiding him while backing up. LPC Comments & Recommendations: Driver Refresher Training (Supervisor did not fill out report) P-27-04 DAVID KLINGSPORN, POLICE 07/26/04 Description of Incident: While stopped in traffic(W/B Devon east of Nerge),the rear of Squad#233 was struck by the front bumper area of the car that was behind the squad. Supervisor's Comments & Recommendations: This crash was unavoidable. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 16 of 18 F-28-04 LT. MARK PERCUDANI, FIRE 07/2804 Description of Incident: While on an auto extrication at Elmhurst Road and Devon Avenue, the sensor from Life Pack 12 was broken into three pieces. Supervisor's Comments & Recommendations: The damage may have been avoided if the sensor was removed from the patient, prior to freeing him from the vehicle. LPC Comments & Recommendations: Concur P-28-04 RICHARD GIGANTE, POLICE 08/01/04 Description of Incident: After stopping a traffic violator, the driver put his vehicle in reverse and backed into Squad #235. Damage to left front quarter panel. Supervisor's Comments & Recommendations: The accident was captured on the officer's MVR. It clearly shows the actions of the officer attempting to put the driver's vehicle in park by reaching in through the open window. The officer had no control regarding the other driver's inability to secure his vehicle. The officer took action in attempt to prevent any damage. Recommendation none. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 17 of 18 F-29-04 JORGE SIGLER, FIRE 08/01/04 Description of Incident: While changing personnel on E-10, it was noticed that the crystal display screen on the portable radio was damaged. It is unknown how or when this damage occurred. Supervisor's Comments & Recommendations: Exercise due caution and care with the portables. LPC Comments & Recommendations: No Comment P-29-04 THOMAS DANAHY, POLICE 08/17/04 Description of Incident: The Motorola Model #MW520 computer monitor in Squad #232 was accidentally struck with a flashlight, which resulted in the screen breaking. Supervisor's Comments & Recommendations: Unavoidable accident LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 9, 2004 Meeting Page 18 of 18 F-30-04 WILLIAM REID, FIRE 08122/04 Description of Incident: During a check out on Quint 8, it was noticed that there was a deep scratch on the frame on driver's side steps up to turntable. Supervisor's Comments & Recommendations: Unknown how or when damage occurred. If hand tools caused the damage, they should be kept in tool belt. LPC Comments & Recommendations: No Comment
