Chairman Shapiro called the meeting to order at 6:00 p.m. in the Charles J. Zettek Building,
901 Wellington Avenue, Elk Grove Village and noted for the record that the following were
present: Chairman Alan J. Shapiro, DDS, and Secretary E. Butch Ehrke, and Commissioner
Michael P. Kontek.
It was moved by Commissioner Kontek and seconded by Commissioner Ehrke that the
minutes of March 12, 2009 meeting be approved as submitted and that the Board dispense
with the reading thereof. The motion was voted upon by acclamation whereupon Chairman
Shapiro declared the motion carred.
PRESENTATION AND APPROVAL OF INVOICES
1. Daily Herald, Invoice T4141614 dated March 10, 2009 in the amount of$23.00
for publication of meeting change.
2. Daily Herald, Invoice T4148636 dated April 17, 2009 in the amount of 821.85 for
publication of meeting cancellation
It was moved by Commissioner Ehrke and seconded by Commissioner Kontek that the Board

authorize the disbursement of funds for payment. The motion was voted upon by acclamation
whereupon the Chairman declared the motion carried.
CORRESPONDENCE
1. Memo to Village Clerk advising that the Regular Meeting of the Fire & Police
Commission scheduled for April 21, 2009 has been cancelled.
2. Memo to Human Resources Coordinator advising that Ofc. Miguel Mosqueda has
satisfactorily completed his probationary period and is granted permanent
employee status.
3. Written Reprimand dated April 20, 2009 to police officer for violating Rules and
Regulations of the Elk Grove Village Police Department, Section 26.1.1A, Rules
of Conduct, Rule 26.1.09 (Inattention to Duty).
4. Written Reprimand dated May 6, 2009 to police officer for violating Rules and
Regulations of the Elk Grove Village Police Department, Section 26.1.1 A, Rules
of Conduct, Rule 09 (Inattention to Duty).
5. Memo from Chief Schmidt dated May 7, 2009 requesting that the Commission
promote Ofc. Michael Garrison to the rank of Sergeant to replace Sgt. Mark
Waclawski who retired effective May 1, 2009. j
6. Memo to Chief Miller requesting comments and constructive criticism regarding
the performance of(7) probationary firefighters hired on June 23, 2008 (Liska, i

Peterson, Domin, Seegers, Melendy, Ritchie and Evert). Final probationary
interviews are scheduled for June 16, 2009.
7. Memo to Chief Schmidt requesting comments and constructive criticism regarding
the performance of Probationary Ofc. Bryan Lyp who was hired on March 24,
2008. Ofc. Lyp's final probationary review is scheduled for June 16 2009.
8. Letter to Ofc. Michael Garrison appointing him to the rank of Police Sergeant
effective June 1, 2009.
9. Retirement congratulations letter to Sgt. Mark Waclawski who retired on May 1,
2009 after twenty-seven plus years of service.
10. Retirement congratulations letter to Ofc. Wesley Gustafson who retired on May 1;
2009 after twenty-eight plus years of service.

NEW BUSINESS
1. Reviewed draft of memo to firefighters requesting to be notified of their intent to
participate in the Fire Lieutenant Promotional Process.
2. Meeting with Chief Miller
a. Discussed Fire Lieutenant Promotional Process
b. Discussed the possibility of using the CPAT for our next firefighter testing
cycle. Chief Miller will survey surrounding Fire Departments.
c. Discussed the possibility of modifying our education requirement to require
Illinois paramedic license. Chief Miller will survey surrounding Fire
Department.
ELIGIBILITY LIST/ HIRING STATUS
1. Police Officer—two vacancies on hold at this time. Twenty-seven
names remain
on the current eligibility list scheduled to expire on August 1, 2010.
2. Firefighter—no vacancies. Thirty-six names remain on the current eligibility list
scheduled to expire on January 18, 2010
3. Police Sergeant Eligibility List—Ten names appear on the new eligibility list
which will expire on February 4, 2012.
4. Fire Lieutenant List—Two names remain on the current eligibility list scheduled to
expire on June 24, 2009.

ADJOURNMENT
It was moved by Commissioner Ehrke and seconded Commissioner Kontek that the meeting
be adjourned. The motion was voted upon by acclamation whereupon Chairman Shapiro
declared the motion carried and the meeting adjourned at 6:45 p.m.
