Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 3 of 13 Review of Current of Reports F-23-06 Reported by: July 2006 LYNN COX - FIRE DEPARTMENT Description of Incident: Lightning strike at Landmeier and Arlington Heights Roads in early July damaged two(2) Strobecom One heads and the OPIC-4 card required repair. All items required re- installation. einstallation. Cost of Repairs $1,603.58. Supervisor's Comments & Recommendations: Damage was weather related LPC Comments & Recommendations: Concur S-09-06 JAMES SCHWARTZ— STREETS 08/04/06 Description of Incident: Seasonal employee, who is responsible for this incident, left on 08/11/06 to go back to college. When removing, barricades from the back of truck #703 a leg hit the rear window, which caused it to break. Estimated cost to repair $500.00 Supervisor's Comments & Recommendations: Barricades were not placed on truck properly. Truck has tailgate lift, should have been unloaded from rear not side. SWEENEY - POLICE DEPARTMENT 08/26/06 Description of Incident: Officer responded to the Belvedere Banquet hall in reference to 15 intoxicated subjects that were fighting. Upon arrival two of the main combatants were identified and arrested. One subject was placed into the rear seat of squad#245, who then proceeded to kick the rear driver's side door, breaking the glass and denting the door. Estimated cost is $500. Supervisor's Comments & Recommendations: Prisoners should be seat belted to restrict their movements. LPC Comments & Recommendations: Concur F-24-06 F/F LAIRD - FIRE DEPARTMENT 08/30/06 Description of Incident: When Q8 was pulling out of 1000 Oakton Fire Station on an emergency response Employee failed to release the air hose. $50 to replace the coupling. Supervisor's Comments & Recommendations: Better continuity of operational/functional changes over regular shift changes and also in the daily personnel movements. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 5 of 13 F-25-06 06/34 — FIRE DEPARTMENT 09/02/06 Description of Incident: Firefighter was performing rig check. He opened the engineer's compartment door and a play pipe fell out on his left toe. Sustained contusion to toe. Cost approximately$300. Supervisor's Comments & Recommendations: Replace the appliance holder. LPC Comments & Recommendations: Concur P-39-06 OFC. PRINDLE — POLICE DEPARTMENT 09/06/06 Description of Incident: Officer was eastbound Mark Street when he observed a M/W frantically waving at him. Officer believing person to be in distress attempted a U-turn to go westbound on Mark Street to assist the person. The front right rim/tire of squad #232 struck the curb causing a scrape on the rim. Repair cost to be determined. Supervisor's Comments & Recommendations: To avoid attempting a U-turn when there is not an adequate amount of room to do so. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 6 of 13 S-10-06 TOM VENTURA - PUBLIC WORKS 09/13/06 Description of Incident: Hand held radio fell off the employee's belt and has not been found. Cost will be close to $3,000 to replace radio. Supervisor's Comments & Recommendations: Unfortunate incident. Employee needs to be more cognizant of the whereabouts of such high priced items such as this radio. It may turn up, but it has been taken off line at central dispatch. LPC Comments & Recommendations: Concur F-26-06 06/35 — FIRE DEPARTMENT 09/14/06 Description of Incident: Employee was sitting in the front seat of Tower 7, when a wasp flew in the front cab and stung him in the face (right cheek). Went to Occupation Health where he was given antihistamines. Cost approximately $150. Supervisor's Comments & Recommendations: This accident was unexpected. Can't determine when a wasp may sting unexpectedly. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 7 of 13 P-40-06 OFC. CARLSON — POLICE DEPARTMENT 09/20/06 Description of Incident: Squad #238 struck a curb with the right front tire. Cost to replace tire is $100.75 Supervisor's Comments & Recommendations: Ofc. Carlson, who was the last driver of the vehicle, needs to pay closer attention to surroundings. It is noted that Ofc. Carlson was not driving the vehicle during this incident. The driver who caused the damage was unable to be identified at the time of this report. Ofc. Carlson was disciplined for not inspecting his vehicle prior to his shift. LPC Comments & Recommendations: Concur F-27-06 F/F SHARP —FIRE DEPARTMENT 09/25/06 Description of Incident: While backing vehicle #155 into Station 7 the side view mirror struck the side of the overhead door opening. Cost to be determined. Supervisor's Comments & Recommendations: Be aware of your surroundings. Back into the building slowly, checking both side view mirrors as well as the rear view mirror. The driver should also be turning backwards occasionally to check the entire perimeter. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 8 of 13 F-28-06 06/36 — FERE DEPARTMENT 09/26/06 Description of Incident: While responding to an incident, a car abruptly stopped at Landmeier and Higgins. To avoid a collision the brakes were applied and employee sustained a strain/sprain to his thumb on his right hand. His discomfort became progressively worse throughout the day. He was examined at ABMC Occupation Health Clinic. and diagnosed with a sprained right hand & right thumb. Cost approximately $300. Supervisor's Comments & Recommendations: While responding to emergency incidents, anticipate the actions of others. Maintain a safe interval in traffic. LPC Comments & Recommendations: Concur S-11-06 06/37 —STREETS 09/28/06 Description of Incident: Employee climbed up the back of 705 to switch it to purge mode. He slipped and banged his head on the bucket rest. He sustained an open wound to the back of his head. Occupational Health Clinic took x-rays and administered stitches. Approximate cost$300. Supervisor's Comments & Recommendations: Install something to improve footing because of oils dripping on the deck. LPC Comments & Recommendations: TABLE.......Did they put "something" on the deck to prevent slipping? Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 9 of 13 BS-04-06 Reported by: DUANE WESTBERG — BLDG SERVICES 10/03/06 Description of Incident: Entire pole, light, mounting base and mounting studs were knocked down in the parking lot at 954 Town Center(fountain area). Unknown how this incident occurred. Estimated cost $1,200. Supervisor's Comments & Recommendations: Slow down, pay better attention. LPC Comments & Recommendations: Concur P-41-06 OFC. KUZYK— POLICE DEPARTMENT 1-0/12/06 Description of Incident: Officer was proceeding southbound on Arlington Heights Road north of Cosman when he looked at what he believed to be an expired registration plate sticker. As the roadway was curving, the officer then apparently turned the steering wheel too severely and struck the center median. Damage to left front rim and sidewall of tire. Estimated cost $90. Supervisor's Comments & Recommendations: Knowing the roadway was curving, officer should have been more patient and waited for the roadway to straighten or the subject vehicle to stop. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 10 of 13 P-42-06 POLICE DEPARTMENT Report made- 10/13/061 Description ade:10/13/06Description of Incident: While conducting a squad check, Officer Greenberg noticed that the right rear bumper had an approximately 2 inch long tear. The damage was not consistent with S.M.A.R.T. trailer, which had been attached to the vehicle. Cost to repair $482.80 Supervisor's Comments & Recommendations: None, unknown how to prevent. LPC Comments & Recommendations: Concur P-43-06 OFC. McINTYRE - POLICE DEPARTMENT 10/14/06 Description of Incident: Officer McIntyre was executing a right turn around the northeast corner of the building at 1060 Pauly, when he scraped the right rear (passenger side) wheel well area of squad #238 against the structure. Supervisor's Comments & Recommendations: It was Officer McIntyre's first day operating a squad car. He needs to be more familiar with the size of a squad car and its limitations. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 11 of 13 P-44-06 06/38 — POLICE DEPARTMENT 10/19/06 Description of Incident: Officer was handling an injured rabbit call in the backyard of 831 Milbeck when one of the complainant's dogs bit him in the left leg. Employee received tetanus shot & antibiotics and returned to full duty. Cost was $150. Supervisor's Comments & Recommendations: The homeowner should have known that the dog was not friendly and warned the officer or the officer could have asked about the dog's disposition. LPC Comments & Recommendations: Concur F-29-06 Disovered by; 10/21/06 F/F NEJEDLO — FIRE DEPARTMENT Description of Incident: The SPO2 Monitor Finger Sensor was discovered missing when A10 crew was performing the ambulance EMS check. Replacement cost $160. Supervisor's Comments & Recommendations: Crew members should be more diligent regarding equipment. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 12 of 13 P-45-06 06/39 — POLICE DEPARTMENT 10/24/06 Description of Incident: While entering the women's locker room, employee tripped either on the carpet or on the transitional threshold. Employee fell to the ground striking her right & left knee and twisting her right ankle. Sprained right ankle & knee. Medical cost about $500. Supervisor's Comments & Recommendations: Employee needs to pay closer attention when the flooring transitions between tile and carpeting. LPC Comments & Recommendations: Concur W-03-06 JOSH FARMER- PUBLIC WORKS 10/27/06 Description of Incident: While.excavating to repair leaking water main at 1440 Hodlmair Lane, an incorrectly marked natural gas service pipe was struck and damaged. No cost, not EGV fault. Supervisor's Comments & Recommendations: Locating marks by Nicor's agent were off by approximately 2 1/2' at the point of damage. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for November 9, 2006 Meeting Page 13 of 13 F-30-06 06/40 — FIRE DEPARTMENT 10/31/06 Description of Incident: Tower7 was dispatched to an MVA entrapment. During this incident the employee assisted in an extrication, assisted in pushing a pick up truck out of the way, used hydraulic omni tool, hydraulic spreaders, hydraulic cutter and pry bar. F/F is not sure what move or lift caused his pain, but immediately after completion of tasks he felt back strain and weakness. Sprain/strain to lumbar area. Cost is yet to be determined. Supervisor's Comments & Recommendations: On this incident we utilized sufficient manpower to physically move the pick up truck. The hydraulic spreader was operated by two firefighters. The hydraulic omni tool and cutter is a one-man operation. The patient removal was done with sufficient manpower to make the evolution as safe as possible. Unfortunately this type of work and evolution can be done in the most safe and efficient manner and there can still be the opportunity for injury. LPC Comments & Recommendations: Concur
