The meeting was called to order at 8:35 A.M. at the Village Hall.

F-04-06 KAREN BENSON = FIRL DEPARTMENT 12/13/05
Description of Incident:
Upon return to station, firefighter was hooking up plymovent and noticed damage(3 small
dents/cuts in aluminum) to the roll up doors on the officer's side of the vehicle.
Repair costs not yet received from Fleet.
Supervisor's Comments & Recommendations:
I am positive that Tower 7 did not strike any type of object (2"' shift) and cause this
damage. This damage was not noticed during morning check, but they are small
dents/cuts and it could have been easily overlooked. It is unknown how this damage
occurred and it is also unknown how to prevent similar occurrences in the future.

P-07-06 MARK WACLAWSKI — POLICE DEPARTMENT 12/31!05
Description of Incident:
On the annual 2006 inspection of the building by Cmdr Zinnel, damaged bench cushions
were located in the lobby by the I.C. Holes were punched by unknown person(s).
Repair/Replacement cost $285.
Supervisor's Comments & Recommendations:
Use furniture in the lobby that inhibits this type of damage.

P-08-06 LARRY HAMMAR— POLICE DEPARTMENT 03102/06
Description of Incident:
Motorola brand i60C cell phone (Serial #919XDJOLJV) and belt clip fell off sometime
during the day at an unknown location. Replacement cost is $200.
Supervisor's Comments & Recommendations:
Cell phone belt clips that do not attach to the duty belt or pants in manner that provides
maximum security need to be supplemented by special Velcro strap designed to
overcome that issue when a wide belt is worn. Employee now using that Velcro strap in
spite of 10+years of not having lost cell phones or pagers in the past when fastened with
less than adequate belt clips.

S-02-06 GEORGE MURTAUGH — PUBLIC WORKS 03102/06
Description of Incident:
While pulling small trees down in the ditch line at 260 Martin, a limb fell down and cracked
the mirror on the gradhall. Repair cost not yet known — Estimate $600.
Supervisor's Comments & Recommendations:
We can tuck the mirrors in, since they are on a swing arm and it will prevent this from
happening again. Protect mirrors when working in this habitat.

P-14.06 MICHAEL CARLSON — POLICE DEPARTMENT 03114/06
Description of Incident:
Officer lost a police station key. Replacement cost $2.50.
Supervisor's Comments & Recommendations:
Properly securing individual property is solely the individual's responsibility. Counseling
and written reminders should hopefully be enough to reaffirm with the individual that it is
extremely important to secure property.

SR-04-06 MARK ANGELLOTTI - UTILITIES 03/16106
Description of Incident:
While maneuvering in the garage at Landmeier to install plow, Unit#825 made contact
with another plow blade when backing up causing dent to passenger side door. Waiting
for repair costs from Fleet— Estimate cost is $500.
Supervisor's Comments & Recommendations:
To have fellow employees help guide when not sure of blind spots around vehicle.

P-09-06 ANTHONY,KONETZKE - POLICE DEPARTMENT 0311.7106
Description of Incident:
Damage to the front right lower bumper and right side of bumper where it wraps around
the car. The marks appear consistent with a left turn maneuver striking something like a
concrete wall. The bumper is scuffed in several places and the right side buckled out
from the fender. Waiting for repair costs from Fleet.
Supervisor's Comments & Recommendations:
The driver misjudged the distance and needs to be more conscious of how close an
object is to the car.

Committee feels improper investigation, No effort was made to investigate or follow up at
the time of reporting the vehicle damage.
F-05-06 FIRE DEPARTMENT = 06/08 03/25/06
Description of Incident:
Engine 9 was dispatched to 985 Jefferson Square for a person stuck in the elevator.
Upon arrival firefighter used the elevator key for the door. The door opened 'forcing the
firefighters left hand against the doorjamb pausing a compression injury to.his middle
finger. About $800 for the ER visit and shots.

Extreme care performed when working on malfunctioning elevators.

S-03-06 DAVID VALERIO - PUBLIC WORKS 03/28/06
Description of Incident:
While sweeping northbound Ridgewood Avenue, north of Oakton, driver of#724 did not
allow enough room to turn away from a parked car thus making contact with vehicle
causing damage to bumper and trunk of a Toyota Camry. No damage to the Village
sweeper, but $2,300 in repairs to the Toyota Camry.

Pay closer attention whenever behind the wheel of a vehicle.

Driver Refresher Course
GO-01-06 FINANCE DEPARTMENT = 6/06 03/31/06
Description of Incident:
Employee lifted/transferred a box of Xerox paper with 3 reams inside, from the mailroom
to the finance administration printer. Employee noticed afterwards her right index finger
joint area turned black and blue and had swelled to twice its size. Village cost of$580 for
ER visit and x-rays.
Supervisor's Comments & Recommendations:
Possible improper carrying of the box with the reams of paper. Employee should exercise
proper way of carrying and/or handling.

P-10-06 EDWARD HANSEN - POLICE DEPARTMENT 04/02/06
Description of Incident:
Police unit #227 was parked and unoccupied when struck by another vehicle as the
striking vehicle was backing out of a parking space in the municipal parking lot.
Offending driver's insurance paid for squad repair cost of$957.31

P-11-06 POLICE DEPARTMENT = 06105 04102106
Description of Incident:
Officer was involved in a foot pursuit of an offender. During the foot chase the officer
tackled the subject and became involved in a short wrestling match. During the incident
the officer tore his pants and suffered scrapes on both knees. Officer stated there was
muscular pain on the right side of his chest To date costs are $3,000.
Supervisor's Comments & Recommendations:
These injuries are unavoidable. Due to a resistive subject being involved in the incident,
the injuries were suffered.

P-12-06 LAWRENCE NAGEL - POLICE DEPARTMENT 04/04/06
Description of Incident:
Someone keyed obscenities into the passenger rear door and quarter panel of squad -
#253. Repair cost is $498.30
Supervisor's Comments & Recommendations:
This incident was not an accident. It was the intentional criminal act of a person who
caused damage to the vehicle.

F-06-06 FIRE DEPARTMENT = 06107 04106106
Description of Incident:
During a fire training exercise at 1850 Arthur Avenue the employee was walking down
the hallway,zero visibility(artificial smoke)tripped over something planted other foot and
twisted left knee. Employee will require knee surgery and therapy. Approx cost$28,000

Use greater care walking in these situations.

F-07-06 ROBERT MARSHALL - FIRE DEPARTMENT Recv'd Report
on 4114106
Description of Incident:
Unknown damage to bumper of Unit#151. Only aware of damage after informed by the
D/C who was told by Fleet Services when the car was in for service for 2 days. Vehicle
repair cost $436.60
Supervisor's Comments & Recommendations:
Inspectors should visually check assigned car before leaving the Administration Building
parking lot and upon return.

P-13-06 MYLES GREENBERG - POLICE DEPARTMENT 04/20106
Description of Incident:
Officer was running stationary radar in the parking lot at Byrd School. When exiting the
parking lot to pursue a violator,the front right bumper of the squad struck a handicapped
sign post. Repair cost not yet known.
Supervisor's Comments & Recommendations:
Officer must always be aware of his location and any dangers that may present
themselves when putting the vehicle into motion.

H-01-06 HEALTH DEPARTMENT = 06109 04126106
Description of Incident:
Lid of top loading preparation cooler was not properly locked in place by food service
employee at the Sunny Side Up Pancake Restaurant at 1142 E. Devon. Therefore,when
the Health Inspector went to show employee that food was in violation, lid came down
onto inspector's finger causing contusion on right index finger. $600 for x-rays&dr.visits.
Supervisor's Comments & Recommendations:
Accident was unavoidable. Employee did not have control of equipment while conducting
a routine inspection of the kitchen.

Concur...Be more aware of equipment and surroundings.
