Review Current Reports: 31 Tabled Reports: F-41-07 Jorge Sigler. This was an unavoidable accident. FF Sigler was given no warning that the other vehicle would pull out in front of him. Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 5 of 29 P-07-08 OFFICER PATRAS — POLICE DEPARTMENT 02/05/08 Description of Incident: Officer was patrolling and made a right turn into the private parking lot at 572 E. Devon. Officer indicated that although he was driving slowly in the ice and slush covered lot, he struck a cement base of a light pole causing damage to the right side front and rear passenger doors. Supervisor's Comments & Recommendations: This is the Officer's first recorded crash involving a squad car. The Officer was counseled as to safe and proper driving. The Officer also received a Corrective Action Report for the damage to the squad. The Officer acknowledged his responsibility in the incident. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 6 of 29 FS-01-08 TOM MORONY— FLEET SERVICES 02/06/08 Description of Incident: While plowing the parking lot at 1635 Biesterfield, Tom bumped the rear bumper of squad #247 with the bucket of loader#717. The damage was a minor mark on the bumper and touched up at the Fleet Services garage. Supervisor's Comments & Recommendations: Tom stated that he accidentally missed the brake peddle while maneuvering close to car#247, which resulted in the accident. Although, if the parking lot is cleared of vehicles before plowing, this type of accident could have been avoided. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 7 of 29 IS-02-08 MARK ANGELLOTTI — PUBLIC WORKS 02/06/08 Description of Incident: Vehicle 2 was at the stop sign on Pratt turning N. bound onto Tonne. Vehicle 1 (718) hit a patch of ice and slid into the rear of vehicle 2 striking the rear of vehicle 2 with the plow, causing a minor dent and the rear window was blown out. Supervisor's Comments & Recommendations: I believe the intense snow storm and road conditions were a huge contribution to this occurrence. Employee possibly should have given himself more room to brake safely. However, the patch of ice was hidden under snow cover and could not be anticipated. I do not believe the operator (Mark) to be negligent in this accident, but I have spoken to Mark and advised him to double his stopping distance in hazardous conditions. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 8 of 29 LF-01-08 VICTOR MORALES — PUBLIC WORKS 02/06/08 Description of Incident: Vehicle 2 (722) was inside the turnaround, waiting to make a left hand turn onto JFK Blvd. Vehicle 1 (car) traveling east bound on JFK, approaching the turnaround, slid into the rear spinner assembly on the salt truck rendering it un-functional. Supervisor's Comments & Recommendations: No recommendations on the Village's part, our driver did nothing wrong to cause this accident. The driver of the car was cited for the accident; no insurance and a suspended operator's license. The report was taken at the Police Department station due to the number of calls due to the snowstorm. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 9 of 29 F-05-08 B/C SELLERS — FIRE DEPARTMENT 02/11/08 Reported b Description of Incident: A sub-tractor for the cell phone tower was trenching with a backhoe just North of the tower to install a 12" PVC pipe for a future cell phone line. He struck and cut through the lawn sprinkler system. They had used JULIE locating service but failed to check in with the Fire Department. (Triple "L" Excavating). Supervisor's Comments & Recommendations: Before excavation work begins, require the contractor to contact the property owner and ask if there are other hazards/utilities. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 10 of 29 GO-01-08 08/05 - FINANCE DEPARTMENT 11/27/07 Description of Incident: Employee bumped her right elbow on 11/27/08 while placing a box back on the pile of boxes to be destroyed when her right elbow came down on a metal furniture dolly standing by the boxes in the 901 Wellington-Village Storage Room. Employee did not report his incident to her supervisor until 2/15/08. Employee states pain has been getting progressively worse and that she cannot extend her right arm or close her right hand without pain shooting up to her elbow. Also says she is in pain when writing. Per Alexian, employee has "Epicondylitis" which is pain/tenderness to right elbow that can be caused by a significant single blow. Employee is currently being treated with medication & hourly "self' exercises prescribed by the doctor. Supervisor's Comments & Recommendations: Employee could have completed a work order for Public Works to bring the storage box to her desk, and then they could have returned the box to storage after she was done with it. The employee would have thereby avoided being in the storage room. If not, need to be aware of surroundings to be careful. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 11 of 29 P-08-08 OFFICER PELAYO — POLICE DEPARTMENT 02116/08 Description of Incident: Officer was backing Police squad car#233 North bound along west edge of front driveway at 1325 Pratt Boulevard towards the street after finishing an alarm call. The right rear corner of the rear bumper and the exhaust pipe struck a frozen snow bank. The bumper sustained minor damage and the exhaust pipe was pushed forward and sideways against the inner edge of the right rear wheel. The police squad had to be towed from the scene. Supervisor's Comments & Recommendations: It is always incumbent upon the driver of a vehicle to use all due care and caution when backing a vehicle. In this case there was more than ample room to safely back the squad out of the driveway and onto the street without coming close to any obstructions. There was also more than ample room to turn the squad towards the street without having to back up the entire length of the driveway. Officer Pelayo must use more care and caution when backing his police squad car in all future situations. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 12 of 29 P-09-08 OFFICER SACOMANO — POLICE DEPARTMENT 02/18/08 Reported b Description of Incident: Officer inspected vehicle 242 at the beginning of his shift and observed that there was damage to the lower portion of the front bumper on the left side. Officers listed on the drivers log were questioned about the damage and no information regarding what caused the damage was learned. Supervisor's Comments & Recommendations: Officer needs to be constantly aware of the surroundings, especially during inclimate weather. LPC Comments & Recommendations: Because it is unknown who was at fault, committee has no comment Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 13 of 29 F-06-08 08/06 — FIRE DEPARTMENT 02/19/08 Description of Incident: Employee was on the treadmill taking a hazmat stress test when he felt a cramp behind his left knee. The employee advised the doctor running the test and she ended the test. Employee diagnosed by Alexian as probably sustaining a left knee/leg strain/sprain although he may also have an inflamed cyst in the back of his knee. Supervisor's Comments & Recommendations: Test was performed under doctor supervision. LPC Comments & Recommendations: No comment FS-02-08 BRUCE ROBINSON — FLEET DEPARTMENT 02/21/08 Description of Incident: Employee was pulling the International Vactor truck over the truck hoist to perform repairs when the inside of the right front tire rubbed against the front hoist saddle. This action punctured a hole in the tire. Supervisor's Comments & Recommendations: Always have another person guide while driving a truck onto the large truck hoist. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 14 of 29 FP-10-08 08/07 - POLICE DEPARTMENT 02/28/08 Description of Incident: Officer was carrying evidence out of the garage, slipped and fell on a clear patch of ice on the driveway. Officer sustained an abrasion on his left knee. Officer's left pant leg was also torn due to the fall. Supervisor's Comments & Recommendations: Sgt. Jensen observed the clear patch of ice and reminded Officer to be aware of hazards such as snow and ice in his intended pathway due to the inclement weather. Sgt. Jensen salted the area where the fall occurred. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 15 of 29 P-11-08 OFFICER KELLY SWEENEY— POLICE DEPT 02/29/08 Description of Incident: Officer was traveling southbound on Arlington Heights Road north of Landmeier Road in the left lane. A deer ran across Arlington Heights Road. Ofc. Sweeney attempted to swerve around the deer, but struck it with the front passenger side of her vehicle. Supervisor's Comments & Recommendations: Accident was unavoidable, due to the actions of animal (deer) involved, which crossed the street in front of the squad. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 16 of 29 F-07-08 MELVIN STILLSON — FIRE DEPARTMENT 03/06/08 Description of Incident: While F/F Stillson was backing up and turning, Engine 10 (V1115) hit the refuse dumpster with the right front bumper. F/F Stillson had F/F Wind backing him. A/Lt. Tranchita was located in the officer's seat of Engine 10 (V/115). Engine 10 sustained a minor dent to the right side of the front bumper. This dent was approximately the size of a silver dollar. There was no apparent damage to the refuse dumpster. Supervisor's Comments & Recommendations: Driver needs to check all areas prior to moving vehicle. A/Lt. Tranchita needs to better communicate with his crew and needs to have a better knowledge of surroundings prior to moving any vehicle. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 17 of 29 F-08-08 08/08 — FIRE DEPARTMENT 03/07/08 Description of Incident: Employee was walking in the parking lot, slipped and fell on some ice that had frozen around a storm sewer drain. She sustained confusions to her left leg and complained of a sore ankle. Supervisor's Comments & Recommendations: A small amount of ice had frozen around a storm sewer in the fire admin parking lot. Employee failed to avoid the icy area, slipped and fell. The entire parking lot was clear of ice and snow with the exception of the sewer drain area. In the future, avoid areas of ice and snow when possible. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 18 of 29 F-09-08 LT. RICK GARCHIE — FIRE DEPARTMENT 03/07/08 Reported b Description of Incident: E-10 was working at the house fire at 259 Walnut Lane. We brought our Thermal Imaging Camera to the front of the house while we awaited an assignment. We entered the building and FF Reid took the camera out of the case and left the case on the front bumper of Quint 8. At the end of the incident, we were packing up our equipment and realized the case was missing. We were the last unit to leave the scene. I called all the other stations looking for the case, and no one had it. B/C Denna retraced Q8 route with no luck. Supervisor's Comments & Recommendations: Take the camera out of the case, hook it to someone's coat and leave the case in our vehicle. We were trying to protect the camera as long as we could. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 19 of 29 P-12-08 OFC. JOHN WILLIAMS — POLICE DEPT 03/08/08 Reported b Description of Incident: An arrestee, who was being housed in the north interview room inside the processing area, intentionally damaged the drywall. Supervisor's Comments & Recommendations: The actions of the arrestee could not be predicted and the incident could not be avoided. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 20 of 29 P-13-08 08/09 — POLICE DEPARTMENT 03/09/08 Description of Incident: Officer was investigating the scene of a Domestic Battery and Criminal Damage to Property incident. He was walking around inspecting the damage on victim's car, when he slipped on snow covered ice and fell to the ground. His left middle finger struck the ground and was severely injured by being twisted and bent after striking the ground. Supervisor's Comments & Recommendations: While walking around checking and observing the damage to the victim's car, he slipped on snow covered ice fell to the ground. The only way to prevent this type of injury is to be very observant of the environmental conditions, and try to walk in a manner that will.prevent a fall on icy ground surfaces. A police officer must go on crime scenes that pose slip and fall conditions at times and due caution must always be used in these conditions. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 21 of 29 P-14-08 08/10 — POLICE DEPARTMENT 03/12/08 Description of Incident: While assisting a non-compliant arrestee to the ground, Officer bent his left ring finger back. Supervisor's Comments & Recommendations: There were no unsafe actions that contributed to the sustaining of the Officer's injury. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 22 of 29 P-15-08 OFC. RANDAL SCHUMACHER— POLICE DEPT 03/12/08 Description of Incident: Ofc. Schumacher responded to assist Officers who were attempting to apprehend a battery offender who fled on foot through the residential area near Walnut and Cypress. The offender was running through yards and climbing over fences moving northbound through the area. Responding officers including Officer Schumacher closed the perimeter around the fleeing suspect until he was forced out of the yards in the area of Fern and Clearmont (208 Fern). Officer Schumacher used his vehicle to block the suspect's path by pulling across the parkway and into the yard at 208 Fern Drive. Officer Schumacher utilized his squad to block the path of the fleeing battery offender. Officer Schumacher drove his squad off the roadway, onto the parkway, and ultimately into the yard at 208 Fern Drive. Officer Schumacher's vehicle left ruts in the wet parkway grass as well as the resident's grass at 208 Fern Drive. Contact was made with the resident, Scott Stiff of 208 Fern Dr. EGV (847-344-4271) reference the damage. Mr. Stiff was not upset and simply requested the damage be fixed. Grass damaged in parkway and residents yard at 208 fern Drive, EGV. Police vehicle left ruts in the wet grass of the parkway and the resident's yard. Supervisor's Comments & Recommendations: Although Officer Schumacher's enthusiasm towards his job is greatly appreciated, he will be advised to utilize greater discretion while operating a Police vehicle. Though this incident resulted in no permanent damage, the possibility of injury or significant damage to property was evident. Officer Schumacher was issued a Corrective Action Report for his involvement in this incident. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 23 of 29 P-16-08 OFFICER JASON GOTTWALD — POLICE DEPT 03/12/08 Description of Incident: Officer Gottwald was checking the LaSalle Bank parking lot when he struck a curb, resulting in a flat tire. He added that he did not see the curb prior to striking it. Supervisor's Comments & Recommendations: Officer Gottwald was advised that he must exercise caution while operating a Police vehicle. He was also advised to take the necessary steps to minimize the possibility of a re-occurrence of this type of incident. It was noted that this is Officer Gottwald's first incident involving damage to a squad. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 24 of 29 W-01-08 08/12 — PUBLIC WORKS - UTILITIES 03/13/08 Description of Incident: While exercising underground water valve with valve key, felt tightness in lower back. Employee followed-up with Alexian Corporate Health Clinic on 3/14/08 & was diagnosed with "lumbar sprain". Supervisor's Comments & Recommendations: In the future, more warm up exercising is probably a good practice. The nature of exercising valves always required a lot of twisting and turning and force exeration when exercised by hand. In the future, obtaining a valve-exercising machine would be very beneficial, but some valves will always be exercised by hand. LPC Comments & Recommendations: Public Works had a valve-exercising tool. Do we still have it? Jack will check. Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 25 of 29 P-17-08 08111 — POLICE DEPARTMENT 03/15/2008 Description of Incident: Officer O'Neill had been assisting the Officer on a traffic arrest by having Canine Tyson conduct a search of a vehicle. As Tyson exited the car, he became startled by the Officer and growled at him. Officer O'Neill corrected Tyson and believed the incident was over. The Officer then approached Tyson to pet him. Tyson allowed this and then placed his paws on the Officer's arm to allow him to continue to pet him. Officer then began to scratch Tyson's jaw. When the Officer did this, Tyson began to growl and as he moved his hand away, Tyson bit the left hand causing an abrasion to the top of the hand. Supervisor's Comments & Recommendations: Officer O'Neill is still in the beginning phases of becoming a handler and continues to develop a relationship with his partner. It is imperative for Officer O'Neill to be constantly in control of his canine especially in situations where the canine can be in contact with a person. Officer O'Neill must be aware of the warning signs that his partner gives him that he is uncomfortable. In this situation Tyson has interacted with the Officer many times, and although Tyson initially growled at him when he was startled, once his handler told him the Officer was not a threat, Tyson allowed himself to be petted and in fact jumped into Officer's arms. This behavior allowed both Officers guard to be down. Officer O'Neill must remember that he must always be prepared to take control of Tyson in a moments notice. Based on the investigation it is apparent that Tyson does not like his mouth touched and Officer O'Neill must ensure that anyone handling Tyson not touch him in this area. Additionally, Officer O'Neill must take steps to ensure that Tyson is properly socialized by allowing him to interact with more people in control environments. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 26 of 29 P-18-08 OFC. M. MCGREAL-- POLICE DEPT 03/19/08 Description of Incident: Radar unit fell out of the dashboard mounted bracket and struck the "shift" and "A" keys of the Panasonic Toughbook keyboard. Officer advised that while making a left turn the radar unit mounted on the dashboard fell out of the bracket. Supervisor's Comments & Recommendations: Officers using radar units need to ensure that the units are properly secured in their brackets. LPC Comments & Recommendations: Jack Gray advised putting a bungee cord around the unit as a simple repair to take care of the problem. IS-03-08 UNKNOWN - PUBLIC WORKS DEPT 03/20/08 Description of Incident: There is a dent on the top of the lower unit's cab. Supervisor's Comments & Recommendations: Be more aware of your surroundings. LPC Comments & Recommendations: No comment Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 27 of 29 S-04-08 STEVE STORC — STREET DEPT 03/20/08 Description of Incident: Backing out of the rear garage hit the mirror on driver's side on the garage door opening causing the mirror and frame to brake off truck door. Supervisor's Comments & Recommendations: Be more aware of your surroundings. LPC Comments & Recommendations: Driver Refresher Course P-19-08 08/1.4— POLICE DEPARTMENT 03/25/08 Description of Incident: Officer was conducting training with his canine partner (Tyson) at the above listed location. After finishing a drug search exercise, Officer bent down to pick up the canine's leash, which was lying on the ground. The canine bit the Officer on his right hand. After a verbal and physical correction on the canine, it released the Officer's hand. However, the canine immediately went after Officer a second time, and bit his right hand again. Officer was finally able to control the canine and muzzle it. Supervisor's Comments & Recommendations: Predicting the actions of an animal is difficult. I recommend the Canine Program cadre review canine Tyson's history and performance to ascertain if changes are needed. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 8, 2008 Meeting Page 28 of 29 1-FS-03-08 ANTHONY QUICK— FLEET SERVICES 04/02/08 Description of Incident: While pulling into the lot with Fire dive vehicle #141, Tony misjudged the distance between his vehicle (#141) and vehicle #220 which was parked in the parking lot. The right rear of#141 scraped the right rear of#220. Supervisor's Comments & Recommendations: Be more aware of objects/vehicles around you when driving through a parking lot. Use the side mirrors and proceed with caution if distance is an issue. LPC Comments & Recommendations: Driver Refresher Course VM-01-08 RAYMOND RUMMEL— 04/02/08 VILLAGE MANAGER'S OFFICE Description of Incident: Upon leaving Village Hall for the evening, the lower left rear door of a parked Village vehicle was found to be creased. Based upon the damage, it appears to have been caused by a vehicle backing out of adjoining spot and striking the parked municipal vehicle. Damage involves a crease in lower portion of rear door. No paint was transferred from offending vehicle. Supervisor's Comments & Recommendations: No recommendation. Another driver struck parked vehicle. PATRAS — POLICE DEPARTMENT 04/11/08 Description of Incident: On 4/12/08 damage was found on the rear bumper of squad 231. The last driver of the squad was Ofc. Patras on 04/11/08. Ofc. Patras advised that he didn't cause the damage to the squad; however, he admitted that he did not do a thorough inspection of the vehicle at the start of his tour of duty on 04/11/08.
