Members Present: Jack Gray, Chairman At Vrshek, Deputy Fire Chief Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Leslie Sass, Asst to Chief of Police-Non Member(Attending for L. Hammar) Excused: Lary Hammer, Deputy Police Chief Debbie Liberatore, Loss Prevention Secretary The minutes were approved for the meeting of May 11, 2006 Review Current Reports: 24 Incidents Tabled Reports: None Old Business: "Defensive Driving Course and Sexual Harassment" June 28, 29 & 30 at the Fire Department is completed. Nex classes,w is area continuation of these classes are scheduled for August 23, 24 & 25. New Business: • Pending training for new PW supervisors on incident/accident investigation and safety orientation for new full time/seasonal employees. Director of Public Works notified. • Pending sexual harassment for Police personnel. Waiting for reply from Chief Schmidt. • 2nd annual Public Works Safety Expo Day is scheduled for March 14,2007 at Fire Admin. • Driver's Refresher Class for Police personnel only is scheduled for Wednesday(8/02/06). • Driver's Refresher Class for all other personnel is scheduled for Thursday (12/07/06). • Consideration for training for front counter personnel who deal with the public. Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 3 of 15 Review of Current of Reports P-14-06 MYLES GREENBERG - POLICE DEPARTMENT 04/22/06 Description of Incident: Officer in squad #244 was stopped, waiting for traffic to clear in the traffic aisle of the Shawnway Plaza at 664 Meacham Road, when unit #1 backed out of a parking space and struck squad #244. Repair costs $0. Supervisor's Comments & Recommendations: Driver of unit#1 did not exercise all due caution prior to backing her vehicle up. Accident was unavoidable as it relates to driver#2. LPC Comments & Recommendations: Concur PAS-06 POLICE DEPARTMENT 04127/06 Description of Incident: While officer was wearing a "bite sleeve" during a K-9 demonstration, Sjowy bit the sleeve, but missed the padded portion. Subsequently the officer suffered a bruise type injury to the left forearm. Medical costs $0. Supervisor's Comments & Recommendations: This was an unpreventable accident. The officer is trained in the use of the bite sleeve and the dog reacted as trained. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 4 of 15 P-16-06 )FFICER ROBERT INDURANTE - POLICE DEPT. 05101106 Description of Incident: Officer had executed a traffic stop on a vehicle. The officer placed the gear selector in park on squad #231 and while exiting the squad, realized the selector had not engaged, which resulted in the squad rolling forward and colliding with the violator's vehicle. Repair Costs: squad $1,730.33 — other vehicle $1,717.62 (Total $3,447.95) Supervisor's Comments & Recommendations: Officer's safety will always remain a top priority. In this incident, it was merely a case of misjudgment on the placement of the gear selector by the officer. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 5 of 15 P-17-06 POLICE DEPARTMENT 05/03/06 Description of Incident: Officer was parked in the lot at Gaylord Street & Higgins Road. He reached over the center console of squad #245 to retrieve the laser radar gun on the floor in the front passenger compartment. He heard and felt a snap in his neck. He reported significant pain to the right side of his neck as well as numbing in his right arm. Medical costs to date approximately $1,300. Supervisor's Comments & Recommendations: Officers should not reach over the center console to retrieve equipment from the floor. The center consoles of the squads contain radios, electrical switches and a computer terminal. All of this equipment would make it very difficult to reach an item stored on the floor of the squad. Officers should exit the vehicle and walk to the passenger side to retrieve items. This practice would prevent future injuries or the chance that equipment might be damaged by an officer leaning or laying over them. LPC Comments & Recommendations: Concur S-04-06 reported by: RICH EDWARDS — PUBLIC WORKS 05/04106 Description of Incident: An overthe road trucker, who was lost, pulled into the rear yard at 666 Landmeier and hit unit#818. Total vehicle repair cost$68.50, which was collected from trucking company. Supervisor's Comments & Recommendations: Truck driver was not familiar with his surroundings. LPC Comments & Recommendations: No Comment Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 6 of 15 F-08-06 FIRE DEPARTMENT 05/05/06 Description of Incident: While re-bedding the fire engine, the hose table set-up tipped and the hose table struck the F/F's left shin. The firefighter tried to avoid the table and subsequently twisted his left knee. There are two areas of pain, the left shin and the left knee. Ice applied and transported to ARMC. Medical costs to date approximately $1,000. Supervisor's Comments & Recommendations: We need to purchase hose rolling tables for all fire stations. We have one at station 9 and all stations should have the same table since it is designed specifically for the purpose of providing a stable worktable for unloading hose. LPC Comments & Recommendations: No comment at this time. Fire Department to investigate further and report back at next meeting. FS-01-06 PUBLIC WORKS — FLEET SERVICES 05/10/06 Description of Incident: While employee was working under the dash of vehicle#510 he started getting a tingling in his lower back. One hour later he was in pain. He was instructed to go to the clinic. He ignored this direction and opted to go to his own doctor. Supervisor's Comments & Recommendations: No unusual or unsafe conditions were present. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 7 of 15 P-18-06 POLICE DEPARTMENT 05/10/06 Description of Incident: While conducting a traffic stop at Landmeier and Perris, the officer slammed the ring finger on his right hand in the car door of squad #239. As the officer walked around the car door, he began closing it with his right hand. The window on the squad was down, allowing officer to grasp the door by the window frame. As he swung the door closed, he failed to remove his hand quickly enough and the ring finger was caught in the doorjamb. The finger had swelling and some slight discoloration. Medical costs to date $0. Supervisor's Comments & Recommendations: The officer's attention was focused on the traffic violator, which is how he is trained to respond. The ordinary act of closing his squad door caused the injury, primarily due to his inability to remove this hand quickly. In the future the officer needs to remain attentive to other minor acts going on around him in order to help avoid any future injuries. LPC Comments & Recommendations: Concur P-19-06 POLICE DEPARTMENT 05/20106 Description of Incident: Officer was driving squad #240 and was proceeding westbound on Laurel at Love Street when another vehicle failed to yield at a stop sign. The right front corner of the other car collided with the left front corner of the squad. The squad was then forced into a parkway tree. Officer sustained a cervical "sprain/strain". Medical costs $611.49 to date. Supervisor's Comments & Recommendations: Accident was unavoidable. VERONICA GARCIA— POLICE DEPART. 05/20/06 Description of Incident: Officer was driving squad #240 proceeding westbound on Laurel at Love Street, when another vehicle proceeded into the intersection not yielding to the stop sign. The right front portion of the other vehicle then collided with the left front portion of the squad. The squad was then forced into a parkway tree. Squad almost totaled. Repair cost$17,593.50 Supervisor's Comments & Recommendations: Accident was unavoidable. LPC Comments & Recommendations: Concur P-21-06 QFC. KELLY SWEENEY— POLICE DEPARTMENT 05/24/06 Description of Incident: While executing a U-turn to pursue a traffic violator on Biesterfield near Beisner,the right front tire came into contact with the curb leaving a scuffmark on the tire and possibly causing damage to the steering assembly. Repair cost not known at this time. Supervisor's Comments & Recommendations: If appropriate, a 3-point turn should be completed. CHRIS TORRES — POLICE DEPARTMENT 05/26/06 Description of Incident: Officer was stopped in traffic westbound Biesterfield on the 1290 overpass when his vehicle (squad #231) was struck from behind. No cost to repair unit. Supervisor's Comments & Recommendations: Officer was stopped in traffic in a fully marked squad car when struck. Officer could do nothing to prevent incident. LPC Comments & Recommendations: Concur FS-02-06 JACK GRAY — BLDG & FLEET SERVICES 05/26/06 Description of Incident: Employee was backing unit#148 with a spotter guiding him. Upon moving forward to exit the facility, the right rear swung out to the right and came into contact with the left rear taillight lens of#158, which was parked adjacent to#148. The departure angle of this unit is significantly greater than any other normal vehicle. The spotter could not see the right side while on the left. No cost to repair-we have a supply of used lenses in stock. Supervisor's Comments & Recommendations: Exercise more care when operating the large vehicles by moving surrounding vehicles to create more space to maneuver. Driver Training Class a must. DENIS MURPHY — POLICE DEPARTMENT 05/30/06 Description of Incident: Black leather off duty wallet containing an off duty police star and commission card was lost/misplaced/stolen. Approximate replacement cost $75. Supervisor's Comments & Recommendations: Instruct officer to take better care of his department issued property. LPC Comments & Recommendations: Concur E-01-06 SENIOR CLERK— BLDG/ENGINEERING 05/31/06 Description of Incident: While sifting at the typewriter the senior clerk turned on the swivel chair to answer phone. Her left foot hit the corner of the desk leaving a bruise on top of her foot. Medical costs to date $0. Supervisor's Comments & Recommendations: "No comments shown on form by supervisor' LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 11 of 15 P�24-06 POLICE DEPARTMENT 06/05/06 Description of Incident: Officer and his K-9 partnerwere involved in a Character Counts demonstration at Admiral Byrd Elementary School. During the demonstration our K-9 Vasco was distracted by another dog being walked in the area and the canine officer initiated a correction on Vasco, at which time Vasco turned and bit the officer's left wrist and left thigh. Medical costs to date approximately $1,000. Supervisor's Comments & Recommendations: Officer will be directed spend additional time in training for corrections and in basic obedience training. In addition the use of the electronic correction collar will be immediately utilized for K-9 Vasco. Efforts will be made to ensure future K-9 demonstrations are conducted in a controlled environment when possible. LPC Comments & Recommendations: Concur F-09-06 reported by: F/F HANSELMAN — FIRE DEPART. 06/05/06 Quint 8's Bushnell Ensign 10 X 50 power binoculars were found with the plastic body broken allowing lens to fall out. They were inside the storage compartment behind the driver's seat. Approximate replacement cost $79. Supervisor's Comments & Recommendations: Purchase new binoculars, which would include lens covers and a hard plastic case. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 12 of 15 W-01-06 SCOTT SANTILLE — PUBLIC WORKS 06107/06 Description of Incident: While backing the front-end loader into its assigned parking stall, the rear left wheel of the front-end loader came in contact with the front right fender of parked unit#824. No spotter was available to assist in backing up. Repair cost approximately $400-600. Supervisor's Comments & Recommendations: Be more aware of locations of vehicles and where vehicle being operated will be traveling. Additional training and familiarity with this piece of equipment will help employee better judge and safely operate the equipment. LPC Comments & Recommendations: What type of training do you give new/seasonal employees on Public Works equipment? Public Works please respond. F-10-06 F/F TOM REINHARDT — FIRE DEPARTMENT 06/09/06 Description of Incident: Firefighterwas operating the aerial ladder for morning test and had rotated it and lowered the ladder below the level of the vehicle. The ladder was in a position that the bottom of the ladder hit the metal trough for the 2 '/2 inch hose, which had been hit before by another operator. There was a second bend placed in the metal at the top. No other damage to the ladder. Repair cost unknown at this time. Supervisor's Comments & Recommendations: The operator of the ladder/quint needs to be familiar with the limitations of the ladder on that vehicle and always pay close attention to possible obstructions, especially when lowering the ladder close to or below the vehicle height. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 13 of 15 F-11-06 PARAMEDIC — FIRE DEPARTMENT 06/09/06 Description of Incident: While on an emergency medical call at 930 Wilma Lane, the F/F stepped off the edge of the sidewalk, which had been converted into a handicap accessible ramp and started to fall into the rock covered flower garden. He twisted his back as he tried to control the stretcher and keep from dropping the patient as he stumbled into some bushes. ABMC diagnosed a "back strain". Medical costs to date approximately $600. Supervisor's Comments & Recommendations: Since stretcher operation is at the minimum a two person operation, personnel need to communicate with each other and control the stretchers speed on uneven terrain,which will then allow for safe foot placement. This will aid in controlling the stretcher and reduce the possibility of injury. LPC Comments & Recommendations: Concur P=25-06 POLICE DEPARTMENT 06/10/06 Description of Incident: Officer was running to assist a woman trapped in a car that was on its side from a traffic crash when his right foot slid out from under him due to vehicle fluids on the roadway by Elk Grove and Tonne . He caught himself against the vehicle and felt his knee snap and was in pain. Possible strain/sprain or tear to the knee, which MRI will further diagnose. Medical costs (surgery, therapy etc.) projected to be $30,000. Supervisor's Comments & Recommendations: Officer's primary role was to assist the crash victims upon his arrival and when performing this function did not see the oil on the roadway. Even in haste there is a need to watch where we all need to step or stand especially in unfamiliar surroundings. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 14 of 15 S-05-06 GEORGE MURTAUGH - PUBLIC WORKS 06/19/06 Description of Incident: Gradall was digging a pavement patch. Waste Management truck parked in the middle of the street when previously asked to wait to pass. Without warning,the truck parked in the middle of the street. Gradall driver sitting in the back cab of the excavator, moved back and scraped the side of the Waste Management truck. Supervisor's Comments & Recommendations: Gradall driver must take more care when backing up the Gradall excavator and not assume that traffic around Gradall has not moved. Look in both directions to make sure that traffic is clear to move forward or backwards with the Gradall. Have an employee on the job site work traffic control to make sure that roadways are clear. LPC Comments & Recommendations: Driver Refresher Course P-26-06 INV. RUSSELL SULLIVAN — POLICE DEPART. 06/19/06 Description of Incident: While walking along Lake Michigan, the department's Nextel cell phone dropped into Lake Michigan and was lost. Approximate replacement cost $300. Supervisor's Comments & Recommendations: Employee must use due care in properly securing his equipment. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for July 13, 2006 Meeting Page 15 of 15 P-27-06 POLICE DEPARTMENT 06/20106 Description of Incident: Officer was removing the metal target stands from the range at the National Guard Range in Marseilles, Illinois and cut the palm of his left hand. Medical costs to date $0. Supervisor's Comments & Recommendations: If officer used gloves to remove the metal targets this may have prevented the injury from occurring. LPC Comments & Recommendations: Concur S-06-06 SEASONAL SUMMER EMPLOYEE 06/20/06 STREETS/BUILDING/FLEET Description of Incident: Employee was applying emulsion (5 gallon can tar sealer equal to 40 lbs)to the walls of a street patch. While setting down the can the employee felt a sharp pain in his upper arm and shoulder. Medical costs to date $500. Supervisor's Comments & Recommendations: I have no recommendations to prevent this type of occurrence. This is a muscle strain that could happen at any time or to anybody. Employee was performing a task he was instructed to do and did it in a proper manner. Investigate the purchase of smaller emulsion buckets to prevent further injuries.
