Loss Prevention Committee Minutes for May 12, 2005 Meeting Page 3 of 12 Review of Current of Reports S-06-05 GEORGE, MURTAUGH, Public Works 02/10/05 Description of Incident: On Lively just south of Arthur, the Pace bus stop glass pane was broken by unknown vandal. Repair cost $200.00. Supervisor's Comments & Recommendations: Believed to be vandalism LPC Comments & Recommendations: Concur F-07-05 RICHARD DAVIS, Fire Dept. Inspector 02/18/05 Description of Incident: Village vehicle #152 was traveling in southbound lane of 1-290 near Thorndale, when wheel cover came off. Due to heavy highway traffic, vehicle was unable to stop in a safe position to look for wheel cover. No charge for repairs. Supervisor's Comments & Recommendations: The wheel covers may be checked daily for proper attachment. This type of property loss is sometimes unpredictable. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 12, 2005 Meeting Page 4 of 12 F-08-05 JOHN LODEWYCK, FIRE DEPT. 02124/05 Description of Incident: Pager was lost during the course of the shift, since several calls were run that day. FF has to put the fire gear on and off several times and during the course of this the pager was lost. Replacement cost about $100.00. Supervisor's Comments & Recommendations: We should purchase the clips that properly attach to our wide belts we wear so that the pager cannot come off. Tethering the pager to one's belt is another alternative. LPC Comments & Recommendations: Concur W-04-05 Reported by: SCOTT NAILOR, Public Works 02125/05 Description of Incident: At the corner of Charlela and Nerge a vehicle struck the ballard post, which protects the Rohlwing lift station. Post knocked over but can be straightened. Repair cost $33.00. Supervisor's Comments & Recommendations: No supervisor comments shown. Loss Prevention Committee Minutes for May 12, 2005 Meeting Page 5 of 12 P-10-05 MATT FIORITO, POLICE DEPT. 03/01/05 Description of Incident: Officer was involved in a K9 training session. Officer was acting as decoy for another K9 team when he was caught on the sleeve pad and was jerked, which put stress on his shoulder resulting in pain to the right shoulder area. Went to ABMC. Supervisor's Comments & Recommendations: Officer was working in a clean, safe environment without obstructions. Officer was wearing a protective K9 "bite sleeve"during the training session. Although officertook all practical steps to minimize risk during K9 training, he could not anticipate the actions of the aggressive police dog and the resulting stress to his shoulder. It was noted that aggression work with animals has inherent risk factors, however, officer was advised to take any steps necessary to minimize that risk. LPC Comments & Recommendations: Concur P-11-05 MARK MARUSICH, POLICE DEPT. 03/03/05 Description of Incident: Officer was walking around the front of a vehicle, which was parked in the east parking lot of 2400 Arthur. The vehicle had been broken into earlier and victim contacted the police. Officer advised he stepped into a pothoRe, which he did not see, causing him to lose his balance and fall. He landed on his left side, hitting with his shoulder first and then his head. As a result he suffered minor abrasions to the left hand and pain in his neck, head and back. Supervisor's Comments & Recommendations: Officer is responsible to watch his footing in all environments. Accidental tripping is difficult to prevent, due to the fact it is by nature "accidental." LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 12, 2005 Meeting Page 6 of 12 W-05-05 ROBERT GRAUF, WATER & SEWER 03/11/05 Description of Incident: While exiting truck #811 at Route 83 lift station, employee slipped on ice and fell. Supervisor's Comments & Recommendations: Be more aware of conditions and take more caution. LPC Comments & Recommendations: Concur P-12-05 PAUL PETZOLD, POLICE DEPT. 03/11/05 Description of Incident: Parking Enforcement Officer pulled into the parking lot located at 1615 Greenleaf in order to turn around to check on a parking violation. After turning around he exited the private drive and started to turn right. During this maneuver the lower passenger side of his vehicle struck a concrete sign base that was located in the roadway near the curb. Repair cost totaled $498.00. Supervisor's Comments & Recommendations: Be more observant, LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for May 12, 2005 Meeting Page 7 of 12 W-06-05 RONALD EVANS, Water & Sewer Maintenance 03/'17/05 Description of Incident: It is believed the employee's right wrist and forearm was injured while using wrenches to tighten bolts on a fire hydrant on the afternoon of the 17`h. Employee was at Well #13 located at 931 Mississippi. He does not recall a specific moment when the injury occurred, but he did notice soreness and a small bruise later that evening. When he awoke on the morning of the 18'h, the pain was significantly greater and the bruise had become quite large. Supervisor's Comments & Recommendations: As the mechanical joints of underground.fittings and appurtenances are made by hand in accordance with industry standards and endorsed by material manufacturers and the American Water Works Association, the use of pneumatic and/or hydraulic tools for this task is not advisable. Therefore, I have no recommended alternatives for completing this task. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 12, 2005 Meeting Page 8 of 12 F-09-05 MICHAEL PESTEL, FIRE DEPT. Verbally reported on 03/21/05 Description of Incident: Incident occurred on 08/05/04, but was verbally reported on 03/21/05. While treating and unloading a patient at ABMC emergency ramp FF Pestel experienced "intermittent pain". He described pain as a "cramping and burning pain in the middle of my chest". He discussed the feeling with his partners, who said he should be treated in the ER, ABMC determined not a cardiac episode, but symptom stemmed from acid reflux. Supervisor's Comments & Recommendations: A medical condition. LPC Comments & Recommendations: It was determined by Gallagher Bassett that this was a Workmen's Comp Case. W-07-05 ROBERT SPOERLEIN, Water & Sewer 03125105 Description of Incident: While removing the coring machine at Virginia Avenue, the bit bound up and employee strained muscles in his lower back and pain shot down right leg. Supervisor's Comments & Recommendations: Not really preventable, just something that happened while operating machinery. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 12, 2005 Meeting Page 9 of 12 P-13-05 STEPHEN SCHMIDT, POLICE CHIEF 03/25/05 Description of Incident: Squad#260 was parked unoccupied at the end of the driveway.at 1066 Gloria Drive. The. squad was struck by a vehicle, which was backing out of the garage. Due to the height of the striking vehicle and downward slope: of the driveway, the driver did not observe the parked squad, prior to the collision. LPC Comments & Recommendations: Committee does not agree with the comments made by the supervisor. The form was not properly executed, it should have been signed by S. Schmidt's supervisor. P-14-05 JOHNELL WILLIAMS, Police Dept. 03/25/05 Description of Incident: The officer was in the west parking lot of the Higgins Charhouse at 800 Higgins Road assisting another officer on a traffic crash. The officer began backing the vehicle in order to reposition his squad, at which time he struck a concrete light pole base. Waiting for repair estimate. Supervisor's Comments & Recommendations: Pay closer attention to surroundings. RICHARD GARCHIE, Fire Dept. 04/01/05 Description of Incident: The water tank fill hose that hangs from the ceiling apparently came loose and fell onto the top of Engine#112. While vehicle was driving out of the building the hose caught on the left side scene light. The scene light broke from the vehicle and the copper supply pipe broke at the ceiling. Waiting for repair estimates. Supervisor's Comments & Recommendations: Either provide a more secure way of securing the fill hose to the pipe, or remove the hose and pipe. The tank can be filled by the garden hose on the ground or by a hydrant. No police report was made by approval from D/C Miller. LPC Comments & Recommendations: Per D/C Forde this is still the most convenient way to fill the tank. The key factor is to pay more attention to removing the hose when the tank is filled. FS-01-05 JOHN MEYERS, Fleet Services 04/05/05 Description of Incident: A Milwaukee brand portable saw with case and blades and one Dremel brand hand grinder with case are missing from the 1635 Biesterfield Road Fleet Service garage. Total cost for tool replacement is $280.50. Supervisor's Comments & Recommendations: By locking tools in a room it would be reasonable to assume that they would be secure. There are a lot of tools at the garage and to lock them all up in cabinets would not be practical. The Fleet Service garage is accessible by almost all Village employees. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 12, 2005 Meeting Page 11 of 12 W-08-05 JOHN NICHOLAS, Public Works - Utilities 04105/05 Description of Incident: During the course of excavation at the southwest corner of Arkansas and Nerge, employee hand dug area looking for utilities. Utilities were not present, so he guided the backhoe operator to remove material to the level he hand excavated. While the operator was moving the bucket, it came in contact with the cables (SBC telephone and Comcast CATV) that were stretched to the point of breakage. The cables were not following the same elevation and routing in the area of damage and the backfill material was different. Supervisor's Comments & Recommendations: Accidents of this nature are extremely hard to avoid without hand excavating the entire repair, which is impractical. Employee used reasonable care and caution in exploring the excavation and using accepted methods. No visible indicators that the cable routing and elevation had changed significantly a few feet away. Hazard of excavations even with reasonable care taken. LPC Comments & Recommendations: Concur SGT. KATHERINE ENDRE, Police Dept. 04/08/05 Description of Incident: The prongs, which get inserted into the wall outlet are broken off on the electrical battery charger for the Kwik Start 650 Qump start unit). Repair cost is $12.00. Supervisor's Comments & Recommendations: The charger is plugged into the wall inside the gun closet. The outlet and charger are located in a vulnerable location. The less lethal gun boxes are stored in the same general area. Possibly store the Kwik Start and charger in another location. MARK WACLAWSKI, Police Dept. 04/16/05 Description of Incident: The employee was injured during a rapid deployment scenario. While conducting an exercise involving an uncooperative suspect, the employee pulled the role-player away from the door. During this movement he scraped his right forearm on the grip of another officer's weapon, causing an abrasion to his arm. Supervisor's Comments & Recommendations: The nature of realistic scenario based training may on a rare occasion result in accidents to those officers participating. The training conducted was performed in a safe manner and there are no actions recommended to prevent a similar occurrence. All safety factors were taken into consideration prior to conducting the training. LPC Comments & Recommendations: Concur F-11-05 NATHAN GAC, Fire Dept. 04/19/05 Description of Incident: While treating a patient in the rear of the ambulance, the employee bumped a glucometer that had blood on it causing it to splash in his left eye. He was examined at ABMC. Supervisor's Comments & Recommendations: Reminder to wear safety glasses and to be cautious may prevent this incident from reoccurring. LPC Comments & Recommendations: Concur
