Present: Jack Gray, Chairman John Nowacki, Commander Police Department Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Al Vrshek, Deputy Fire Chief Karen Turner, Loss Prevention Secretary Excused: Scott Miller, Deputy Fire Chief The minutes were approved for the meeting of March 12, 2009 Review Current Reports: 17 Tabled Reports: F7-09/06 — Fire Department Description of Incident: Assisting in the lifting of a very large female patient from the ground, injury to employee occurred. Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 3 of 11 W43 SCOTT BERNHOL>DT PUBLIC WORKS. 01/23%09 Description of Incident: While reaching into vehicle to retrieve flashlight, radio was .pulled off employee's belt possibly by coat, falling into water and subsequently being washed.downstreet. Radio remained submerged until water was shut-off .approximately 45 minutes later. Supervisor's Comments & Recommendations: Employee should seek to position radio in a location that it will not be pulled or knocked off by clothing or movement, or try to leave radio in vehicle and position vehicle so that emergency work activities can be accomplished and still hear and access the radio as needed. LPC Comments & Recommendations: Concur f=11 09/12 FIRE QEPARTMENT`'. 2/24/09 : Description of Incident: E7 responding to a residential fire alarm in District 10 when approaching the intersection of west bound Biesterfield at Wellington had contact with a vehicle that approached from the north. Employee was a rear passenger jump seat, with his full turnout gear and seat belted in his seat. Employee had no complaints of injury at the scene and assisted with patient assessment and treatment prior to Ambulance arrival. No complaints after being transported to ABMC. Supervisor's Comments & Recommendations: Employee was a rear jump seat passenger and had no involvement in the operation of the vehicle to prevent or stop the crash. In the future, all passengers can assist with observing traffic. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 4 of 11 F=12 = :-- 09/14;=:FIRE DEPARTMENT--:' 02/24/09 Description of Incident: E7 responding to a residential fire alarm in District 10 when approaching intersection of west Biesterfield at Wellington, made contact with a vehicle that approached from the north on Wellington with both vehicles coming together in the intersection. Employee was the operator of the fire apparatus (E7), E7 emergency lights, federal Q and electronic siren all operating and in use at the time of the crash. The driver of the vehicle involved was assessed and treatment started prior to first ambulance arriving on location. No injury reported by firefighters as scene or after in the ER at AMBC. Supervisor's Comments & Recommendations: Make all attempts to clear intersection of approaching or moving vehicles to the best of our ability with use of emergency devices. LPC Comments & Recommendations: Concur F13 Reported=By 02/2409 -L --H KEENAN >FIREDEP,T Description of Incident: E7 dispatched on a residential fire alarm in District 10. E7 with emergency lights Federal Q and electric siren active, approached the intersection of West Biesterfield Road at Wellington and made contact with a vehicle that approached from north Wellington into the intersection. Intersection control device was activated as we turned out of the station onto west Biesterfield. E7 Officer Lt. Keenan reading the map book to look-up the residential street during impact. One civilian injury from the vehicle involved in the crash, a male driver who was assessed, treated and transported to ABMC. All 3 firefighters transported to ABMC for evaluation. 'All firefighters cleared to return to work by ER doctor. Supervisor's Comments & Recommendations: Continue to wear seat belts and continue to have all emergency warning devices activated anytime we respond to emergency calls. Use a safe defensive driving approach while daily driving or while responding to emergency calls. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 5 of 11 P-13 OFFICER`MARK-MARUSICH POLICE DEPT. 0.3/03%09._ Description of Incident: Officer Marusich was driving S/B Arlington Heights Road while approaching a green light at Cosman Road when a vehicle traveling N/B Arlington Heights Road attempted to turn left onto Cosman and struck vehicle 224 on the driver's side causing damage. Supervisor's Comments & Recommendations: The accident was not avoidable. The damage was caused by the unpredictable actions of another. LPC Comments & Recommendations: Concur P=14: 09113'- POLICE;DEPT 03/03%09`: Description of Incident: While driving a police squad, Officer was injured after his vehicle was struck by another vehicle, which had turned left into his vehicle striking the driver's side. Supervisor's Comments &-Recommendations: Officer was injured while operating a police vehicle during normal driving conditions. Injury was not a result of any action taken by the officer. Officer's vehicle was struck by another at fault driver. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 6 of 11 LF 1 RANDY BLOCK.' PUBLIC:.WORK3: 3/05/09J Description of Incident: Randy was informed on March 5th that a broken passenger rear tail light lens and cover had been broken/cracked. As the truck was situated it looked like possibly it was backed into. There was very minimal damage done but it still requires an incident report be created. I asked around and nobody took ownership of this damage so it is unsolved. Supervisor's Comments & Recommendations: I really have no opinion on that because I do not know how this occurrence even happened. I will say though, when causing damage to a piece of equipment, it is always better to own up to.it so we can determine better ways to avoid these occurrences from happening together. LPC Comments & Recommendations: Concur P`1 5 :09115;- POLICE DEPT - 03/06/09 Description of Incident: Officerwitnessed a battery, which occurred at the VFW Hall, 400 E. Devon Avenue, Elk Grove Village. Upon Officer ordering the subject to stop, the offender fled northbound into the Elk Grove Village residential area. Following a chase through several yards over several fences, Officer attempted to clear the fence in the backyard of 282 Walnut but caught his shins on the top of the fence while falling over, resulting in swelling and bleeding to both shins. Officer caught the offender in a nearby yard, which terminated the foot pursuit. Supervisor's Comments & Recommendations: Officer called out a foot pursuit and provided updated locations throughout the foot pursuit allowing responding Officers to close on Officer's location. Officer crossed residential yards and fences maintaining a visual on his offender the entire time of the pursuit. Officer followed the offender until the offender faltered and fell to the ground. Due to fatigue, Officer scraped his shins crossing the last fence before he apprehended the fleeing subject. I am aware of nothing the Officer did incorrectly which could be changed to avoid this outcome. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 7 of 11 F_14 FIRE DEPARTMENT 03/16%09: Description of Incident: Employee was lifting a stretcher loaded with two EMS bags and ECG monitor up two steps thru a doorway. The load shifted to one side. Employee tried to correct the shift and in so doing injured his back. Supervisor's Comments & Recommendations: The stretcher could be unloaded prior to stepping. A physical fitness program emphasizing core strength may prevent such an injury. LPC Comments & Recommendations: Concur FOSTER FIRE DEPT 03!21%09_ Description of Incident: When leaving the building after a fire at the above address, the microphone cord caught on the door handle of the house tearing the cord off the connectors. Supervisor's Comments & Recommendations: Pay closer attention to where all equipment is at all times. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 8 of 11 - GO 1 _ 09/1- = FINANCE DEPARTMENT:_ 03/26/09:. Description of Incident: While getting into the high chair at employee's workstation, employee used hand for support (right hand on counter and left hand on chair arm rest). Employee felt wrist pop followed by pain. The footrest was already broken, but nobody reported that the chair was damaged. Supervisor's Comments & Recommendations: Called furniture supplier. He is ordering a new base for the chair (chair is possibly still under warranty). He said the footrest is not designed as support to gain access to the chair. He suggested we purchase a heavy-duty chair and build up floor or supply a step stool. LPC Comments & Recommendations: Concur FS=3 JACK-GRAY BUILDING &;FLEET DEPT 03/23%09= Description of Incident: While delivering unit #137 to the body repair facility listed above, upon making a left turn from the curb lane, due to a tractor trailer parking in a position that this unit was to be parked, the right rear hand rail came into contact with two mailboxes while making a left turn. No damage to unit #137. The mailboxes are owned by the vendor who will be making the body repairs to the fire apparatus and they indicated that they would make the required repairs at no cost to the Village. Supervisor's Comments & Recommendations: Driver should be more aware of any and all surroundings. LPC Comments & Recommendations: Driver Refresher Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 9 of 11 S-4 UNKNOWN=:PUBLIC=WORKS - = 04/02/09'. Description of Incident: It is unknown-exactly how the damage occurred but it appears to-have happened when the dump body was raised up and the tarp system came in contact with an unknown object. Supervisor's Comments & Recommendations: Caution all dump truck operators of the importance of proper clearance around the vehicle when raising and lowering the dump body. LPC Comments & Recommendations: Concur 097.18 - FIRE DEPARTMENT 04/04/09: Description of Incident: Employee was using a sharpening steel in the kitchen. The blade came off cutting him in the webbing between the thumb and index finger. The employee was sent over to ABMC ED for stitches. He was seen right away and was released back to duty. Supervisor's Comments & Recommendations: When working in the kitchen pay closer attention to what you are doing. This was a true accident that better hand placement on the sharpening steel probably could have been avoidable. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 10 of 11 SR 2 - : 09/19 STREET/BU,IWING/FLEET DEPT 04/08/09 Description of Incident: While running flusher hose with nozzle, employee attempted to adjust spray nozzle when the nozzle split causing injury to right and left hand. Supervisor's Comments & Recommendations: Employee should pay greater attention to pressures on equipment that they are working with to ensure that they are operating correctly and.safely. Additional retraining on this equipment will be conducted. LPC Comments & Recommendations: Concur F :17 09/20 FIRErDEPARTMENT _04/08/09: Description of Incident: E10 was working a patient who was on the floor in full- arrest. Employee stepped around.the patient who was lying on the dining room floor and strained his right knee. Employee felt the strain with pain but did not lose balance and continued to perform CPR and ALS interventions without difficulty. Supervisor's Comments & Recommendations: As a precaution and to document the incident for future reference, employee immediately reported the occurrence to a Lt. He did not want to receive treatment and had little.pain afterwards. He did not have difficulty in performing duties the rest of the shift. By the morning, employee was pain-free. Occurrence of potential injury could not be avoided; employee acted within the guidelines and performed within his job-description and scope of practice. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 14, 2009 Meeting Page 11 of 11 -BRIAN MCGEE :,UTILITIES DEPT.' 04128L09. Description of Incident: While flushing fire hydrants, Brian entered passenger side of van to retrieve a hydrant wrench. While stepping into the van to reach over the passenger's seat, he slipped and fell backwards, tripping over the curb and pushed against the door. The door was pushed back over center and sprung. He was not hurt. He then pushed the door in the closed position. Supervisor's Comments & Recommendations: While out on the street performing hydrant flushing, personnel must be fully aware of the surrounding terrain and the fact that there is a fast moving current of water. One must take extreme caution to everything in the general vicinity while performing this operation. LPC Comments & Recommendations: Concur
