Parrin, Village Manager All Department Heads FROM: Loss Prevention Committee SUBJECT: Loss Prevention Committee Meeting Minutes — November 10, 2005 The meeting was called to order at 8:40 A.M. at the Village Hall. Present: Jack Gray, Chairman Leslie Sass, Asst. to Chief of Police Al Vrshek, Fire Deputy Chief Diane Hunter, Finance Department Secretary Excused: Larry Hammar, Police Deputy Chief Pam Kuc, Housing Inspector The minutes were approved for the meeting of September 8, 2005 Review Current Reports: 8 Incidents Tabled Reports: None Old Business: With regards to the discussion, which occurred at our last meeting regarding HIPAA, the Loss Prevention Committee has made changes to ensure the privacy for each employee. The new revised IOD forms are in place. The employee's SSN, home address, home phone, birth date, minor dependent information, marital status, signature and personal medical information have been deleted. The following system has been devised to take the place of an employee's name, which should resolve any privacy issues. On the IOD form and the Equipment Damage/Lost/Vehicle Accident Report form the employee's name will now be replaced with a log number, which Diane Hunter assigns. This number will also replace the employee's name in the heading portion of the meeting minutes sent to all department heads. rhe committee's log number (located on the left side of each heading), which Jack Gray has always assigned for identification purposes and also used as a tracking function to show how many incidents occur per department in a year, will remain as part of the Loss Prevention Meeting Minutes. Loss Prevention Committee Minutes for November 10, 2005 Meeting Page 3 of 6 Review of Current of Reports P-32-05 A-05-33, POLICE DEPARTMENT 09/22/05 Associate this report with P-33-05 Description of Incident: Employee lost control of the vehicle while traveling on wet pavement. Vehicle left the roadway and struck the ComEd high-tension wire metal support. Value of "totaled" squad $12,175. Approximate cost to purchase new squad $25,000. ComEd high-tension wire cost to repair is between $30,000 & $60,000. Supervisor's Comments & Recommendations: Employee needs to use caution when operating a vehicle on wet pavement. LPC Comments & Recommendations: Driver Refresher Course P-33-05 05/36, POLICE DEPARTMENT 09/22/05 Associate this report with P-32-05 Description of Incident: Officer involved in a single vehicle accident while responding to a call. Vehicle struck a ComEd high-tension wire support. Strain to left shoulder, neck and cervical spine. Medical costs $5,000 to date (clinic, specialist, MRI, misc. tests and physical therapy) Supervisor's Comments & Recommendations: Officer needs to be aware of potential hazardous road conditions during inclimate weather. LPC Comments & Recommendations: No additional comments...Employee is taking Driver Refresher Course (See P-32-05) Loss Prevention Committee Minutes for November 10, 2005 Meeting Page 4 of 6 F-22-05 A-05-34, FIRE DEPARTMENT 10/05/05 Description of Incident: Tower ladder (unit #137) was responding to an activated fire alarm heading eastbound Elk Grove Blvd at Tonne Road. When approaching the intersection there were approximately 4 cars in the left lane. The truck entered the intersection from the right lane and started a right turn, but the Allsteer was in fireground and the left rear corner kicked out to the left hitting the first car in line on the right side. Unit was repaired in house for less than $100. Other vehicle will cost about $3,500 to repair. Supervisor's Comments & Recommendations: If you don't have room for the vehicle to swing out in Ailsteer either go straight through the intersection and go a different route or wait until traffic clears the intersection so the vehicle has room to make the turn. LPC Comments & Recommendations: Driver Refresher Course P-34-05 A-05-35, POLICE DEPARTMENT 10/10/05 Description of Incident: The squad (unit#233)was traveling westbound in the right lane of Devon Avenue when a tractor trailer made a wide right turn without a turn signal into a driveway running northbound off of Devon in the path of the squad, which was traveling westbound in the right lane of Devon. Repair cost for squad is $5,681.73 Supervisor's Comments & Recommendations: This was unavoidable by the driver of the squad car, but the tractor-trailer driver needs to be more alert to his blind spots. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for November 10, 2005 Meeting Page 5 of 6 F-23-05 A-05-36, FIRE DEPARTMENT 10119105 Description of Incident: Ambulance (A9) was responding to a call at 1800 Nicholas. The firefighter opened the passenger side door and the wind blew the door wide open, at which time the employee closed the door. After the 911 call, the employee opened the passenger side door, but it would only open slightly. Employee noticed the door was touching the right front fender area. Repair estimate has not yet been received as of 11/03/05. Supervisor's Comments & Recommendations: Be aware of wind conditions. When opening, control the door appropriately. LPC Comments & Recommendations: Concur W-13-05 05-37, WATER & SEWER 10/20/05 Description of Incident: As the employee and a partner were unloading a 3" pump from vehicle #812, he felt a sharp pain in his mid to lower back. Employee sustained a mid to lower back strain/sprain. Medical costs about $500 Supervisor's Comments & Recommendations: This task is performed routinely using the best possible methods and practices. No recommendation in this situation. LPC Comments & Recommendations: Committee questions "Were proper lifting techniques used?" Loss Prevention Committee Minutes for November 10, 2005 Meeting Page 6 of 6 F-24-05 05-38, FIRE DEPARTMENT 10/24/05 Description of Incident: Employee entered main entrance of Fire Admin Building and tripped on carpet runner, which apparently was folded over. She fell and injured her left hip and right ankle. She was able to stand and walk to her desk with assistance. Employee refused "formal medical attention". Supervisor's Comments & Recommendations: When entering into common areas, be aware of your environment. Check to see if the carpet runner is creating a hazard. If a problem exists, avoid the area, take the time to correct the hazard or notify a supervisor of the problem. LPC Comments & Recommendations: Concur LF-03-05 A-05-37, PUBLIC WORKS 10/31/05 Description of Incident: Driver was backing into leaf dumping barn at Groot Industries. It was dark and there was foreign matter in the pile. The spreader box on the rear of the truck was bent and will need to be straightened. Repair estimate not received as of 11/03/05. Supervisor's Comments & Recommendations: It was very dark in this facility and there seemed to be a foreign object in the pile and employee hit this object. This was really in my opinion unavoidable LPC Comments & Recommendations: Concur
