Municipal Complex Fire Drill: A fire drill was also held the first week of September. Online access will be available at a later date. New Business: Gallagher Bassett inspected the safety practices for workers on roof and ladders at the Municipal building. The outlet that the shop vac was plugged into was protected by the UPS. Apparently the initial inrush of current when the shopvac was switched on in conjunction with the existing power loads of the Water & Sewer SCADA equipment, public works network server, network equipment, wireless communication equipment, and voip phone equipment were too large for the UPS, rendering it inoperable. Supervisor's Comments & Recommendations: When outlets are protected by this type of equipment, some means to differentiate this protection is required. There needs to be visual cues and written warnings or some other form of protection to avoid overloading protective circuits. It is likely that the replacement unit will not be wired in a similar fashion, but if it is then explored appropriately, signage and warnings on the outlets and equipment. Supervisor's Comments & Recommendations: No recommendations LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 11, 2008 Page 4 of 17 F-17 08120 FIRE`DEPT Date reported: 7/3/08 Description of Incident: No specific incident or job task identified causing F/F double hernia. Examined by Physician, employee was diagnosed with double hernia. Supervisor's Comments & Recommendations: Though no specific act or instance can be singled out as causing the herniation that employee now has, it would be difficult to give any recommendation for actions. LPC Comments & Recommendations: Concur P-34, JOHN WILLIAMS - POLICE-DEPT 7/3/08 Description of Incident: While backing in the private drive at 220 S. Lively Blvd. Officer Williams struck a raised concrete barrier near the loading dock of the building. Damage was done to the passenger side front and rear doors, trim under doors, tire and rim. Supervisor's Comments & Recommendations: Officer Williams was advised to be more conscientious when operating his vehicle, especially while backing. LPC Comments & Recommendations: Driver Refresher Course i I Loss Prevention Committee Minutes for September 11, 2008 Page 5 of 17 F-18 BRIAN MCVEIGH — FIRE DEPT 7/5/08 Description of Incident: Tower 7 was returning from a detail at Station 8. While completing the turn in the rear of Station 7, the driver's side rear handrail made contact with the construction fence. Minor paint damage and a minor abrasion to the rail occurred. Supervisor's Comments & Recommendations: Require a spotter prior to the turn being initiated. LPC Comments & Recommendations: Driver Refresher Course BS-3 Reported By: 07/14/08 DUANE WESTBERG — BUILDING SERVICES Description of Incident: Water was leaking in, came down the wall and through parts of the ceiling onto our workbench where our cordless tools are kept. The drills were on the top shelf where most of the water came in. They got soaked with water; we let them dry for several days before trying them. They no longer work. Supervisor's Comments & Recommendations: MTI and EGV will attempt to minimize any future water infiltration into the existing complex. LPC Comments & Recommendations: Concur i i I i I Loss Prevention Committee Minutes for September 11, 2008 Page 6 of 16 BS-4 Reported by: 7/14/08 DUANE WESTBERG — BUILDING SERVICES Description of Incident: Truck (605) was lent to another division; when it was returned, it came back with the damage. This truck was in perfect condition prior to being lent out. Supervisor's Comments & Recommendations: This is a brand new vehicle which was permitted to be used by the Land & Forestry Division. They have denied causing the damage. However, whether or not they are aware of the damage, they should have taken responsibility for it. Therefore, because of this action, no recommendation can be made. LPC Comments & Recommendations: Concur P-35 OFC. RICHARD MEYERS — POLICE DEPT 07/15/08 Description of Incident: Officer Meyers was involved in canine training and the phone came off his duty belt and fell on the concrete. When he removed the phone from the holder he noticed that the battery case cover was broken. Supervisor's Comments & Recommendations: Ensure the phone is secured in its case on his duty belt or secured in a pocket. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 11, 2008 Page 7 of 17 P-36 STEPHEN O'NEILL—.POLICE DEPT .07115/08, Description of Incident: While driving southbound on 1-53 at 1-90, Ofc. O'Neill said he noticed a piece of steel in the roadway. He was unable to swerve to avoid running over the steel when the driver's side tires of Squad 248 drove over the steel. The front left tire went flat and there was severe damage to the rim. The rear left tire also had severe rim damage and the tire had lost some of its air. The piece of steel was reported to be about 3 feet in length and about 4 x 4 inches in width and depth. Supervisor's Comments & Recommendations: Attempt to be more attentive to the roadway and any objects in his path. LPC Comments & Recommendations: Concur PAA 08/21 - FIRE DEPT. 07/17/08 Description of Incident: The employee experienced an irregular heart rhythm and elevated blood pressure while at the fire station. Supervisor's Comments & Recommendations: Employee experienced some cardiac related symptoms while at work. F/F has been advised to follow-up with his/her physician to determine if he/she has any physical conditions that may affect he/she ability to perform daily tasks. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 11, 2008 Page 8 of 17 P-37 CHRISTOPHER TORRES — POLICE DEPT 07/17/08 Description of Incident: Officer Torres was climbing up a ladder to photograph a scene when the flash came loose from the hotshoe and fell to the ground. Supervisor's Comments &-Recommendations: Officer was advised to conduct a visual inspection of any equipment prior to deployment in the field. Additionally the Officer was advised to take any steps necessary to minimize the possibility of any future damage. LPC Comments & Recommendations: Concur F-20 08123 - FIRE DEPT 07/21108 Description of Incident: Employee was on a code 1-ambulance call and was aiding his crew with the movement of a large patient from the bed to the stretcher. Employee was stretching across the bed in an attempt to control the patient movement and as he/she stretched, felt a strain in the back and lower abdomen. Supervisor's Comments & Recommendations: On calls of this nature all personnel on the scene should be utilized and additional equipment and manpower should be called if the situation allows for the delay. Coordination of patient movement is critical which I was told occurred in this situation. Injuries of this type on these types of call are very common in emergency services. We should consider acquiring a nylon board for sliding patients. Similar to what the Hospitals utilize. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 11, 2008 Page 9 of 17 P-38 THOMAS DANAHY — POLICE DEPT -07/21108- Description of Incident: Officer was attempting to maneuver his squad between a parked truck and a garbage dumpster when passenger side of squad made contact with dumpster. Supervisor's Comments & Recommendations: Officer should have used alternative route to exit the parking lot. LPC Comments & Recommendations: Driver Refresher Course S-8 _SEAN MCNULTY (Seasonal Employee) 07/24/08- STREET Description of Incident: While backing up truck, Sean jackknifed trailer into rear of 758 hitting tool box with the corner of lift gate frame. Truck was not damaged, tool box was caved in. Supervisor's Comments & Recommendations: Chris Connelly was working with Sean and should have guided Sean when he was backing up. Sean should have made a wide U-turn when traffic was clear to do so. LPC Comments & Recommendations: Driver Refresher Course { I i i i Loss Prevention Committee Minutes for September 11, 2008 Page 10 of 17 SR-1 08/22 (SEASONAL EMPLOYEE) 07/25/08 WATER & SEWER Description of Incident: Employee was instructed in the proper way to disassemble and assemble barricades. While removing 3/8" bolt with a box end wrench and a '/2' socket wrench, the socket wrench slipped off the bolt hitting employee in the nose. Supervisor's Comments & Recommendations: Reviewed with employee safe and proper way to use hand tools on this assignment. LPC Comments & Recommendations: Concur P-39 CATHLEEN CARR — POLICE DEPT 7/30/08 Description of Incident: Cathleen Carr while driving vehicle 226, N/B Arlington Hts. Road at Oakton St. was involved in a rear end accident. Licensing Officer Carr's vehicle was the striking vehicle causing damage to the front of vehicle 226. Supervisor's Comments & Recommendations: Licensing Officer Carr should use more caution when operating her vehicle in traffic. The following distance was inappropriate and did not allow Licensing Officer Carr enough stopping distance to avoid contact. j I LPC Comments & Recommendations: Driver Refresher Course i I Loss Prevention Committee Minutes for September 11, 2008 Page 11 of 17 W-3 Reported By: Scott Nailor 8/5/08 PUBLIC WORKS DEPT Description of Incident: MDS Radio repeater for Sewer SCADA system telemetry was hit by lightning. Supervisor's Comments & Recommendations: Unavoidable LPC Comments & Recommendations: Concur P-40 08/30 - POLICE DEPT 8/06/08 Description of Incident: Officer reported that on 8/6/2008 after participating in physical training class during his/her assignment to the police academy, he/she experienced "tightness" in the lower back region of his body. Officer related that when officer reported this to the academy the officer was advised to call this department. Officer went on to state that he/she could not attribute the "tightness" to any particular exercise. Officer further informed me that he/she did not seek any medical attention and was now fine. Supervisor's Comments & Recommendations: Due to the fact that the Officer advised that he/she could not attribute this tightness to any particular exercise and had "cooled down" prior to experiencing this discomfort, no recommendation can be offered at this time. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for September 11, 2008 Page 12 of 17 I i P-41 Reported By: Stephen Schmidt 08/11/08 POLICE DEPT Description of Incident: While inspecting vehicle 260 a dent was noticed on the right front fender along with a broken side light lens. It is unknown when this happened, but damage may have been caused when an unknown object hit the vehicle while on the 1-290 a few days ago. Supervisor's Comments & Recommendations: Unknown when or where damage occurred. Always be aware of surroundings especially on expressway. LPC Comments & Recommendations: Concur G-02 08/24 - FINANCE DEPT8/12/08 Description of Incident: At front counter on high chair, got off chair and ran into open drawer. Resulted in large welt on upper log instantly. Supervisor's Comments & Recommendations: _Advised to keep drawers closed when not accessing them. LPC Comments & Recommendations: Concur I i i Loss Prevention Committee Minutes for September 11, 2008 Page 13 of 17 P-42 08/26 - POLICE DEPT 08/13/08 Description of Incident: Employee while exiting his squad, felt his left hip "pop" while he was turning to check traffic. Supervisor's Comments & Recommendations: In the opinion of this supervisor, there were no unsafe acts to unsafe conditions that contributed to this incident. The employee has no pre-existing condition and this injury could not have been anticipated or prevented. LPC Comments & Recommendations: Concur F-21 DOUG SIMPSON — FIRE DEPT 8/16/08 Description of Incident: While driving northbound on IL 53 just past Algonquin Road the tongue of the boat trailer bent on its own. The failure of the trailer tongue made the trailer not capable of being towed, because it was touching the road surface. Supervisor's Comments & Recommendations: I Lessen the weight on the tongue. Dive team needs a larger vehicle to carry our equipment. i LPC Comments & Recommendations: Concur I i I I Loss Prevention Committee Minutes for September 11, 2008 Page 14 of 17 I F-22 08/25 - FIRE DEPT 08/16/08 Description of Incident: E-10, vehicle 115 needed to be taken out of service. Vehicle 111 was being put into service. In doing so, 3 lengths of hose needed to be rebedded because it had water in it. While bedding the hose the employee was holding on to the engine with his/her right hand. Employee lost grip and stepped backward off of the engine and stepped right on a set of bunker pants and boots sitting on the floor. Employee lost his/her balance and fell backwards to the floor. Employee heard their knee pop and now had pain in the lateral part of the left knee. We called central and told them to assign E10 and A10 on a code 1 at station 10 for an injured firefighter and patient was transported to ABMC. Supervisor's Comments & Recommendations: A platform of some type that a firefighter could stand on would make the job of loading hose on the cross lays on the two older engines safer. If a step ladder is used you are too far away from the hose bed and need to lean away from the ladder to perform the task. This is not a safe position. A sturdy handhold might be the best idea at this point in time. These two engines are not long for the fleet and will be replaced as new engines are added. Moving the bunker gear may have also prevented this accident. F/F thought that just stepping backward off the engine from the running board when he/she lost their grip. It is possible that if he/she had a flat surface to step down onto, employee may not have been injured. LPC Comments & Recommendations: Concur i Loss Prevention Committee Minutes for September 11, 2008 Page 15 of 17 F-23 08/27 - FIRE DEPT 8/19/08 Description of Incident: Employee was training with a 1 3/ inch fire hose at 200 GPM when employee lost control of the nozzle. Lt. was behind employee and in his attempts to regain control of the hose both people fell to the ground. Supervisor's Comments & Recommendations: More hose line control work at lower pressures first and then work up to the higher pressures. Use control devices such as webbing or rope hose tools when working with higher pressures. Know an employee's.limitations and not exceed them. LPC Comments & Recommendations: Concur F-24 08/29 - FIRE DEPT 8/19/08 Description of Incident: F/F was operating the nozzle, in front of employee on a hose line. F/F lost control of the hose line and tried to reestablish control of the hose line and shut it down. He also tried to protect himself and anyone else close-to them. He.reached for the hose line, and took F/F and himself to the ground, impacting his left shoulder. Supervisor's Comments & Recommendations: Care needs to be taken during training to assure everyone's safety. More time needs to be spent reinforcing basics of hose line handling. LPC Comments & Recommendations: Concur I I i i Loss Prevention Committee Minutes for September 11, 2008 Page 16 of 17 F-25 08%28 FIRE DEPT 8/21.108 Description of Incident: While using charged hose line during drill. F/F overstressed right knee while handling the hoseline. Shortly thereafter F/F noticed knee was getting sorer as day progressed and notified his officer. As the day progressed further, outside of right leg from knee down became sore also. Supervisor's Comments & Recommendations: Some actions will cause knee to become sore from previous injury. LPC Comments & Recommendations: Concur P-42 Reported by: Officer Veronica Garcia 8/24/08 POLICE DEPT . Description of Incident: Greg Krasowski M/W DOB 06/21/82 punched security agent of Belvedere Banquets, Tim Kendre M/B in the face. In the process of being arrested, Krasowski damaged squad 234 by punching the hood of the vehicle. Krasowski had what appeared to be a seizure while in custody and treatment by FD who was already on the scene, then transported to AMBC for additional treatment. BAC was .204. Ofc. Marusich processed the vehicle and also took digital photos of the damage. I Supervisor's Comments & Recommendations: i Due to the nature of police work, we encounter numerous intoxicated, mentally unstable and agitated people on a daily basis. It would be impossible to predict how or when a subject may attempt this irrational criminal behavior again. STEPHEN O'NEILL - POLICE DEPT 8/25108 Description of Incident: Officer O'Neill had parked his take home squad car in his driveway. Sometime between 2300 hours on 08/24/2008 and 02:30 hours on 08/25/2008, unknown offender(s) spray-painted all four sides, trunk, hood, and windows of his squad car. Supervisor's Comments & Recommendations: Although the vehicle was parked near a yard light, it is parked toward the end of the driveway and can't be seen from Officer O'Neill's residence. I would recommend that the vehicle be either placed in the garage or if that is not possible to have it parked as close to the home as possible. If possible, some type of motion light would also serve as a deterrent. LPC Comments & Recommendations: Concur I i i i i I I
