The meeting was called to order at 8:45 A.M. at the Village Hall.

• The Loss Prevention Committee will meet the 3`d Thursday of each odd month of the
year in 2010.
• The Loss Prevention Committee discussed the On-the-Job Injury Report 09/40 that
occurred on 10/18/09. The Fire Department needs to execute more caution when
responding to this individual due to his excessive weight. On any future calls to this
resident, more manpower should be present (if possible) to grab the cargo net to lift
him, to prevent any injury to personnel. D/C Miller will personally go to this
resident's apartment (if/when there is another call) and evaluate the space. He will
speak with this resident and ask him to explore the possible purchase of a portable
lift (through his own insurance) to keep in the apartment. It was also suggested that

maybe Social Services could visit this individual to urge him to move to the first floor.
It would be very difficult to move him out of the building, and could endanger him in
the event of a fire.
• The December 12, 2009 Accident Refresher Class has been postponed due to the
Public Works Winter Driving Course. The new date to be announced.

F-28 JOHN GUGLIELMOA F/F LAIRD 8/31/09
FIRE DEPARTMENT
Description of Incident: Lt. Guglielmo was operating 142 to connect it to the
confined space trailer. Firefighter Laird was guiding Lt. Guglielmo. They were lining
up the ball in the bed of the trailer with the gooseneck receiver. The alignment has to
be correct in order to make the connection. F/F Laird had directed Lt. Gulielmo to
move forward a little and then back up again moving the ball to line up better with the
receiver. F/F Laird had closed the gate to give him more room in guiding the truck, but
did not relay this to Lt. Guglielmo. As Lt. Guglielmo moved forward, the gooseneck
receiver hit the tailgate.
Supervisor's Comments & Recommendations: Both the driver and the backer
must communicate back and forth whenever there are any changes that could impact
movement or cause an accident. This is vital in situations where the driver cannot see
the tailgate when looking in the side mirror as in this case.

F-29 09/36 FIRE DEPARTMENT 9/7/09
Description of Incident: Firefighter was at the foot of the stretcher pulling a patient
with his left arm while F/F was pushing from the head. Emergency Department
Security Officer opened the door from the inside as EGVFD crew approached with a
patient on the stretcher. After the ED doors opened and the F/F proceeded through,
the doors suddenly closed on him as he was in the doorway and struck him on the
right shoulder causing him to twist in an un-natural position. F/F experienced
discomfort about 5 to 10 minutes after the incident and mentioned it to Lt. Naida.
After arriving back in quarters, F/F stated to Lt. Naida that the discomfort did not
subside.
Supervisor's Comments & Recommendations: Recommend that the doors be
checked for proper safety detection device to prevent doors from closing on personnel
when in the doorway. j

=F=30 09/37 FIRE DEPARTMENT 09/1-7/09
Description of Incident: Firefighter responded on Al for a Code 3 report of something
burning at 2 Oakwood Drive. Upon arrival, the exited the ambulance and put on his
SCBA to assist Q8 in the investigation when he reported to the Lieutenant the odor was
burning his nose and eyes. Fire companies searched and found no reason for the odor
at which time the F/F informed Battalion Chief Sellers he was having a burning sensation
in his throat and chest with a non-productive cough.
Supervisor's Comments & Recommendations: When fire companies are investigating
unusual odors of an unknown nature they need to utilize their breathing apparatus even in
an outside environment especially if they personally find the odor irritating.

P-34 09/38_-:POLICE DEPARTMENT 09/22/09
Description of Incident: While investigating a domestic dispute/criminal damage to
property report, Officer and I were advised by the offender that the property we were
inquiring about (3 guitars) were in a fire pit in the backyard. The offender further stated
that we should check and validate this information. Upon checking the pit, we observed
the burnt remnants of the items in the pit. The officer then grabbed one of the pieces but
when she grabbed the second piece, she sustained burns to the thumb and index finger
of her left hand. There were no flames or smoke visible and as the officer had already
grabbed one of the items did not realize that the second item was hot.
Supervisor's Comments & Recommendations: Due to the fact that there were no
visible indicators that the evidence was hot, officer was not aware that the time required
special handling.

"P=35. OFC: GREENBERG = POLICE DEPARTMENT 10/11-L09
Description of Incident: Officer Greenberg, while conducting random patrol, pinched
the left rear tire against a curb when he turned into a driveway. Contact with the curb
caused damage to the tire.
Supervisor's Comments & Recommendations: Ofc. Greenberg was counseled
regarding the importance of being aware of his surroundings while traveling in areas
which he frequently does not drive.

F`=31 09/40 FIRE-DEPARTMENT 10/1:8709
Description of Incident: Lieutenant was assisting seven other members lift a patient
using the cargo net. The patient weighted in excess of 425 lbs. The patient wanted to be
placed in his bed which was approxi 36 inches off the ground. While placing the patient
in bed; Lieutenant twisted his back. The Lieutenant complained of left sided back pain
from his shoulder blade to his waist. At the time of this report, Lieutenant declined being
evaluated in the ER following the current Village policy regarding reportable injuries.
Supervisor's Comments & Recommendations: We have had this patient several
times before. We have tried to take every precaution possible. it appears that he is
gaining more weight. I would suggest that we have a second engine company respond in
order to balance the height of our personnel due to the height of the patient's bed.

'P-36 09/39 — POLICE DEPARTMENT 10/18/09
Description of Incident: Officer was westbound on Touhy Avenue approaching Tonne
Road when he applied his brakes. As the squad came to a stop, the rifle released from
the rack and struck Officer in the rear of his head, causing a laceration. .
Supervisor's Comments & Recommendations: This squad has been written up in
the past for the same malfunction of the rifle rack. The wiring in the car needs to be
checked again or replaced to avoid future occurrences.

zP=37t OFC. FINNEN_--POLICE-DE PARTMENT - 10/23/09
Description of Incident: Ofc. Finnen was checking squad 238 prior to the start of his
shift and located what appears to,be fresh damage on_the lower center of the rear
bumper. It appears something hit the bumper causing a gouge and small crack. The
vehicle had just been picked up by the second shift at the Meacham garage where it
had been since 10/19/09. The vehicle was picked up by the second shift and driven
directly to the station without incident. It is unknown where or how this incident
happened.
Supervisor's Comments & Recommendations: An unknown object came in contact
with the bumper and an unknown time/place. Unable to determine if vehicle was
stationary or in motion at the time of the incident.

1#42 09%41 - FIRE DEPARTMENT 10/26/09
Description of Incident: Firefighter awoke with right upper back pain at approximately
0600 AM. Firefighter stated he was unsure of what caused the pain. He was not able
to continue on AT He was released and replaced at 0700 AM.
Supervisor's Comments & Recommendations: Firefighter may need to re-evaluate
his sleeping quarters and mattress.
