Present: Jack Gray, Chairman William Zimmerman, Commander Police Department Al Vrshek, Deputy Fire Chief Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Debbie Liberatore, Loss Prevention Secretary Excused: The minutes were approved for the meeting of March 8, 2007 Review Current Reports: 22 Incidents Tabled Reports: SR-01-07 (07/19 — Sewer Department) Old Business: Public Works Safety Day was held on March 20. 34 employees attended. It was well received and successful. Subjects covered at each booth were Confined Space Entry, Ladder Safety, Lockout/Tagout, General Safety Awareness, Temperature Extemes, Back Care, Right To Know, Personal Protective Equipment, and Blood Borne Pathogens. Repair estimate still pending. Supervisor's Comments & Recommendations: With all the busy days with painters, carpeting and furniture at the Village hall, this report slipped through the cracks and was just brought to my attention. Employee was going to fast, did not turn all the way. Snow removal on the shorter and narrower sidewalks should be done by hand. The large equipment can't make the turns. LPC Comments & Recommendations: Concur 12-08-07 OFFICER ANSEM - POLICE DEPARTMEf�1 ' 07.12"1/0.7 Description of Incident: During a narcotic arrest, the suspect was in possession of an American bulldog. The dog escaped from the suspect and charged an officer. That officer(different department) shot the dog. The dog proceeded to run toward Ofc. Jensen's squad #251, jumped in and bled all over the backseat. Fleet services cleanup was $597.25 Supervisor's Comments & Recommendations: If possible, close squad door prior to encounter with a vicious animal. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 10, 2007 Meeting Page 4 of 14 P-09-07 OFFICER THOMAS - 00 LICE DEPARTMENT' a2/271{It? Description of Incident: Officer was involved in the search and apprehension of two subject wanted for armed bank robbery out of Hoffman Estates. He deployed a X26 Taser at one fleeing offender and dropped the taser to transition to his firearm to cover arresting officers while they handcuffed a felon. The taser suffered damage to the two side levers. One lever is broken and the lever controlling the light does not function. Supervisor's Comments & Recommendations: Under normal use the taser should not be dropped. However, in this high stress situation, where the arrestee is believed to be armed and fleeing from the police, the safety of officers and the general public is paramount. Ofc. Thomas transitioned to his duty weapon to ensure that safety. LPC Comments & Recommendations: Concur 12- 0-0? 07194 , POLICE IJEPARTMENT t:2l27 07 Description of Incident: Officer was loading equipment into his squad at beginning of his shift and slipped on "black ice" and fell onto his right side. He suffered a scraped right knee, a blow to the head which struck an AED case and a strain to his lower back when his hip contacted the pavement. Estimated medical costs $500. Supervisor's Comments & Recommendations: Make sure that more salt is applied to walk areas during potential "black ice"conditions. LPC Comments & Recommendations: Committee recommends the Street Department apply generous amounts of salt in a timely manner in the parking lot. Description of Incident: Employee was trying to locate a unit in Parcel C at 773 Wellington when she slipped on "black ice", lost footing and strained her neck and left shoulder. No medical treatment. Supervisor's Comments & Recommendations: Employee should ensure footing is safe while walking. LPC Comments & Recommendations: Concur 1211-07 CASEY KOWALCZYKC— POLICE DEPARTMENT 03/01/07 Description of Incident: When employee was exiting a private driveway at 2451 Lively in vehicle#255, when the right side of his vehicle struck a landscape rock, which was covered by snow. Repair Cost $482.01 Supervisor's Comments & Recommendations: Driver further forward into the street before exiting the private driveway. LPC Comments & Recommendations: Driver's Refresher Course Loss Prevention Committee Minutes for May 10, 2007 Meeting Page 6 of 14 F-Q7 07 07J16, FIRE DEPARTMENT t33�071 Description of Incident: After performing a rescue of 3 men trapped in a machine at 2550 American Lane, employee complained of a moderate headache. He was breathing off his S.C.B.A. during incident of hazardous atmosphere and it was an extremely physically taxing rescue. Transported to Northwest Community Hospital for precautionary measures. Estimated medical cots $600. Supervisor's Comments & Recommendations: Employee was wearing all his P.P.E. including S.C.B.A.. The area was small and only 1 rescuer could fit in the space at a time. Employee expended a lot of energy during rescue, which may have lead to his headache. LPC Comments & Recommendations: Concur F-4$-(I7 LT. FERGUSON:.,-. FIRE DI=f'ARTMENT /(l4107 Description of Incident: Employee opened the sliding glass window in Unit #139 (behind the officer seat) and when closing the window the glass shattered. Repairr cost was $385.50 Supervisor's Comments & Recommendations: It is unknown why the glass failed. The age of the glass may have been a factor. LPC Comments & Recommendations: Replaced the glass. Original glass wasn't safety glass. Examined car and saw no damage. On 3/19 had car washed and noticed some paint on left side of bumper and upon closer inspection there was a 3'/z inch crack in the bumper. Estimate still pending. Supervisor's Comments & Recommendations: Utilize greater care when backing any vehicle during winter months do not park close to curb. If needed get out of vehicle to observe area before getting too close to curb. '07/l8 POLICE aEPARTMENT .03/21107 Description of Incident: Officer was called,to the scene (5 Woodcrest) of an attempted suicide by a 14 year old girl. His responsibility on scene was to collect evidence and record pertinent information about the scene. While measuring the scene, employee was working in a confined area underneath a raised bed and when he turned his head his face struck the corner of a shelf. This caused a small mark under his right eye. No medical treatment or cost. No time missed from work. Supervisor's Comments & Recommendations: Employee was advised to take whatever action is necessary to attempt to avoid a reoccurrence of this type of incident. Awareness of the environment around him and cautious procedural operation was recommended. LPC Comments & Recommendations: Concur F-1D-fl7 07/20 - FIREDEPA`R?HENT 03I26107 Description of Incident: While transporting restrained patient to ABMC the patient was able to work oxygen mask off his face,which was being used as a barrier, and spit. The spit contained blood,which hit the employee's face. Several blood tests are needed, costs are on-going. Supervisor's Comments & Recommendations: All personnel need to be constantly cautious and anticipatory to all possible actions of a patient and protect themselves accordingly. Supervisor's Comments & Recommendations: All personnel need to be constantly cautious and anticipatory to all possible actions of a patient and protect themselves accordingly. LPC Comments & Recommendations: Concur F-12-07 f#;EQ HAMSELMANI - FIRE DEPARTMENT 43f29/07 Description of Incident While returning to the Greenleaf station after a call, employee was backing up to the overhead door and misjudged the distance to the door, hitting it with the light bar as it went up. The light bar was broken and door was dented. Estimate still pending. Supervisor's Comments & Recommendations: Must keep a safe and sure distance from overhead doors and objects when preparing to backup. Do not precede any further until properly backed up by another individual while maintaining visual contact at all times. LPC Comments & Recommendations: Driver's Refresher Course Loss Prevention Committee Minutes for May 10, 2007 Meeting Page 10 of 14 i f-13-0? fIJIICHAEL PESTEL - FIRS DEPiARTMENT 0319f� Description of Incident: Employee stopped in front of station 9 in ambulance 126 to let cars go by and then proceeded to back the ambulance on the front ramp. He did not see Jack Gray's car on the ramp and bumped unit #601. No repairs needed for either vehicle. Supervisor's Comments & Recommendations: Firefighters must make sure they have visual assistance from the rear while backing up ambulances and other large fire apparatus. LPC Comments & Recommendations: Driver's Refresher Course SR-01-07 07/119 - WATER& SEWER D"EPARTMI NT 04/02107 Description of Incident: While operating a small handheld grinder to remove deteriorated bolts from a valve, the grinder bound momentarily and jumped forward striking the employee on the left index finger, which was steadying the valve at the time. Employee sustained a lacerated finger & required stitches. Estimated medical costs about $500. Supervisor's Comments & Recommendations: Employee was wearing gloves when the injury occurred, This is the most effective method for removing deteriorated bolts that can no longer be wrenched off. Employees will be warned to exercise greater caution to ensure that their body parts are not within the likely path of the grinding disc should a similar occurrence happen. LPC Comments & Recommendations: TABLE - Need more clarification Loss Prevention Committee Minutes for May 10, 2007 Meeting Page 11 of 14 i SR-02AU 0�2 WATER & SEVI/ERDEP-ARTIVI;Ehi�' U4/QZ14 Description of Incident: Employee had a rash on both arms & legs and suspected it might be related to his job, per ABMC. Blood tests were run and ABMC advised rash was not related to work and stems from another source. EGV will pay for initial visit & blood tests. Supervisor's Comments & Recommendations: I really don't know, the employee was wearing long sleeve work clothes and gloves. LPC Comments & Recommendations: Concur F-7'4""-07 DIANA LAPORTE - FIRE DEPARTMENT 04/07147 Description of Incident: While searching mobile home at 400 Touhy for four victims the plastic casing and loss of cord integrity for a Motorola HT 1000 VHF portable communication radio occurred.There is nothing the F/F could have done to prevent this damage. The heat from the fire was very severe and there is no special case the radio can be placed in for protection. There were four individuals in the trailer so time was of the essence. Repaired in house $50. Supervisor's Comments & Recommendations: As long as EGFD members enter structures that are burning to perform rescue or fire suppression, exposure to heat is inevitable. However, when reasonable risks are taken and safety to members is stressed these types of accident can be minimized. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 10, 2007 Meeting Page 12 of 14 F-15-0 EDWARD 10'OREK - FIRE'DEPAR�'MEh1T 04/07117 Description of Incident: While searching mobile home at 400 Touhy for four victims the plastic casing and loss of cord integrity for a Motorola HT 1000 VHF portable communication radio occurred.There is nothing the F/F could have done to prevent this damage. The heat from the fire was very severe and there is no special case the radio can be placed in for protection. There were four individuals in the trailer so time was of the essence. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 10, 2007 Meeting Page 13 of 14 S-05-07 RICH ENGLER11 - PUBLIC 1NQRKS, Description of Incident: Forestry department was dispatched to 88 Kennedy to remove a very large tree that was hindering the efforts of a sewer collapse repair. The chain saw (14" Stihl MS200T) kicked back and fell out of Rich's hand and down on the parkway. The plastic housing and handle cracked. No injuries reported. Total repair cost was $176.45 for parts. Supervisor's Comments & Recommendations: We have teather straps that anchor the saw to the operator, but most guys do not trust them including myself. They can be double edge sword. If the saw comes loose and is dropped the strap can cause the saw to swing back into the operator's body. I myself feel it is better to drop a saw then to have a running chain saw swing down against my body. The strap does have its positive points also so we pretty much leave it up to personal preference. I suppose the only avoidance of this would be to hang onto the saw tighter. LPC Comments & Recommendations: Concur P- 347 07122 - POLICE DEPARTMENT111147 Description of Incident: Employee was firing a pistol in the range while performing this task a spent shell casing ejected and bounced off a wall and lodged between employee's prescription glasses and his right eye. Employee refused formal medical attention, therefore no medical cost. Supervisor's Comments & Recommendations: To prevent or to reduce the chance of a future type of incident from occurring, safety glasses, which offer additional protection, should be worn over prescription glasses. LPC Comments & Recommendations: Concur. Committee recommends & encourages the use of safety glasses. i Loss Prevention Committee Minutes for May 10, 2007 Meeting Page 14 of 14 I P- 4-07 OFC. INDURANTE PC)L10E DEPJIRTMENT 04/2107 - , . ; I Description of Incident: Employee was on a traffic stop at York Road 200 feet south.of Supreme Drive with emergency lights and arrowstick activated, when a car approached in the right S/B lane towards the squad and sideswiped the left side. The driver admitted he was looking down at his heater switch and was distracted. Estimate still pending. Supervisor's Comments & Recommendations: Ofc. Indurante was on a traffic stop of a speeding violator with emergency equipment and arrowstick properly activated and visible. His car was half in the right lane and half on the shoulder. The next street up was at least a 1/ mile away. The speeding violator had gone quite a distance before stopping. The violator chose the place of the stop. An officer can not always choose the safest place for a stop. Traffic was light. The officer placed squad as safely as possible, but driver distraction caused accident. LPC Comments & Recommendations: Concur P-15-07 0117 POLICE DLPARMENT /D9/Q7 Description of Incident: Employee dropped his keys in the PD lobby and when he attempted to pick kthem up he slipped on wet floor. While trying to regain his balance he developed a groin strain or possible hernia. Estimated medical costs $700. Supervisor's Comments & Recommendations: Employee was advised to be more careful when outside conditions cause wet floors and wet shoes.
