Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 3 of 12 Review of Current of Reports P-35-05 05/39 - POLICE DEPARTMENT 11/04/05 Description of Incident: Officer was taking part in a "ground fighting"training exercise. Employee executed a hand block and jammed his finger into the other student's collarbone. His pinkie finger on his right hand is dislocated at mid-knuckle. To date $2,500 in medical bills (on-going). Supervisor's Comments & Recommendations: Employee was involved in a physical training exercise. The general nature of this subject matter has the potential to result in an injury. In this particular case the injury could most likely have been avoided if employee: performed the training movement in a more controlled fashion. LPC Comments & Recommendations: Concur P-36-05 E-05-22 - POLICE DEPARTMENT 11/04/05 Description of Incident: Officer lost key ring, containing EGVPD master key and squad car key,while responding to a domestic violence call for service at 1250 Hemlock. $5.00 in replacement cost. Supervisor's Comments & Recommendations: The officer responded to a domestic violence call. He and other responding officers could not locate the complainant and were checking the front and rear yards of the address. At some point the officer's key ring fell from his duty belt. He and other officers searched for the item, but were unsuccessful. This is an isolated incident. LPC Comments & Recommendations: The committee feels the supervisor's comments are improper. It will be explained to the supervisor the committee's expectations regarding the proper wording to be used for the comments and recommendations section of the reports. Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 4 of 12 F-25-05 A-05-39 - FIRE DEPARTMENT 11/04/05 Description of Incident: The firefighter had just completed backing Tower 7 at the Station 9 parking lot using the "fire ground" mode of All Steer. As the firefighter was going forward the left rear bumper struck a car in the parking lot due to the straightening of the wheels from the fire ground mode. $3,000 in damage to other vehicle and $0 to Tower 7. Supervisor's Comments & Recommendations: Be more aware of clearance and turning radius of vehicle when in fire ground mode. LPC Comments & Recommendations: Driver Refresher Course F-26-05 E-05-23 - FIRE DEPARTMENT 11/06/05 Description of Incident: A7 responded to a MVA and was providing care for an ALS female patient. While cutting and removing clothing from the arms and shoulders one of the V-leads for the 12-Lead Monitor was cut. $157.00 in Damage costs for repairs. Supervisor's Comments & Recommendations: When removing clothing from a trauma patient, every effort should be made to visualize what will be cut with the trauma scissors. if possible, cut clothing prior to applying the cardiac monitor leads. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 5 of 12 F-27-05 1-05-38 - FIRE DEPARTMENT 11/06/05 Description of Incident: Village vehicle (unit 150) was parked in front of 236 Parkchester Road. Person backed into the parked Village vehicle causing minor scrape to left fender in front of the wheel well. $0 damage to the vehicle. Supervisor's Comments & Recommendations: Person was not paying attention while backing out of their driveway. Keep Village vehicle on the driveway as much as possible. LPC Comments & Recommendations: Concur S-09-05 05/42 - STREETS 11/15/I05 Description of Incident: The gate release on truck #712 (dump truck) did not operate correctly. The employee lowered the bed and pulled up on the handle and midway through the handle sprung back pinching his left hand between the pull handle and the bed. It is believed the pressure of the load against the gate prevented the gate to release. After the load finally dumped, the gate functioned properly. Approx. $600 in medical bills to date. Supervisor's Comments & Recommendations: Just one of those things that happens. It was an accident. LPC Comments & Recommendations: Committee does not agree with the supervisor's comments. We feel the employee should be more aware how the load is positioned and take that into consideration when opening the gate. Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 6 of 12 SR-05-05 05140 - WATER AND SEWER 11/16/05 Description of Incident: Backhoe operator was excavating the site at Brookhaven & Cypress and placed a tooth from one edge of the bucket on the edge of the sidewalk. Employee was standing on the square of sidewalk monitoring the proximity of the bucket to the zone of tolerance around a NICOR gas main. Employee failed to observe the other side of the bucket had a tooth on the edge of the sidewalk. When the operator applied downward force with the bucket, the ground under the sidewalk gave way, tilting the sidewalk upwards, which launched the employee into the air. Employee landed on his back, hand and elbow. Employee complained of discomfort at the site, but elected to wait until the following morning to see how he felt prior to seeking medical care. To date $2,500 in medical bills. Supervisor's Comments & Recommendations: During excavations and anytime when working around operating equipment, employees should be cognizant of their proximity to the equipment and the surfaces the equipment is impacting. Likewise operators of equipment need to exercise extreme care when working around employees to ensure their actions will not harm others. Employee that was guiding the backhoe operator (this was not the injured employee) needs to take into account the bucket placement and the anticipated actions when the bucket moves. Greater awareness on the part of all employees working in and around equipment and excavation sites. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 7 of 12 P-37-05 E-05-24 - POLICE DEPARTMENT 11/26/05 Description of Incident: Squad (unit 241) was traveling southbound on Perrie Drive towards Landmeier Road. Officer advised he became distracted and took his eyes off the road. While doing so the squad struck the divided highway street sign/pole located at the beginning of the raised median. $400 for pole/sign repair and $500 to repair squad. Supervisor's Comments & Recommendations: Driver error. Driver was not paying attention. Officer needs to have complete awareness when operating a vehicle. He needs to be familiar with the surroundings and have complete control of his vehicle. As a suggestion the two ends of the raised median would be more visible if they were painted yellow. LPC Comments & Recommendations: Driver Refresher Course BS-05-05 A-05-40 - STREETS 11/29/05 Description of Incident: Sandbags (used for the tree lighting ceremony) were left on the dock. The sandbags needed to be put back onto the pallets where they are stored. While picking up the 50-60 pound bags, the employee hurt his back. To date $1,500 in medical bills, still on-going. Supervisor's Comments & Recommendations: It has always been my recommendation to all my employee's to use proper lifting techniques while lifting heavy objects and/or ask for assistance. This should help prevent future injuries. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 8 of 12 P-38-05 A-05-40 - POLICE DEPARTMENT 12/01/05 Description of Incident: Officer had just finished assisting another officer on a traffic stop. He pulled into the parking lot at 1501 Tonne to make a u-turn, exit the private lot and return to patrol duties. The front of the officer's squad (unit 267) struck a curb causing minor damage to bottom of front bumper. The curb was 14 inches tall. Approx. cost to repair squad is$1,134.26. Supervisor's Comments & Recommendations: Officer was advised to exercise caution when operating a squad. Officer further advised to take any necessary steps to minimize the possibility of the situation occurring again. LPC Comments & Recommendations: Driver Refresher Course P-39-05 A-05-41 - POLICE DEPARTMENT 12/04/05 Description of Incident: Officer in squad 245 slowed down at westbound Nerge and Meacham with lights and siren activated waiting for the traffic to stop. He then proceeded through the intersection on red light when a southbound vehicle failed to see or hear the squad and proceeded through intersection. Repair cost for squad is approximately $3,358.58. Supervisor's Comments & Recommendations: Check and double check the intersection before proceeding as many motorists ignore the police lights and siren. LPC Comments & Recommendations: The committee has a second recommendation: The officer out ran the Opticom before it had a chance to register. When driving a "flat top" style squad, extra precautions should be taken when entering an intersection. Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 9 of 12 P-40-05 A-05-42 - POLICE DEPARTMENT 12/05/05 Description of Incident: Squad 230 was parked in the roadway directly behind a disabled vehicle on westbound Landmeier Road approximately 70 feet east of Arlington Heights Road. All emergency lights were activated on the squad. A motorist driving westbound hit the passenger side mirror on the squad with the driver's side mirror of her vehicle. No damage to the squad. Supervisor's Comments & Recommendations: All precautionary measures were taken by the officer. Within reason, the accident was unavoidable. No recommendation. LPC Comments & Recommendations: Concur P-41-05 E-05-25 - (reported by) POLICE DEPARTMENT 12/05/05 Description of Incident: The exterior rear glass door to police/fire employee entrance had a four-way crack in it. Grayish marks were visible from the interior side, which could mean the door was struck from the inside. It was during shift change and several officers were exiting the door with different types of cases. The building maintenance person was also using the door at that time to roll out large garbage cans. Repair cost for door was $200. Supervisor's Comments & Recommendations: Replace the lower portion with a material other than glass and caution employees to be more careful. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 10 of 12 GO-01-05 05/43 - FINANCE DEPARTMENT 12/06/05 Description of Incident: Employee bumped left knee on side of desk when getting up to wait on a customer at the front counter. Seems to be getting more painful each day. To date no medical bills. Supervisor's Comments & Recommendations: No recommendations or prevention. No unsafe condition existed. Accidentally bumped knee on desk when getting up from chair. LPC Comments & Recommendations: Committee disagrees with supervisor's comments: Committee recommends the employee should be aware/careful of their workspace area and surroundings. P-42-05 A-05-44 - (reported by) POLICE DEPARTMENT 12/08/05 Description of Incident: Officer reported he found damage on squad 239. The center of the front bumper,where the license plate bracket is, came in contact with an unknown hard object causing the bumperto crack. Over the last several days the temperatures have been below freezing, which could have contributed to the bumper cracking on impact. $499 damage to squad. Supervisor's Comments & Recommendations: Not knowing if this damage occurred while in actual operation or if struck while parked, no recommendations can be made at this time. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 11 of 12 �BS-06-05 A-05-46 - PUBLIC WORKS 12/08/05 Description of Incident: Driver#1 was backing out of a spot in the parking lot at 901 Wellington and struck Driver #2 (employee) in a Village plow truck. Driver#2 was backing up to complete his plowjob in the parking lot of the municipal buildiing. Unknown damage amount at this time. Supervisor's Comments & Recommendations: Each driver must pay close attention when backing out of the parking spot and backing up a plow truck to avoid accidents such as this in the future. LPC Comments & Recommendations: Concur F-28-05 A-05-43 - FIRE DEPARTMENT 12/09/05 Description of Incident: Quint 8 was pulling into the station at 1000 Oakton through the back overhead door. Quint 8 had axle locked, but the axle was not straight when it was locked out. The rear end of the Quint moved to the right hitting the building with the rear left side handrail. The handrail was pulled loose from bracket. Damage to Quint 8 is $107. Supervisor's Comments & Recommendations: It is very important that drivers of all-steer vehicles perform proper procedures when entering the fire station. It is most important that the rear wheels are straight before locking out and turning the power key off for the all-steer. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for January 12, 2006 Meeting Page 12 of 12 P-42-05 E-05-26 - POLICE DEPARTMENT 12/13/05 Description of Incident: Sometime between 12/10/05 and 12/13/05 the officer misplaced his tape recorder. Value of lost recorder was $25.00 to replace. Supervisor's Comments & Recommendations: Officer was advised to be aware of his department issued equipment and to maintain control over said equipment at all times. LPC Comments & Recommendations: Concur P-43-05 A-05-45 - POLICE DEPARTMENT 12/17/05 Description of Incident: Officer was leaving the area of a house fire at 280 Edgeware. As the officer departed,fire equipment was parked on the north side of the street. A pick up truck with a snowplow blade was parked on the south side of the street. As the officer attempted to maneuver around the vehicles, the right rear and front passenger door of squad 243 grazed the edge of the plow blade. Repair costs for the squad is $500. Supervisor's Comments & Recommendations: Due to the emergency lights hindering the officer's night vision and poor lighting on the roadway, extra caution must be used to be aware of the vehicle's surroundings.
