Jack Gray would like this approved and in effect approximately by January 1, 2008 • Village Employee driver's license status checks was brought up again as to who will be responsible to check them and how often will it be done. However, this year has been unusually busy with the relocation of the Police department and the demolition of the old facility. I apologize for the tardiness of this report. The membership of the committee has changed with the addition of Scott Miller (Fire) who replaced Al Vrshek and William Zimmerman (Police) who replaced Larry Hammar. We continue to meet every other month. The incident totals remain constant, therefore, bimonthly meetings will continue. In the next table, is a representation of the total incidents for each department for 2006. LPC, only tracks our internal totals of incidents. Please keep in mind that our internal totals do not equal or match Gallagher Bassett's, because LPC tracks all* incidents, regardless of whether it is a successful workman's compensation claim or not, or an incident with an outside party. Gallagher's yearly totals will always be lower. Description of Incident: While assigned to squad#245 officer was traveling E/B on Greenleaf approaching Lively. Area is under construction with variances in the height of the roadway. Higher roadway is paved and lower roadway is still gravel. The front bottom of the squad made contact with the gravel causing damage to the undercarriage and lower bumper. Vehicle repair cost totaled $566.97 Supervisor's Comments & Recommendations: At this particular intersection, the height variance was adjusted by the construction company, who placed a small patch of asphalt to reduce the difference in the roadway heights. The officer needs to anticipate various driving conditions, especially while in construction areas. During the incident, Greenberg pulled into the driveway of a house located in the Cook County Forest Preserve. When leaving he performed a three-point turn. In the process the squad "bottomed out" catching the rear bumper on the left side causing a vertical tear of approximately 6 inches to the bumper cover. Repair was done internally for zero cost. Supervisor's Comments & Recommendations: Prior to driving on unfamiliar areas, the officer should examine the area for any potential hazards. Morrison Blvd. When the stop was completed, he continued E/B in the W/B lanes. Squad #232 (Officer Gustafson) was exiting the driveway of Rainbow Falls, turning right (westbound) onto Rev. Morrison Blvd. Repair cost for unit #260 was $731.71 Supervisor's Comments & Recommendations: After completing a traffic stop in the wrong lanes of traffic either proceed as safely and quickly to the proper lanes of traffic or use emergency lights when moving. Morrison Blvd. when the left front of squad #232 struck the left front of squad #260. Repair cost for squad #232 was $497.00 Supervisor's Comments & Recommendations: Although squad #260 should not have been traveling E/B in the W/B lanes of Rev. Morrison Blvd, Officer Gustafson could have checked one last time for any unexpected traffic to his right before proceeding. Employee was transported to ABMC/ Total medical costs not yet known. Supervisor's Comments & Recommendations: All Citizen Fire Academy instructors and students were wearing safety glasses or goggles in the demonstration area. High-pressure hydraulic hose failure is a known possible hazard. Equipment and hose worked fine up to the moment of the rupture. The equipment was checked earlier prior to the start of the class. I believe the pressure where the hose burst was strong enough an hit the safety glasses at the right angle to dislodge them and expose the firefighter's eyes to hot oil. Without safety glasses injury would have been worse. Recommend the continued procedure of wearing protective eyewearto prevent or reduce exposure to injury in the future. Firefighter was wearing eye protection and his full protective clothing at the time of the incident. LPC Comments & Recommendations: Concur that goggles, not glasses should be used to cover the eyes for protection. ' . Description of Incident: Right knee was injured during a training arrest at Suburban Law Enforcement Academy. Total medical costs not ynt known ABMC determined knee strain. Description of Incident: Officer was assigned to 1596 Texas to take a stray cat into custody and transport to Hollywood Kennel. While removing the wild cat from cage wearing heavy duty gloves,the cat was able to scratch his right forearm, causing two deep 2" long scratches. ABMC ER doctor ordered the rabies shots (series of 4) to be given to the officer. Supervisor's Comments & Recommendations: Longer gloves, which protect the forearm would have helped. A cage/trap which allows transfer of the animal without having an officer having to handle the animal would help. LPC Comments & Recommendations: It has been decided that if animal is brought to the kennel after hours the officer will leave the animal in the cage for a kennel employee to remove. ON Description of Incident: Officer was turning left onto E/B Higgins from S/B Bond, a truck was turning N/B onto Bond from W/B Higgins and another vehicle was behind the semi. Officer attempted to avoid the vehicle, but the motorcycle struck the right rear of the vehicle. Repair of mUtUrcycle was $774.25. Other party did not contact EGV regarding repairs. Supervisor's Comments & Recommendations: Waiting for traffic to clear to provide an unobstructed view. Vehicle #157 hit the Expedition by the driver's side door. Vehicle #157 repair cost was$5,274.01, which was collected from Expedition driver's insurance. F/F was treated at ABMC. Total medical costs not yet known. Supervisor's Comments & Recommendations: Whenever possible, remove dog from room and secure it behind closed doors. Request police to handle the removal of animal. LPC Comments & Recommendations: Concur g, as Y A ' Description of Incident: During Aggression Control Training officer was taking K-9 of the bite when his left middle finger became lodged under the pincher collar causing a prong to cut his finger. Officer received 5 stitches. Total medical cost not yet known. Supervisor's Comments & Recommendations: It is recommended that officer use caution with his hand placement when taking his K-9 off of a bite during Aggression Control Training. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for December 6, 2007 Meeting Page 11 of 23 P-38 07 -07/59;-- PQLIGE DEPARTMENT :` 09/12/1x7: Description of Incident: Officer was assisting a 1046 motorist who had run out of gas. Officer while using a pocketknife made a funnel out of a cup and while closing the knife he accidentally cut the tip of his middle finger on his right hand. The cut needed 7 stitches. Total medical cost not yet known. Supervisor's Comments & Recommendations: The injury could have been avoided. Officer in the future needs to be more careful when handling a sharp edged knife. LPC Comments & Recommendations: Concur Description of Incident: Officer was responding W/B on Thorndale at 83 to an accident with injuries at Thorndale and Lively. Officer was proceeding through the intersection on a recd light usirry his emergency lights and sirens. He noted several semi trucks in the left turn lane, who had stopped, but as he proceeded his squad was struck on the rear passenger side bumper by a vehicle traveling S/B 83 in the center lane. The driver thought tie had a yellow liylrt. Repair cost, which was collected from other driver's insurance was $499.80 Supervisor's Comments & Recommendations: Unfortunately, this is a hazard of the job. Officer stated in his memo that he proceeded with caution through this large intersection. Several vehicle drivers at the intersection observed the squad and stopped. The driver of the striking vehicle was traveling very fast and only noted the squad just prior to the crash. Employee pulled into the nearby parking lot to wait for the driver, but she did not stop. They waited 5 minutes then began driving through the parking lot of the hospital and office complex to look for her. Driver returned to the area, stopped the employee and looked at vehicles. No damage to#821 and some scratches on the hood of the minivan and what appeared to be old damage. Zero cost for repairs. Supervisor's Comments & Recommendations: As there is no way to determine that the can came from the truck, the only action that could prevent this type of occurrence is to ensure that the materials on the truck are secured prior to travel. Employees routinely check their vehicles prior to travel. On the roof of this building were over thirty golf balls. Cost to repair windows is $995.00 Supervisor's Comments & Recommendations: We will replace the broken windows with a tempered glass, which will be stronger and the golf balls hopefully will not break the glass. LPC Comments & Recommendations: Concur Description of Incident: Arrestee was placed into an interview room within the processing area of the Police Department. Arrestee damaged the door handle by placing his entire weight downward on the door. This caused door handle mechanism to move downward and open the door. Handle needs to be repaired/replaced. $186 in damage to door handle. Supervisor's Comments & Recommendations: A heavy duty door handle would prevent further incidents. Description of Incident: An intoxicated subject was placed into the rear of squad #240 for transportation to the station. He kicked the driver's side rear window until it "spider webbed", but not broken out. Repair cost was $499 (uncollected since arrestee never showed up to court). Supervisor's Comments & Recommendations: No recommendation. Persons are transported in the rear of squad cars by policy and sometimes they will cause damage. Winkler to destroy the listed property, which was a black 30gb Apple iPod. ET waited the required time period, followed proper procedures and destroyed the item. Cost to replace$247.88 Supervisor's Comments & Recommendations: Evidence Technicians will re-check with Investigators and all officers about destruction of property prior to doing so. Description of Incident: While excavating to repair a leaking water main, the streetlight cable and duct were struck by the backhoe bucket. Estimate pending for repairs. Supervisor's Comments & Recommendations: No recommendations as the crew was following accepted methods and practices, but were unaware that the duct had shifted to that extent. Methods and practices are being re-evaluated by staff for re-digs. Repair cost is $408.20 Supervisor's Comments & Recommendations: Officer was advised to ensure the area behind his vehicle is clear before he begins backing. Officer was issued a corrective action reference the accident. LPC Comments & Recommendations: Driver Refresher Course Description of Incident: Squad //267 was parked unoccupied in the Quality Inn driveway at 100 Busse Road. Officer Meyers was driving squad#246 and backed into squad #267. Estimate pending. Supervisor's Comments & Recommendations: Officer Meyers should have checked the area surrounding his squad before initiating his maneuver. Estimate pending. Supervisor's Comments & Recommendations: Steve ha all safety lights, flashers and headlights on and sounded his air horn when Mr. Pumphrey backed into the leaf loader. Mr. Pumphrey was ticketed for failure to look both ways before backing up. This was not our driver's fault. He was doing his job the correct way as he has for many years. Unavoidable by employee. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for December 6, 2007 Meeting Page 18 of 23 F- 2D7: 71fi4 FZ!'RE DEPAR�MEtt RY 2 _ Description of Incident: Upon cleaning A7 after a motor vehicle accident, FF picked up an IV that appeared to be retracted but the tip was sticking out and scraped his fingertip on the right ring finger. There was patient's blood on the IV but no bleeding occurred from the scrape on finger. Medical cost pending. Supervisor's Comments & Recommendations: FF was wearing medical gloves and had taken precautions to limit exposure to possible blood borne pathogens. No blood was exposed from the scrape on finger. The accident was an isolated incident. Exposure protocol was followed as a precaution and for documentation. With careful scrutiny and knowing that the guard on the IV may fail, FF knows to handle any IV catheter as if it is not retracted. Furthermore, the rest of E7 and A7 crew are aware of the incident and will be mindful to take appropriate precautions. LPC Comments & Recommendations: Concur Description of Incident.- While ncident:While backing loader (unit#750) into parking space in garage, the tire of the loader hit the bumper of unit #824. Repair cost is still pending. Supervisor's Comments & Recommendations: Side mirror should have been readjusted to improve visibility when backing into the stall. Operator should familiarize himself with surroundings prior to backing into a stall. Lt. Naida blew the air horn for the crew, but did not see FF approx. 5 feet from the horn when it was blown. After 2 weeks the ringing in his ears had not gone away and FF was sent to ENT for an audiogram. Total medical cost not yet known. Supervisor's Comments & Recommendations: Lt. Naida reviewed the incident with FF and determined that before the air horn is sounded the area should be checked to make sure all is clear. Per Fire Chief, no use of air horn in building for any reason. Having thought the vehicle directly in front of his had turned right, Ofc. Homfeldt moved forward and struck the rear of another vehicle. Estimate pending for both vehicles. Supervisor's Comments & Recommendations: Officer will be more attentive to his surroundings when in traffic. Jump seat member got out to be a spotter. E10 struck the pilling mounted in the center of the drive on the front driver side of the engine during backing procedures. Our spotter was on the drier's side, but was unable to view the front of the engine due to the position the engine was in. Estimate pending. Supervisor's Comments & Recommendations: The property rep can place reflective tape or a higher marker on the post indicating the presence of the post. Truck was parked outside the garage door kitty corner and as he backed out around corner Frank hit the corner of unit#711 salt box in the rear. Minor damage was incurred to the auger box,which can be bent back in place. Minimal damage to be repaired in-house.. Supervisor's Comments & Recommendations: I believe if Frank would have gotten a spotter to assist him back out or maybe even peeked out the garage door to make sure there were not vehicles in harms way. Estimate pending. Supervisor's Comments & Recommendations: We all need to make sure that all doors are completely closed when we are finished in the gear lockers, especially on windy days such as today was.
