6. CONSENT AGENDA
a. Consideration of a request from Mikerphone Brewery to waive the fee for a temporary
Class D liquor license ($2,100) for the Pale Pauper Day fest, benefiting the Elk Grove
VFW on May 13, 2017.
(The fee for a Class D Liquor License is $2,100.
(Mikerphone Brewery is partnering with the Village to help raise funds for the Elk Grove
VFW Hall.
(Proceeds from the event will help fund the installation of an elevator at Elk Grove VFW
Post 9284.)
b. Consideration of a request from the Elk Grove Park District, located at 499
Biesterfield Road, to waive FY 2017/18 vehicle sticker fees in the amount of$2,250.
(The Elk Grove Park District is seeking a waiver of FY 2017/18 vehicle sticker fees
for 5 passenger vehicles ($125), 13 Class-B trucks ($650), 17 Class-D/F trucks
($1,275), and 2 Class-H/J/K trucks ($200) for a total waiver of$2,250.
(It has been past practice of the Village Board to grant fee waivers for governmental
and non-profit organizations.
(The Director of Finance recommends approval.)

c. Consideration to waive the formal bidding process and award a purchase contract to On
Time Embroidery, Inc. of Elk Grove Village, IL for Fire Department uniforms and
accessories, in the amount of$34,290 from the General Fund.
(On March 21, 2017, the Village opened sealed bids for the purchase of Fire Department
uniforms and accessories.
(A legal bid notice was published in the Daily Herald on March 6, 2017.
(The current vendor was provided a bid packet and chose not to re-bid.
(One bid was received, from On Time Embroidery, Inc. of Elk Grove Village, IL.
(The Fire Department has purchased items from On Time Embroidery, Inc. and has a good
working relationship with the firm.
(The initial term of the contract is from May 1, 2017 through April 30, 2018.
(Adequate funds are budgeted for this contract in the FY 18 Fire Department General
Fund Budget.
(The Fire Chief recommends approval.)
d. Consideration to waive the formal bidding process and award a service contract to JG
Uniforms of Chicago, Illinois for the purchase of police officer uniforms in an amount not
to exceed $45,000.
(The Police Department has purchased uniforms and equipment from JG Uniforms for the
last 3 years.
(For the 2018 budget, JG Uniforms has agreed to extended our contract with no price
increase from the current year.
(Some of the benefits provided by JG include: they offer hand measured and sewn ballistic
vest carriers; they have the ability to work with several manufacturers of uniform specialty
items which hold costs down; they provide uniforms to approximately 100 other police
departments which allows JG to provide discounts due to high volume and also provides
acceptable delivery expectations as most of the uniform items are in stock; they possess all
of the Department's specialty embroidery set ups; and they do not charge for delivery or
restocking fees.
(The Chief of Police recommends approval.)
e. Consideration to contract with Victorin/Heartland Business Systems of Lisle, IL in the
amount of$45,548 for the purchase of the Hewlett Packard desktop computers contained
in the Village's FY 2018 budget.
(The Village previously established a four year rotating replacement cycle for its PC
hardware.
(The Village also previously standardized on a hardware vendor(HP) whose units have
been proven to handle the demands of our increasingly complex networks.
(Each desktop will come with a 256 gb solid state hard drive, Windows 10, and a 3-
year warranty, while each laptop will come with a similar configuration but a 4-year
warranty.
(The favorable price of these units is available to the Village by virtue of Hewlett
Packard's contract with the State of Illinois (contract number MNNVP-133).

(The Director of Information Technology recommends approval.)
f. Consideration of the following for fiscal year 2017-18:
- to award a professional services contract to Gallagher Bassett for the administration of
the Village's self-insured liability plan in an amount not to exceed$52,551; and
- to approve the Liability Insurance Premiums for fiscal year 2017-18 submitted by Arthur
J. Gallagher Risk Management Services at a premium level as determined for the following
coverage:

(The contract with Gallagher Bassett fluctuates based on the amount of claims over a three
year period.
(This year's total premium renewal represents an increase of 4.9%.
(High Excess Liability Pool (HELP) provides excess liability coverage in the amount of
$13 million in excess of the Village's underlying $2 million coverage.
(The Director of Finance recommends approval.)
g. Consideration to adopt Ordinance 3491 designating the representative to the Northwest
Suburban Municipal Joint Action Water Agency (JAWA) for the Village of Elk Grove
Village.
(This Ordinance designates and appoints Craig B. Johnson as Director of the Agency and
Jeffrey C. Franke and Samuel L. Lissner as its Alternate Directors for a two-year term
expiring on April 30, 2019.)

h. Consideration to adopt Ordinance No. 3492 amending Title 3. Chapter 2, Section 3-2-8,
Simplified Municipal Telecommunications Tax, of the Village Code of the Village of Elk
Grove Village, by increasing the Simplified Telecommunications Tax by 1% for a total
Simplified Telecommunications Tax Rate of 6%.
(The Village Board authorized the increase of the Simplified Telecommunications Tax in
the amount of 1%, for a total tax rate of 6% as part of the Elk Grove 2025 long range
construction program.
(The 1% increase will be earmarked for the Capital Projects Fund.
(This Ordinance amends a portion of Ordinance 3488 duly passed and approved on
February 28, 2017, amending the start date to July 1, 2017.)
i. Consideration to adopt the following:
• Ordinance No. 3493 amending Chapter 2, Section 12, Municipal Gas Use Tax of
Title 3 Business and License Regulations of the Village of Elk Grove Village.
• Resolution No. 22-17 authorizing the Mayor and Village Clerk to Execute a Tax
Collection Agreement between Northern Illinois Gas Company (Nicor) and the
Village of Elk Grove Village.
(The Village Board authorized the increase of the Municipal Gas Use Tax in the amount of
$0.010 per therm for a total tax rate of$0.020 per therm as part of the Elk Grove 2025 long
rage construction program.
(The $0.010 per therm increase will be earmarked for the Business Leaders Fund.
(The Tax Collection Agreement formalizes the necessary procedures for the tax collection
process and is an update to the existing agreement for the collection of the Municipal Gas
Use Tax by Nicor.)
j. Consideration to adopt Ordinance No. 3494 granting a variation of Section 3-7-:1)(3) of the
Zoning Ordinance to permit the construction of a six (6) foot high wood fence extending
approximately seventeen (1 T) beyond the nearest front corner of the principal structure
located on the adjacent single-family residential lot for property located at 520 Montego
Drive, Elk Grove Village.
(This item was previously discussed at the April 11, 2017 Village Board Meeting and
currently appears under Unfinished Business.)
k. Consideration to adopt Resolution 23-17 designating a Director and Alternate Directors to
the Northwest Municipal Conference for the Village of Elk Grove Village.
(This Resolution designates and appoints Craig B. Johnson as Director to the Northwest
Municipal Conference; and James P. Petri, Raymond R. Rummel, Matthew J. Roan, and
Maggie A. Jablonski as Alternate Directors to the Northwest Municipal Conference.)

m. Consideration to adopt Resolution No. 25-17 approving and authorizing the appointment
of a Director and Alternate Directors to the Solid Waste Agency of Northern Cook
County, a Municipal Joint Action Agency (SWANCC).
(This Resolution designates Craig B. Johnson as the Director on the Board of Directors of
the Agency; and appoints James P. Petri and Raymond R. Rummel as Alternate Directors,
for a two-year term expiring April 30, 2019 or until his or her successor is appointed.)
n. Consideration of the following items for Fiscal Year 2017-18:
• To authorize a 2.25% market adjustment plus a .25% equity adjustment to non-
union step and specific rate plan positions;
• To authorize a 2.25% market adjustment plus a .25% equity adjustment to the merit
pay plan positions;
• To continue the additional open merit pay eligibility features of 0%to 2% merit
based adjustment for open merit employees and 0%to 2% general wage
adjustment;
• To adopt Ordinance No. 3495 amending wages by 2.5% for part-time positions of

Custodian, Porter, Health Code Enforcement Officer,
Fire Inspector, and Water Meter Reader; Temporary Part-Time Administrative
Interns and Summer Seasonal Employees third year; and School Crossing Guards
of the Village of Elk Grove Village; and
• To adopt Ordinance No. 3496 amending the Position Classification and Salary
Plans: the Classification Plans: and Salary Range for, Merit Pay Plan Positions,
Step Pay Plan Positions, and Specific Pay Plan Positions of the Village Code of the
Village of Elk Grove Village.
(The Ordinances above update salary ordinances for Fiscal Year 2017-2018.)
(Ordinance No. 3495 and Ordinance No. 3496 will be available in the Village Clerk's
Office.)
o. Consideration to adopt the following Resolutions:
• Resolution No. 26-17 approving the Plat of Vacation identified as 225 Bond Street
L.L.L., 225 Bond Street; and
• Resolution No. 27-17 approving the Plat of Resubdivision identified as 225 Bond
Street L.L.L., 225 Bond Street.
(This item was discussed at the JPZ Committee Meeting on April 7, 2017.
(The purpose of the resubdivision is to facilitate the connection of two existing buildings
into one for use by Jackson Spring& Mfg. Company.
(The JPZ Committee recommends approval of the Vacation and Resubdivision.)

a. Consideration to adopt Ordinance No. 3497 amending Title 3, Business and License
Regulations, of the Village Code by adding thereto a new section with regard to conflicts
with certain Home Rule County Ordinances.
(Pursuant to Home Rule authority, the Village of Elk Grove finds it in the best interest of
the Village to amend the Village Code to clearly define the sick leave and minimum wage
regulations that apply to employers located in the Village of Elk Grove Village.
(This item was discussed at the April 11, 2017 Committee of the Whole Meeting.)
