The meeting was called to order at 8:40 A.M. at the Village Hall.

• Committee discussion regarding the CPR masks (in keychain form) for all village
employees versus putting some at the front counters. Conclusion was made that all
employees do not need one. Jack Gray will contact Tony Konetzke regarding cost of
this item. Need Village Manager to reconsider obtaining funding from the Wellness
Committee.
• The Fire Department completed training regarding hydraulic stretchers on the
ambulances.
• The Village needs to contact Alexian Brothers Medical Center regarding the existing
signage above the entrance to the parking garage. The signage needs to be corrected
to display the lowest height.

FIRE DEPARTMENT
Description of Incident: During recovery efforts, a pike was used to assist in the
removal of a victim from an 18,730 Ib. coil of aluminum. The extent of contamination
and the porous nature of the wood prevented adequate decontamination and the pike
was disposed.

P-38 BRIAN VIVONA= POLICE DEPT. 1112/09
Description of Incident: While driving through the parking structure a halogen light that
protrudes.above the roof of Unit 220 struck the bottom of a concrete beam. This action
caused the pole attached to the halogen light to bend which caused damage to the roof
of Unit 220.
Supervisor's Comments & Recommendations: Even though there was not proper
signage indicating a height change, Ofc. Vivona must be aware of any objects that may
be lower than the highest point on the vehicle.

P-39 RANDY SCHUMACHER— POLICE DEPT. 11/05109
Description of Incident: Ofc. Schumacher was responding to an I.L.E.A.S. call out in
Schaumburg, IL, and while he was westbound on Higgins Rd. the cell was upgraded to
a N.I.P.A.S. call out. Ofc. Schumacher activated his emergency equipment to expedite
his response. As he approached Plum Grove Rd., westbound traffic was backed up in
all lanes of traffic which caused him to use the shoulder to pass the stopped vehicles.
Ofc. Schumacher merged back to the center lane at the intersection and a vehicle
immediately in front of him slammed on its brakes and he struck the back of that
vehicle.
Supervisor's Comments & Recommendations: Per D/C Zinnel's 11/9/09 email,
Officer Schumacher was NOT at fault for this accident.

W-8 STEVE JOHNSON — PUBLIC WORKS 11/19/09
Description of Incident: While opening door to exit vehicle, a passing car mirror hit
Unit 817 door, breaking the mirror on other vehicle.
Supervisor's Comments & Recommendations: Pay more attention to surroundings
before exiting vehicle.

KEITH WILCOX—POLICE DEPT.
Description of Incident: On 12/2/09, Fleet Services advised that while servicing
Squad 224 it was noticed that the oil pan had damage. It appears that the vehicle had
struck a hard object (curb or rock) which caused the oil pan to dent. Fleet Services last
serviced the vehicle on 11/2/09 and no damage was reported. Fleet Services estimated
that the damage appears to have occurred with a week's time of being discovered.
Fleet Services related that they will not be repairing the damage. Shift supervisors have
been informed to check with noted drivers on their shifts to identify a driver.
Supervisor's Comments & Recommendations: Drivers need to use additional
caution when driving to be aware of objects or high barriers which may cause damage.

P49 OFC. PELAYO -POLICE DEPT._ 1218/09
Description of Incident: Officer Pelayo was legally parked facing S/B in the rear (east)
parking lot at 635 Meacham Rd. (Strawberry Hill Plaza) when a vehicle backing out of a
parking space struck the left (driver's) side rear quarterpanel of Squad 232. During a
review of the scene it was observed that it appeared the striking vehicle backed straight
out of the space, although there was adequate room for the vehicle to angle out of the
space.
Supervisor's Comments & Recommendations: Officer Pelayo was legally parked
with his lights on and striking vehicle driver would have walked within 25 feet of the
squad to enter her car.

F-34 09/42 — FIRE DEPT. 12/8/09
Description of Incident: Firefighter was assisting the transfer of an extremely obese
patient from the ambulance cot to the E.R. bed. The head of the cot was placed in line
with the foot of the E.R. bed. Firefighter was on the patient's left side at the middle of
the body. While attempting to slide the patient forward, firefighter had to step around
one of the wheels of the cot, causing him to turn his feet outward to avoid the obstacle.
Firefighter encountered pain in his back.
Supervisor's Comments & Recommendations: This situation creates a challenge for
personnel due to the size of the patient and the tight areas to move patient. Firefighters
must be careful not to twist back when moving heavy loads like this patient.

Description of Incident: Plowing parking lot pushed snow against fence and damage
two sections of wooden fence.

Description of Incident: She was leaving for lunch and she fell forward in the
handicapped hallway at EGV Hall — bilateral knee contusions.
Supervisor's Comments & Recommendations: Reminder to please use railing.

F-35 DAN RYAN& DEAN JAMROZEK'=-FIRE DEPT. -12/17/09
Description of Incident: While loading the patient into the ambulance, the front
loading wheel was caught in the frame of the door. The ambulance was parked on a
_ slope due to the natural slope of the street. As Firefighter Ryan and Firefighter
Jamrozek advanced the stretcher into the ambulance, the wheel broke.
Supervisor's Comments & Recommendations: Have a third person observe the
loading of the patient if the slope of the street is a factor.

P-42 OFC. MYLES GREENBERG — POLICE DEPT. 12/30/09
Description of Incident: Ofc. Greenberg was E/B on Touhy Avenue at N/B Tonne Rd.
and was stopped at the stop sign. He checked for N/B vehicles in traffic and it
appeared clear of vehicles. He proceeded into the intersection at which time he
observed a vehicle through the right front passenger window located in the left N/B lane
of Tonne. He saw that there was going to be an imminent collision. He applied the
brakes and was then struck by the left front corner of that vehicle in the right front
bumper and turn signal assembly of Police Squad 231.
Supervisor's Comments & Recommendations: It is always incumbent upon officers
to use all due caution when driving a vehicle. He was responding to a verbal domestic
dispute call and driving at normal speed to the call without using any emergency
equipment. He stopped at the stop sign and attempted to ascertain that it was clear to
proceed. Ofc. Greenberg related the video camera obstructed a portion of his view. In
closing, this crash.was avoidable and officers must use all strategies to work and look
around equipment present in the vehicle.

F-1-10 ROBERT MARSHALL — FIRE DEPT. 01/5/10
Description of Incident: Unknown damage to bumper. Only aware of damage after
informed by the Deputy Chief after car was in for service for 2 days.
Supervisor's Comments & Recommendations: Inspectors should visually check
assigned car before leaving the administration building parking and upon return.

F-2-10 10/01 - FIRE DEPARTMENT 01/7/10
Description of Incident: Firefighter was working at the scene of a structure fire. Fire
was out and ventilation and overhaul were in progress. Firefighter low-air alarm
sounded and he exited the building. He stepped out of the building, onto the rear patio
(snow-covered) and his left leg went forward and he felt a sharp tearing pain in his left
calf. Firefighter notified his partner and then came to notify his officer. He was advised
to seek medical treatment with an ambulance outside and this officer notified command
of the situation.
Supervisor's Comments & Recommendations: Trips and falls are a common hazard
when working at a fire scene with snow and freezing temperatures. Firefighter stepped
on to wet/snow covered wood decking and his foot slipped on this slippery surface. I
advised the firefighter that we must take our time and do our best to keep solid footing
underneath us, even when the conditions make that impossible to accomplish.
