SUBJECT: Loss Prevention Committee Meeting Minutes — November 13, 2008
The meeting was called to order at 8:32 A.M. at the Village Hall.

The minutes were approved for the meeting of September 11,'2008
Review Current Reports: 18 Reports were reviewed.
Tabled Reports: None
Old Business:
The enhancements have not been made to the "On the Job Injury Entry Screen". Diane
Hunter is working with Janet Bean on this issue.
New Business:
Committee members received the latest version of the Elk Grove Village "Emergency
Preparedness Plan" brochure. LPC members would like all employees to receive a
copy. Scott Miller will look into this matter.
The Driver Refresher Course scheduled for November 19th has been cancelled. Jack
Gray looking into an online Driver Refresher Course to take the place of the current
Course due to overtime cost issues.

Review of Current of Reports
W-4'. JIM_:SCHAUDEK— UTILITIESIWATER 01/21/08
Description of Incident:
Report filed on 9/26/08 because of recently received bill from AT&T for damages to.
phone line located at 201 Tower Lane on Charing Cross side. Started excavation for
leaking house service repair at 201 Tower at approximately 8:00 AM on 1/21/08.
Removed 2 squares of sidewalk, made the proper repair. There were no damages
observed to any utilities during the repair operation. The excavation site was
backfilled with grade 9 gravel at approximately 1:30 PM. Not sure of the size of the
excavated area.
Supervisor's Comments & Recommendations:
The indications are we did not cause damage during excavation. Excavation was
done in the general area of the cable damage. There is remote possibility while back
filling the hole a chunk of frozen gravel may have clipped the line.

F-26 08/31 — FIREDEPT: 08/27/08
Description of Incident:
While providing patient care in back of ambulance 8, employee was given direction for
a procedure by ARMC. Employee was using a 60 cc syringe to give patient a fluid
bolus. While accessing an IV port, the needle went through the port and punctured
employee's left thumb.
Supervisor's Comments & Recommendations:
If this procedure were to be used again, employee was advised to either use a blunt
needle or have the ambulance driver stop the vehicle in a safe area.

Description of Incident:
While excavating for a b-box repair, cut through an un-marked AT&T phone cable.
Supervisor's Comments & Recommendations:
Unavoidable as the line was not marked.

F-27 WILLIAM.KNIPP FIRE:DEPT 09/01/08
Description of Incident:
Unknown how incident/damage occurred. Dent on the top of the right front quarter
panel.
Supervisor's Comments & Recommendations:
On 9/1/08 at approximately 0845 AM, F/F Knipp, F/F Nejedlo and F/F Reid were
washing A10 (124). During which time a dent was detected on top of the right front
quarter panel. The vehicle had not been driven or taken out of the station yet that
shift. Nothing was on continuity from the previous shift in regards to the damage. Lt.
Gutzmer was notified of the damage and F/F Knipp completed a driver inspection form
that was forwarded to PW.

Description of Incident:
Officer advised me that on today's date, he and Officer O'Neill were participating in a
K-9 demonstration at Christus Victor Church explained that during the demonstration
he was wearing a "bite suit" taking a bite from "Kim" when he rolled his ankle due to a
dip in the grass. The officer related that he did not realize the extent of his injury until
he returned home and removed his boot. The then observed that the affected area
was swollen and black and blue.

Officer did not carefully study the terrain he was standing on and did not see the dip in
the grass.

Description of Incident:
Employee was walking up the stairs to do the mail when she lost footing with her right
foot. Employee hit the granite stairs with her left upper thigh hip and twisted right
knee. Her left shoulder and face hit the glass plating under the handrail. Lip was
swollen/cut from impact with glass. Tooth possibly loose.

Employee was walking up the stairs when she lost her footing. Nothing could be done
to prevent injury.

Description of Incident:
Office flooded over the weekend. IDSD computers not operating (replacement of
motherboard and memory cards required).

Description of Incident:
Due to heavy rain, and all sewers were backed up, the sanitary.sewer line backed up
into the building (901 Brantwood, Fire Admin Bldg.).

The rain came down so fast and heavy, all sewers, and sewer lines backed up fast.

Description of Incident:
Officer was re-locating a barricade. While he was removing it from the trunk of his
squad car (#235), he banged his left wrist. Officer was advised that at this time he did
not believe the injury was serious enough to warrant medical attention, however he
wished the incident documented.

P-46 OFC. KEVIN FINNEN_- POLICE DEPT: 09/14708
Description of Incident:
Ofc. Finnen was driving E/B on Cosman approaching Charles in adverse weather
conditions and poor lighting.. He (Squad 238) entered what turned out to be a large
body of water, which he was unable to detect, as there was no warning of the
impending hazard. As he was transverse to this water hazard, Squad 238 stalled and
had to be towed from.the scene to the Meacham Garage.
Supervisor's Comments & Recommendations:
In the future, Ofc. Finnen should use due caution when operating a motor vehicle in
adverse weather conditions. It is imperative that he operates a motor vehicle within
his ability to safely perceive potential hazards.

Description of Incident:
Officer Moody advised that on 09/15/08 he could not get the Panasonic computer to work
(power issue). Squad 223 was written up for repair. On 09/17/08 Fleet services advised
that the Panasonic Toughbook battery was missing from the computer.
Supervisor's Comments & Recommendations:
Employees are to be reminded to lock the police vehicles at all times to reduce the
opportunity of theft.

P48 OFC. PAUL SYVERSON POLICE.DEPT 09/19/08
Description of Incident:
Ofc. Syverson while traveling on the north apron of 1600 Oakton with Squad 239,
struck a log which was extending over the curb line causing damage to the drivers
side front quarter panel.

Officer was counseled regarding the safe operation of a police vehicle during
assignments.

P-49 OFC. MIGUEL MOSQUEDA— POLICE DEPT 09119%08
Description of Incident:
Ofc. Mosqueda related he was driving Squad 239 northbound on Elmhurst from Arthur
in the curb lane of traffic. Ofc. Mosqueda was preparing to change lanes and stop a
vehicle for a traffic violation he observed in the median lane of traffic. While his
attention was drawn to the violators in the median lane of traffic, the vehicle in front of
him stopped suddenly. Ofc. Mosqueda was not able to stop his Squad 239 before
making contact with the rear of the car in front of him. The damage to both vehicles
was reported to be minor.
Supervisor's Comments & Recommendations:
Observing traffic violators is an important aspect of this job. However, minor traffic
violations should be weighed against the risk created to enforce such violations.
Accidents of this nature can be avoided by officers taking an extra second to scan the
area around their squad car. This would allow for an extra margin of safety by
allowing the officer to process their surroundings for possible traffic hazards or threats.

Description of Incident:
After Inv. Mason finished using his phone, he attempted to clip it to his belt when it fell
to the floor. He then discovered that the antenna was broken.

Ensure the phone is properly clipped in place.

Description of Incident:
Heart monitor was cut by ABMC ED staff while leads were on a full-arrest patient and
PT was transferred to the hospital ED bed vehicle CPR was in progress.

If allowed, FD members pull the leads from patient before staff cuts off shirt.

Description of Incident:
Employee was helping strap the patient in on the stretcher when she spit in his face.

Wearing personal protective equipment is the safest thing for the employee. It is not
always the best thing to make the patient feel comfortable.

Description of Incident:
Request was made to fix dead battery on vehicle. Later informed that there was a
scratch on front drivers side fender, drivers side passenger door, and underside of front.
I've been informed that the scratch on the drivers side passenger door occurred
sometime ago, a Police Report was filed, and that the scratch was suppose to be
repaired. Also informed that the scratch on the underside of front occurred some time
ago as well — before R. Rummel had possession of the car. M. Roan was unaware of
any new scratches.

Description of Incident:
Officer twisted his left foot during a foot pursuit of a murder suspect. After a vehicle
pursuit, the suspect ran from his vehicle down a steep hill. Officer gave chase,and
twisted his left foot in the process.
