Present: Jack Gray, Chairman Larry Hammar, Deputy Police Chief Scott Miller, Deputy Fire Chief Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Debbie Liberatore, Loss Prevention Secretary Excused: None The minutes were approved for the meeting of November 9. 2006 Review Current Reports: 20 Incidents Tabled Reports: S-11-06 Streets Incident occurred on 9/28/06 "Did they put "something" on the deck to prevent slipping?" Response: Yes..slip prevention pad was put on the bottom of deck and an element has been devised to correct dripping oil. Old Business: Driver Refresher Training Course was held on 12/07/06 non police personnel. New Business: Driver Refresher Training: Police personnel course is scheduled for 2/14/07. Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 3 of 12 Review of Current of Reports O6-P-46 06/42 - POLICE OFFICER 10/19/06 Description of Incident: Arrestee decided to act out against the officer when getting out of the back seat. He threw his body into the officer's upper torso and arm. Officer sustained a sprain/strain to his right wrist, hand & thumb. Estimated medical costs $400. Supervisor's Comments & Recommendations: When dealing with arrestees it is difficult to predict their behavior. In this case the person was handcuffed and believed to be under control, but the subject decided to act out in the back lot, which was unexpected. The space between parked cars does not always give enough room to get out of the way. If an officer feels a person is threatening or agitated then an assist officer could be requested. LPC Comments & Recommendations: Concur 06-LF-02' 06/41 - MAINTENANCE WORKER 11/01/06 Description of Incident: Employee exited the sweeper to talk to other employees during the leaf program to ask about his radio. While getting back into the sweeper he walked near the pile of leaves and stepped on a large rock that was under the leaves, fell and injured his left shoulder and his left hip joint is sore. No Medical attention — No cost. Supervisor's Comments & Recommendations: Employee should be more cognizant of the surrounding area on a job site. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 4 of 12 06-SR-06 06/43 - MAINTENANCE WORKER 11/06/06. Description of Incident: While removing a beehive style storm sewer grate to rescue a lost kitten, employee felt sharp pain in lower back on his right side. Employee was lifting in approved manner, but the location made movement awkward. Estimated medical costs $1,800. Supervisor's Comments & Recommendations: Injuries of this nature are unpredictable. This employee has performed this same activity in both better and worse environmental conditions without any problem. Employee is aware of his physical limitations and operates within them. Employee, and employees in general should try to limit lifting if necessary, but lifting was required in this instance. Unavoidable aspect of the job. LPC Comments & Recommendations: Concur 06-P-47 OFC. BRANDI MATHIS - POLICE DEPARTMENT 11/07/06 Description of Incident: Officer was as 2424 Landmeier Road in squad 236, completing a police report. Upon being dispatched to a reported battery, the officer backed out of her parking space and struck a landscaping rock in the lot. Repair cost to squad is $400. Supervisor's Comments & Recommendations: Officer Mathis was advised to always confirm the area behind her vehicle is clear of people and property before backing up. Rapid response to a call should not supersede safe motor vehicle operation. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 5 of 12 06-SR-07 RICHARD ENGLER - UTILITIES 11/13/06 Description of Incident: Employee was in front of 124 Turner breaking out curb with a skid steer for inlet replacement. The skid steer ran over a piece of concrete pinching it between road surface and tire, which shot it out making contact with a car parked about twenty feet away. Cost to repair other vehicle $422.17. Supervisor's Comments & Recommendations: We could put up a temporary barrier that may reduce future incidents or occurrences, but this method might not always be feasible. LPC Comments & Recommendations: Concur 06-P-48 06/44 - POLICE OFFICER 11/13/06 Description of Incident: K-9 officer was involved in a traffic stop/narcotics investigation. Upon searching the vehicle, officer located pills suspected to be Ecstasy. While conducting a field test using the Narco Pouch test kit, when cracking the test vial, a small piece of glass punctured the officer's left thumb, resulting in minor bleeding. Wound was cleaned and treated. Due to minor wound, no formal medical treatment was needed. $0 medical cost. Supervisor's Comments & Recommendations: Although the incident was not attributed to an error on the officer's part, officer was advised to attempt to use due caution when testing narcotics with the Narco Pouch test kit. The use of gloves or blunt solid object to break the glass vials was suggested. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 6 of 12 06-13-54 06/45 - POLICE OFFICER 11/14/08 Description of Incident: Officer reported that he was involved in NIPAS training at Maryville Academy. He and others were kicking one another while wearing pads. He believes he injured his foot while involved in this activity. Officer didn't realize he injured himself until he returned home at 1600 hours. Estimated medical cost between $200 and $300. Supervisor's Comments & Recommendations: Officer's injury was a result of specialized training for NIPAS. Safety equipment was being used correctly. The injury was minor and was an isolated incident. LPC Comments & Recommendations: Concur Reported By: 06-F-31 MICHAEL ROGERS -- FIRE DEPARTMENT 11/16/06 Description of Incident: While a citizen was visiting Station 7, upon exiting the station via the overhead door, his winter jacket sleeve caught the edge of the bug deflector and broke off approximately 8 inches of the deflector. $0 cost to repair. Supervisor's Comments & Recommendations: Supervise civilians more closely. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 7 of 12 O6-F-32 06/46 - FIRE DEPARTMENT 11/28/06 Description of Incident: While employee was participating in a search and rescue drill, descending down the stairwell, he began to have difficulty breathing. At the bottom of stairs he exited the building and removed his SCBA mask. Treated at ABMC ER for"fume/smoke inhalation" and released. Estimated ER cost $2,000. Supervisor's Comments & Recommendations: The employee should wear his SCBA mask and regulator when entering the fire training building. LPC Comments & Recommendations: Concur Reported by: 06-P-49 :)FC. BRANDI MATHIS -- POLICE DEPARTMENT 11/29/06 Description of Incident: Prisoner was being held in interview room #1. Prisoner stood on a chair and punched a small hole in the ceiling. Prisoner was charged with Criminal Damage to Property in addition to his original arrest. Repair cost $167.50 Supervisor's Comments & Recommendations: Continued routine prisoner checks will be made of subjects holding in police interview rooms. Officers will be advised to forward any information reference possible prisoner problems to the sergeants as early as possible in the detention process. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 8 of 12 06-P-50 06/47 - POLICE OFFICER 11/29/06 Description of Incident: Officer was in rear lot of 1203 Pagni investigating a burglaryto motor vehicle. The officer pushed open the door to his squad #245 and began to exit the car when it bounced back into the officer, the window frame struck him under the right eye causing a minor cut and swelling. Officer chose not to seek formal medical attention. $0 medical cost. Supervisor's Comments & Recommendations: While attempting to complete his assignment at an expeditious pace, the officer misjudged his posture and hand position in reference to his closing car door, resulting in the accident. The officer was advised that although his effort to rapidly complete his assignment was admirable, safe procedures and actions should always be the goal. LPC Comments & Recommendations: Concur 06-S-12 BRIAN MISIAK - BLDG SERVICES 12/01/06 Description of Incident: While plowing corner intersection of Longboat and Schooner, Brian stopped to let a car go around him. Car passed and Brian backed up not seeing another car was behind him. There was front bumper and side fender damage to car that was hit and no damage to Village truck #712. $1,380.70 to repair other car. Supervisor's Comments & Recommendations: Be more aware of his surrounding. When backing up, driver is always at fault. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 9 of 12 06-F-33 06/48 - FIRE LIEUTENANT (Parademic) 12/03/06 Description of Incident: On arrival at 1180W. Devon employee stepped from E10 into a snowdrift,then to parking lot surface. Surface was frozen, employee's foot slid and he twisted his knee. Next day knee was sore and progressively became worse. Diagnosed with tendonitis in left knee by Alexian Corporate Health. Estimated medical costs $300. Supervisor's Comments & Recommendations: Check for good safe apparatus placement, proceed with caution based on conditions. LPC Comments & Recommendations: Concur O6-P-51 OFC. MOODY — POLICE DEPARTMENT 12/05/06 Description of Incident: Officer stopped a 1992 Oldsmobile Bravada for a traffic violation at Lively and Estes. After they pulled away from stop, officer stopped to let a car turn left in front of him into a driveway. His ticket paperwork fell to floor, he went to pick it up while squad 236 was in motion and struck rear end of the Oldsmobile Bravada that was in front of him. $1,924.12 to repair squad. Supervisor's Comments & Recommendations: Police officers must not allow themselves to become so distracted as to take their attention away from vehicles on roadway in front of their police squad car while it is in motion. Drivers of other vehicles cannot be counted onto drive safely and reasonably at all times, so officers must be extra vigilant and cautious. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 10 of 12 06-W-04 Reported by: JOHN WESA - PUBLIC WORKS 12/05/06 Description of Incident: At southbound Arlington Heights Road and Biesterfield a Chevy Cavalier struck Village vehicle #817. $0 damage to Village vehicle. Supervisor's Comments & Recommendations: Other vehicle failed to reduce speed and struck Wesa. Accident not caused by Village employee unavoidable. LPC Comments & Recommendations: Concur 06-F-34 Reported by: MIKE PIANTKOWSKI -- FIRE DEPARTMENT 12/07/06 Description of Incident: Upon receiving Ambulance 8 (vehicle #128) back from the garage for safety inspection work the laptop computer screen was found damaged. The screen was not damaged prior to going to Fleet Services. Estimate pending for repair of HP laptop computer screen. Supervisor's Comments & Recommendations: Non-qualified personnel should not touch the computer screen or remove the laptops before sending vehicle to service providers outside of the Village garage. LPC Comments & Recommendations: Concur - Working on features to better protect laptops in vehicles. • Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 11 of 12 06-P-52 OFC. PRINDLE — POLICE DEPARTMENT 12/14106 Description of Incident: Officer was backing squad #237 from middle northbound lane of Arlington Hts. into left northbound lane to position himself behind two stopped vehicles in the left northbound lane. As he was backing into position another car pulled up in the left northbound lane behind the cars. The right rear bumper of the police squad collided with the front end of that car. $2,283.07 cost to repair other vehicle. Still waiting for Village vehicle estimate. Supervisor's Comments & Recommendations: It is always the officer's responsibility to safely back up his police squad. He did not have his emergency lights activated while backing up from one lane into the other lane. Officer must take all due care when backing his squad and ascertain that the roadway is clear before backing into another lane. The emergency lights should have been activated. LPC Comments & Recommendations: Driver Refresher Course Reported by- 06-P-53 SGT. NELSON -- POLICE DEPARTMENT 12/16/06 Description of Incident: Unknown who is responsiblp, probably a prisoner scratched gang symbols into the back of a plastic chair located in the interview room. $0 repair cost. Repair done in-house. Supervisor's Comments & Recommendations: Appears that damage could have been done by a fingernail. Remove all chairs from interview rooms unless they are being used by officers. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for January 18, 2007 Meeting Page 12 of 12 06-F-35 BRADLEY MENAS — FIRE DEPARTMENT 12/26/06 Description of Incident: Incident occurred on response to Algonquin Road and Pennsylvania in Des Plaines. During response Q8 needed to make a right hand turn onto Pennsylania, but overshotthe street. Lieutenant instructed Menas to initiate a U-turn at the next intersection. During execution, Q8 clipped either a branch or street sign, which detached the side rear handrail from the body of 08. Estimate costs are pending. Supervisor's Comments & Recommendations: Continued apparatus driving experience. This vehicle requires constant awareness on the reaction of the rear end during any type of turn. More time behind the wheel is the only way for a driver to become efficient in operating this type of equipment. LPC Comments & Recommendations: Driver Refresher Course 07-P-01 07/01 -- POLICE DEPARTMENT 01/03/07 Description of Incident: Employee exited the word processing room when her left shoe hooked on the carpet runner in the hallway causing her to fall to the floor. Taken to ABMC ER for sprain on top of right foot. On-going case, to date cost is $1,000. Supervisor's Comments & Recommendations: Consider the removal of the carpet runners in the hallways. They are only effective during inclement weather. LPC Comments & Recommendations: Concur
