The meeting was called to order at 8:40 A.M. at the Village Hall.

➢ Confined Space Training for Fire and Utilities: J. Forde will speak to J. Denna with
regards to selecting a date.
➢ "On Your Watch" was offered on December 8, 2004 with six (6) employees in
attendance.
➢ Driver's Refresher Class was held on December 8, 2004 with seven (7)employees in
attendance. At our next LPC meeting (March 10, 2005) Mrs. Sally Bulicek will visit
us to discuss the continuation of this class.

F-38-04 JORGE SIGLER, FIREFIGHTER 10/23104
Description of Incident:
While on a call at 1848 Fox Run, F/F Sigler was carrying the stretcher down a few stairs
when he felt a pull/discomfort in his groin. He was examined at ABMC.
Supervisor's Comments & Recommendations:
Attempt to have more crew members assist in moving the cot at all times.

P-40-04 MYLES GREENBERG, POLICE OFFICER 10/26104
Description of Incident:
Laser radar unit (serial #UX002297) was left inadvertently on the roof of his squad and
drove away. The radar unit fell to the ground, affecting internal operations. Repair cost
estimated at $100.00
Supervisor's Comments & Recommendations:
Inattentiveness on the officer's part leaving the radar unit on the roof of the squad and
then driving away. Officer should not place equipment on any exterior surface of the
vehicle while loading at the beginning of the shift. This lends itself to these types of
incidents which occur much too frequently.

VMO-01-04 RAY RUMMEL, VILLAGE MANAGER'S OFFICE 10/31/04
Description of Incident:
While backing vehicle out of garage at 611 Meadow at a low rate of speed, the right front
fender barely struck rubber protected garage post. Barely visible indentation on the front
fender and no damage to the post.
Supervisor's Comments & Recommendations:
The accident resulted from the driver not paying full attention to the surroundings while
backing the vehicle. Driver must be aware of surroundings when backing even at low
speeds.

FS-05-04 TOM MORONY, PUBLIC WORKS 11/01/04
Description of Incident:
Employee closed the west overhead door at 1635 Biesterfield (Fleet Services) with his
remote control opener, not knowing that a vendor "Wentworth Tire' was approaching.
NOTE: the "vehicle damage" refers to the vendor's truck. Vendor never contacted the
Village to request repair. Finance is submitting this as a claim through Gallagher Bassett
because the overhead door damage totaled $1,330 & is over our $1,000 deductible.
Supervisor's Comments & Recommendations:
Door does have a reversing feature on the lower leading edge. However, damage can
occur before the door reverses itself. An infrared safety would eliminate this from
reoccurring. This will be added to all doors at this facility that are of a high use.

P-41-04 SHEARA GARRISON, POLICE OFFICER 11/05/04
Description of Incident:
Officer finished fueling squad#256 at the gas pump located in the parking lot behind 101
Biesterfield. She began backing up when she realized that she had not disconnected the
gas nozzle from the vehicle. The fuel door bent and caused damage to the area in front
of the fuel door.
Supervisor's Comments & Recommendations:
Officer failed to complete the fueling process by not disconnecting the fuel pump nozzle
from the vehicle before placing the vehicle in motion. Officer should pay greater attention
to detail during the fueling process.

FS-06-04 BRUCE ROBINSON, FLEET SERVICES 11/12/04
Description of Incident:
Employee states that he has pain and numbness in right hand and forearm. Unknown to
what could have caused this type of injury. Employee has sought medical attention via
his private physician. Gallagher Bassett is reviewing to determine whether or not it
qualifies as work related.
Supervisor's Comments & Recommendations:
Since it is unknown how this injury occurred or in what time frame, only a doctor could
recommend preventative measures.

F-39-04 GEORGE EILERS, FIREFIGHTER 11/13/04
Description of Incident:
F/F Eilers was at the front of the stretcher that had a heavy patient on it. F/F Eilers went
to place the stretcher in the loading position by raising up the front and he felt a pull in his
groin area.
Supervisor's Comments & Recommendations:
Given the size of the patient, F/F Eilers could have waited until there was another person
there to help with the lifting of the stretcher into the loading position.

F40-04 THOMAS WIND, FIREFIGHTER 11/14/04
Description of Incident:
F/F Wind was performing overhaul operations at a house fire (394 South Spruce Drive,
Wooddale). He was removing drywall and insulation when he got something in his left
eye. His eye was irrigated on the scene by on scene medics and he was then
transported to ABMC. He did not have his goggles on.
Supervisor's Comments & Recommendations:
In order to prevent further incidents like this, it is important that the protective goggles be
worn when performing this type of function.

S-07-04 STANLEY MUNSON, SEWER 11/17/04
Description of Incident:
Employee was operating brush chipper when a tree limb smashed his finger against the
chute of the machine. ABMC clinic x-rayed and bandaged left pinkie finger.
Supervisor's Comments & Recommendations:
No unsafe acts. Just one of the hazards that can happen with a brush chipper.

SR-04-04 PETE SAGARTZ, SEWER (Seasonal Employee) 11/29/04
Description of Incident:
While gaining access to water meter remote,the employee moved several branches from
shrubbery when one branch released and struck employee in right eye. He was referred
to an ophthalmologist who described his injury as a "corneal abrasion".
Supervisor's Comments & Recommendations:
To reduce the likelihood of reoccurrence, whenever a location requires branches to be
physically moved, the employee should don safety glasses.
LPC Comments & Recommendations:
It is the recommendation of the LPC Committee to have Public Works put a request in the
Village Newsletters regarding the problem. Article should state the residents need to
clear snow and debris, as well as trim shrubbery and vegetation around the meters for
easier and safer access by the Village meter reader.

F-41-04 JOHN FORDE, FIRE 11/29/04
Description of Incident:
John Forde reported that the wood door hardware and hinge is bent on dumpster shed.
Hardware does not line up with holes in ground. $100.00-$200.00 to repair.

Better care when removing dumpster from it's shed.

F-42-04 WILLIAM SELLERS, FIRE 12/02/04
Description of Incident:
Touch key pad to enter northeast employee entrance at 1000 Oakton (Fire Station #8)
broke loose from wall, but still functions. $125.00 to repair.

P-42-04 SHAUN BERENS, POLICE OFFICER 12/03/04
Description of Incident:
During the arrest of a combative offender at 142 Wildwood lace, the officer injured his
groin while wrestling with offender. ABIVIC diagnosis was a strain to left hip/groin area.
Supervisor's Comments & Recommendations:
Officers arrested combative subject in a narrow halfway and had little room to maneuver
when subject resisted arrest. No action could have prevented this injury, since a physical
confrontation was created by the arrestee.

P43-04 PAUL SYVERSON, POLICE OFFICER 12/06/04
Description of Incident:
While at Oakton and Kent on a traffic stop, the car video transmitter and case fell off the
officer's garrison belt.
Supervisor's Comments & Recommendations:
While entering and exiting the vehicle many times a day, certain items on the Garrison
belt get caught on various parts of the door. This may have caused the case to unsnap
and fall off unknowingly. Check the equipment on the belt frequently especially when
exposed to rigorous duty.

F-43-04 .JOHN LODEWYCK, FIREFIGHTER 12/07/04
Description of Incident:
F/F Lodewyck was starting an IV line for ALS patient. While he was advancing the
needle, the patient moved her arm forward, causing the needle to pop through the skin at
the point where he was securing the arm and the needle punctured his left thumb.
Supervisor's Comments & Recommendations:
In order to try to prevent this situation from occurring in the future, it is important to watch
for hand placement that might be in line with the IV needle.

F-44-04 CARLA PENN, FIREFIGHTER 12/11/04
Description of Incident:
F/F Penn was carrying the drug bag with the shoulder strap to the scene of a medical
emergency, when the Velcro securing the strap unfastened and caused the buckle end of
the strap to fly up and strike her in the right eye. This incident resulted in a
subconjunctival hemorrhage & facial contusion of the right eye area. Went to ABMC.
Supervisor's Comments & Recommendations:
Velcro holding the strap should be checked to be sure it is secure. Strap could still come
loose if turned or bumped in certain directions.

S-08-04 KENNETH LOBOCKI, STREETS 12/15/04
Description of Incident:
When taking plow off truck#702, Mike Burke had a hard time removing the pin from plow
frame. Ken tried to help Mike when walking to the other side of plow he hit his knee on
plow frame. No medical treatment.
Supervisor's Comments & Recommendations:
Be more aware of work area.

S-09-04 ED FRANZEN, PUBLIC WORKS 11/23/04
Description of Incident:
Street light cable at 950 Arthur and 800 Devon was cut during the digging process. The
locate required both sides of the street to be flagged and only the north side of street was
located. Estimated cost to repair is $1,000.00
Supervisor's Comments & Recommendations:
Double check all location requests in the field before informing of an all clear. Any other
way is unacceptable because prints are not always correct.

P-44-04 GEORGE MOUJOUROS, POLICE OFFICER 12117/04
Description of Incident:
Squad#267 was parked unattended in the parking lot at 565 Landmeier. The officer had
parked the squad at 1505 hours to take his break at the 7-Eleven. When the officer
returned 15 minutes later, he discovered that person(s)unknown had broken out the right
outside rear view mirror on the squad. The mirror was inspected and there was no
indication that the mirror had been damaged as a result of another vehicle coming into
contact with the mirror. The damage was apparently intentional. $100.00 to replace.
Supervisor's Comments & Recommendations:
Squads should be parked in areas that can be seen by officers or the public. This vehicle
was parked on the side to afford customers the close parking spaces.

W-08-04 JOHN WESA, WATER 12121104
Description of Incident:
When digging up a B-Box across from 1234 Aspen the backhoe bucket made contact
with the cable, thus cutting the line.
Supervisor's Comments & Recommendations:
Try to be more accurate when marking street lights.

F-45-04 BELINDA GUGLIELMO, FIREFIGHTER 12/25104
Description of Incident:
F/F Guglielmo was performing an Ice Rescue of unresponsive victim and was pulling the
victim and the other rescue paramedic out of the water. The F/F had EMS gloves on and
the rope caused bruising to her hands due to the pressure and weight on the rope.ABMC
diagnosed "left hand contusion".
Supervisor's Comments & Recommendations:
To help prevent this type of injury in the future, it is recommended that when using ropes
the firefighter should wear rescue or fire gloves. This will help to prevent the rope from
crushing in on the hand.

P-45-04 ELIZABETH BAUMBACH, POLICE CSO 12/27104
Description of Incident:
CSO Baumbach was operating squad #247 making a left turn from E/B Biesterfield onto
N/B Arlington Heights. Road conditions were icy and during the turn,the squad veered to
the right, causing the vehicle to strike the curb.
Supervisor's Comments & Recommendations:
More care by vehicle operator during poor road conditions. Also, earlier notification to the
proper salting authority to salt intersections.
LPC Comments & Recommendations:
TABLE REPORT -- Further investigation in regards to the "pop" sound mentioned in
CSO Baumbach's memo will be made prior to the committee making a final decision.

W-09-04 JOSH FARMER, WATER 12129/04
Description of Incident:
While digging main break at 241-247 Crest, the bucket made contact with the street light
cable, thus cutting cable.
Supervisor's Comments & Recommendations:
According to AS-BUILTS street light cable should not have been in dig site area. Be
more accurate in mapping.
LPC Comments & Recommendations:
TABLE REPORT — Further investigation by the Engineering Department will be made
prior to the LPC Committee making their final decision.
P-46-04 EDWARD HANSEN, POLICE SGT. 12/29/04
Description of Incident:
It appears as if the Remington Model 581, .22 caliber bolt action rifle (Serial #1071311)
had fallen off of or been knocked off of the shelf in the storage locker. $50.00 to repair.
Supervisor's Comments & Recommendations:
Damage appears to be accidental. Purchase a gun case and/or storage rack for the rifle.
LPC Comments & Recommendations:
The Police Department will supply the proper gun case to store the rifles, which will then
alleviate the problem.
