The meeting was called to order at 8:35 A.M. at the Village Hall.

➢ Confined space training for Fire and Utilities is still pending on a date.
➢ "On Your Watch" class is being offered again on December 8, 2004 and will be
held at the Fire Administration facility in the Fire Prevention Conference Room.

F-31-04 WILLIAM KNIPP, FIREFIGHTER 6/09/04
Description of Incident:
While treating a patient enroute from Jensen Park to ABMC, patient began seizure
activity and vomited. Vomit hit F/F Knipp in the face and mouth. The crew was not able
to anticipate all possible developments that took place. The crew was unable to don full
protective equipment because of an unexpected seizure activity.

F-32-04 JAMES JOHNSON, FIRE LIEUTENANT 06/09/04
Description of Incident:
While transporting a patient from Jensen Park to ABMC, the patient began seizing, cut
her hand and hit Lt. Johnson in the mouth with her hand.
Supervisor's Comments & Recommendations:
The accident was unavoidable. The patient just started seizing unexpectedly while being
secured.

P-30-04 MICHAEL McGREAL, POLICE OFFICER 08/15/04
Description of Incident:
While parking squad #245 behind an officer's personal vehicle in the parking lot at 901
Wellington,the front bumper of the squad made contact with rear bumper of the vehicle in
front of the squad. The license plate frame and retaining screw damaged plastic bumper
cover on rear of the other vehicle.
Supervisor's Comments & Recommendations:
The officer needs to use due diligence and safety when parking behind another vehicle.
Also, the officer's personal car is not allowed to be parked in the location where the
accident occurred.

F-33-04 THOMAS REINHARDT, FIREFIGHTER 08/29/04
Description of Incident:
While assisting in lifting Hurst equipment on TWR7 at EGFD Station #7, F/F Reinhardt
felt a popping in his back and fell to his knees on the ground in pain.

P-31-04 THOMAS KREJCZYK, POLICE OFFICER 09/04/04
Description of Incident:
Employee was attempting to turn around in the parking lot at 1027 Charlela Lane in
Squad #239. As employee backed up vehicle, he struck a parked vehicle at less than 5
mph. Employee thought he had more room to complete his maneuver.
Supervisor's Comments & Recommendations:
Limit backing of vehicles to areas with adequate space.

P-32-04 DION ZINNEL, CMDR. POLICE 09/07/04
Description of Incident:
Criminal damage by unknown offender in Interview Room #3. Two metal pieces were
pulled off the vent. It is unknown if offender was contemplating possible escape,
contraband hiding, or use of metal as weapon.
Supervisor's Comments & Recommendations:
All officers have to be aware of the condition of interview rooms and potential damage
that can occur. We need to better secure these ceiling vents. This was a potential
deadly weapon that could have harmed an officer if the offender acted. In a perfect
situation we could have video monitoring in the interview rooms.

Unavoidable. LPC recommends due to financial resources, possibly installing a "fake"
camera to deter offenders.

Description of Incident:
Ofc. Suarez was the driver of Squad #233. He was backing out of a parking stall when
Ofc. Carow opened the right front door to speak to the officer. The officer did not notice
that the door was opened until it struck Squad #242.
Supervisor's Comments & Recommendations:
Opening the door to the squad while it was still in motion was the major cause. Stop this
type of activity.

P-34-04 KEVIN FINNEN, POLICE OFFICER 09/11/04
Description of Incident:
Squad #257 was parked in the parking lot at 437 Perrie Drive while conducting an
investigation. The officer didn't notice until he was filling the vehicle with gas that the
antenna, which is attached to the rear window was missing.
Supervisor's Comments & Recommendations:
This type of theft is very difficult to prevent, unless an officer constantly watches his
vehicle at all times.

P-34-04 JOHN FORDON, FIREFIGHTER 09/13/04
Description of Incident:
While employee was pulling an unconscious driver out of a car thru the passenger's door
(vehicle was against a "K rail" on driver's side), he felt something pull in his back.
Supervisor's Comments & Recommendations:
Awkward position of the patient, along with necessity to get him out rapidly in a life
threatening situation.

SR-03-04 MARK BLANCHFIELD, UTILITIES 09/14/04
Description of Incident:
Five village vehicles at 666 Landmeier Road had been entered. Two tires and one fire
extinguisher needed to be replaced.

P-35-04 THOMAS FROELICH, POLICE OFFICER 09/14/04
Description of Incident:
Ofc. Froelich was attempting to make an arrest at 611 Landmeier Road on a
uncooperative, combative subject. The officer used his pepper spray for compliance.
The pepper spray was blown back into his face, causing a burning sensation to his eyes.
Supervisor's Comments & Recommendations:
It is this supervisor's opinion this incident was unavoidable. Ofc. Froelich was complying
with policy and procedures in regards to "use of force'. The injury caused to Ofc. Froelich
was primarily due to weather conditions.

P-36-04 CHARLES WALSH, SGT. POLICE 09/20/04
Description of Incident:
In the process of restraining a subject at 2411 Landmeier Road, who was threatening
harm to himself, Sgt. Walsh had to chase and hold subject to the ground for paramedics.
In process of control his right knee hit the parking lot pavement and suffered a small
contusion/cut. He did not desire any medical treatment for the injury, which should heal
without medical attention.
Supervisor's Comments & Recommendations:
Directed officers at scene to keep closer watch on subject until inside ambulance to
prevent running away as an option.

F-35-04 TODD RISHLING, FIREFIIGHTER 09/23/04
Description of Incident:
The shoreline plug was run over(destroyed) by the engine, as response was made to an
ambulance call. Incident occurred at 11355 Greenleaf-Station 9.
Supervisor's Comments & Recommendations:
When switching rigs, the proper shoreline must be placed on the electrical drop rather
than plugging one into another.

P-37-04 JEFFREY PRIOR, SGT. POLICE 09/26/04
Description of Incident:
While assisting with a criminal investigation, Sgt. Prior was walking quickly through the IC,
when he felt a sharp pain in his left knee.

P-36-04 MARC SKOWRONSKI, FIREFIGHTER 09/29/04
Description of Incident:
The portable pulse ox was left within the patient bedding when the patient was transferred
from the stretcher to the ER bed.

Do a more thorough equipment inventory prior to leaving the hospital.

W-05-04 DAVID WROBLE, WATER & SEWER 09/29/04
Description of Incident:
While performing maintenance to lift station check valve at 1031 Nerge Road (Rohlwing
Sanitary Lift Station) employee was removing debris from body of valve when a
hypodermic needle punctured his left second finger.
Supervisor's Comments & Recommendations:
Additional caution will need to be taken in the future. This is a fairly common task, we
will take as much caution as possible in the future.

W-06-04 BOB GRAUF, PUBLIC WORKS 10/01/04
Description of Incident:
While turning around the vehicle at the Well and Pump House#6 at 1650 Greenleaf, the
employee failed to see support wire for telephone pole.
Supervisor's Comments & Recommendations:
Wrap the wire with plastic tubing to provide additional visibility.
LPC Comments & Recommendations:
Concur...LPC would like the Utilities Division to contact Com Ed to check on who should
wrap wires (Com Ed or Village) to prevent this from happening again.
W-07-04 JOSHUA FARMER, UTILITIES 10/02/04
Description of Incident:
A case tractor pulling out of the Public Works facility at 666 Landmeier on to Tonne was
involved in an accident. A car going west on Landmeier went thru a red light causing
case tractor to stop quickly and van #812 hit the case tractor. No damage to case tractor.
Supervisor's Comments & Recommendations:
Should keep a safe distance behind vehicle in front.

P-38-04 LYNN ATKINSON, CMDR. POLICE 10/05/04
Description of Incident:
Employee was using the bathroom, when the Nextel 1000 Cell phone fell into the toilet.
Supervisor's Comments & Recommendations:
This is a rare situation, however, it has occurred before. The only way to absolutely
guarantee that the phone won't hit the water is to remove the equipment and relocate itto
a safe location prior to any bathroom activities. No negligence on the part of this
employee.

F-37-04 LYNN COX, FIRE DEPT. ADMIN Reported
10/05/04
Description of Incident:
Thirteen (13) older Motorola Minitor radio pagers were stolen from the Fire Admin Bldg.
Supervisor's Comments & Recommendations:
Unknown as to why anyone would want this old equipment. The equipment had been
inventoried and was in employee's office waiting for shipment to a broker.

P-39-04 CASEY KOWALCZYK, POLICE OFFICER 10/23/04
Description of Incident:
Officer responded to a reported retail theft at 8:23 pm at the Dominick's Finer Food Store.
He approached suspects as they entered their car. Officer ordered subjects to stop,
offenders started car and attempted to strike officer as they fled the lot. Officer was
forced to take evasive action to avoid being struck by the vehicle. Officer felt no initial
pain, but began to feel his back tighten. Officer advised sergeants of his back pain while
completing paperwork that evening at about 11:30 pm.
Supervisor's Comments & Recommendations:
Officer was on scene in the Dominick's parking lot when the call was dispatched for retail
theft. Officer had offenders on scene in sight, which prohibited the officer from waiting for
back up units that were already enroute to his location. Officer attempted a safe
approach to the vehicle knowing he had an unknown threat with two offenders in the
vehicle. Subjects were uncooperative raising the threat level. Officer did his best to
control the situation until assistance arrived. When officer observed offenders driving at
him in an apparent attempt to strike him and escape the area, the officer made the
appropriate response by getting himself out of the path of the oncoming vehicle. The
officer was the victim of a set of unavoidable circumstances that resulted in his injury.

VMO-01-04 DEBRA WEBER, VILLAGE MGR CLERICAL 10/26/04
Description of Incident:
Employee was carrying an armload of files and mail. As the paperwork slipped out of her
arms, her finger was sliced by the metal clasp on a Kraft envelope. Medics at Station #7
applied bandages.
Supervisor's Comments & Recommendations:
Perhaps carried too many files. Two trips may have been safer, but the metal clasp on
the envelope can't be eliminated.

SR-04-04 RICHARD ENGLER, WATER & SEWER 11/01/04
Description of Incident:
While trying to remove a metal from rack, metal slipped and caused hand to go upward
making contact with rack above. Contusion of left hand per ABMC.

To be aware of heavy objects you are lifting.

After further review,John Wesa is not being sent for the Driver Refresher Course since there was a
misunderstanding regarding his involvement according to the traffic accident report. The proper
person to attend the Driver Refresher Course is Joshua Farmer.
