The meeting was called to order at 8:40 A.M. at the Village Hall.

Old Business:
Driver Refresher Class was held with 12 employees attending on December 17, 2008.
Jack Gray is working with Tony Konetzke regarding an online course for the Driver
Refresher Class.
New Business:
• Public Works "Flagger" training scheduled for February 27, 2009
• Public Works "Safety Expo" scheduled for March 12, 2009
• Public Works "Flagger" training Session 11 scheduled for April 2, 2009

BS-6 Duane Westberg — Building Services 10/13/0`8
Description of Incident:
About 325-350 feet of fencing damage by Vandals.
Supervisor's Comments & Recommendations:
Re-install fence using steel raps, instead of aluminum raps.

P-51 Officer Myles Greenberg — Police Dept. 10/29/08
Description of Incident:
While conducting an inspection of squad car #230 at the beginning of his shift, Officer
Williams observed that the right (passenger) side front tire rim was bent. Subsequent
to reporting the damage to me, I observed that the damage was consistent with hitting
a curb. It was also noted that the damage was difficult to see and was dependant
upon the position of the wheel. A check of the driver log was conducted and the
initials "MG" was the last entry. "MG" is believed to be Myles Greenberg from the
second shift.
Supervisor's Comments & Recommendations:
Occasionally vehicles will suffer wheel damage during normal police activity. Officers
are to be reminded to inspect their vehicles and report any damage to their supervisor
to ensure safe operation.

P-52 Officer Johnnell Williams — Police Dept. 10/31108
Description of Incident:
Officer was traveling westbound Biesterfield Road with emergency equipment
activated. Officer slowed prior to entering intersection. Officer proceeded through
intersection and was struck in the right/rear quarter panel by a vehicle traveling
southbound Rohlwing Road.
Supervisor's Comments & Recommendations:
.Though the Officer was operating his vehicle within department policy and procedures,
he was reminded to take any steps necessary to promote the maximum possible level
of safety for both himself an the community.

P-53 Officer James Chmelik — Police Dept. 10/31/08
Description of Incident:
Ofc. Chmelik, acting on radio traffic regarding the pursuit of a homicide suspect, was
westbound Biesterfield approaching Rohwing. The pursuit was east bound on
Biesterfield approaching Rohlwing. He advised, his intention was to clear the
intersection so the pursuit could make it safely through. After the pursuit cleared the
intersection, he would assist in the pursuit if needed. Ofc. Chmelik operating Squad
241, entered the intersection, stopped and turned on his emergency lights. At almost
the same instance, a vehicle he just passed made contact with the rear of his squad.
Supervisor's Comments & Recommendations:
Officer Chmelik should have turned his lights and siren on, as it may have given the
motoring public a chance to yield the right of way to him.

W-6. Scott Bernholdt — Public Works 11/03/08
Description of Incident:
Unknown part or parties damaged the fence between the period of October 31st &
November 3rd. Police Department report number 08-18687. It appears that someone
with possibly a trailer backed into the fence bending several poles and rails, when they
pulled away it appears the fence material was caught on the vehicle which resulted in
the fence material being pulled off of almost the entire south side of the fence. The
company to the south reported the damage in the morning of November 3, 2008.

H-2 Michael Cavallini — Health & Comm. Services 11/04/08
Description of Incident:
Village cell phone charger is assigned to Health and Community Services Director to
ensure cell phone is available during periods away from the Village. Director's
personal vehicle was broken into and Village cell phone charger was stolen during
theft. The vehicle was parked in the Pavilion parking lot during normal Village
business hours..
Supervisor's Comments & Recommendations:
Ensure personal and Village vehicles are locked properly at all times.

F-31 08137 — Fire Dept. 11/04/08
Description of Incident:
A monthly drill that consisted of supplying the arial with a large supply line from an
engine. There is a short 10" piece of 5" hose that is laid on the ground behind the
tower to facilitate this process. When the hose was charged with water, this piece of
hose had kinks in it. The F/F saw this and went to straighten out the hose. When he
did this the hose rolled over onto his foot and knocked him off of his feet. The hose
was still on his left foot when he fell down and this caused him to twist his ankle.
Supervisor's Comments & Recommendations:
This hose is very large and heavy. It weighs about 10 pounds per foot when filled with
water. The fact that it can move needs to be understood by everyone in the
department. I had just finished explaining this fact to two of our new firefighters when
this accident occurred. The F/F expressed to me that he did not know that the hose
could move this far this fast. We- need to make sure that everyone is aware of what
can happen with LDH.

E-1 Bruce Barnett— Community Development 11/04/08
Description of Incident:
Minor scratches on paint on rear bumper.

Since vehicle was at a complete stop, the accident could not have been avoided by
the village vehicle.

P-54 Officer Thomas Krejczyk — Police Dept. 11/04/08
Description of Incident:
Officer Krejczyk while driving police squad #242 was attempting to pull into a marked
parking space to the rear of 321 Bond St. when the right front end of squad #242
came into contact with parked unoccupied police squad #268.
Supervisor's Comments & Recommendations:
Officer needs to be more attentive while operating a vehicle.

F-32 James Denna Sr. — Fire Dept. 11/11/08
Description of Incident:
Battalion Chief Denna.was responding to a motor vehicle accident at Arlington Heights
Road and Shelley in the northbound lanes-, with lights and siren. As Battalion Chief
Denna approached the accident, the northbound lanes were stopped. There were
police vehicles stopped ahead in the southbound.lanes. Battalion Chief Denna pulled
into the southbound lanes in order to approach the scene. Just south of the scene,
Battalion Chief Denna's vehicle was struck in the rear bumper by a driver that was
attempting to make a U-turn from the northbound lanes into the southbound lanes.
Supervisor's Comments & Recommendations:
Very difficult to state that this could have been prevented by Battalion Chief Denna. It
could have been prevented if the vehicle attempting to make*a U=turn that struck the
Battalion Chiefs vehicle had complete control of their vehicle.

LF-2 Edward Garry _ Public Works 11/18/08
Description of Incident:
Mr. Garry was removing small stumps from the parkway and restoring the damage.
He was in the small end loader 717. Ed was sitting still against the curb waiting for the
strap to be brought to the stump site while the woman driving a Saturn Vue backed
out of her driveway and proceeded to pull forward and struck the bucket of 717 with
her drivers side front quarter panel. There is absolutely no damage to our equipment.
After interviewing both parties, Ed said when she got out following the contact, she
explained how it was her fault, and her windshield was icy.

Nothing on Ed's part and I cannot speak for the driver of the other vehicle.

Description of Incident: .
Damage to the LED lens and possible internal electrical damage.

Instruct F/F to be more careful with meter and use strap when possible to prevent it
from falling.

P=55 Inv. Sheara Garrison —Police Dept. 11/21/08
Description of Incident:
The driver of a vehicle on which this device was mounted discovered its existence and
destroyed and discarded it.

Ensure solid magnetic contact between device and vehicle.

P-56 Officer Russell Sullivan — Police Dept. 11/22/08
Description of Incident:
On 11/22/08 at 14:30 hrs in the rear parking lot of 321 Bond Street, Officer Sullivan
was placing the Less Lethal shotgun with case into the trunk of squad 243, when the
case accidentally struck the passenger side rear red tail-light cover subsequently
cracking it.
Supervisor's Comments & Recommendations:
Officer Sullivan has to be careful and take his time when placing large items into the
trunk to avoid causing damage to the squad.

S-9 Steve Storc — Public Works 11/25/08
Description of Incident:
While collecting leaves with the leaf loader trailer and maneuvering around a parked
vehicle, the employee misjudged the space between the loader side broom causing
,the side broom to strike the bumper and a piece of trim on the side of the vehicle.
Supervisor's Comments & Recommendations:
While it is understood that operating this time of equipment takes a great deal of
concentration and focus, the operator needs to recognize and ensure that every effort
is made to avoid damage to property, both public and private, at all times. The
operator has safely completed this action numerous times prior to, and subsequent to,*
this incident and fully understands the importance of safety and accuracy to avoid
repeat incidents.

F-33 F/F.Lodewyck — Fire Dept. 11/28/08
Description of Incident:
While hanging Christmas lights at Sta8, Aerial Ladder struck left rear spotlight and
broke handle. .Light itself still works.

Remind operators of possibility of striking outside add on spotlights.

P-57 Officer Efren Pelayo— Police Dept. 1:1129/08
Description of Incident:
Officer Pelayo began his tour of duty by completing a squad inspection of squad 240.
During the inspection, Officer Pelayo noted that he had possession of the transmitter.
At the end of his tour he noted that he misplaced the transmitter. Officer Pelayo made
two traffic stops during the shift. Video was reviewed and it was noted that there was
no audio on either stop. It is believed that Officer Pelayo lost the transmitter between .
his squad inspection at 1430 hours and 1701 hours, the time of his first traffic stop.
Supervisor's Comments & Recommendations:
Officer Pelayo will be sure that all department equipment is secured to his person in a
manner that will avoid the loss of said property.

P-58 Officer Gary Rosko — Police Dept. 12102%0$
Description of Incident:
Officer Rosko was making a left turn from E/B Higgins onto N/B Bond St. A red 2001
Ford F-150 pick-up IL/Reg 32407N(B) was stopped at the stop sign of S/B Bond and
Higgins St. The pick-up proceeded forward to make a left turn onto E/B Higgins and
made contact with the left front wheel area of the squad.

P-59. 08/38 - Police Dept. 12/02/08
Description of Incident:
Officer was making a left turn from E/B Higgins onto N/B Bond St. when his squad
was struck by a vehicle trying to make a left turn from S/B Bond St. onto E/B Higgins.
Supervisor's Comments & Recommendations:
Officer was properly operating his squad when it was struck by another vehicle.

P-60 Officer Kelly Sweeney — Police Dept. 12/04/08
Description of Incident:
Prisoner placed in booking area interview room used a cigarette lighter to damage the
wall surface area and one ceiling tile.
Supervisor's Comments & Recommendations:
Proper searching of prisoners would have prevented the damage. Prisoners should
not have stated item in unsupervised room.

Description of Incident:
During a medical call, with a combative seizure patient, the plastic sensor was
stepped on and broken.

P-61 08/39 — Police Dept: 12/1.5/08
Description of Incident:
Officer was investigating an on-station vehicle crash. He went outside to observe the
extent of the damage to the vehicles. While stepping off of the curb, Officer slipped
on the ice, falling on his left elbow, left hip, and left side of his back.
Supervisor's Comments & Recommendations:
Additional caution and care should be taken during and after periods.of adverse
weather condition that cause icing.

P-62 08/40 — Police Dept. 12/17/08
Description of Incident:
Officer was walking from squad to crime scene and slipped on ice packed sidewalk,
falling to ground.
Supervisor's Comments- & Recommendations:
There is no action that could have prevented this injury.

F-35 08/41- Fire Dept. 12/21/08
Description of Incident:
While exiting E7 upon arrival at ambulance call stepped off and slipped on the ice and
injured his right elbow and shoulder. After patient care was completed, E7 drove him
to ER.
Supervisor's Comments & Recommendations:
During winter with snowfalling and icy conditions with temperatures below freezing, we
need to remind everyone of possible hazardous foot placement.
