The meeting was called to order at 8:32 A.M. at the Village Hall.

New Business:
■ The March 29, 2006 Public Works "Safety Days' will be held. A new format will be
tried. A hands on/exhibit hall format will be used instead of a lecture format.
■ "Sexual Harassment' class will be held April 12''for Municipal Complex employees
■ "Sexual Harassment' class will be held April 19'" for Public Works employees
■ "Defensive Driving Course and Sexual Harassment' will be held for the Fire

Review of Current of Reports
LF-03-05 A-05-47 = PUBLIC WORKS 12/16/05
Description of Incident:
The Public Works Chevy pickup is listed as the 3rd vehicle involved in this accident. 1"
vehicle was traveling southbound on Meacham turning left on a yellow light and was
struck by the 2nd vehicle headed northbound on Meacham in the curb lane. This caused
the 1 s'vehicle to spin out and strike the 3'vehicle (PW Truck). The front plow on the PW
truck was hit. The PW truck was in the left turn lane on westbound Biesterfield.
No damage to Unit #709, therefore no cost to Village.

SR-01-06 E-06-01 = PUBLIC WORKS (Utilities) 01/03/06
Description of Incident:
During excavation of area at 547 Sussex Court to repair a leaking water service, an
unidentified electric line was damaged. Repair cost not yet received from public works.

Increase consistency when reviewing maps and drawings prior to excavations.

SR-02-06 E-06-02 = PUBLIC WORKS (Utilities) 01/04/06
Description of Incident:
During excavation of the area of 30 Smethwick Lane to repair a leaking water main, the
operator was removing the concrete sidewalk when the backhoe bucket struck a gas
service line. No invoice received from NICOR to date.
Supervisor's Comments & Recommendations:
The operator needs to carefully monitor the depth of each pass made with the backhoe
bucket, especially when the exact location of underground facilities are uncertain.

SR-03-06 A-06-01 = PUBLIC WORKS (Utilities) 01/05/06
Description of Incident:
As the Village employee was backing up Unit # 816 to the excavation area at 275
Thorndale Avenue, the roadway collapsed and the rear end of the vehicle fell into the
hole. Muffler and brake line dryer were damaged. Approximate repair cost is $250.
Supervisor's Comments & Recommendations:
Drivers should continue to use extreme caution when operating a vehicle near an
excavation and be sure to focus on the guiding personnel with the best vantage point
when any backing operation is required.

LF-01-06 A-06-02 = PUBLIC WORKS (Forestry) 01/09/06
Description of Incident:
Employee was cutting down a tree in the parkway at 945 Hartford and a branch came
down to the ground and the butt end hopped up and put a small dent in the fiberglass
side box, rear passenger side of Unit #705. Repair cost is $280.
Supervisor's Comments & Recommendations:
Be a little more careful when dropping the branches in such a tight area. It is hard to
judge if the limb will bounce or twist. R. Edwards comment: If limb can do damage, rope
it off and lower by hand thus preventing any potential damage.

Committee concurs with R. Edward's comment.
P-01-06 E-06-04 = POLICE DEPARTMENT 01/18/06
Description of Incident:
Officer was issued a new commission card. He retrieved and replaced his old commission
card from his off duty wallet, which was located in his vehicle that was parked in the
police department parking lot. On 01/22/06 the officer noticed that his off duty wallet with
badge #146 and commission card were no longer in his vehicle. Replacement cost
approximately $89.
Supervisor's Comments & Recommendations:
Although the wallet previously had been stored in the door's deep side pocket without
incident in the past, I would recommend that this practice be stopped.

P-02-06 06/02 = POLICE DEPARTMENT 01/22/06
Description of Incident:
Officer advised he was investigating a domestic battery at 950 Jefferson Square. While
exiting his police vehicle he slipped and fell on the pavement. The pavement was
covered with a thin layer of ice. Upon losing his balance the officer's right shoulder and
hip struck the pavement. Officer related he had some soreness. He indicated that he did
not need medical attention and anticipated he would be fine. $0 medical cost to date.
Supervisor's Comments & Recommendations:
Officer unfortunately slipped on the icy pavement. Officer was reminded to be aware of
his environment.

P-03-06 06/02 = POLICE DEPARTMENT 01/31/06
Description of Incident:
While the officer was using a fire extinguisher on a car fire at Meacham and Nerge
Roads, some molten plastic flew back and burned his right index finger. $0 medical costs.
Supervisor's Comments & Recommendations:
Officer was extinguishing a car fire in traffic with the department issued fire extinguisher
when some molten plastic from the car flew back and burned his right index finger and
melted the left jacket sleeve. Using the fire extinguisher from a different angle may help
in the future.

P-04-06 E-06-03 = POLICE DEPARTMENT 01/31/06
Description of Incident:
While using the fire extinguisher on a car fire some molten plastic "blew back" from the
force of the CO2 causing his department issued police spring jacket sleeve to melt and
also caused a burn to the officer's right index finger. Estimated replacement cost $140.
Supervisor's Comments & Recommendations:
Try a different angle of approach when using the fire extinguisher.

F-01-06 E-06-05 = FIRE DEPARTMENT 02/02/06
Description of Incident:
While conducting the monthly ambulance inventory check it was noticed that the inflation
hoses were missing. $0 replacement cost. Hose company provided a free replacement.

P-05-06 A-06-03 = POLICE DEPARTMENT 02/02/06
Description of Incident:
Officer was executing a U-turn from westbound Higgins Road to eastbound Higgins Road
when the right front tire came into contact with the curb causing a small tear in the
sidewall of the tire. Repair estimate not yet received.
Supervisor's Comments & Recommendations:
If roadway is of insufficient width to accommodate a U-turn, a three-point turn should be
completed.

F-02-06 06/03 = FIRE DEPARTMENT 02/05/06
Description of Incident:
While providing Advanced Life Support care for the patient in the rear of the ambulance
while enroute to ABMC, paramedics were performing a blood glucose test. A drop of
blood flicked off the end of the Gluco stick and hit employee in the face, possibly his left
eye. Immediately blood was removed with a towel and area was cleansed with alcohol.
Upon arriving at ABMC employee was checked in as a patient, seen by ER doctor, had
base line blood draws and his left eye was irrigated. He was released back to service
and instructed to follow up at the ABMC Clinic on 02/08/06. Estimated cost $1,500 for
medical treatment & he will require periodic blood tests due to blood exposure.
Supervisor's Comments & Recommendations:
This employee, as well as all Firefighters involved, was instructed to wear safety eye
protection in emergency medical calls as soon as the need is identified.

P-06-06 06104 = POLICE DEPARTMENT 02/10106
Description of Incident:
While on a call at 1162 Westminster, the officer was confronted and attacked by a
mentally unstable subject. When the two fell to the ground the officer's left hand was
jammed on the sidewalk. ER later diagnosed him with a left wrist sprain. Total medical
treatment cost $730.
Supervisor's Comments & Recommendations:
Officer was standing by waiting for back-up when the subject bolted out the front door and
confronted the officer, who followed all department procedures. The subject was
unstable and the situation was not preventable.

F-03-06 E-06-06 = FIRE DEPARTMENT 02/11/06
Description of Incident:
While connecting the shoreline to ambulance 7, employee received a small shock from
the cord. The shock caused his hand to jump, bending the plug hatch and causing the
flap section to break off. Shoreline was repaired immediately. Part repair cost is
approximately $20.
Supervisor's Comments & Recommendations:
Due to snow and rain the exterior of the ambulance was wet. Employee was counseled
on electrical safety if hands, vehicle or plug is wet. This may or may not have caused the
shock. The wires being frayed could have caused the shock.

S-01-06 E-06-07 = PUBLIC WORKS 02/22/06
Description of Incident:
Incident occurred at the drainage ditch running along the railroad tracks north of Devon
Avenue in the 2400 block. Operator obtained grade on both sides of the service to
determine depth. When he tried to penetrate the ice on the surface the bucket made
contact with the electric service. Repair cost will probably be over $2,500.
Supervisor's Comments & Recommendations:
J.U.L.I.E. will not remedy this situation. It's a question of depth, since we can see that a
service is running down the pole. From now on we will stay five feet away from either
side of a power pole. J.U.L.I.E. shall be called for ditch cleaning activities and caution
shall be exercised by probing or hand digging around utilities to determine depth.
