Present: Jack Gray, Chairman Larry Hammar, Deputy Police Chief Dave Miller, Deputy Fire Chief Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Christine Tromp, Director of Finance Excused: Al Vrshek, Deputy Fire Chief The minutes were approved for the meeting of July 14, 2005 Review Current Reports: 15 Incidents Tabled Reports: None Old Business: Since there have been quite a few questions regarding HIPAA, the Loss Prevention Committee consulted with the Village Attorney, Finance Director and Human Resources for their input. From discussions with the above individuals the committee has made the following revisions: ► Revising the IOD forms for the employee's privacy requirements. The following fields will be removed: SSN, home address, home phone, birth date, minor dependent information, marital status, and personal medical information. ► All employee's names will be removed from the LPC minutes in the header, as well as in the narrative. Each incident which is being discussed will be known to the committee as well as the department heads as a log number only. ► Minimize the amount of people viewing IOD reports, which will ensure a more secured procedure for each individual's privacy. Loss Prevention Committee Minutes for September 8, 2005 Meeting Page 3 of 10 Review of Current of Reports S-07-05 A-05-29 = PUBLIC WORKS 07/01105 Description of Incident: Seasonal employee was cleaning the vehicle in the wash rack at 666 Landmeier Road when he backed out of the garage door with the truck door open. Total damages for this incident $2,640.51 (door - $1,035 and truck - $1,544.31) Supervisor's Comments & Recommendations: When questioned about the accident, the seasonal employee laughed. The employee was sent home and told that I would get back to him. The employee was terminated from the Village on 0705/05. LPC Comments & Recommendations: SERIOUS CONSIDERATION TO RESTRICT SEASONAL EMPLOYEES FROM OPERATING CERTAIN VEHICLES. F-19-05 E-05-17 = FIRE DEPARTMENT 07/12/05 Description of Incident: After filling up engine #7 at 666 Landmeier Road, the fuel keys were left at the pump. Later the employee went back to look for the keys and found that they were missing. Supervisor's Comments & Recommendations: Engineer forgot to put fuel keys back into engine. The employee needs to make sure that he removed key from the pump before leaving the fuel pumps. LPC Comments & Recommendations: r The keys were found and returned per D/C Miller. Employees must be more careful not to misplace keys. Loss Prevention Committee Minutes for September 8, 2005 Meeting Page 4 of 10 W-12-05 E-05-20 = WATER & SEWER 07/13/05 Description of Incident: While employee was working on gas chlorination equipment,chlorine gas was released in the room. Employee immediately exited. Chlorine gas is extremely corrosive in this state and ate away at the furnace components until the Fire Dept. was able to shut off cylinder and ventilate the room properly. Furnace was in fair to poor condition. Total expenses $4,258.11 (furnace replacement- $348.43, Fire Dept. hazmat clean up costs-$3,758.11 and ABMC ER costs for firefighter needing treatment - $500.00). Supervisor's Comments & Recommendations: Employee's actions allowed chlorine gas to escape. Employee used poor judgment. Standard operating procedures have been revised to prohibit this type of action. Employee did correctly follow procedures related to a chlorine leak that provided for his safety and the safety of others working in the facility. LPC Comments & Recommendations: Procedures have been revised since this incident. Committee concurs. F-20-05 5/33 = FIRE DEPARTMENT 07/13/05 Description of Incident: Firefighter was a member of an entry team in Tyvek suits and SCBA. He was the first to enter the chlorine room. He felt a burning sensation at the front of his neck. Treated at ABMC for skin irritation due to chlorine exposure (Cost $500.00). Supervisor's Comments & Recommendations: Hazmat Techs should investigate utilizing the proper resources to determine what would be the proper clothing (PPE)for the entry, back-up, and decon teams. The hazmat techs should then meet with the incident command to explain the approach recommended by the hazmat team. LPC Comments & Recommendations: Hazmat team selected wrong suit for situation. Committee concurs. Loss Prevention Committee Minutes for September 8, 2005 Meeting Page 5 of 10 F-21-05 E-05-18 = FIRE DEPARTMENT 07/19/05 Description of Incident: The power extension cord ejected from the fire engine as designed when the rig was started. The cord landed up under the front driver side wheel and was run over when the rig exited the building. The driver was watching his mirrors when exiting but never saw the cord land under the wheel. Approximate cost to repair/replace is $150.00. Supervisor's Comments & Recommendations: It's that rare occurrence where the extension cord hits the ground and bounces underthe driver side front wheel. My recommendation would be to make sure the cord is not too long giving it the opportunity to land up in the wrong place. LPC Comments & Recommendations: Concur S-08-05 A-05-30 = PUBLIC WORKS 07/25105 Description of Incident: The driver of the vehicle backed out of his driveway on 827 Spring Creek Court and struck the Village trailer(unit # 759). Streets superintendent stated there is no damage to the Village trailer, but the driver's car sustained damage. No damage costs per public works. Supervisor's Comments & Recommendations: No comment by the supervisor LPC Comments & Recommendations: No comment Loss Prevention Committee Minutes for September 8, 2005 Meeting Page 6 of 10 LF-02-05 5/30 = PUBLIC WORKS 07/28/05 Description of Incident: Employee was cutting trees at the Martin Street ditch line and got poison ivy all over his body, which does not show up for 4-5 days. Medical costs to date are $350.00 Supervisor's Comments & Recommendations: Wait until winter to clear ditches so the poison ivy will be dormant. LPC Comments & Recommendations: Employees should be better trained in observing poisonous plants. LF-01-05 5/32 = PUBLIC WORKS 07/28/05 Description of Incident: Employee was cutting trees at Martin Street ditch line and unknowingly had a run in with poison ivy. Medical costs to date about $350.00 Supervisor's Comments & Recommendations: Wait until winter to do these projects when poison ivy plants are dormant. LPC Comments & Recommendations: Employees should be better trained in observing poisonous plants. Loss Prevention Committee Minutes for September 8, 2005 Meeting Page 7 of 10 P-29-05 E-05-19 = POLICE DEPT. 08/02/05 Description of Incident: Police Department bicycle was discovered damaged (left hand brake is hanging off the handle bars) while conducting an inspection in the garage. Per police dept. the bicycle will not be repaired. Supervisor's Comments & Recommendations: Proper care should be maintained cat all times when utilizing police department equipment. If damage was caused accidentally, it should be reported ASAP. Sign off sheet might be useful to determine which officer was the last to use a bicycle. LPC Comments & Recommendations: Concur BS-04-05 5/30 = PUBLIC WORKS (seasonal employee) 08/04/05 Description of Incident: While sanding a piece of copper pipe, the employee received a puncture wound to his right thumb. Medical costs to date about $500.00. Supervisor's Comments & Recommendations: The employee should have been wearing protective gloves while working with fresh cut pipe. Always wear protective gloves when handling any type of metal. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 8, 2005 Meeting Page 8 of 10 W-10-05 A-05-31 = WATER & SEWER 08/05/05 Description of Incident: While parking unit#828 at 666 Landmeier, the employee backed into the bumper of unit #814 hitting the license plate and bumper with the stabilizer of#828. No damage costs for vehicles. Supervisor's Comments & Recommendations: If employee has trouble backing up in the parking space, have someone guide him. Proper parking of other vehicles along that wall of the garage. LPC Comments & Recommendations: Enforce policy to use a spotter.....Driver Refresher Course W-11.05 A-05-32 = WATER & SEWER 08/09/05 Description of Incident: While backing out of a parking stall at 1199 Biesterfield, after checking mirrors, unit#811 struck a passing vehicle. The backup alarm and lights were operating. No damage to EGV vehicle. Damage on other vehicle between $1,500.00 and $2,500.00. Supervisor's Comments & Recommendations: Improper backing out of parking space. Take more caution when backing up. LPC Comments & Recommendations: Enforce policy to use a spotter .....Driver Refresher Course Loss Prevention Committee Minutes for September 8, 2005 Meeting Page 10 of 10 P-i E-05-21 = POLICE DEPT. 08/28/05 Description of Incident: Employee reports having previous problems with the design of the belt carrier for the cell phone. At an unknown location, at an unknown time over the weekend, the phone became disconnected from the belt carrier and was lost. Estimated cost $100.00 Supervisor's Comments & Recommendations: Since this type of belt clip design has proven to be risky in terms of the phone becoming disconnected from the clip at inopportune times, we should refrain from using this type of belt clip in the future. We will explore alternatives in providing a more secure belt- mounted carrier system for this model cell phone. No neglect or irresponsibility is noted. LPC Comments & Recommendations: Concur
