Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 3 of 18 Review of Current of Reports yy, q Description of Incident: E7 and A7 responded to a patient in arrest with CPR being performed at 901 Biesterfield. Crew began ALS protocols; CPR, cardiac monitor attached, IV line connected to existing port, drugs were given. A week later FD was informed patient was diagnosed and confirmed to have TB and members of E7 and A7 need to be tested. At no time was the crew given information of possible exposure problem. Medical costs pending. Supervisor's Comments & Recommendations: Again this possible exposure is an unfortunate hazard that faces all EMS providers. If the patients condition had been know,the appropriate level of personal protection would have been used. However, we cannot assume and be prepared for the unknown. No cost to Elk Grove Village. Supervisor's Comments & Recommendations: Officer acted in the correct manner. The autistic teenager was out of control to a point where ha had already scratched his mother while she was operating a vehicle. The parking lot was near a busy intersection and the officer had to help restrain the teen, not only for the teen's safety, but in anticipation of the arrival of the paramedics. After reviewing the incident, the injury could not be prevented. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 6 of 18 Kms- ix . 0-0 u.,rs a•. :. -� „'v a, IN Description of Incident: Officer was stopped in traffic S/B Busse at Arthur in the far right lane near the shoulder when his squad (#236) was struck on the driver's side door by a vehicle, which made an intentional right turn into his squad from the middle lane of S/B Busse. The officer suffered a left upper arm/shoulder contusion. Medical costs pending. Squad totaled. Supervisor's Comments & Recommendations: Officer had positioned his squad in preparation to affect an arrest of a subject who had fled and eluded police. As the subject's vehicle and the officer's squad were stopped in traffic S/B Busse north of Arthur Avenue,the subject intentionally accelerated towards the officer's squad and subsequently struck the driver's side door, causing injury to the officer's shoulder and substantial damage to squad#236. Officer was stopped and could not prevent his squad from being damaged after it was intentionally struck. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 7 of 18 NO Ott IF Description of Incident: Officer was driving squad #236 he located a subject in a vehicle who fled and eluded police. Subject was stopped in traffic in middle lane of S/B Busse north of Arthur and officer was in the far right lane when the subject accelerated towards the officer and intentionally rammed the driver's side door, causing extensive damage to the squad and injury to the officer. Supervisor's Comments & Recommendations: Officer Medina could not avoid the crash from occurring. The subject intentionally rammed Officer Medina's squad, which was stopped in traffic. LPC Comments & Recommendations: Concur a Description of Incident: While loading/unloading pallets of salt the corner of the tailgate got bumped and bent. Repaired in-house, no cost to the Village. Supervisor's Comments & Recommendations: Be sure truck is all the way back to the dock before loading or unloading. To be sure the person loading the supplies has placed the plate or dock leveler properly before driving onto the truck. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 8 of 18 Description of Incident: The employee was attempting to sit in her chair and slid off it sideways and landed on the floor. As she was failing she bent her right thumb back trying to brace herself and hit her head on the file cabinet behind her chair. The employee did not need medical attention. Supervisor's Comments & Recommendations: The employee must secure the chair behind her before she sits in it. LPC Comments & Recommendations: Concur Description of Incident: Zimmerman was westbound on Landmeier just east of Arlington Heights Road in the left turn lane. He was proceeding straight ahead on a solid green when the vehicle in front of him stopped in traffic. He attempted to stop but the roadway was covered in ice and he was unable to stop before striking the vehicle in front of him. $2,875 to repair other vehicle. Driver may file medical claim for injury to finger. Estimate for EGV vehicle still pending. Supervisor's Comments & Recommendations: When driving in adverse conditions one must take extra care while driving. The driver must adjust their speed and distance between vehicles to allow more time to stop. Description of Incident: Backhoe bucket struck and broke a plastic service line at 46 Avon. Per gas company representative, there will be no cost for repairs due to bad locate from the gas company. Supervisor's Comments & Recommendations: This incident is not avoidable unless the gas company locator correctly marks the service lines. Successful efforts were made to identify the location of the service based on the location marks by hand digging. Reasonable care was taken by the Village and there was no reason or indications to expect a service line other then where the old steel line was marked, located and found. LPC Comments & Recommendations: Concur < Y Description of Incident: It was discovered after the offender was removed from squad #236 that the door handle located on the inside rear passenger side was broken off. The offender was seated in the back seat for a ten minute period. The offender was later charged with criminal damage to vehicle. Repair cost $86.35 Supervisor's Comments & Recommendations: The officers on the scene said they were standing outside the vehicle and watching as the offender was seated in the back seat area. It is recommended that they be more watchful for potential damage. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 10 of 18 Description of Incident: While in Romeoville on a MABAS Dive the sonar unit was being used for an underwater search and while being raised to the surface it was discovered that one leg had fallen off the tripod. Repair cost is $155. Supervisor's Comments & Recommendations: Better ways to secure pins with duct tape or chem. tape or going to a nut/bolt system. LPC Comments & Recommendations: Concur F gg 'E3RR 3: Description of Incident: Officer placed a subject in the rear of his squad to stay warm while a search of a vehicle was being conducted. The subject pulled a radio cable wire out from the headliner and broke it. Repair cost $132.70 Supervisor's Comments & Recommendations: At the time the subject was placed into squad#241 and was not under arrest so therefore was not handcuffed. While the subject was in the car he was calm and with the exception of pulling on the headliner and pulling the cable out gave no advanced warning to the officers to believe there was a risk. Given the cold weather and the need to separate the subjects to complete the investigation it is my belief this incident was unavoidable. Description of Incident: Employee was closing an overhead door at the Fleet Service garage at 1635 Biesterfield. As he reached for the control panel on the wall he knocked over a 20-25 pound brake chamber off a plastic barrel and it landed on his right foot. His big toe on his right foot became swollen, sore and the nail turned purple. Employee was not wearing safety shoes. He continued with his assignments and took aspirin for pain for pain. Employee didn't require outside medical attention or time off of work. No cost to the Village. Supervisor's Comments & Recommendations: Employees must always wear proper protective steel-toed shoes when working, especially when in the shop area where there is heavy equipment, moving vehicles, heavy parts and tools. This type of work shoe is provided by the Village. Employee was instructed to ALWAYS wear his Village safety shoes while at work. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 12 of 18 Description of Incident: Farmer was putting salt in the dome at the State yard and as he entered the dome with an overloaded bucket, Farmer articulated to the right and doing so on the uneven ground forced the loader to tip onto two wheels, coming to rest on the side of the salt dome. Farmer admitted he may have had the bucket overloaded which made the situation more dangerous. Safety warning dome light was destroyed and repair cost is $662.49 Supervisor's Comments & Recommendations: To slow down and think about what you are about to do and how dangerous a piece of equipment like a loader can be if operated without the utmost care. LPC Comments & Recommendations: Concur Description of Incident: K-9 Tyson was left secured in his cage while officer was out of the vehicle (#248). Tyson was able to reach the headrest by putting his snout between the bars and chewed the headrest. Repair costs are pending. Supervisor's Comments & Recommendations: A heavy duty plastic should be secured to the cage bars and gate. The heavy duty plastic will have to have holes drilled in it to allow air to pass into the rear cage. This was done several years ago for K-9 Mike. A thin low quality plexi-glass will not be adequate. Also if Tyson can reach the edges of the plastic he will chew it,which could cause serious injury to the canine. In addition Officer O'Neill will continue to train in this area. Unknown object hit inside fender while plowing. Repair estimate still pending. Supervisor's Comments & Recommendations: Cannot be avoided. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 14 of 18 Description of Incident: Employee stated he was helping to move a piece of the garage door when he felt as if he possibly twisted his right knee. He let go of the garage door and fell against the file cabinet, which broke his fall and held him up so as not to fall to the ground. He seemed to become more sore after doing cleanup duty and notified his supervisor. Employee sustained a possible right knee sprain per ABMC. Total medical costs pending. Supervisor's Comments & Recommendations: Prior activities for the F/F that day were assisting with hand jacking of 300' of large diameter hose, primary interior search, interior ventilation, overhaul of garage roof and 2nd story exterior wall, overhaul of area of origin and he assisted with clean-up of scene. The garage door that we were moving was a standard residential two-car garage door. The door was intact on the area of origin, but off track and springs failed. Tower 7 used a K- 12 to cut the door in half and we removed the two sections from the area. Neither section of the door was excessive in weight, but it was rather large, bulky and not rigid. The four employees not only lifted the door, but also partially dragged the door from the area of origin. F/F stated that he felt as if he twisted his knee during our movements. The lighting was poor, the footing was poor and the housekeeping was poor. Although unfortunate, that F/F injured his knee, I do see this as an injury that is avoidable, but in the same breath I would say that this type of injury will happen because of the nature and environments that we must work. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 15 of 18 Description of Incident: While emptying the garbage at 901 Wellington the employee opened the lid on the dumpster, then the lid came down on his arm. ABMC diagnosed severe bruising and fracture. Needs surgery on right arm. Total costs pending. Supervisor's Comments & Recommendations: When dumping garbage, the lid on the garbage dumpster must be completely open. This way the lid cannot fall back down. LPC Comments & Recommendations: Concur Description of Incident: An officer who was going to use the camera noticed the battery compartment cover on the Nikon Coolpix 3200 Digital Camera was broken. Cost to replace is $112.71 Supervisor's Comments & Recommendations: Caution should be used in handling the camera. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for February 14, 2008 Meeting Page 16 of 18 01 ME, 0 . 0-Y 11, Description of Incident: Officer fired two shots from his carbine rifle to euthanise an injured deer in the roadway. He experienced ringing in his ears and subsequent dull pain and muffled hearing. ABMC performed an audiogram, which showed mile to moderate hearing loss. Prognosis is excellent for full recovery. Total medical costs pending. Supervisor's Comments & Recommendations: Removing the earpiece from his ear before firing the weapon. Since this was a non- emergency situation it is also recommended that ear protection be worn. Foam ear plugs can be placed in the patrol and supervisor vehicles. LPC Comments & Recommendations: Concur e� g asrc y Description of Incident: While pulling into rear of Station 7 in Unit #137, employee struck overhead doorframe with two right rear hubcaps causing the fake lug nut covers to come off and damaged right rear spotlight. Estimate pending. Supervisor's Comments & Recommendations: Move construction fencing to allow more room to turn into building. Per D/C Vrschek: Drive with caution. The fence has been in place for quite some time, without any previous accident. Description of Incident: Officer explained that he was parked completing paperwork and his metal clipboard was balanced partially on the dashboard and partially on the steering wheel. Officer advised that when he reached across his body for an item, his nee accidentally moved the steering wheel causing the clipboard to fall, striking the left shift key on the keyboard of the Panasonic Toughbook. Supervisor's Comments & Recommendations: Officer Williams was advised that he should not keep his clipboard on the dash of his squad. LPC Comments & Recommendations: Concur ups�i� �.ra _' � 's• '� � -� r Description of Incident: During patient transfer from bed to stretcher the plastic SPO2 sensor attached to the monitor was broken. Sensor was broken beyond repair. Cost to replace is $243.00 Supervisor's Comments & Recommendations: Remove sensor before patient transfer. LPC Comments & Recommendations: Concur Be more aware of this piece of equipment when moving patient. Unknown when incident actually occurred. Estimate still pending. Supervisor's Comments & Recommendations: This appears to be accidental damage and therefore would be unavoidable. LPC Comments &Recommendations: Concur u Description of Incident: Officer was attempting to handcuff an emotionally disturbed female. While doing so,the teeth on one of the cuffs scratched the officer's middle finger on his left hand causing minor injury. No medical cost to Elk Grove Village. Supervisor's Comments & Recommendations: The officer's minor injury could not be prevented because the female he was handcuffing was not cooperative and resisting.
