The meeting was called to order at 8:35 A.M. at the Village Hall. Present: Jack Gray, Chairman David Naumann, Sgt. Criminal Investigations Scott Miller, Deputy Fire Chief Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Debbie Liberatore, Loss Prevention Secretary Excused: William Zimmerman, Commander Police Department The minutes were approved for the meeting of May 10, 2007 Review Current Reports: 25 Incidents Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 2 of 16 Tabled Reports: SR-01 The following explanation was given regarding the physical injury to water division employee on 4/02/07 by Scott Bernholdt: The employee was wearing appropriate personal protective equipment (leather gloves). This method of removing severely deteriorated bolts is accepted throughout the industry, but is not without risks such as that experienced. The unsafe method (accepted) box and arrangement hazardous box were checked because there is inherent hazard to this removal method. The unsafe act boxes were marked because in the.supervisor's opinion there are other means to steady the valve body. Employee had methods, other then his hands or other body parts, to steady the valve hence the improper use of hands and unsafe position. To avoid similar occurrences in the future, employee was instructed to use other methods and to always remain cognizant of the expected path of both debris from the cutting operation as well as the likely path of the tool should it bind again. Old Business: • Fire Department back training completed on July 24, 25 & 26. 59 employees attended and it was a successful class per Gallagher Bassett. New Business: • Annual report from Gallagher Bassett was discussed by committee. • Not all employees went to the emergency location during the tornado warning on August 8th. Jack Gray will be addressing this issue. • Committee suggests that Northwest Central Dispatch be asked to notify employees on their radios to alert of the severity.in weather when they are out on the road to know to drive to a safe location. • Procedure or policy to validate employee' driver's license. This would become an issue especially for employees that it is a "condition of employment" when driving a Village vehicle. Fire and Police validate yearly. Two suggestions were made regarding how to validate each Village employee on a regular basis. The first would be at the employee's semi-annual review. The second suggestion would be to give the Police Department each employee's driver's license number and have it run at the Information Desk. It was also discussed that checking driver's license validation could be a union issue and it will be checked. Human Resources may be the responsible Department for this procedure. • Topics for future training opportunities. Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 4 of 16 Review of Current of Reports F-15-07 _ Reportedby: 'FRED HANSELMAN— FIRE DEPT. 03/06/07 Description of Incident: During morning check of equipment, the Thermal Imaging Camera did not function properly. A hairline crack to the casing was noticed. Cost to repair is $8,500. Supervisor's Comments & Recommendations: Be aware of sensitive electronic nature of equipment and use more care in handling these tools. LPC Comments & Recommendations: Concur. In the future the cameras will be kept in cases to protect when not in use. F-16-07 SCOTT PETERSON - FIRE DEPT 03/07/07 Description of Incident: Hose was used for a fire at 130 Martin Lane. Due to the hazardous chemicals on premise, which overflowed during the fire onto the hoses, causing contamination to the hose. Repair cost for 2-50 ft. lengths of 1 3/ fire hose is $250. Supervisor's Comments & Recommendations: This incident was unavoidable due to fire location. Just be aware of these occupancies and try to avoid the hazard areas if possible. LPC Comments & Recommendations: Concur Note: Diane Hunter will be checking if it was or should have been charged back to the company who had the Hazmat incident. This item may be reimbursable. Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 5 of 16 W-03-07 MARK ANGELLOTTI PUBLIC WORKS 04/11/07 Description of Incident: While pushing gravel into the storage bin, the gravel under the right side of the vehicle gave way causing the vehicle to tip. The right side front tire and headlight housing came into contact with the storage bin wall, which caused the headlight bracket to bend and headlight housing to crack. Repair cost was $350. Supervisor's Comments & Recommendations: Operator should ensure the stability of the ramp as the task progresses. LPC Comments & Recommendations: Concur F-17-07 07/24 - FIRE DEPARTMENT 94W/07 Description of Incident: While in the ambulance FF was in the process of gaining IV access when he stuck his right middle finger with the needle. When the patient moved on two separate occasions. FF treated for needle exposure/puncture at ABMC ER. Supervisor's Comments & Recommendations: Many medics use the two-handed technique when using the new IV needles. More attention to when the needle locks into the protective shield, may have prevented this accident. There are other alternatives to stopping or slowing the flow of blood from a catheter Using the protective shield (with needle inside) may not be the best option for solving this problem. LPC Comments & Recommendations: Change in needles ordered by ABMC can cause confusion to the technique to use. Possibly more training and familiarization with the equipment. Committee concurs it was unavoidable because patient jerked and moved causing this injury. Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 6 of 16 P-15-07 OFC.EDWIN MEDINA - POLICE DEPT. 05/07/07 Description of Incident: While parked in the lot of the 7-Eleven (565 Landmeier) the officer repositioned his driver's side mirror to view two subjects having a loud discussion. The subjects left and the officer then began to back his squad, but forgot to reposition his mirror. The rear, driver's side bumper of squad 242 struck the rear driver's side bumper of a parked and unattended 1998 Chevy S10 pick-up truck. $480 cost to repair squad and $865 to repair other vehicle. Supervisor's Comments & Recommendations: Officer Medina understands he backed his vehicle without using due care. He assures the Department that he will exercise due care in the future. LPC Comments & Recommendations: Driver Refresher Course LF-01-07 PATRICK TORTORICI — PUBLIC WORKS 05/14/07' Seasonal .Employee, Description of Incident: While operating dump body (Unit 709), the tailgate was chained open. The top of the tailgate then came in contact with the hitch and the ball hitch dented the top of the tailgate. $1,300 in damage costs. Supervisor's Comments & Recommendations: Pat was slightly rusty being a summer worker, but he should have been more aware of the gate being chained open when he dumped so it wouldn't hit the hitch. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 7 of 16 S-06-07 07/25 - STREETS 05/16/07 Description of Incident: Employee stepped out of vehicle #715 and slipped on mud on the black top surface by the dirt bin at 666 Landmeier Road and fell on right arm hitting the running board. Supervisor's Comments & Recommendations: Exit vehicle where there is safe ground, thus avoiding slipping and or injury. LPC Comments & Recommendations: Concur P-16-07 07/26 - 'POLICE OFFICER 05/17/07 Description of Incident: Employee observed a vehicle going S/B in the N/B lanes of Busse Road. Employee in a marked squad (#229) with emergency lights activated positioned vehicle several 100 ' yards ahead of the wrong way vehicle in order to get him to stop. Employee was partially blocking traffic lane the wrong way vehicle was in so he could prevent the vehicle from causing a head on collision with other vehicles on the road going the correct direction. Wrong way driver failed to stop and struck squad#229. Employee sustained strain/sprain to left shoulder& upper arm. Treated at ABMC and returned to full duty.$6,000 in squad repairs and on-going medical for employee. Supervisor's Comments & Recommendations: Employee put himself in harms way to prevent a head-on collision from possibly occurring. If a head-on collision took place on that stretch of roadway, it could have resulted in a serious or fatal injury. Employee was protecting the public and carrying out his sworn duties. LPC Comments & Recommendations: Concur. Per Diane Hunter this will end up being a very costly incident that the Village will end up paying the total cost. MATTHEW J.-FIORITO POLICE DEPT. 05/17/O7 Description of Incident: Employee observed a vehicle going S/B in the N/B lanes of Busse Road. Employee with emergency lights activated positioned his vehicle (#229) several yards ahead in order to get the vehicle to stop. Wrong way driver failed to stop and struck the employee's vehicle (#229). $6,000.00 to repair and on-going medical costs for employee. Supervisor's Comments & Recommendations: Employee put himself in harms way to prevent a head-on collision from possibly occurring. If a head-on collision took place on that stretch of roadway it could have resulted in a serious or fatal injury crash. Employee was protecting the public and carrying out his sworn duties. LPC Comments & Recommendations: Concur P-#8-0707127 - POLICE OFFICER 05/17/07 Description of Incident: Officer arrested an intoxicated subject for battery. Officer had his left arm in an escort position on the arrestee's right shoulder, when the arrestee pushed towards the officer and attempted to kick him. Officer attempted to stop this activity and upon doing so both the officer and arrestee fell to the ground. The next morning when the officer woke up he felt left shoulder pain. Supervisor's Comments &Recommendations: Officer followed all policies and procedures while affecting this arrest. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 9 of 16 F-18-07 07/28 FIREFIGHTER 05/22107 Description of Incident: FF was cleaning the cot, the handle at the foot of the cot sticks and upon attempting to manipulate the handle so it could be cleaned, it swung free and hit FF in the upper lip. Lip showed slight swelling and had a 2mm laceration. Later when FF pulled the cot from the ambulance to clean the MISU floor and cot, they noticed what appeared to be blood covering the black plastic area surrounding the handle and on parts of the handle itself. They sprayed the area and wiped it with a cloth to confirm that it was not just a discoloring on the plastic. The rust colored stain, when wiped with the cloth appeared to be blood. FF then reported the possible exposure to his supervisor and then went to ABMC to follow proper procedure for exposure. On-going medical expenses at this time. Supervisor's Comments & Recommendations: 1. Make sure all equipment is in proper working order upon placing in service. 2. All equipment must be thoroughly cleaned after any EMS call which the patient's bodily fluids were exposed to our equipment or vehicle. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 10 of 16 F-19-07 07/29 FIREFIGHTER 05/22/07 Description of Incident: At 2411 Landmeier Road (In Town Suites) E9 and A9 were treating a patient that was a suspected overdose. A NRM (non-rebreather mask)was put on the patient to administer oxygen and protect against fluid contamination. The patient began to vomit in the mask. Employee began to wipe patient clean to clear the airway and patient began to shake head from side to side violently and vomited again. A spray of vomit came in contact with the employee's face, eyes, nose and mouth area. He immediately rinsed his face in the ambulance and had further evaluation in the ER. Supervisor's Comments & Recommendations: All PPE precautions were taken. Be aware and.cautious of the unexpected reaction of the patient. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 11 of 16 F-20-07 07132 , FIREFIGHTER/PARAMEDIC 05/29/07 Description of Incident: A patient E10 & A10 treated and transported to ABMC has tested positive for TB. Fire Department was notified three weeks after the exposure by ABMC. Five employees involved in this incident. Supervisor's Comments & Recommendations: Nothing could have been done to limit exposure short of masking everyone up on every call no matter the nature of the EMS call. The dispatch gave no indication of the seriousness of the existing medical condition of this patient. What is preventable is the prolonged time from the date and time of this call and our notification from ABMC (3 weeks). If we are positive for TB and even if we are not, we in turn have or have not exposed our fellow firefighters, family and friends to TB. I recommend a quicker way of notification to EMS, Hospital and Fire Service Personnel of TB exposure. LPC Comments & Recommendations: Concur. Per DIC Miller it was found that a very minimal amount of exposure occurred. F-21-07 D7133 - FIR EFIGHTER/PARANIEDIC 05/29/07 Description of Incident: Exposure to TB on a call at 963 Richmond with four (4) other employees. Supervisor's Comments & Recommendations: Same supervisor comments as 7/32 above. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 13 of 16 P-19-07 OFC.`OMER HOUSTON - POLICE DEPT. 06/06/07 Description of Incident: Officer was driving squad #222 westbound on Elk Grove Blvd at Ridge and stopped for the stop sign when he was struck in the rear by a Mercury Mountaineer. No cost for repairs. Supervisor's Comments & Recommendations: No fault on the part of the officer. LPC Comments & Recommendations: Concur F-25-07 07/30 - FIREFIGHTER 06/07/07 Description of Incident: FF was operating a K-12 saw to gain entry into afire involved dumpster. He sustained a wrist injury when the saw blade made a twisting motion. Per ABMC sprained left wrist. Supervisor's Comments & Recommendations: Possibly additional training and being more cautious when operating these saws may prevent future injuries. LPC Comments & Recommendations: Committee agrees that bending and reaching using the saw made it difficult to use. Additional training would be advantageous. Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 14 of 16 LF-02-07 JAMES SCHAUDEK PUBLIC WORKS 06109/07 Description of Incident: Employee was stopped at the traffic light at Biesterfield and Rohlwing eastbound lane while the driver of a Lincoln Continental rear-ended him. Waiting for estimate. Village and driver of vehicle #1 are being sued by passenger from vehicle #1. Claiming truck stopped to fast, but truck was not moving and already stopped when incident occurred. Supervisor's Comments & Recommendations: None LPC Comments & Recommendations: *TABLED for supervisor to add proper comments. S-07-07 Reported by; 06/12/07. TOM VENTURA—STREETS Description of Incident: Tire on truck blew out causing asphalt on street to be thrown into a glass storm door breaking the glass. Tire blow out cost not yet known. Storm door repair cost $130.20 Supervisor's Comments & Recommendations: Unavoidable LPC Comments & Recommendations: Was it assumed that the Village vehicle caused this damage? We have not replaced any tires on a Village vehicle. *TABLED for lack of information Loss Prevention Committee Minutes for August 9, 2007 Meeting Page 15 of 16 P-20-07 07/31 - POLICE OFFICER06119!07 Description of Incident: Employee was attempting to make contact with a suspect and as she walked from the front porch, a dog came from the backyard to the front and bit her in the left leg. Supervisor's Comments & Recommendations: Employee could not prevent this from occurring. The dog was unsecured and came around the house from the backyard to the front. LPC Comments & Recommendations: Concur 12,21-07 Reported;by, ` 06/20/07: SGT, KEITH WILCOX - POLICE DEPT. Description of Incident: Sgt. was notified by Fleet Services, while providing normal service and detailing to squad #238 it was noticed when the vehicle was lifted off the ground there was a small dent to the passenger side rocker panel in front of the rear wheel. The dent is difficult to notice when vehicle is in a normal driving position. Damage most likely occurred after the last service date of 5/18/07. Approximately $500 to repair Supervisor's Comments & Recommendations: Presently is not known how the damage occurred. It is recommended that officers who feel that. they may have driven over debris or may have struck a curb or item should carefully inspect their vehicle for damage. DETTLOFF STREETS 06/22/07 Seasonal Employee Description of Incident: Truck #740 was backing out of a private parking spot at 609 Meacham and hit a 2006 Saturn in rear bumper. Saturn repair cost $1,000. Waiting for estimate on truck #740. Supervisor's Comments & Recommendations: Take time to look around the truck and have a spotter back you up. LPC Comments & Recommendations: Driver's Refresher Course BS-02-07 Reported by: 06/25/07 DUANE WESTBERG - BUILDING SERVICES Description of Incident: Window in the tower at the shopping center was broken. Estimated cost is $1,100. Supervisor's Comments & Recommendations: Possibly with the new video cameras being installed, this may not happen again. LPC Comments & Recommendations: Concur
