The meeting was called to order at 8:40 A.M. at the Village Hall.

Emergency & Tornado Evacuation Plan given to all Department Heads and now in
effect. There may be a drill in July or August 2008.

The Committee raised the question regarding adding/deleting employees to the On The
Job Injury Reports. Jack Gray will follow up with Human Resources.

Description of Incident:
While plowing, the strobe light was struck by tree branches.
Supervisor's Comments & Recommendations:
Ensure that property owners properly maintain these trees or have Village personnel
trim the trees and back charge the property owner for the work performed.

Description of Incident:
On 2/15/08 at approximately 3:00 P.M., minor damage consisting of a scratch and
dent were located on the lower portion of the front passenger side door of squad 240.
The damage was documented in the squad damage report book.

Description of Incident:
On 7/11/08, in preparation of the move to the temporary Police facility, equipment was
boxed up. During the end of the year inventory, it was discovered that Trijicon ACOG
rifle scope (serial #11452) was missing.
Supervisor's Comments & Recommendations:
To avoid and/or reduce the chance of a reoccurrence of this nature, it is
recommended during the next move that the number of individuals involved in the
process should be reduced. This should help increase and maintain accountability of
the equipment. Adequate secured space should also be made available at the
relocated location to handle the storage of all the equipment so the chance of leaving
items unsecured are eliminated completely.

P-23 OFFICER MCINTYRE — POLICE DEPT 04/13/08
Description of.lncident:
Ofc. McIntyre was investigating a motor vehicle accident with a suspected intoxicated
driver. He placed the PBT (portable breath test) on the hood of his car and failed to
secure it, leaving it on the hood of the vehicle.
Supervisor's Comments & Recommendations:
In the future, taking in account any officer safety issues, Officer McIntyre should
secure his equipment immediately. Officer McIntyre could have simply placed the
PBT in one of his cargo pants pockets prior to placing the subject under arrest. Prior
to leaving any scene, officers must ensure that all equipment is accounted for.

F-10 SCOTT CORDOVA — FIRE DEPT 04/16/08
Description of Incident:
A-10 turned on to the access road between St. 7 and Walgreens. Construction crane
was parked in the.lane beside St. 7 so A-10 pulled up behind the crane. A-10 waited
for oncoming traffic to clear so it could pull around the parked crane and pass it. After
traffic cleared, A-10 began to pull into left lane. A-10 failed to clear a piece of steel
rigging that was hanging off the back of the crane. As the ambulance pulled forward,
the rigging scraped along the passengers side of the ambulance box just below the
roofline.
Supervisor's Comments & Recommendations:
Don't allow construction equipment to block the entrance to the rear of station 7.
Perhaps a wider turn would have been in order if the room on the street existed.

P-24 POLICE DEPT 04/17/08
Description of Incident:
Employee was walking S/B on the sidewalk parallel to Wellington Avenue heading into
the Village Hall 903 door from the parking lot when a motorist pulled over and asked
for directions. This caused some distraction and when employee put their right foot
down to stop and talk, employee's right foot was only half on the sidewalk causing the
right foot to quickly bend over to the right at a 90-degree angle. After some pain and
swelling started, the employee was sent for an exam and it was determined that there
was a small fibula fracture (of right lower leg).
Supervisor's Comments & Recommendations:
Check surroundings before walking and limit distractions if possible, also raise level of
turf to be more level with the sidewalk.

P-25 OFFICER TORRES — POLICE DEPT 04/18/08
Description of Incident:
Officer Torres had initiated a traffic stop on a vehicle on Devon Ave. in front of 1200
Devon EGV. He was out of his vehicle talking to the driver of the vehicle he had
stopped when he heard tires_squealing. He looked back to observe a pickup truck
strike the rear of his stopped squad #223. It was noted that Officer Torres had his full
emergency equipment activated at the time of the incident. Follow-up investigation at
the scene resulted in the arrest of the driver of the vehicle, which struck the rear of
squad #223; for drunk driving.
Supervisor's Comments & Recommendations:
Officer Torres properly positioned his vehicle to affect the safest traffic stop possible.
Officer Torres acted within Police Department protocol and violated no department
policy or procedure. Short of relocating the stop that was not practical in this incident,
there was nothing Officer Torres could have done to predict or prevent this incident.

F-11 AL VRSHEK — FIRE DEPT 04/22/08

Description of Incident:
A crack in the windshield .was noticed on April 22, 2008. The crack location is on the
lower right side of the windshield. It is unknown as to how the crack occurred.
Supervisor's Comments & Recommendations:
These types of damages to vehicles are difficult to impossible to prevent. The
condition of the roads this year are much worse than in recent past years. Small
debris becomes airborne very easily.

P-26 OFFICER PELAYO — POLICE DEPT 04/23/08
Description of Incident:
Officer Pelayo was backing westbound from Elmhurst Road onto the entrance drive of
1124 Elmhurst Road in order to park there and monitor traffic. While backing, the rear
of Squad 238 struck a steel pole along the edge of the drive.
Supervisor's Comments & Recommendations:
Instead of backing from the roadway onto the driveway, Officer Pelayo could have
driven forward into the parking lot and circled around to park on the entrance drive to
face the roadway.

W-2 MATTHEW VICTOR - UTILITIES 05/02/08
Description of Incident:
While backing 820 into parking stall (passenger side rear) made contact with driver's
side 818 front fender.
Supervisor's Comments & Recommendations:
If not sure of how much clearance he has when backing up, he could wait and have
someone guide him back.

P-27 POLICE DEPT 05/25/08
Description of Incident:
While attempting to apprehend an offender of a battery, employee lunged toward the
suspect as he ran away, causing a strain in the stomach area.
Supervisor's Comments & Recommendations:
This injury occurred in the performance of duty while attempting to affect an arrest.
No recommendations.

F-12 FIRE DEPT 05/19/08
Description of Incident:
Firefighter stated he/she didn't have any specific time or situation when the injury
occurred. The sudden onset of lower back pain occurred while performing station duties.
The lower back had become worse throughout the day. Firefighter came to me and
advised he/she felt he/she would be unable to perform duties because of the pain.
Supervisor's Comments & Recommendations:
Employee should try to protect his back from further injury and follow doctor's advice.

F-13 FRED HANSELMAN — FIRE DEPT 05/25/08
Description of Incident:
Firefighter dropped the camera while at the scene trying to get the camera to work.
The film compartment door will not stay closed.
Supervisor's Comments & Recommendations:
Hold on to the camera better and/or use neck strap.

F-14 JOSEPH MCENERY — FIRE DEPT 05/28/08
Description of Incident:
Responding to a call —while driving on the left shoulder to move past traffic, the
driver's side roof gutter was scratched by a road sign. Driver allowed for clearance of
mirror past post, but large overhead sign, being out of sight caused scratch to the
paint and rear chevron decal.
Supervisor's Comments & Recommendations:
Stop in traffic and wait for the light to change and traffic to clear. This would prevent
them from having to pass on the shoulder of the road.

S-6 KEVIN FIDORUK- PUBLIC WORKS 05/30/08
Description of Incident:
Kevin was cleaning off storm sewer grates that requires the tailgate to be in the down
or flat position for height reasons. As Kevin went back to the yard to dump when the
dump body went up in the air it came down against the trailer hitch and bent the top of
the tailgate itself. This has happened many times before so we are going to take off
the permanent hitch and put on a removable telescopic one.
Supervisor's Comments & Recommendations:
Remove the permanent trailer hitch on the truck and replace it with a mobile hitch or a
telescopic removable hitch so it can be removed when not hauling a trailer. This way
it will be out of the way of the dump body all together.

Description of Incident:
Village cell phone and leather case lost at theater or restaurant. Replaced at ($0) cost
to EGV because we had an extra phone & case— value of phone $75.00 — value of
case $25.00.

DOUGLAS SIMPSON — FIRE DEPT 06/01/08

Description of Incident:
While coming off the football field service drive and transporting a patient to the
hospital, the driver's side rear mud flap caught on the curb. The mud flap was torn
from its bracket.

P-23 KATHRYN ENDRE — POLICE DEPT 06/03/08

Description of Incident:
An unknown offender shattered the front windshield of the vehicle while it was parked
and unattended in the parking lot of Wickstrom Chevrolet. The damage occurred
sometime between 06/03/08 and 06/04/08. The vehicle was parked and unattended.
Supervisor's Comments & Recommendations:
This was a crime of opportunity. Roselle PD reports that two neighboring commercial
businesses also suffered from criminal activity during the same time frame.

P-29 POLICE DEPARTMENT 6/5/08
Description of Incident:
Employee was in training class at the Suburban Law Enforcement Academy at the
College of DuPage to learn about pepper spray. As part of the training, employee was
sprayed in the face area with the pepper spray by a training instructor to learn how it
affects a person. Employee then suffered from chemical conjunctivitis in both eyes
and also suffered a corneal abrasion in the left eye.
Supervisor's Comments & Recommendations:
Since this is a required part of a Police Academy Training class, this physical injury is
not necessarily avoidable. As a suggestion, the trainees may not need to take as
much of a "hit" with the pepper spray in order to learn how it affects people.
LPC Comments & Recommendations: Suggestion made by the Committee that in the
future, perhaps the Academy could use a diluted version of the pepper spray.
P-30 SGT. NAUMANN — POLICE DEPT Submitted on

Description of Incident:
Vehicle #258 was dropped off for service at the Meachum Public Works garage on
Friday, 06/13/2008. While it was in for service, the plastic front splash pan and lower
bumper was discovered cracked and loose. It was unknown when or where this
damage occurred.

P-31 SGT. HANSEN - POLICE DEPT 06112/08

Description of Incident:
Police squad #233 was in for monthly servicing at fleet services when it was
discovered the right front rim was dented. The mechanic servicing the squad stated
he visually inspected and also test-drove the squad indicating that at this time, the
contact damage did not extend beyond the rim. No apparent induced damage.

Driver needs to be more alert.

P-32 FRANK GOTTARDO — POLICE DEPT 06/14/08
Description of Incident:
Officer Gottardo did not notice any damage to the window during his vehicle
inspection at the start of his tour. Ofc. Gottardo returned to the PD, and when he
rolled up the front passenger side window, he noticed a small crack.

F-16 FIRE DEPT 06118/08
Description of Incident:
Employee was assisting in the packaging and lifting of a DOA when employee felt a
pull in the groin. Employee completed the task and once back at the station, noticed
discomfort. We took the engine OOS and took employee to the ER at ABMC for an
exam. The DR stated employee did not have a hernia and was able to return to work.
Supervisor's Comments & Recommendations:
Employee followed all proper lift techniques in this incident, this was unpreventable.

P-33 SGT. GASPARI — POLICE DEPT 06/17/08

Description of Incident:
Enough force was used when opening door to cause the handle to strike the wall,
creating a hole in the gypsum board.

Install a doorstop along the wall to prevent a reoccurrence.
