A vehicle traveling Northbound on Lively Blvd. sliding on the ice - and hitting the plow blade of 720. Supervisor's Comments & Recommendations: When salting intersections in these situations, travel North to South and North end and avoid U-turns whenever possible. LPC Comments & Recommendations: Driver Refresher Course 5=11 08/42 STREETS - 12/15108.- Description of Incident: Employee was salting Lively when a car hit him in the front plow carriage. Employee complained of neck stiffness and went to the clinic. Supervisor's Comments & Recommendations: Employee made a U-turn. In the future, plow and salt through intersections then turn around to the opposite lane. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 5 of 23 F=37 08/43 =FIRE DEPARTMENT 12/25%08 Description of Incident: Firefighter lifted an unresponsive patient from the floor with the aid of firefighter. The injury occurred at this time. Firefighter was assessed at the ABMC ER and was advised to visit Occupational Health on 12/26/09 for reassessment. Supervisor's Comments & Recommendations: A physical fitness program emphasizing core strength would help to prevent a back injury. However, a situation such as this is a common occurrence in EMS. I.e., awkward lifting positions, poor handholds, restricted space to properly lift, and emergency time. LPC Comments & Recommendations: Concur P-63 : OFC. TEV&ROHMAN P-_OLICE DEPARTMENT 12/25/08. Description of Incident: While Officer Rohman was closing the trunk on Squad 235, after retrieving the AED, the edge of the trunk struck the box causing the molding on the trunk to break. Supervisor's Comments & Recommendations: Officer Rohman has to make sure that when he removes items from the trunk, those items are clearly out, so when he closes the trunk he will not cause damage to the items or the trunk. Burn marks on trunk from flammable liquid. Supervisor's Comments & Recommendations: Department vehicles are temporarily parked during the week at the Pavillion parking lot due to the construction at the Village Hall. Once construction is completed, the vehicles will be parked behind the Police Station which will be a deterrent in itself. Other prevention alternatives would be to find a more secure location to park the cars, more police presence and monitoring of the area, car alarms on Village vehicles and surveillance cameras in the parking area. LPC Comments & Recommendations: Concur E3 _ PAP ER COMMUNITY DEVELOPMENT 12129/08: Description of Incident: Vandalism. Burn marks in area of gas cap. Lost bracket and plate. Supervisor's Comments & Recommendations: Ofc. Thomas stated that he did not notice the front license late missing during his squad check at the start of shift due to the squad being parked in close proximity of another squad making it difficult to see the front bumper. Ofc. Thomas was advised to make sure he makes a more thorough check of his squad in the future. LPC Comments & Recommendations: Concur P=2 SGT JAMES=LANCASTER POLICE DEPT. 01/07/09:' Description of Incident: Arrestee threw a chair at the interview room door causing two dents. Supervisor's Comments & Recommendations: Officers should have removed chairs from the interview room. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 8 of 23 P=3 OFC_ ERIC=PERKINS - POLICE`DEPARTMENT:. 01/08/09. Description of Incident: Officer Perkins was backing squad #239 from a parking space in the parking lot of 286 E. Devon, when he struck a snow bank which had iced over. Officer Perkins explained that he did not see the portion of the snow bank he struck because the height was lower than the level of the squad trunk. The collision caused the protective paint layer on the left (driver's) side of the rear bumper to crack and fall off. (for information, there was prior damage of a similar nature reported in December of 2008 in the same area of this squad). Supervisor's Comments & Recommendations: Officer Perkins should have exercised more caution while backing up and should have been more aware of his surroundings. LPC Comments & Recommendations: Driver Refresher Course FS 1 = DEREK STURGULEWSKI :FLEE-T SERVICES 01/09/09;: Description of Incident: Derek backed #705 into a snow bank by the Fleet services wash bay door and bent the rear aluminum bumper. I Supervisor's Comments & Recommendations: Always have a spotter when backing around obstacles/vehicle or visibility is poor. Supervisor's Comments & Recommendations: Have someone clear snow piles from side of building so truck can fill up as needed. Have someone or yourself check when moving in areas unsure of. LPC Comments & Recommendations: Driver Refresher Course P-4 SGT MICHAEL GASPARI POLICED PT01/15/09= Description of Incident: The door to the battery compartment of this digital camera was discovered to be broken. It will not remain shut and the camera will not operate unless the battery door is completely shut. It may have occurred by the camera being bumped or dropped, causing a small plastic clip to break off. It is unknown exactly how or when this incident occurred. Supervisor's Comments & Recommendations: Obtain a protective case to enclose the camera to avoid damage. Description of Incident: While plowing inside the gates & pumphouse #7 employee backed into concrete retaining Wall. Supervisor's Comments & Recommendations: Employee needs to be aware of surroundings and use caution when plowing village water and sewer facilities. LPC Comments & Recommendations: Driver Refresher Course S=1 -STEVESTO,RC -.STREETS. 01/16/09. Description of Incident: Backing up #719, plow hit front right fender of#819 causing damage to the truck's fender. Supervisor's Comments &-Recommendations: To be more aware of surroundings when backing up and ask for someone to guide him when backing up. Evans advised, on 01/17/09 at 2200 hrs, he reported for duty with his star on his outer vest carrier. On 01/18/09 at 0400, he cleared desk relief. At that point, he noted, his star was missing from his outer vest carrier. Ofc. Evans re-traced his movement throughout the night however, was unable to locate the missing star. Supervisor's Comments & Recommendations: Embroider uniform badges would significantly reduce lost badges. As would an improved or redundant clasp system. LPC Comments & Recommendations: Concur F=1 _ KAREN BENSEN FIRE DEPARTMENT. _ 01/23%09 Description of Incident: E7 was returning from 840 Wellington trying to navigate between the snow covered islands we heard a brief sound under the right front bumper. When back in the station, we noticed a small buckle in the right front chrome skirt near the front bumper. Supervisor's Comments & Recommendations: Removal of the snow mounds around islands by property management. LPC Comments & Recommendations: Driver Refresher Course I Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 12 of 23 F_S 2 _ PAUL-FRIEDMAN .= FLEET SERVICES 01125/09 Description of Incident: Paul was repairing a windshield wiper system on vehicle #262. Upon removal of the wiper transmission from the cowl vent area, it nicked the bottom of the windshield lip and formed a crack. Supervisor's Comments & Recommendations: Mechanic must be very cautious around glass edges, and if there is a possibility of a part or tool coming in contact, the edge or surface of the windshield must be protected (i.e., duct tape, cardboard). LPC Comments & Recommendations: Concur P'6 1- 4,POLICE_DEPARTMENT (WORD P )_ 01/25109. Description of Incident: Employee reported that she was in the process of moving the plastic floor mat located in the word processing area when her right foot caught on the mat. Employee further related that she consequently fell forward striking her head on the partition and skinning her right knee. Employee was offered that an ambulance be summoned but she declined, advising that she just had a headache. Supervisor's Comments & Recommendations: It is recommended that the word processor seek assistance when moving equipment. LPC Comments & Recommendations: Concur i I I i Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 13 of 23 P_7 .. _ 09%02 P,OLICE_DEPARTMENT_ 01/28/09:: Description of Incident: Officer was sitting in his vehicle doing paperwork when he noticed a strong odor of exhaust fumes in the vehicle. Officer began to feel ill and developed a headache. Officer was brought to the emergency room for treatment. Supervisor's Comments & Recommendations: Once Officer realized that exhaust fumes were entering the passenger compartment of the vehicle he immediately opened all windows to ventilate the vehicle. The vehicle was then removed from service. The vehicle did not show any obvious signs of defects when Officer inspected the vehicle at the beginning of his tour. This incident could not be prevented by the officer since there was no indication of a problem until he detected the exhaust fumes inside the vehicle. LPC Comments & Recommendations: Concur FS:3 MARK KMIECIK FLEETSERVICE$ _ 01%28/09=: Description of Incident: Mark was pulling into the parking lot of Dave & Jim's Auto Body to drop off another employee. As he entered the parking area, he noticed a large landscaping rock to his right and an extended length van parked to his left. He continued to drive through the very small area between these two objects and in doing so hit the landscape rock with the trucks running board, damaging it. Supervisor's Comments & Recommendations: Employee should not have taken a chance attempting to drive through an area that was too congested to drive through safely. This was very poor judgment on his part. i LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 14 of 23 P=8 --MICHAELJEDLINK -POLICE DEPT: 01/28/09 Description of Incident: Ofc. Jedlink was traveling N/B on Lively, approximately 200 feet south of Oakton St., when while executing a U turn from N/B to S/B, the right front end of the squad came into contact with a snow bank on the west side of lively causing damage to the passenger side of the front bumper. Supervisor's Comments & Recommendations: By exercising better judgment. LPC Comments & Recommendations: Driver Refresher Course S 2 ROBERT:000KE STREETS 01/28/09. Description of Incident: Mr. Cooke was plowing and stopped for a school bus that was picking up children at the corner of Virginia and Richmond. As Mr. Cooke was waiting to proceed another vehicle slid into the rear of Unit 707. Supervisor's Comments & Recommendations: No recommendations as the Village have little control over the actions of other drivers. LPC Comments & Recommendations: Concur I I I Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 15 of 23 S-3 : TODDrGORE PUBLIC WORKS 01/29/09; Description of Incident: Todd was up in the bucket truck removing a parkway tree about 30 ft. in the air. As Todd was cutting the branch, the chain was bound in the crotch of the branch as the limb began to fall to the ground. The saw was stuck in the crotch of the branch, which forced the saw out of Todd's hand and plummeted to the ground along with the branch. Saw was damaged. Supervisor's Comments & Recommendations: For safety reasons and serious consideration, we have not been tethering the saws to the bucket. Through seminars and talking with Stihl reps, it is more dangerous to tie yourself or the saw with a tether strap. In that particular case, Todd could have been dragged out along with the branch and the saw. Todd reacted like he was taught and nobody was hurt. The ground crew was not under the bucket like they are trained not to do, so again nobody was injured. LPC Comments & Recommendations: Concur P=9 09/03 POLICE:DEPARTMENT 01/30/09 Description of Incident: After exiting vehicle, Officer slipped on the ice. Officer fell landing on his left shoulder. Supervisor's Comments &Recommendations: The continued unpredictable winter weather conditions and freezing temperatures this season have caused icy conditions in the parking lot. Officers need to be reminded to be careful and continued efforts to remove the snow and ice is recommended. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 16 of 23 F.'2 09/04 FIRE DEPARTMENT, 01/31/09 Description of Incident: Employee acting officer on E7 responded for the report of basement fire. His crew first in on fire attack advanced a pre-connected line into the basement. Crew encountered high heat conditions and had to retreat. Sometime during the advancement and retreat, employee slipped on the basement stairs and injured his back. Supervisor's Comments & Recommendations: Based my review of this incident at times these types of injuries due occur during firefighting activities. In the future all precautions should be reviewed and reinforced. LPC Comments & Recommendations: Concur F3 LT R FERGUSON FIRE DEPARTMENT _ 01/31L09: Description of Incident: Axe was left near front entrance when E10 assigned to take over the attack line. Heat from structure fire caused damage to the axe. I Supervisor's Comments & Recommendations: Purchase wood handle axes. LPC-Comments & Recommendations: Concur i Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 17 of 23 GUSTAVSEN -'FIRE_DEPARTMENT • 0:1/31/09 Description of Incident: The camera was being used by the Tower 7 crew while interior on the first floor of 61 Avon. The fire conditions deteriorated rapidly and an immediate evacuation was called. During the evacuation, visibility became zero and contact with the camera was lost. The first floor was breached with a large ventilation hole and multiple burn through areas. The camera is presumed lost in the rubble and water in the basement. Under these heat and collapse conditions, the camera is not designed to survive even if found. Supervisor's Comments & Recommendations: An extendable lanyard on all cameras would help prevent loss especially when there are multiple users during an incident. LPC Comments & Recommendations: Concur F-5 LANCE'HARRIS FIRE DEPARTMENT. 01/31/09:: Description of Incident: The axes were left in the basement of a house with rapidly deteriorating fire conditions in the basement and first floor. Multiple hose lines, personnel, zero visibility and equipment made it difficult to account for the axes during the evacuation phase of the fire. Supervisor's Comments & Recommendations: Possibly less equipment per crew member to account for each tool in an unpredictable environment. LPC Comments & Recommendations: Concur j i I I I I Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 18 of 23 SR 1_, STEVE J_OHNSON'.—,_UTILITI_ES._. ._ 02/09/09_ Description of Incident: Mr. Johnson was stock-piling trench spoil and debris into a bin (a three-walled concrete structure) and he was raising the loaded bucket to dump atop the pile. This action caused the right front side of the loader to sink into the debris more than the left side. As the loader began leaning, the mirror bracket came in contact with the bin sidewall. As the bracket was being crushed, it pushed the mirror into the windshield causing both to crack/shatter. Supervisor's Comments Recommendations: Remind all operators of the importance of continually monitoring the stability of the base material when operating this equipment. Especially when ascending or decending a stockpile. LPC Comments & Recommendations: Tabled F=6 09/05 FIRE DEPARTMENT :02/04/09. _. . _ I Description of Incident: While exiting Tower 7, employee slipped and fell to the ground, injuring his right wrist. Supervisor's Comments & Recommendations: Take care in entering and exiting fire apparatus. i LPC Comments & Recommendations: Concur i I i i I Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 19 of 23 P-10 . : 09/08 POLICE DEPARTMENT 02/13/09: Description of Incident: Employee was walking north at 860 Bonnie near an area between the sidewalk and the dock apron when she slipped and fell striking left forearm, left hand, and left leg on the concrete sidewalk. Supervisor's Comments & Recommendations: Employees must be aware of outside environmental conditions that could cause or lend itself to accidents. In this case, the small gravel was revealed when the snow melted recently. LPC Comments & Recommendations: Concur F-7 09/06 FIRE DEPAR_TNIENT 02/14/_09 I Description of Incident: A large female patient (approx. 275-300 Lbs.) was lying supine on the bathroom floor. Patient has hx. of M.S. and unable to control either leg foot. C-Spine cleared and we began by moving patient into the sitting position. A lifting strap was applied to patient (yellow waist strap/black leg strap) and made a lift on the patient. When the patient was almost to the Y2 to 3/ standing position, the black leg strap (of the lifting strap) slid off patient's legs and rear end, and patient started to make a rapid descent towards the floor. They were unable to hold this patient from falling. Lifting strap was reapplied and patient was transferred to here electric scooter. Both Lt. and employee injured their backs during the lift. j Supervisor's Comments & Recommendations: We are often put into difficult situations for patient care. Crew used safe lifting techniques and.proper equipment, and there was still an injury. The lift strap has been used many times and is an effective tool for difficult lifts. This strap was applied properly, but with the shape and size of the patient, the black strap slipped out of position and an injury resulted. A possible modification of the black strap, may have prevented its movement (possibly having the black strap work in the same fashion as our KED, with buckles). i LPC Comments & Recommendations: Concur i Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 20 of 23 F,78-: :. : 09/07 FIRE DEPARTMENT, 02/14109. Description of Incident: Lt. was assisting in the lifting of a very heavy patient from the ground. The patient was placed in a lifting strap and as the patient was being lifted, the strap slipped off and the patient's weight shifted back towards the Lt., causing him to have to catch the bulk of the weight all of a sudden. This caused him to have a painful twinge in his lower back. Supervisor's Comments & Recommendations: After talking with the Lt. and others on the scene, the only recommendation that can be made to help prevent this incident from occurring again would be to have all personnel aware of the strap locations, and to watch carefully for any movement of the strap while lifting the patient. LPC Comments & Recommendations: Tabled F 9 I(YLE-SEEGERS _FIRE DEPARTMENT 02/16%09 Description of Incident: Employee was assigned to bring Hazmat 8 out to the scene of a Hazmat call at 150 Northwest Point. While responding, employee failed to disconnect the airline that is constantly hooked up to the apparatus. While pulling out of the station, the force tore the air hose line from the station wall fitting, damaging it and tore the air hose line. There was no damage done to the vehicle. I Supervisor's Comments & Recommendations: 1) Doing a 360 check of reserve and j special response vehicles (Hazmat, Dive, etc.) for doors, power cords, air lines, etc., prior to pulling them out of the station. 2) A sign will be put on the steering wheel of Hazmat 8 to remind everyone to disconnect the airline prior to pulling out of the station. LPC Comments & Recommendations: Concur j Loss Prevention Committee Minutes for March 12, 2009 Meeting Page 21 of 23 P,.=11 OFC Chris,Crum -Police Department. 02/19/09=: Description of Incident: Officer Crum was S/B on Bond Street approaching the stop sign at Higgins Road when his vehicle slid through the intersection on the ice covered roadway as he attempted to brake, striking a fully loaded Mack bomber Dump truck. The right rear tandem of the truck struck the left front of the squad. Supervisor's Comments & Recommendations: Caution must be exercised when driving in any condition to anticipate the unexpected. LPC Comments & Recommendations: Driver Refresher Course P 12 09109POLICE DEPARTMENT ,02/19109 S. Description of Incident.- Officer ncident:Officer Crum was S/B on Bond Street and slid on slick roadway past stop sign and into tractor-trailer. Vehicle airbags deployed and employee was transported to ABMC ER. Supervisor's Comments & Recommendations: Officer is reminded to operate Village vehicle in safe manner during all weather and road conditions. Supervisor's Comments & Recommendations: Employee will be more careful when closing the printer lid in the future. LPC Comments & Recommendations: Concur OFC:=WARREN MOODY-='POLICE-DEPARTMENT`- 02/23/09_ Description of Incident: Ofc. Moody was driving through the parking lot to backup Ofc. Jedlink who was in process of a Traffic arrest when he.cut a parking lot median curb too close and struck a decorative boulder on the raised median with center bottom of the right front door of Squad #237, causing dent to door. Supervisor's Comments & Recommendations: Officer must use more caution and due diligence when driving, especially in parking lots with landscape decorations that can cause harm to his moving squad car. Supervisor's Comments & Recommendations: Drivers should proceed slower thru intersections; assuming that other drivers will not stop even though they may see our flashing lights and hear our sirens.
