The meeting was called to order at 8:45 A.M. at the Village Hall.

• The LPC Committee discussed the importance of purchasing pole wraps for designated
column beams in the Police Department garage due to the 5 cars that have hit a pole or
electrical box, and have been damaged. .
• Discussion regarding the offensive odor coming from the soil of the indoor plants,
located on the lower level in front of Engineering and Community Development,
which is causing headaches and affecting employees with allergies. The LPC
Committee suggested that the service be conducted off hours.or on the weekend.
• The LPC suggested that an employee Suggestion Box be placed in an accessible
area for their input regarding safety issues or other.

Description of Incident: While loading the man lift, the lift rolled forward into the cab
of the truck.
Supervisor's Comments & Recommendations: Putting some type of blocking near
the front of the bed so it will not hit the cab if it was to roll forward.

Description of Incident: Backing out of the garage employee did not see 706 parked
outside of garage directly across from garage door and backed into 706S front driver's
side fender.
Supervisor's Comments & Recommendations: Ask for someone to guide him backing
out of garage.
LPC Comments & Recommendations: Elk Grove supervisors need to review Elk Grove
Village policies and procedures with all season employees, and have them sign their name
stating they understand and have-read them.

Description of Incident: Officer was.attempting to maneuver the squad car from its
assigned stall and struck a support pole in the garage area.
Supervisor's Comments & Recommendations: Officer needs to use better judgment
of the obstacles present in the garage.
LPC Comments & Recommendations: Driver Refresher Course. LPC Committee strongly
urges and recommends that the proper department head expedite action to resolve this
problem.

Description of Incident: Officer Bazukas had observed a traffic violation. The motorist
pulled over in the parking lot of Towne Air Freight located at 952 Fargo, Elk Grove Village,
IL. After completing the traffic stop, Officer Bazukas realized he had to maneuver his
squad around a wooden gate arm, which was in the down position. While attempting to
drive around the gate arm, the front passenger side of his squad (24) caught the tip of the
gate arm, which caused a dent on the front passenger side panel. The gate arm was not
damaged.
Supervisor's Comments & Recommendations: Officer Bazukas should have activated
the wooden gate arm so it would have been in the raised position, subsequently making
his exit clear.

Description of Incident: Officer Finnen was using the copier and as he lifted the top of
the copier to put the form on the scanner, his hand slipped hitting the document feeder.
A small tab used to hold the feeder broke off.
Supervisor's Comments & Recommendations: In the future, Ofc. Finnen should
ensure that he has a firm handle of the copier door attempting to lift it.

P 27 OFC PATRICK EVANS :POLICE DEPT 07/10109_:
Description of Incident: Ofc. Evans was patrolling the north side of the parking lot and
when he turned left to patrol the west side of the lot he struck a pole with the left rear
passenger door of his vehicle.
Supervisor's Comments & Recommendations: Ofc. Evans must be diligent in
scanning his route of travel so he is aware of any potential hazards.

Description of Incident: While investigating a subject who was suspected of
purchasing alcoholic beverages for minors, the subject fled the scene. Officers
engaged in a foot pursuit and during the apprehension Officer sustained abrasion type
injuries to his left wrist and left knee.
Supervisor's Comments & Recommendations: Officer was in foot pursuit of a
suspect and took the necessary steps to apprehend him.

Description of Incident: No body damage, just scratches to the paint on the right
rear corner of ambulance. This damage was not witnessed by Fire Department
personnel.
Supervisor's Comments & Recommendations: In an,attempt to keep the vehicle
from being struck by passing vehicles, care should be taken to park the vehicle as far
away from moving traffic as possible. Also, the vehicle should be left unattended,
shortest duration as possible.

Description of Incident: Sometime between the evening of July 20, 2009 and the
morning of July 21, 2009 the portable radio was stolen from the Village vehicle parked
in the Village Hall parking lot.
Supervisor's Comments & Recommendations: The user of the radio and the
village vehicle must routinely make sure the car is locked each time he/she leaves the
vehicle. In the case of the radio, the user must make sure that he/she either keeps the
radio with them at all times or locks the radio in the charger/cradle installed in the
vehicle.

Description of Incident: Firefighter was returning from a call (A10) when he
observed a vehicle traveling East bound on Biesterfield cross the median and entered
the Forest Preserve. EGVPD was first on the scene and A10 pulled to the side of the
road. EGVPD broke the passenger side window of the vehicle. EGVPD waved for
A10 to enter the scene. EGVPD and EGVFD removed the patient from the vehicle
and placed the patient in the ambulance. Once inside the ambulance, F/F noticed
blood on his right forearm. After transfer of care F/F reported to me that he had an
abrasion on his right forearm and he may have been splashed with vomit from the
patient while doing CPR. F/F was advised to follow the NWCH System exposure
Policy.
Supervisor's Comments & Recommendations: Firefighter was performing CPR per
System SOP's. The patient vomiting is a normal occurrence while performing this
action. F/F was wearing gloves and safety glasses. Blood borne pathogens and the
System Exposure Report was reviewed with F/F.
LPC Comments & Recommendations: The LP Committee would like management
to consider a "Rescue Key" (mouth to mouth preventive device from fluids when
administering CPR) be given to all EGV employees. Estimated quantity is about 200.

Description of Incident: While cleaning the chute door on vehicle 708, the door slid
down pinching the employee's right index finger. Employee initially thought it would
just bruise but the swelling overnight became quite painful.
Supervisor's Comments & Recommendations: Employees should ensure that
chute door is propped open before removing asphalt.

Description of Incident: Ofc. Moody was removing a Taser from its storage bin
when he lost his grip on the taser and it fell to the floor thereby damaging the safety
switch on the taser.
Supervisor's Comments & Recommendations: Employee to be more attentive
when handling equipment.

Description of Incident: While excavating for 6" water main repair plastic gas'service
for 69 Grange was cut.
Supervisor's Comments & Recommendations: Seeing as there is no way of
knowing what material the gas lines are made of and how they are installed, hydro-
excavating for all utilities might be an option when there are no time constraints and
the ground conditions are favorable.

F 23: 09/32 -:FIRE-DEPARTMENT. 07%29109`:
Description of Incident: Injured middle and index finger of left hand while using
Hurst Tool to extricate two passengers from severely damaged auto that was involved
in an accident. F/F sprained these fingers when tool twisted during extrication. He
noticed that they were sore on morning of 7/30/09; however on the morning of
7/31/09, they were noticeably swollen and he then went to Emergency Department of
Edward Hospital where he was treated.
Supervisor's Comments & Recommendations: Auto was severely damaged and
used best method possible to gain entry. Due to damaged condition of door, tool
twisted while attempting to gain entry. Attempt to train on damaged autos and warn
employees that told may twist during extrications.

P/31 POLICE DEPARTMENT 7/30/09:
Description of Incident: While in the back of the transport van, employee was
attempting to lift the wheels of the stretcher over the raised metal frame in order to get
the stretcher into the van and felt a slight pull in lower back. Although she was not in a
lot of pain she wanted to report the incident.
Supervisor's Comments & Recommendations: Have the raised metal frame
removed.

Description of Incident: While rebedding hose on E10 at a Mutual Aid Box Alarm
Fire at 1000 Stevenson Court in Roselle, IL, informed supervisor he didn't feel well.
Upon return to 676 Meacham (Fire House #10) in EGV, again said he didn't feel well
and clutched his chest. He was moved to Amb. 10 and Paramedics provided ALS
care and transportation to Alexian Brothers Medical Center Emergency Room.
Supervisor's Comments & Recommendations: Recommend individual employees
identify and control the risk factors which contribute to Heart Disease. A formal
education plan for improving our life choices could also be beneficial, i.e.: healthy
eating habits, exercise, and limitations for alcohol and cigarette consumption.

Description of Incident: Ofc. Rohman said that while backing his vehicle in the
parking garage his attention was focused on the support post to the right of him and
he did not see the one directly behind him. Ofc. Rohman said as he backed up he
started to turn his front wheels and struck the support post with the left rear corner of
his vehicle.
Supervisor's Comments & Recommendations: Ofc. Rohman needs to pay more
attention to his surroundings while backing his assigned vehicle from its parking
space.

Description of Incident: Crews were in the kitchen area of the fire station when
Firefighter PM entered the room advising that A7 was going to take employee to the
hospital. At time of report F/F was not able to discuss his condition with me.
Supervisor's Comments & Recommendations: F/F advised one of the medics from
A7 that he was not feeling well. The medic started treatment and then advised that
they would be transporting. The medic should have advised the rest of the crew
earlier instead of waiting until they were ready to transport.

Description of Incident: When backing up from the parking spot, hit the electrical
box sticking out on the steel beam.
Supervisor's Comments & Recommendations: Pre-trip inspection, before moving
any vehicle, take a walk around the vehicle, look for any hazards.
LPC Comments & Recommendations: Drive Refresher Course. Village needs to
look into purchasing protective measures for the garage.

F=26 = AL VRSHEK FIRE DEPARTMENT 08/14/09
Description of Incident: Deputy Chief Al Vrshek was backing his car from his
assigned parking spot when he noticed some workers blocking the exit in front of him.
He then decided to back up and exit in the opposite direction, when he struck the
support column located behind and to the North of his vehicle.
Supervisor's Comments & Recommendations: Driver should utilize caution when
backing a vehicle because of the blind spots of the vehicle and the numerous columns
located in the garage in the Village Hall.
LPC Comments & Recommendations: Protective measures for certain poles needs to
be addressed asap.
09/35 WATER=-&-SEWER;' = - 08118/09.
Description of Incident: While excavating by hand with a shovel around a main
valve box, employee struck a buried rock with the shovel causing his hand to lose its
grip on the shovel, travel downwards and get smashed between the shovel and the
valve box.
Supervisor's,Comments & Recommendations: Use of a longer handled shovel or
the hydro-excavator may have lessened the possibility for smashing the digit, wearing
gloves potentially could have absorbed some impact and mitigated the injury.

Description of Incident: Control vault for the water wall at the Village Hall flooded
due to the drain valve on the sand filter was left open after maintenance was
performed.
Supervisor's Comments & Recommendations: Employee must pay better
attention to make sure all operating conditions are met before leaving the vault.

Description of Incident: Officer was backing squad #239 out of designated parking
space in the parking garage when the left front fender came into contact with the
support column located directly next to 239's designated space.
Supervisor's Comments & Recommendations: Parking spaces in the garage are
narrow in width and maneuvering a squad in and out of the spaces next to the support
columns is challenging. Recommend the support columns be wrapped with a cushion
which would absorb a minor impact, thereby preventing any damage to the column
and the vehicle making contact.
LPC Comments & Recommendations: Driver Refresher Course. The LPC Committee
strongly suggests that the pole be cushioned.

Description of Incident: Sometime after vehicle was parked in the lot (approx. 10:45
am) a fire started under the driver's side seat. Scott Bernholdt noticed discoloration of
the vehicle windows at approximately 5:40 pm. Police and fire were called to the.
scene. The fire had extinguished itself as the doors and windows to the vehicle were
closed. Interior of vehicle was significantly damaged.
Supervisor's Comments & Recommendations: Employees should be careful not to
leave objects on the floor of the vehicle that may become lodged under the seat.
LPC Comments & Recommendations: Concur
F=27, - SCOTT PETERSON'= FIRE.DEPT:: 8/26/09=
Description of Incident: After returning to quarters from a call, Lt. Peterson exited
the vehicle and was taking off his turnout gear. The front passenger door would not
shut completely and would not go-past % open. Lt. Peterson attempted to open the
door fully and the window on the front passenger side door shattered. The window
was in.the down position. As the window shattered the door, the door did open freely
and close freely.
Supervisor's Comments & Recommendations: Investigate cautiously, if still an
issue, call Fleet Services to evaluate issue.
LPC Comments & Recommendations: Reconsider charging this to maintenance
instead of insurance due to a mechanical issue.
