Present: Jack Gray, Chairman Larry Hammar, Deputy Police Chief Al Vrshek, Deputy Fire Chief Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Excused: None The minutes were approved for the meeting of May 12, 2005 Review Current Reports: 24 Incidents Old Business: Tabled report: W-04-05 reported by Scott Nailor, Public Works originally did not have any supervisor comments. Description of Incident: At the corner of Charlela and Nerge a vehicle struck the ballard post, which protects the Rohlwing lift station. Post knocked over, but can be straightened. Repair cost $33.00 Supervisor's Comments & Recommendations: Extremely hard to avoid, when damage is the result of an item performing its intended. function. Additional police monitoring on a 24-hour basis would only provide a means to apprehend not to avoid. Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 3 of 14 Review of Current of Reports F-12-05 HANS GUSTAVSEN, FIREFIGHTER Approx. 08/04 Description of Incident: A Motorola XTS 3000 Portable radio 800 MHZ was dropped into the lake during a rescue drill in Busse Lake. The radio is unable to be repaired because of water damage. Replacement cost $3,470.00. Damage & repair diagnosis was not until 4 months after incident actually occurred. Supervisor's Comments & Recommendations: All portables to be secured in a holder either shoulder sling, belt or apparatus fixed, not just handheld. All information was obtained over the past year. LPC Comments & Recommendations: Concur F-18-05 PHILIP HAVERLAND, FIREFIGHTER 02/16/05 Description of Incident: At 676 Meacham, employee was beginning to conduct the Emergency Generator Test. As he depressed the test button, the battery to the generator exploded. The resulting noise from the explosion hurt employee's ears. Employee was transported to ABMC by Ambulance 10. Repair costs to the generator totaled $160.60. Supervisor's Comments & Recommendations: Insure that the batteries and charging systems are functioning properly and are of newer technology instead of being dated. These automotive type batteries should be replaced on a schedule instead of waiting for a failure. LPC Comments & Recommendations: Problem resolved. Batteries will be replaced routinely, instead of when damage occurs. Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 4 of 14 BS-03-05 DUANE WESTBERG, BLDG. SERVICES 04/22105 Description of Incident: The flood lights at 901 Wellington on the front lawn area were broken off from the base. Repair costs totaled $295.80. Supervisor's Comments & Recommendations: We have raised these lights up so they do not get hit from a lawnmower, by just going around the lights. But, these may have taken a direct hit LPC Comments & Recommendations: Concur F-13-05 JOHN LODEWYCK, FIREFIGHTER 04/24/05 Description of Incident: Ambulance 8 was returning from the hospital and was at Winston South and Bradford Circle, when the ambulance antenna got caught in a kite string, which was entangled in the trees on both sides of the road. The results to this entanglement, was the antenna breaking off of the ambulance. Repaired in-house at no cost to the Village. Supervisor's Comments & Recommendations: An almost invisible kite string was affixed to vegetation in the path of the ambulance. Due to the minute diameter of the fiber and wind conditions, it was not detected until impact with the moving ambulance was made. Limit the use of kites in Elk Grove Village. LPC Comments & Recommendations: No Comment Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 5 of 14 F-14-05 JOSEPH HOLLERBACH, FIREFIGHTER 04/27/05 Description of Incident: Broken clip on the E9 SPO2 finger probe. Cost to repair is $175.00. Supervisor's Comments & Recommendations: The broken finger probe was found during the morning rig check. Just the movement of the equipment bag in and out of the EMS compartment could have caused the damage. The location of where this unit is stored and carried may have to be changed. LPC Comments & Recommendations: Concur P-17-05 TIM KLEINFELDT, POLICE OFFICER 05/12/05 Description of Incident: Employee was attending a week-long police motorcycle training course.While conducting an emergency braking exercise the officer fell off the motorcycle. As a result the officer sustained a joint separation of the right shoulder— no fracture. Assoc. with P- 18 -05 Supervisor's Comments & Recommendations: Officer was attending training to improve his ability to safely operate a police motorcycle under dangerous and hazardous conditions. Falls and minor crashes are common for this type of training. Officers are taught what the limits are to their motorcycle. Teaching these limits will occasionally result in crashes, which I feel are for the most part unavoidable. In this incident the only recommendation I would have, would be for the motorcycle operator not to release rear brake until rear tire comes into line with front of motorcycle. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 6 of 14 P-18-05 TIMOTHY KLEINFELDT, POLICE OFFICER 05/12/05 Description of Incident: Motorcycle Unit #274 was damaged when involved in a motorcycle training exercise at the College of Lake County. Repair cost was $384.90, Assoc. with P-17- 05 Supervisor's Comments & Recommendations: To avoid being thrown from motorcycle in this type of situation the operator should not release rear brake until rear tire is in line with the motorcycle. LPC Comments & Recommendations: Concur FS-02-05 THOMAS MORONY, FLEET SERVICES 05/16/05 Description of Incident: While pulling a mobile truck hoist jack, employee felt something happen in his back. Pain developed soon after and he asked to go to the clinic to have his back checked. Medical expenses to date have totaled $4,000.00 Supervisor's Comments & Recommendations: Using proper lifting and pulling techniques while working in the shop are a must to help prevent back injuries. It does not appear that the employee did anything wrong to cause the injury. This could have been an aggravation from a previous injury that is on file. LPC Comments & Recommendations: Concur. Employee should ask for assistance, especially to prevent any reoccurrence of back injury. Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 7 of 14 P-19-05 MATTHEW FIORITO, POLICE OFFICER 05/23/05 Description of Incident: Officer was conducting a traffic stop at Brantwood & Rev. Morrison Blvd. involving seat belt enforcement. At the conclusion of the stop, the officer was dispatched to a burglary in progress. The officer activated his emergency lights and was in the backing up portion of a three- point turn when he struck a vehicle, which was "sneaking" behind his squad. The rear bumper of squad #246 was damaged. Repair cost $470.16 for the squad and $1,151.13 to repair other vehicle and a vehicle rental fee. Village total cost: $1,621.29. Supervisor's Comments & Recommendations: Officer needed to exercise more caution while backing up and be aware that civilians do not always react in the manner anticipated. LPC Comments & Recommendations: Driver Refresher Course F-15-05 GEORGE EILERS, FIREFIGHTER 05/27/05 Description of Incident: F/F stepped out of the ambulance at the: hospital and stepped into a hole, which caused him to severely twist his right ankle. Supervisor's Comments & Recommendations: F/F could not have seen the hole that was there on the ground as he stepped out of the ambulance. The area should have been a level surface and that would have prevented this type of injury. It is also important for all personnel to watch footing when stepping on grassy surfaces where holes may be hidden. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 8 of 14 P-20-05 REBECCA McGREAL, POLICE OFFICER 05/31/05 Description of Incident: While driving Squad #267 at 450 E. Higgins, the officer did not observe a concrete pole when a turn was attempted. The vehicle struck the pole on the driver's side of the vehicle causing damage to the front and rear driver's side doors. Repair cost $966.80. Supervisor's Comments & Recommendations: Driver needs to be more attentive when operating a vehicle in an unfamiliar area. LPC Comments & Recommendations: Driver Refresher Course P-21-05 JAMES CHMELIK, POLICE OFFICER 06/06/05 Description of Incident: Officer was responding to an ambulance call/suicidal subject in squad #245. While entering the intersection at s/b Nerge Road & Devon Avenue, on a red traffic signal, he struck a semi-tractor on the right saddle fuel tank. Squad #245 was "totaled" and given an estimated cash value of$10,500.00;damage to semi-tractor $13,000.00, Fire Dept. clean up cost $750.00. Total replacement & repair costs approximately $40,000.00 Supervisor's Comments & Recommendations: Officer failed to yield/stop before entering an intersection on a red light. Officer should stop before entering an intersection on a red traffic signal to ensure roadway is clear of vehicles. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 9 of 14 F-16-05 WILLIAM REID, FIREFIGHTER 06/07/05 Description of Incident: While making a left hand turn in Quint 8 during "fireground" mode it-struck a parkway tree, which damaged the rear corner passenger side of vehicle. Repair cost_to exceed $15,000.00. Supervisor's Comments & Recommendations: Narrow street, driver thought he had enough room to maneuver vehicle. Allow more room, watch mirrors and camera, utilize spotters when possible. LPC Comments & Recommendations: The driver should not have been in "fireground" mode at that time. Driver Refresher Course P-22-05 NICHOLAS LANGENDORF, POLICE OFFICER 06/10/05 Description of Incident: While officer was running in a physical training session at the police academy he twisted his left ankle. Injury occurred when officer stepped from sidewalk to uneven grass. Supervisor's Comments & Recommendations: Officer needs to be more aware of his surroundings. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 10 of 14 S-06-05 MATT PETTYJOHN, STREETS 06/13/05 Description of Incident: A van from the right lane on Devon cut in front of Village F-350 Ford truck#740. Rear taillight was damaged. Damage to Village truck $1,034.10. Supervisor's Comments & Recommendations: This was an unavoidable crash for Village truck. LPC Comments & Recommendations: Concur F-17-05 FRED HANSELMAN, FIREFIGHTER 06/13/05 Description of Incident: F/F turned off all apparatus floor lights to change the burned out bulbs. He lowered the fixture door to put in a new bulb. When he went back up to replace the bulb it immediately lit, which surprised him causing him to flinch, hitting his head on the door. this caused the door to fall to the floor, breaking the frame and the glass. Broken aluminum door and glass lens. There was no cost to repair. Supervisor's Comments & Recommendations: F/F used appropriate safety measures in that he shut down power and used the blue stairway. Additional education of employees stating that some lights remain on for safety purposes so they will be less surprised when the bulb lights up will reduce the chance of injury or damage. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 11 of 14 W-09-05 MATTHEW WELSCH, PUBLIC WORKS (seasonal) 06/14/05 Description of Incident: While dumping concrete at west garage, employee raised bed to dump, causing reargate to make contact with pintle hook, damaging top of gate. Top of rear tailgate and pintle hook assembly are damaged. Estimated cost to repair or replace: $1,000.00 Supervisor's Comments & Recommendations: When dumping material larger than gate will allow, driver must set chains on both sides of gate, so that the gate will lay flat with bed. LPC Comments & Recommendations: Concur P-23-05 FRANK GOTTARDO, POLICE OFFICER 06/27/05 Description of Incident: The officer tackled an offender who was fleeing from the police at J.F. Kennedy and Brantwood. Officer has pain in his right hand and suffered scrapes and abrasions to his left forearm, upper arm and elbow. No medical costs to date. Supervisor's Comments & Recommendations: The officer was performing a police function when attempting to take an offender into custody. The offender's actions forced the officer to physically attempt to restrain him via a tackle, which resulted in injury. This type of injury is typical when an officer is forced to physically handle resistive offenders. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 12 of 14 P-24-05 RICHARD GIGANTE, POLICE OFFICER 06/27/05 Description of Incident: While in pursuit-of an offender, who was fleeing the scene at J.F. Kennedy and Brantwood the officer suffered scrapes and abrasions to his right elbow as.a result of falling in the street during the chase. No medical costs to date. Supervisor's Comments & Recommendations: The fall was a result of running at full-speed during a highly dynamic situation. Non- preventable injury. Officer simply fell during a full-out sprint, which is part of his job requirement. LPC Comments & Recommendations: Concur P-25-05 ROBERT INDURANTE, POLICE OFFICER 06/27/05 Description of Incident: Squad #231 was in a parking lot when the other vehicle, which had a flat tire, backed into the front end of the squad. Repair costs currently unknown. Supervisor's Comments & Recommendations: It would be difficult to prevent these except by parking behind a barrier or keeping a greater distance from the other vehicle. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 13 of 14 VM01-05 GARY P-ARRIN, VILLAGE MANAGER 06/27/05 Description of Incident: The right rear panel was"keyed" above the tire area on Village vehicle #401. Unknown where and when the incident occurred. Supervisor's Comments & Recommendations: Unavoidable...car was either at home or public parking lot when vandalized. LPC Comments & Recommendations: Concur P-26-05 TIMOTHY KLEINFELDT, POLICE OFFICER 06/30/05 Description of Incident: The officer was northbound Tonne Road attempting to catch up to a traffic offender utilizing full emergency equipment. Officer was cut off by a vehicle that was crossing Tonne from westbound Touhy. Officer took evasive action missing two vehicles before losing control and dropping the motorcycle. Kleinfeldt was taken to ABMC for treatment. There will be significant medical expenses. Scratches, bruises and shattered wrist. Assoc. with P-27-05 Supervisor's Comments & Recommendations: Officer was operating the police motorcycle within the PD's policy/procedures with both emergency lightsand siren activated. He was wearing the appropriate and required safety equipment. The officer successfully completed a motorcycle safety course,which undoubtedly influenced his ability to successfully avoid impact with multiple vehicles at the scene. The officer could not have forseen or avoided without incident two vehicles pulling out in front of him from a side road. Officer Kleinfeldt had no control over the actions of the other drivers on the road. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 14, 2005 Meeting Page 14 of 14 P-27-05 TIMOTHY KLEINFELDT, POLICE OFFICER 06130105 Description of Incident: Scratches, tears and chips is the extent of damage to the motorcycle, officer's uniform and equipment, which occurred from a motorcycle accident. The officer was northbound Tonne Road attempting to catch up to a traffic offender utilizing full emergency - equipment. Officer was cut off by a vehicle that was crossing Tonne from westbound -- Touhy. Officer took evasive action missing two vehicles before losing control and . dropping the motorcycle. Estimated replacement cost $1,500. Assoc with P-26-05 Supervisor's Comments & Recommendations: Continued utilization of specialty training classes for police vehicles is recommended. Use of all required safety equipment will continue to be stressed. Also stressed will be the fact that officers operating motor vehicles in patrol duties need to be cognizant of their surroundings including attempting to anticipate unknown threats. LPC Comments & Recommendations: Concur P-28-05 EDWARD THOMAS, POLICE OFFICER 07/02/05 Description of Incident: Officer stopped a traffic offender in the right eastbound lane of Biesterfield Road west of David Lane. A vehicle struck the rear of his marked squad (Unit#232)with its emergency lights and arrowstick operating and visible. Probably no vehicle damage costs. Supervisor's Comments & Recommendations: Officers cannot always get a traffic offender to stop in a completely safe place. This traffic stop was on a four lane road with a slow speed limit (25 MPH), a long straight visible section of roadway, an open left lane to pass around squad and light traffic flow. This crash could have been prevented by the striking driver paying attention. LPC Comments & Recommendations: Concur
