Old-Business: --- - - - ■ "Defensive Driving Course and Sexual Harassement"course for the Fire Department on June 28, 29, 30 and August 23, 24 & 25 have all been completed. ■ Pending training for all new Public Works supervisors on incident/accident investigation and safety orientation for new full time and season employees is pending. ■ Sexual Harassment Course for the police is pending. ■ Second annual Public Works Safety Expo Day is scheduled for March 14, 2007. Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 3 of 18 Review of Current of Reports P-28-06 OFC. JOHNNELL WILLLIAMS — POLICE 06/28/06 Description of Incident: Employee was on patrol in the area of Biesterfield and Meacham Roads when he observed a vehicle traveling at a high rate of speed. Upon initiating a U-turn in an attempt to catch the offending vehicle, the front right wheel of squad #230 struck the curb resulting in a dented rim. Vehicle repaired in house at cost of $147.75. Supervisor's Comments & Recommendations: Officer was advised to take the steps necessary to insure the safe operation of his assigned vehicle. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 4 of 18 F-12-06 FIRE DEPARTMENT 07/01/06 Description of Incident: Fire Department was dispatched to 1109 Hickory for three people who suddenly got sick with coughing and burning noses. When firefighters arrived at the scene employee experienced throat irritation and immediately left the home. Employee was taken to ABMC for medical evaluation. Employee was cleared for duty. Medical costs $208. Supervisor's Comments & Recommendations: Personnel need to be aware of all information that is provided to them as they respond and utilize proper protective clothing, including SCBA, whenever potential hazardous environments are encountered. The use of SCBA to enter the house would have prevented this exposure. LPC Comments & Recommendations: Concur F-13-06 FIRE DEPARTMENT 07/01/06 Description of Incident: Fire Department was dispatched to 1109 Hickoryfor three people who suddenly got sick with coughing and burning noses. When firefighters arrived at the scene employee experienced throat irritation and immediately left the home. Employee was taken to ABMC for medical evaluation. Employee was cleared for duty. Medical costs $208. Supervisor's Comments & Recommendations: Use SCBA. Supervisor's Comments & Recommendations: Use SCBA (See F-12 - 06) LPC Comments & Recommendations: Concur S-07-06 KEN LOBOCKI — STREETS 07/05/06 Description of Incident: At Biesterfield Road and 1-290 entrance ramp a vehicle crossed into the lane, which cut off Village truck causing Village vehicle to hit the car. $745.84 damage to EGV vehicle. Village paid since other party's insurance refused to pay and the legal costs would not be worth disputing. Supervisor's Comments & Recommendations: This accident was unavoidable by Village employee. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 6 of 18 F-15-06 F/F CLINT CUNZ — FIRE DEPARTMENT 07/07/06 Description of Incident: Employee took his fire gear off of Ambulance 9 when he was transferred to Station 7. He arrived at the station, removed his gear from his vehicle and noticed his helmet was missing. Both Station 9 and his vehicle were checked for his helmet, but it wasn't found. Helmet replacement cost is $150. Supervisor's Comments & Recommendations: Pay closer attention to his gear. LPC Comments & Recommendations: Concur F-16-06 F/F PATRICK McMANUS — FIRE DEPARTMENT 07/07/06 Description of Incident: When Hazmat 8 was leaving Station 8 on a response to a Level 1, the station's air line which required to be connected to Hazmat 8 to maintain air pressure for the brakes was not disconnected, prior to pulling out,therefore the connection pulled out of the wall damaging the plymovent hookup. $150 to repair broken air line connection. Supervisor's Comments & Recommendations: All personnel, especially those assigned to Station 8, need to rememberto disconnect the air line to Hazmat 8 when leaving the station. A sign had been posted on the dashboard of Hazmat 8 warning of such and reminding to disconnect. This sign was not present prior to this call. Unknown when or why it was removed. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 7 of 18 P-29-06 POLICE DEPARTMENT 07/08/06 Description of Incident: While the officer was attempting to place a combative female prisoner into the rear of his squad she spat into his face and saliva entered his eyes. Officer went to ABMC ER for tetanus shot and a blood test. He will need to return in 6 weeks, 3 months & 6 months due to exposure to the prisoner's saliva/body fluid. Medical cost about $570. Supervisor's Comments & Recommendations: Officer handled the situation very tactically sound. While he was entering the rear squad compartment in order to assist the resistive arrestee in getting her legs in the squad, she suddenly spit in his face. She hadn't spat an any one prior to this so no reasonable preventative equipment (spit mask) could have been deployed. LPC Comments & Recommendations: Concur P-30-06 OFC. GARY ROSKO — POLICE DEPARTMENT 07/08/06 Description of Incident: A prisoner seated in the rear of squad #238 kicked out the passenger side rear window. Vehicle window repair $414 and medical costs for officer were $570. Supervisor's Comments & Recommendations: No recommendation. The prisoner was seat-belted at the time, but due to her small frame, was able to free herself and kick out the window. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 8 of 18 W-02-06 STEVE JOHNSON — WATER & SEWER 07/11/06 Description of Incident: While excavating to repair water service at 581 Charing Cross the telephone cable was cut. At&t sent an invoice of $14,000 to repair the telephone cable. Supervisor's Comments & Recommendations: Use greater care when excavating and hand dig areas when uncertainty exists. LPC Comments & Recommendations: Concur P-31-06 OFC. CHRIS TORRES — POLICE DEPARTMENT 07/12/06 Description of Incident: While working undercover on a theft investigation, the officer left in the rental truck a Nokia cell phone (#356244002898355) No cost to replace since the phone was located at a later date. Supervisor's Comments & Recommendations: Officers must be cognizant of all equipment entrusted to them and return those items in good working order. During the excitement of the investigation and resulting arrests, Officer Torres forgot to retrieve this item from the rental vehicle. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 9 of 18 BS-01-06 Reported by: 07/13/06 DUANE WESTBERG — BUILDING SERVICES Description of Incident: It was observed that the fencing on the west side of 1635 Biesterfield was damaged. Cost to replace fence is $1,650. Supervisor's Comments & Recommendations: This could have been storm damage or kids from the park. It is unknown how this occurred. Very often children from Clark Park lose their Frisbees in our lot and in an attempt to climb the fence it may have been damage. LPC Comments & Recommendations: Concur BS-02-06 CUSTODIAN - BLDG. SERVICES 0717106 Description of Incident: While sweeping the floor in the atrium area, employee leaned the broom on the column. The broom fell on the floor and the employee stepped on it and it popped up and struck the employee in the left testicle. Medical to date at least $5,000. Supervisor's Comments & Recommendations: To be better aware of your cleaning tools, where you leave them and to always watch where you are walking. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 10 of 18 BS-03-06 Reported by: DUANE WESTBERG - BUILDING SERVICES 07/14/06 Description of Incident: While landscaper was cutting the grass the mower hit the floodlight,which was broken off from the base and power supply. Landscaper paid for damage $221 .81 Supervisor's Comments & Recommendations: To follow up behind the contractors when performing any type of work for the Village. LPC Comments & Recommendations: Concur F-17-06 FIRE DEPARTMENT 07/15/06 Description of Incident: E9 drove through tar on Devon Avenue. Upon returning to Station 9 employees began to clean the engine. They started the stations gasoline power washer, which didn't seem to be running correctly. Employee knelt down to trouble shoot the machine. When he began to stand up he placed his left hand on the HOT exhaust system to help push himself up, which caused second degree burns to the second and third joints of all four fingers. Medical costs approximately $400. Supervisor's Comments & Recommendations: The exhaust/muffler can be painted a more obvious color (i.e.: bright orange) and personnel in the future should wear gloves when adjusting the machine and be cautious in their hand placement. LPC Comments & Recommendations: Fleet Services will look at it to see if some type of safety guard can be made. J Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 11 of 18 F-18-06 Reported by: THOMAS REINHARDT - FIRE DEPARTMENT 07/18/06 Description of Incident: Tower 7 responded to several incidents during 2nd shift on 07/17/06 and 07/18/06. Damage to the tower basket and door hinges was found while vehicle was in fire station, but unknown when and where it occurred. Damage has been repaired by Fleet Services Supervisor's Comments & Recommendations: No supervisor recommendations LPC Comments & Recommendations: Unknown how it occurred. Unavoidable P-32-06 SGT. DAVID NAUMANN - POLICE DEPARTMENT 0718106 Description of Incident: Employee was at home cleaning his swimming pool. He was wearing his Motorola Nextel cell phone (#919TDR4T28)on his bathing suit, as required. At some point during the process the employee jumped into the pool. Supervisor's Comments & Recommendations: Employee must be aware that he is wearing electronic equipment that might not be resistant to the elements. Water and electronic equipment do not get along well with each other. His commitment is noteworthy, however, he will be allowed to remove his cell phone while swimming and bathing in the future. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 12 of 18 P-33-06 OFC. BRANDI MATHIS - POLICE DEPARTMENT 07/21/06 Description of Incident: Employee was alone in the entry room to the range, awaiting handgun training. Employee wanted to dry fire her handgun, which is a technique used to help improve shooting skill, Employee failed to clear her weapon of a live round and discharged her handgun. The round fired into the wall. $173.48 to repair bulletin board & wall. Supervisor's Comments & Recommendations: Only training and gun-handling competence can adequately prevent future incidents of this nature. Training is presently on going with this officer. LPC Comments & Recommendations: Concur P-34-06 3GT. DAVID NAUMANN - POLICE DEPARTMENT 07/26/06 Description of Incident: Employee was in the bathroom when his Motorola Nextel (#354TGA60LO) cell phone unclipped from his belt and fell into the toilet. The employee immediately retrieved the cell phone, but it was damaged from the toilet water. $300 to replace phone. Supervisor's Comments & Recommendations: Employee must be aware of his surroundings and the possibility that his phone may unclip from his belt. These cell phones are not waterproof and this employee is well aware of the damage that water may cause. Employee to be issued Letter of Reminder by the Chief of Police. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 13 of 18 FS-03-06 LEONARD STRETCH - FLEET SERVICES 08/01/06 Description of Incident: Taillight lens on unit #602 was cracked when contact was made by one of the forks of a forklift truck while unloading large heavy tires from the pickup bed. No repair costs incurred, per Fleet Services. Supervisor's Comments & Recommendations: A spotter was used. The forklift truck by design does not provide 100%clear site ahead. With the forks protruding over 4' ahead of the lifting mechanism it is difficult to see ahead. The spotter did not notice the direction the forks were traveling in because he was watching the tires and wheels that would be lifted by the forks. Spotter and driver must be aware of the conditions. LPC Comments & Recommendations: Concur S-08-06 SEASONAL SUMMER EMPLOYEE 08/01/06 Description of Incident: Employee was attempting to prune shrubs at Village Hall using the electric pruning shears when he dropped the shears on his left knee and sustained a cut. ABMC treated the cut & advised employee he may need a tetanus shot. Medical costs about $200. Supervisor's Comments & Recommendations: Discuss with the employee the correct technique for using pruning shears. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 14 of 18 F-19-06 FIRE DEPARTMENT 08/01/06 Description of Incident: F/F performed SCUBA at 65' and when he returned to the surface he suffered a nosebleed. Second dive of 25' he again had a nose bleed. In the next few days he began to feel ill with flu symptoms. He missed 2-24 hour shifts. After examination it was determined he had "recurrent barotrauma". Previous problem in 2004. Medical costs $300. Supervisor's Comments & Recommendations: It is with our loss that employee will no longer be able to perform SCUBA related activities. F/F can and will be utilized as a surface rescuer. This type of injury occurs when diving. There is no way to prevent this injury when diving. A diver realizes this has occurred after completing the dive. LPC Comments & Recommendations: Concur P-35-06 POLICE DEPARTMENT 08/02/06 Description of Incident: Officer was involved in a sprinting exercise while at the Police Academy. He pulled a hamstring muscle in the back of his right thigh. No formal medical treatment to-date. Supervisor's Comments & Recommendations: Officer had been doing push-ups and sit-ups prior to this incident. He was instructed to run sprints and advised that his leg muscles were a bit tight prior to pulling this muscle. It appears that this incident was unpreventable. LPC Comments & Recommendations: Concur J Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 15 of 18 SR-05-06 TONY POTUCEK— UTILITIES 08/08/06 Description of Incident: Employee was heading westbound and stopped at the stop sign at Greenleaf and Lively. He needed to back up to allow a semi truck to complete a turn. Employee struck vehicle #2 behind him. No damage to EGV vehicle but cost Village $2,981.98 to repair#2. Supervisor's Comments & Recommendations: To be more aware of other vehicles when entering and approaching intersections. Do not back up unless employee is positive there is no other vehicle present. LPC Comments & Recommendations: Driver Refresher Course W-02-06 JOHN WESA— PUBLIC WORKS 08/08/06 Description of Incident: Someone removed the Chicago Steel MagnaTrak 102 magnetic locator from truck#817 and never returned it. NOTE: Per Tom Cech the locator magically reappeared. Supervisor's Comments & Recommendations: This truck and equipment is used on weekends to perform water meter repairs. It must be available at all times evenings included. It would be hard to keep track of who and when it is being used so it can't be left locked. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 16 of 18 P-36-06 POLICE DEPARTMENT 08/11/06 Description of Incident: Officer was involved in physical defensive tactics training. He suffered bruising to both inner forearms after blocking repeated punches from other recruit officer. Supervisor's Comments & Recommendations: All recruit officers were involved in the defensive tactics training. Officer advised that he and his tactics partner were aggressive in their training. This injury appears to be unavoidable. LPC Comments & Recommendations: Concur P-37-06 POLICE DEPARTMENT 08/13/06 Description of Incident: Officer responded to an incident of smoke coming from a building. Upon arrival officer met with an individual who felt there maybe subjects inside the building. Officer inhaled smoke while in the rear of the building. Officer felt nauseous and tightness in his throat and chest. Medical cost about $208. Supervisor's Comments & Recommendations: Officer's actions and proximity to the building were the result of concern over possible subjects still inside the building. After relaying this information to F/F on scene, officer withdrew from the hazardous area. Although the preservation of human life is priority. Officers will be advised to attempt placing themselves in a safe location at fire scene. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 17 of 18 F-20-06 FIRE DEPARTMENT 08/21/06 Description of Incident: While removing the stretcher and patient from A-120, the stretcher carriage failed to deploy. Both paramedics were holding the stretcher waiting for the carriage to deploy. and then it unexpectedly deployed hitting employee in the right foot, resulting in the trauma injury and also a "rolling" type injury while he was trying to avoid the carriage. Currently on-going case — medical costs to date approximately $2,000. Supervisor's Comments & Recommendations: The stretcher has in the past had similar incidents. The carriage should deploy. It doesn't and then it deploys unexpectedly. The stretcher manufacturer should be contacted to find out if there is a recall or similar incidents with this stretcher model. This stretcher should be repaired, removed or upgraded to a new model. LPC Comments & Recommendations: To date all stretchers have been updated. This incident was not the sole reason for replacement, it was time to order upgraded equipment. F-21-06 THOMAS REINHARDT— FIRE DEPARTMENT 08/25/06 Description of Incident: Apparatus Operator Reinhardt was pulling out and the A/C charge drop cord for Engine 117 was damaged. Repair/replacement cost not yet known to date. Supervisor's Comments & Recommendations: Apparatus drivers, before pulling out of the station, should make sure cord is pulled out of the area, thus when backing up cord will not be in the way. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 14, 2006 Meeting Page 18 of 18 F-22-06 DAN LAMKA— FIRE DEPARTMENT 08/25/06 Description of Incident: While responding to a call out of Station 7 the Shoreline plug for TWR7 was smashed. Repair/replacement cost not yet know. Supervisor's Comments & Recommendations: Poor plug design. This vehicle was responding out of the rear of the station because of concrete work on front ramp. This is an out-of-ordinary response pattern for this vehicle. LPC Comments & Recommendations: Concur
